Email survey in obtaining basic data for the development of clinical practice guideline for infertility in Korean Medicine (불임증 임상진료 지침 개발의 기초 자료 확보를 위한 전자우편 설문 조사)

  • Purpose: This survey was done in order to determine Korean medical doctors (K.M.D)' expertise on clinical practice guideline for infertility and the degree of concordance with the guideline. Methods: The questionnaires were distributed via e-mail to all members of Korean Medical Doctors Society and Korean Gynecology Society from August $30^{th}$ to September $1^{st}$ in 2010. Two hundred sixty nine of completed questionnaires were analyzed through two groups, members of the Gynecology Society and nonmembers of the Gynecology Society. Results: According to the analysis of the outcome of the survey done by 99 members of the Korean Gynecology Society and 170 non-members, two-thirds of the participants had prior information of the clinical practice guideline. 63.6 percent of the participants feel strong necessity of the development of Korean medical infertility clinical practice guideline. Seventy percent of the participants are willing to participate in developing or reviewing a clinical guideline. On the question in active utilization and concordance to the guideline once the Korean medical infertility guideline is developed, 44% answered they would while 34.6% said they would utilize the guideline after some modifications. On the prospect of the impact of the development and application of Korean medical clinical practice guideline for infertility, majority of the participants marked positively that the standardization of clinical practice for infertility will lead to the nation's participation with the Nation's Support Work and that the development will somewhat contribute to the standardization of the clinical practice. Conclusions: The survey confirmed high demand of the clinical practice guidelines for infertility and the degree of concordance for infertility amongst Korean Medical doctors.

  • Diabetes mellitus is reaching epidemic proportions worldwide. The insufficiency of medication to cope with this burden has coincided with a dramatic rise in the prevalence of use of complementary and alternative therapies, especially herbal treatments. This surge in demand presents a challenge to prove the safety and efficacy of these treatments in diabetes. Korean red ginseng (steam treated Panax ginseng C.A. Meyer) is a strong candidate to succeed. It has been shown to possess a multitude of hypoglycemic effects and improve metabolic disturbances related to diabetes in in vitro and animal models. Data in humans is also emerging to support these benefits. Whether these results can be replicated in a rigorous clinical testing program is unclear. We therefore investigated the antidiabetic effects of Korean red ginseng in a series of 2 acute and 1 longterm randomized, double-blinded, placebo-controlled clinical trials. This paper provides the rationale for this program of study, expanding on the problem of diabetes, its management, and the possible role for Korean red ginseng. It then describes the design and expected findings.

Analysis of the Effect of Patients' Clinical Conditions on No-Shows (외래 환자의 임상특성이 예약 부도에 미치는 영향 분석)

  • This study focuses on analyzing no-shows associated with patients' clinical characteristics described by diagnoses in their medical data. A dataset of 7,055 patient-records from a Veterans hospital in the United States was used to test if there is difference on no-shows along with each patient's diagnosed diseases and the number of diagnoses. Patients with mental diseases such as drug dependence abuse and major depression, and chronic diseases such as hypertension are more likely to no-show. In comparisons with the number of diagnoses, the no-show decreases as the number of diagnoses increases up to four and doesn't change significantly afterwards. We provide managerial insights on clinical operations problems from statistical analysis. We believe that our results can be used to develop appropriate solutions on no-shows in clinics.

Study of Stress, Health Promotion Behavior, and Quality of Life of Nursing Students in Clinical Practice and Factors associated with Their Quality of Life (간호대학생의 임상실습 스트레스, 건강증진행위, 삶의 질과 영향요인에 관한 연구)

  • Purpose: The purpose of this study was to identify factors associated with quality of life of nursing students during clinical practice. Methods: A convenience sample of 332 nursing students completed the questionnaire from July to September, 2013. A self-reported questionnaire was used to obtain data on quality of life, stress related to clinical practice, and health promotion behavior. Data was analyzed with independent t-test, one way ANOVA and Scheff${\acute{e}}$'s post hoc test, Pearson correlation coefficients, and hierarchical multiple regression by using SPSS version 20.0. Results: There were significant differences in quality of life scores by age, economic status, interpersonal relationships, daytime somnolence, number of visits in pharmacy and/or healthcare center, health status, and stress management. Quality of life had positive correlation with health promotion behavior but had negative correlation with the level of stress related to clinical practice. Models including these variables explained 34.3% (F=9.77, p<.001) of the variance for quality of life. Age, economic status, sleep amount, numbers of pharmacy/hospital visits, health status, stress during clinical practice, and health promotion behavior were significantly associated with quality of life in nursing students. Conclusion: There is a need to develop and implement new strategies that will result in improvement in nursing students' quality of life.

Prediction and visualization of CYP2D6 genotype-based phenotype using clustering algorithms

  • This study focused on the role of cytochrome P450 2D6 (CYP2D6) genotypes to predict phenotypes in the metabolism of dextromethorphan. CYP2D6 genotypes and metabolic ratios (MRs) of dextromethorphan were determined in 201 Koreans. Unsupervised clustering algorithms, hierarchical and k-means clustering analysis, and color visualizations of CYP2D6 activity were performed on a subset of 130 subjects. A total of 23 different genotypes were identified, five of which were observed in one subject. Phenotype classifications were based on the means, medians, and standard deviations of the log MR values for each genotype. Color visualization was used to display the mean and median of each genotype as different color intensities. Cutoff values were determined using receiver operating characteristic curves from the k-means analysis, and the data were validated in the remaining subset of 71 subjects. Using the two highest silhouette values, the selected numbers of clusters were three (the best) and four. The findings from the two clustering algorithms were similar to those of other studies, classifying $^*5/^*5$ as a lowest activity group and genotypes containing duplicated alleles (i.e., $CYP2D6^*1/^*2N$) as a highest activity group. The validation of the k-means clustering results with data from the 71 subjects revealed relatively high concordance rates: 92.8% and 73.9% in three and four clusters, respectively. Additionally, color visualization allowed for rapid interpretation of results. Although the clustering approach to predict CYP2D6 phenotype from CYP2D6 genotype is not fully complete, it provides general information about the genotype to phenotype relationship, including rare genotypes with only one subject.

Retrospective clinical and radiographic evaluation of restored endodontically treated teeth

  • Objectives: The aim of this study was to perform a clinical and radiographic analysis of endodontically treated teeth (ETT) restored with cast metal posts (CMPs) or prefabricated glass fiber posts (GFPs) and crowns. Materials and Methods: Fifty ETT were restored with 25 CMPs and 25 GFPs at a private dental clinic between 2001 and 2016. The restorations consisted of 12 all-ceramic crowns, 31 metal-ceramic crowns, and 7 composite resin crowns. Demographic data, type of teeth, type of post-and-core system, time of placement, crown restorations, the number of proximal contacts, the type of antagonist, and reports of any complications after post-and-core placement were recorded for each patient. Assessments were performed at baseline (radiographic) and follow-up (radiographic and clinical). Data were analyzed by the McNemar test, the Pearson χ2 test, and Kaplan-Meier survival curves (α = 0.05). The mean follow-up was 67.6 months. Results: No significant difference was observed for any of the radiographic parameters when the baseline and final radiographs were compared. In the clinical evaluation, anatomical form (p = 0.009) and occlusion (p = 0.001) showed significant differences according to the type of crown restoration; specifically, metal-ceramic and all-ceramic crowns outperformed composite resin crowns. Conclusions: CMPs and GFPs showed favorable results for restoring ETT after 6 years of follow-up. All-ceramic and metal-ceramic crowns showed higher survival rates and better clinical outcomes.

Reference ranges for autonomic function tests in healthy korean adults

  • Background: The standardized autonomic function test has become widely available. However, there are no reference data for this test for the Korean population. This study explored reference data for sudomotor and cardiovagal function tests for the Korean population. Methods: The sweat volume by quantitative sudomotor axon reflex test, heart-rate response to deep breathing (HRdb), expiration:inspiration (E:I) ratio, and Valsalva ratio (VR) were measured in 297 healthy Korean volunteers aged from 20 to 69 years. Multivariate regression analysis was performed to evaluate the effects of age, sex, and body mass index on these variables. The 2.5th, 5th, 10th, 90th, 95th, and 97.5th percentile values were obtained for each investigation. Results: The sweat volume was higher in males than in females. The HRdb and E:I ratio were negatively correlated with age, and were higher in males than in females. The VR was negatively correlated with age, but it was not correlated with sex. Conclusions: This study has provided data on the reference ranges for sudomotor and cardiovagal function tests in healthy Korean adults.

Nursing Ethics Values of Nursing Students (간호대학생의 간호윤리 가치관)

  • This study was to investigate the effect of deontology and utilitarianism on the ethical values of nursing students and to provide basic data. Data collection was conducted from May 14 to June 1, 2018 in 180 students of a university nursing student in Daegu who explained their research and agreed to the study. The collected data were analyzed using the SPSS 21.0 program. In order to confirm the difference between the subjects' demographic characteristics and nursing-related characteristics, the independent t-test and ANOVA were used, and the post-test was conducted using the $Sch{\acute{e}}ffe$ post-test. The ethical values of the nursing students were $97.51{\pm}8.96$ out of 150, indicating that they were inclined toward the position of deontology. The first grade students were more dominant, and the higher the grade, the more inclined toward utilitarianism. It is necessary to develop education to establish ethical values so that ethical decision making can be done through philosophical thinking when various nursing dilemma situations in the clinical field are based on the results.

Measurement of Flash Points and Autoignition Temperatures for Xylene Isomers (크실렌 이성질체의 인화점과 최소자연발화온도의 측정)

  • In order to investigate the compatibility of data in MSDS(Material Safety Data Sheet), the flash point of xylene isomer was measured by using Pensky-Martens closed cup (ASTM D93), Setaflash closed cup(ASTM D3278), Tag open cup(ASTM D1310), and Cleveland open cup (ASTM D92) testers. Also, the AITs(autoignition temperatures) of xylene isomers were measured by using ASTM E659-78 tester. The measured the flash points and the AITs were compared with literatures and MSDS in KOSHA(Korea Occupational Safety and Health Agency). The measured the flash points and the AITs were different from those in literatures and MSDS. As a result, this paper is shown that it is needed to investigate combustion characteristics of xylene isomer for the fire safety objectives.

Anti-inflammatory Effects of Low-frequency Stimulator using Superposition of Alternating Microcurrent Wave in the Animal Models

  • Treatment techniques that affect homeostasis by non-invasive regulation in peripheral organs will advance disease research. Here, we demonstrate a non-invasive method of conditioning within an organ using a low-frequency stimulator superposition of alternating microcurrent wave in stages. It is first applied to the inflammatory response in H3N2-infected sinusitis mice. To check the progress of the treatment, mice were sacrificed every week for 3 weeks, nasal tissue was removed, and the inflammatory response was investigated through H & E staining. The low-frequency stimulation treatment group was found to alleviate the proliferation of epithelial cells and invasion of inflammatory cells compared to the control group as the passage of treatment time. The reduction of inflammatory cytokines in the nasal lavage fluid was observed in H3N2-infected sinusitis mice treated with of low-frequency stimulation using superposition of alternating microcurrent wave compared to H3N2-infected sinusitis mice after 3 weeks. These data demonstrate that low-frequency stimulation device in the form of using alternating current wave superposition on within organs provides a new method to regulate specific physiological functions. Therefore, it is necessary to prove the inhibitory effect of low-frequency stimulation using alternating current wave superposition on inflammatory diseases by various methods through further studies and clinical studies.
