체간운동이 하지기능장애 한센노인의 균형능력에 미치는 영향 (The Effects of Trunk Exercises on the Balancing Ability of Elderly Hansen's Disease Patients with Lower Extremity Dysfunction)

  • Purpose : The purpose of this study was to investigate the effects of trunk exercises on the balancing ability of elderly Hansen's disease patients with lower extremity dysfunction. Method : A total of 24 elderly Hansen's disease patients were divided into two groups: 10 without lower extremity dysfunction and 14 with lower extremity dysfunction. The groups exercised for 60 minutes, two days a week, for a total 12 weeks; balancing ability was measured with the one leg standing test, tandem walking test, and timed up-and-go test. The patients were tested and their results were compared both before and after the completion of their exercise programs. Lower extremity dysfunction was assessed according to the following criteria: unilateral foot-drop, toe-loss, and below-knee amputation. Results : After the exercises, participants in both groups showed a positive, statistically significant difference in balance, compared with before the exercises (the one leg standing test, tandem walking test, and timed up-and-go test; p<.05). For comparison purposes, the group with dysfunction and the group without dysfunction were tested before and after the completion of their exercises. Before the exercises, there was a statistically significant difference in the one leg standing test, tandem walking test, and timed up-and-go test (p<.05). However, after the exercises, there was no significant difference in the one leg standing test, tandem walking test, and timed up-and-go test (p>.05). Conclusion : Ultimately, balancing ability was improved in both of the groups after trunk exercises were performed. Although balancing ability was improved, elements of lower extremity dysfunction remained, such as unilateral foot-drop, toe-loss, and below-knee amputation.

Gateway RFP-Fusion Vectors for High Throughput Functional Analysis of Genes

  • There is an increasing demand for high throughput (HTP) methods for gene analysis on a genome-wide scale. However, the current repertoire of HTP detection methodologies allows only a limited range of cellular phenotypes to be studied. We have constructed two HTP-optimized expression vectors generated from the red fluorescent reporter protein (RFP) gene. These vectors produce RFP-tagged target proteins in a multiple expression system using gateway cloning technology (GCT). The RFP tag was fused with the cloned genes, thereby allowing us localize the expressed proteins in mammalian cells. The effectiveness of the vectors was evaluated using an HTP-screening system. Sixty representative human C2 domains were tagged with RFP and overexpressed in HiB5 neuronal progenitor cells, and we studied in detail two C2 domains that promoted the neuronal differentiation of HiB5 cells. Our results show that the two vectors developed in this study are useful for functional gene analysis using an HTP-screening system on a genome-wide scale.

Cardiac Dysfunction Is Not Associated with Increased Reintubation Rate in Patients Treated with Post-extubation High-Flow Nasal Cannula

  • Background: Cardiac dysfunction patients have long been considered at high risk of reintubation. However, it is based on past studies in which only conventional oxygen therapy was applied after extubation. We investigated association between cardiac dysfunction and reintubation rate in situation where high-flow nasal cannula (HFNC) was widely used during post-extubation period. Methods: We conducted a retrospective observational cohort study of patients treated with HFNC after planned extubation in medical intensive care unit of single tertiary center. Patients were divided into normal function group (ejection fraction [EF] ≥45%) and cardiac dysfunction group (EF <45%). The primary outcome was reintubation rate within 72 hours following extubation. Results: Of 270 patients, 35 (13%) had cardiac dysfunction. Baseline characteristics were similar in both groups. There were no differences in the changes in vital signs between the two groups during the first 12 hours after extubation except diastolic blood pressure. The reintubation rates were 20% and 17% for cardiac dysfunction group and normal function group, respectively (p=0.637). In a multivariate Cox regression analysis, cardiac dysfunction was not associated with an increased risk of reintubation within 72 hours following extubation (hazard ratio, 1.56; p=0.292). Conclusion: Cardiac dysfunction was not associated with increased reintubation rate within 72 hours when HFNC is immediately applied after planned extubation.

Ginsenoside Rg1 ameliorates chronic intermittent hypoxia-induced vascular endothelial dysfunction by suppressing the formation of mitochondrial reactive oxygen species through the calpain-1 pathway

  • Background: As the major pathophysiological feature of obstructive sleep apnea (OSA), chronic intermittent hypoxia (CIH) is vital for the occurrence of cardiovascular complications. The activation of calpain-1 mediates the production of endothelial reactive oxygen species (ROS) and impairs nitric oxide (NO) bioavailability, resulting in vascular endothelial dysfunction (VED). Ginsenoside Rg1 is thought to against endothelial cell dysfunction, but the potential mechanism of CIH-induced VED remains unclear. Methods: C57BL/6 mice and human coronary artery endothelial cells (HCAECs) were exposed to CIH following knockout or overexpression of calpain-1. The effect of ginsenoside Rg1 on VED, oxidative stress, mitochondrial dysfunction, and the expression levels of calpain-1, PP2A and p-eNOS were detected both in vivo and in vitro. Results: CIH promoted VED, oxidative stress and mitochondrial dysfunction accompanied by enhanced levels of calpain-1 and PP2A and reduced levels of p-eNOS in mice and cellular levels. Ginsenoside Rg1, calpain-1 knockout, OKA, NAC and TEMPOL treatment protected against CIH-induced VED, oxidative stress and mitochondrial dysfunction, which is likely concomitant with the downregulated protein expression of calpain-1 and PP2A and the upregulation of p-eNOS in mice and cellular levels. Calpain-1 overexpression increased the expression of PP2A, reduced the level of p-eNOS, and accelerated the occurrence and development of VED, oxidative stress and mitochondrial dysfunction in HCAECs exposed to CIH. Moreover, scavengers of O2·-, H2O2, complex I or mitoKATP abolished CIH-induced impairment in endothelial-dependent relaxation. Conclusion: Ginsenoside Rg1 may alleviate CIH-induced vascular endothelial dysfunction by suppressing the formation of mitochondrial reactive oxygen species through the calpain-1 pathway.

Outcomes after Biventricular Repair Using a Conduit between the Right Ventricle and Pulmonary Artery in Infancy

  • Background: This study investigated the outcomes of biventricular repair using right ventricle to pulmonary artery (RV-PA) conduit placement in patients aged <1 year. Methods: Patients aged <1 year who underwent biventricular repair using an RV-PA conduit between 2011 and 2020 were included in this study. The outcomes of interest were death from any cause, conduit reintervention, and conduit dysfunction (peak velocity of ≥3.5 m/sec or moderate or severe regurgitation). Results: In total, 141 patients were enrolled. The median age at initial conduit implantation was 6 months. The median conduit diameter z-score was 1.3. The overall 5-year survival rate was 89.6%. In the multivariable analysis, younger age (p=0.006) and longer cardiopulmonary bypass time (p=0.001) were risk factors for overall mortality. During follow-up, 61 patients required conduit reintervention, and conduit dysfunction occurred in 68 patients. The 5-year freedom from conduit reintervention and dysfunction rates were 52.9% and 45.9%, respectively. In the multivariable analysis, a smaller conduit z-score (p<0.001) was a shared risk factor for both conduit reintervention and dysfunction. Analysis of variance demonstrated a nonlinear relationship between the conduit z-score and conduit reintervention or dysfunction. The hazard ratio was lowest in patients with a conduit z-score of 1.3 for reintervention and a conduit z-score of 1.4 for dysfunction. Conclusion: RV-PA conduit placement can be safely performed in infants. A significant number of patients required conduit reintervention and had conduit dysfunction. A slightly oversized conduit with a z-score of 1.3 may reduce the risk of conduit reintervention or dysfunction.

혈액투석 환자의 기능부전 도관관리 실무지침개발 (Development of a Practice Guideline for Catheter Dysfunction in Hemodialysis Patients)

  • Purpose: This study was done to develop an evidence-based practice guideline for catheter dysfunction in hemodialysis patients. Methods: Development of the guideline process was done according to the De Novo development version 1.0 by NECA which consists of 12 steps. Results: The developed guideline consisted of 5 domains and 14 recommendations. The number of recommendations for each domain were: 3 on catheter dysfunction assessment, 1 on conservative management of catheter dysfunction, 7 on drug management of catheter dysfunction, 1 on catheter function test and 2 on maintenance management. Of the recommendations, 7.15% were marked as A grade, 52.85% of B grade, and 50% of C grade. Conclusion: Findings in this study indicate that this guideline can be added to the evidence-based practice guidelines for fundamentals of practice and that this guideline can be disseminated to nurses nationwide in order to improve the care of hemodialysis patients with catheter dysfunction.

  • The Internet cyber space has become more important as it enters the intelligent information society of the 4th Industrial Revolution beyond the information age through the development of ICT, the expansion of personalized services through mobile and SNS, the development of IoT, big data, and artificial intelligence. The Internet has formed a new paradigm in human civilization, but it has focused only on the efficiency of its functions. Therefore, various side effects such as information divide, cyber terrorism, cyber violence, hacking, and personal information leakage are emerging. In this situation, facing the intelligent information society can lead to an uncontrollable chaos. Therefore, this study classifies the cyber dysfunction of intelligent information society and analyzes social cognition, suggests cyber dysfunction standard of intelligent information society, and examines the seriousness of dysfunction, and suggests technical research directions for future technologies and services. The dysfunctional classification of the intelligent information society was classified into five areas of cyber crime and terrorism, infringement of rights, intelligent information usage culture, intelligent information reliability, and social problems by FGI methodology. Based on the classification, the social perception of current and future cyber dysfunction severity was surveyed and it showed female is more sensitive than male about the dysfunction. A GAP analysis confirmed social awareness that the future society would be more serious about AI and cyber crime

발기부전 치료의 한의학적 접근방법에 관한연구 (Study on the Treatment of Erectile Dysfunction in Oriental Medicine)

  • Erectile dysfunction is defined as the inability to attain and maintain penile rigidity sufficient to allow sexual intercourse. Although erectile dysfunction is usually considered a benign disorder, it has a dramatic impact on quality of life of the patients as well as their sexual partners. And it is common in men between the age of 40 and 70 years, and its incidence increases with age. The prevalence is reported to occupy 10% at the age of 40 years, 20% in 50s, 30% in 60s and 50% at the age of 70 years, and is more prevalent as stress in modern life and interest onsexual behavior increase. This article has aims to investigate and summarize the current trend of treatment for erectiledysfunction so as to suggest the effective and available way to treat the disease. In oriental medicine, erectile dysfunction has been treated with herb medicine and acupuncture with good results. Some of herb drugs have the substances which induce penile erection. So it should be investigated on the neurotransmitter or endothelial mediator which can be included in herb drugs. The acupuncture therapy stimulates the erectile nerves and is reported to be effective for erectile dysfunction. And suppository, locus injection therapy and topical agent are found to be effective as well as stellate ganglion block and biofeedback treatment. So the study on the approach and application of these treatments on erectile dysfunction would be necessary.

여성 성기능장애의 예측 모형 (Construction of an Explanatory Model of Female Sexual Dysfunction)

  • Purpose: Although concerns of female sexual dysfunction (FSD) are increasing in Korea, sexual dysfunction related factors are limited in research studies. The aim of this study was to develop an explanatory model that will further explain the continuously increasing female sexual dysfunction cases in Korea. Methods: Survey visits were conducted to four hundred and eighty five women, over 25 years of age and presently residing in either urban or rural areas. All of them were analyzed using a structured questionnaire. A total of 8 instruments were used in this model. The analysis of data was done with both SPSS WIN for descriptive statistics and AMOS 5.0 for covariance structure analysis. Results: As a result, variables that showed notably direct effects on FSD were: sexual concept (sexual attitude), sexual distress, and psychosocial health (depression, crisis, traumatic life events). On the other hand, variables such as age, educational level, economic status, and marital status showed indirect influences on health-promoting behaviors. Conclusion: By comprehensively addressing the factors related to sexual dysfunction, and comparing each influence, this study can contribute to designing an appropriate sexual dysfunction prevention strategy in tune with the particular characteristics and problems of a client.

급성 진행성 치주염환자에서 백혈구 화학주성 기능부전에 관한 연구 (Peripheral Blood Leukocyte Chemotaxis Dysfunction in Rapidly Progressive Periodontitis)

  • For the assessment of peripheral blood leukocyte chemotaxis dysfunction in rapidly progressive periodontitis, peripheral blood leukocyte of 13 patients were isolated, and tested by the method of blind well chamber technique which was recently developed by Park. Data analysis was performed from the speed of cell migration from the bottom of filter paper to the top for 18 minutes at the condition of chemo-attractant(Zymosan-activated serum). Eight of the 13 patients (62%) exhibited statistically significant inhibition of leukocyte chemotaxis, 4 patients revealed normal function and the other one patients showed elevated leukocyte chemotaxis. Four of the 8 patients in chemotaxis dysfunction showed significant inhibition of random migration and chemotaxis dysfunction both. Two of these 4 patients exhibited random migration impairment, elevation of C.D.I. and chemotaxis dysfunction. The results indicated that the progression of rapidly progressive periodontitis was influenced by the leukocyte chemotaxis dysfunction, and half of these dysfunction was associated with cell-defect of leukocyte.
