A Case Report on Nausea, Retching, Vomiting and Postprandial Fullness Treated with Soshiho-tang (소시호탕(小柴胡湯)을 투여한 오심, 구역, 구토 및 식후포만감 1례)

  • Objectives : "The purpose of this study is to report the effectiveness of Soshiho-tang(小柴胡湯)" on the patient suffered from nausea, retching, vomiting and postprandial fullness. Methods : We diagnosed the symptoms and the signs of the patient as Soshiho-tang syndrome(小柴胡湯證) according to "Treatise on Cold Damage Disease(傷寒論)". We prescribed Soshiho-tang with acupuncture and moxibustion for about 2 weeks. Results : The subjective assessments of the nausea and postprandial fullness had improved. And the frequencies of the retching and vomiting had decreased. Conclusions : We suggest that Soshiho-tang has effectiveness on nausea, retching, vomiting and postprandial fullness in patient diagnosed as Soshiho-tang symptom.

Study of Characteristics of Patients with Chronic Fatigue

  • Objective: This study aimed to produce a description of features of chronic fatigue patients visiting an Oriental Hospital. Methods: 121 patients (75 male and 46 female) who visited a fatigue-care center were enrolled in this study, and the main complaints besides fatigue feeling, symptom differentiation, and health-related factors were analyzed. Results: On a visual analogue scale (VAS), average severity of fatigue was 7.0 (male 6.77 and female 7.4; 0 indicates no fatigue vs. 10 indicates intolerable fatigue), and the main complaints included headache (male and female) and dyspepsia (female). Patients generally slept about 7 hrs and caught a cold 2.4 times per year (same in both male and female). 54.6 % of patients used functional supplements, and red ginseng, vitamin and herbal medicine were chosen in that order. Conclusions: Chronic fatigue is a main target of traditional Korean medicine (TKM). This study can be helpful to design TKM-based treatment for chronic fatigue-related symptoms.

  • Pericarp of Zanthoxylum coreanum Nakai have been used as a crude medicine for the treatment of ozena, rheumatoid, nasal sinusitis, meno-xenia, dyspepsia, toothache, sore throat, pains in the limbs, etc. in Korea. A sterol fraction and two compounds, compound I and II were isolated from the roots, stems, leaves of Zanthoxylum coreanum Nakai. The sterol fraction was identified as a mixture of ${\beta}-sitosterol$ and campesterol. Compound I, colorless prismatic crystals, $C_{13]H_{13}NO_4$, mp. $175{\sim}177^{\circ}$, was proved to be skimmianine which is one of the alkaloids and compound II, $C_{14}H_{28}O_2$, mp. $53{\sim}55^{\circ}$, white powdered crystals was identified as myristic acid.

  • A case of a 38 year-old woman with histrionic personality who had chronic epigastric pain, dyspepsia and alternating bowel habit for more than 10 years was presented in detail on its course of two times of admission and follow-up. The diagnosis was thought as psychophy-siological disorder or gastrointestinal motility disorder of undefined etiology rather than hypo-chondriasis or Briquet's syndrome. She was characterized by sustained illness behavior and combined several physical illnesses. i.e. tuberculosis. anemia and hepatic stone. These physical diseases led to a blurring of psychological and physical boundaries regarding symptom formation. The points on consultation from medical part to psychiatric department were discussed and the supposed causal mechanisms in non-organic functional gastrointestinal disturbances were also reviewed. Physical and psychological modalities for the treatment and the abnormal illness behavior were mainly emphasized in this case.

  • The various species of aconitum contain diterpene (C-20) and norditerpene (C-19) forms of the natural alkaloid aconitine that cause neurologic, gastrointestinal, cardiovascular symptoms. In chinese medicine, these plants has been used as drugs to treat pain, dyspepsia, cerebrovascular disease, and so on. Because the therapeutic window is narrow, poisoning may occur from unintentional exposure, with a variety of toxic effects such as arrhythmia, hypotension, paresthesia, paralysis, nausea, vomiting, and abdominal pain. Aconitine-containing folk remedies are widely used in Korea. We encountered two cases of ventricular arrhythmia in patients who ingested tablets, known as 'Wha-Pung-Dan' made with aconitine extracts and were subsequently admitted to the ED. A 42 year-old man who took 35 tablets presented with ventricualr tarchycardia, and 40 year-old woman who took 40 tablets showed premature ventricular contractions. Both patients were discharged normally without any complications after three days.

  • Aconites distribute widely in northern hemispere of the earth reaching to arctic zone from warm and temperate one. Chinese crude drug 'Cho O'(草烏) has been used as a remedy for apoplexy, dyspepsia, neuralgia, chronic rheumatis etc. Though the botanical origin of the crude drug has been considered to be Aconitum species of Ranunculaceae, there has been no confirmation on it. To clarify the botanical origin of Cho O, the morphological and anatomical characteristics of the roots of Aconitum species growing in Korea, i.e. A. chiisanense, A. ciliare, A. jaluense, A. napiforme, A. proliferum, A. pseudoproliferum and A. triphyllum were studied. As the result, Cho O was proved to be the roots of Aconitum triphyllum, A. jaluense, A. chiisanense and A. napiforme.

  • Purpose : This paper is aim to report the effects of Gamisoyosan(加味逍遙散) on the UL-syndrome(鬱症) Methods : This is two cases report of female patient suffering from headache, chest discomfort, heart burning, insomnia, dyspepsia, anorexia, fatigue and other symptoms due to chronic stress. 52 years old woman who diagnosed UL-syndrome(鬱症) was enrolled in this study. We gave her Gamisoyosan(加味逍遙散) 2 times, each for 3 weeks. During the treatments, we check changes of symptoms and HRSD. Results : After the oriental medical treatments, symptoms of UL-syndrome(鬱症) were decreased or disappeared. Conclusion : This clinical case indicate that Gamisoyosan(加味逍遙散) is effective in treatment of the depression disorder and symptoms of UL-syndrome(鬱症). More clinical data and studies are requested for the treatment of UL-syndrome(鬱症).

Characteristics of Goat Milk and Current Utilizing Trends in Korea (산양유의 특성 및 국내 이용 현황)

  • Goat milk is digested better than cow milk, because the fat globules in goat milk are smaller and the protein is similar to human milk, and assimilated easily. Goat milk is particularly rich in taurine (4.7mg/100mL) and retinol (40mg/100mL). Therefore, it might be recommened to heal dyspepsia and infants allergy by cow milk. However, during winter, supply of goat milk products are unsteady in Korea, because unbalance of demand and supply is resulted from seasonal breeding. Dairy industry for goat milk will be able to grow much more, if goat milk products can keep steady supply without changing by season. This review includes the physicochemical characteristics of goat milk, domestic production of goat milk, domestic goat milk products and future development.

Pharmacognostical Studies on the ‘Hoo Bak’ (후박(厚朴)의 생약학적 연구)

  • 'Hoo Bak' is one of the Chinese crude drugs used mainly to cure a headache, apoplexy and dyspepsia. With regard to the botanical origin of 'Hoo Bak', it has been considered to be Machilus thunbergii of Lauraceae in Korea. But there has no pharmacognostical confirmation on it. To clarify the botanical origin of 'Hoo Bak', the anatomical characteristics of the bark of Machilus thunbergii, Magnolia officinalis and Magnolia obovata were studied. As a result, it was clarified that 'Hoo Bak' from Korea was the bark of Machilus thunbergii of Lauraceae.

  • Chronic abdominal pain (CAP) is a common complaint encountered in pediatric clinics and a great concern for patients and their caretakers as well as health care professionals. A constant challenge is detecting individuals with organic diseases or psychosomatic disorders from the majority of patients who have a functional disorder including functional dyspepsia, irritable bowel syndrome, functional abdominal pain, and abdominal migraine. Beginning with a detailed history and physical examination, physicians must determine a differential diagnosis of CAP by applying the symptom-based Rome III criteria to positively identify a functional disorder. These findings should then be further analyzed based on diagnostic clues and red flags that indicate the presence of specific organic diseases and/or the need for further testing. Once a functional diagnosis has been made or an organic disease is suspected, physicians can initiate an empiric therapeutic trial. Since psychological distress accompanies both organic and non-organic abdominal pain in children, a cooperative diagnostic approach involving pediatricians and psychiatrists is recommended.
