Development of Rating Systems for Power Transmission Bevel Gears (동력전달용 베벨기어의 강도평가 시스템 개발 연구)

  • Rating systems of bevel gears(straight, spiral, and zerol bevel gears) which are commonly used as power transmission devices for non-papallel axes are developed on the personal computer, which analyze and/or evaluate the gear design and the service performance at the point of view of strength and durability. The typical considerations of the ratings are the bending strength, the surface durability, and the scoring resistance. The ratings are carried out using the reliable standards of AGMA & Gleason Works. Therefore, the system is built so that the variables or factors considered differently in those standards and the strength, dura- bility, and scoring partially in Gleason are appraised seperately by each method, and a series of the estimation processes is integrated into the system so as to compare each result. The developed rating systems can be used in the initial stage of gear design process, and also a better design can be performed by the evaluation of the initial design at the view point of gear strength and durability. Additionally, it is useful for the trouble-shooting of bevel gear system and to the purpose of introducing the methods for maintaining design strength in service, with appraising the gear strength after design or with appraising the influencing factor as a whole. Therefore, this rating systems can help the aim of design automation of bevel gears.

An unusual arrangement between the highest denticulate ligament and posterior inferior cerebellar artery

  • The posterior inferior cerebellar artery (PICA) is often involved in pathologies of the posterior cranial fossa. Therefore, a good understanding of the vessel's normal and variant courses is important to the neurosurgeon or neurointerventionalist. During the routine microdissection of the craniocervical junction, an unusual arrangement between the highest denticulate ligament and PICA was observed. On the right side, the PICA was given rise to by the V4 segment of the vertebral artery 9 mm after the artery entered the dura mater of the posterior cranial fossa. The artery made an acute turn around the lateral edge of the highest denticulate ligament to then recur 180 degrees and travel medially toward the brainstem. Invasive procedures that target the PICA should be aware of the variant as described herein.

A RADIOGRAPHIC STUDY OF FIBRO-OSSEOUS LESIONS IN THE JAWS (악골의 섬유성골병소에 관한 X선학적 연구)

  • This study was undertaken to document and better defined this condition to help clarify this clinical and radiographical appearances by the analysis of clinical and radiographical features of fibro-osseous lesions in the jaws. A study was made of a series of 128 cases with fibro-osseous lesions. The obtained results were as follows. 1. Fibrous dysplasia of the jaws occurred with equal predilection for males and females. But the females occurred in 68% of cemento-ossifying fibroma and 75% of periapical cemental dysplasia. 2. 43% of fibrous dysplasia and 32% of cemento-ossifying fibroma occurred in the 2nd decades and 33% of periapical cemental dysplasia in 5th decades. 3. 62% of fibrous dysplasia occurred in the maxilla, 73% of cemento-ossifying fibroma in mandible, 90% of periapical cemental dysplasia in mandible. 4. 98% of fibrous dysplasia occurred in premolar-molar region, 77% of cemento-ossifying fibroma in molar region, 68% of periapical cemental dysplasia in incisor region. 5. In serial radiographic features, mature stage were 55% of fibrous dysplasia, 45% of cemento-ossifying fibroma, 59% of periapical cemental dysplasia. 6. 87% of fibrous dysplasia had monostotic lesion, 67% of periapical cemental dysplasia had multiple lesions. 7. In fibrous dysplasia and cemento-ossifying fibroma, migration of tooth occurred in 61.7% and 36.4%, retention of tooth occurred in 4.3% and 9.1%, loss of lamina dura occurred in 6.4% and 9.1%, and root resorption had not occurred in fibrous dysplasia, but occurred in 18% of cemento-ossifying fibroma, displacement of mandibular canal occurred in 14.9% and 31.8%.

  • The author observed a 35-year-old male patient who came to the Dental Infirmary of Kyungpook National University Hospital who had complained of gradual swelling on both side of the mandible for 15 years. As a result of careful analysis of clinical, radiological and histopathological findings, the authors diagnosed it as cementifying fibroma and obtained the results as follows: 1. In clinical examination, main clinical symptom was facial deformity due to gradual swelling on both side of the mandible. 2. In radiographic examination, radiolucent lesions with central radiopaque foci were seen on both mandibular body areas bilaterally, and cortical thinning and expansion of the mandibular body were seen buccolingually. And loss of lamina dura and root resortption of adjacent teeth were also seen. In histopathological examination, this lesion was composed of delicate interlacing collagen fibers interspersed by the fibroblasts and cementoblasts. And round or oval shaped basophilic masses of cementum-like tissue were observed in the connective tissue.

DIGITAL IMAGE PROCESSING AND CLINICAL APPLICATION OF VIDEODENSITOMETER (실험적으로 제작한 Videodensitometer의 디지털 영상처리와 임상적 적용에 관한 연구)

  • The purpose of this study was to propose the utility which was evaluated the digital image processing and clinical application of the videodensitomery. The experiments were performed with IBM-PC/16bit-AT compatible, video camera(CCdtr55, Sony Co., Japan), an color monitor(MultiSync 3D, NEC, Japan) providing the resolution of 512×480 and 64 levels of gray. Sylvia Image Capture Board for the ADC(analog to digital converter) was used, composed of digitized image from digital signal and the radiographic density was measured by 256 level of gray. The periapical radiograph(Ektaspeed EP-21, Kodak Co., U. S. A) which was radiographed dried human mandible by exposure condition of 70 kVp and 48 impulses, was used for primary X-ray detector. And them evaluated for digitzed image by low and high pass filtering, correlations between aluminum equivalent values and the thickness of aluminum step wedge, aluminum equivalent values of sound enamel, dentin, and alveolar bone, the range of diffuse density for gray level ranging from 0 to 255. The obtained results were as follows: 1. The edge between aluminum steps of digitized image were somewhat blurred by low pass filtering, but edge enhancement could be resulted by high pass filtering. Expecially, edge enhancement between distal root of lower left 2nd molar and alveolar lamina dura was observed. 2. The correlation between aluminum equivalent values and the thickness of aluminum step wedge was intimated, yielding the coefficient of correlation r=0.9997(p<0.00l), the regression line was described by Y=0.9699X+0.456, and coefficient of variation amounting to 1.5%. 3. The aluminum equivalent values of sound enamel, dentin, and alvolar bone were 15.41㎜, 12.48㎜, 10.35㎜, respectively. 4. The range of diffuse density for gray level ranging from 0 to 255 was wider enough than that of photodenstiometer to be within the range of 1-4.9.

Radiographic study of the odontogenic keratocyst (치성각화낭의 방사선학적 연구)

  • Purpose : To acquire the useful diagnostic information through the analysis of the clinical and radiological characteristics of mandibular odontogenic keratocyst. Materials and Methods : The researchers compared and analysed the clinical and radiological features of 112 cases of mandibular odontogenic keratocyst confirmed by histopathlogic examination. Results : Mandibular odontogenic keratocysts occurred more frequently in males than in females and the incidence is the highest in the 2nd and 3rd decades. These cysts occurred in the mandibular posterior area, angle-ramus area and anterior area $51.8\%$, $31.2\%$ and $17.0\%$ respectively. These cysts had undulating border ($69.6\%$) rather than smooth border ($30.4\%$). Most of these cysts had well-defined hyperostotic border ($94.6\%$). These cysts caused cortical thinning or expansion ($78.6\%$) rather than no cortical reaction ($21.4\%$). Loss of lamina dura was observed in $72.3\%$, displacement of tooth appeared in $35.7\%$ and root resolution appeared in $12.5\%$ of cases. In $71\%$ of cases, displacement of mandibular canal was observed. Internal patterns of lesional radiolucency were even ($61.6\%$) or uneven ($38.4\%$). Conclusion : These results would be helpful in diagnosing of mandibular odontogenic keratocyst. (Korean J Oral Maxillofac Radiol 2005; 35 : 51-4)

  • Bony ankylosis is an intraarticular condition where there is fusion of the bony surfaces of the joint : The condyle and the glenoid fossa. It occurs in both children and adults, unfortunately more frequently in the former, in whom early recognition and correction is particularly critical. Trauma is well proven to be the predominant cause of TMJ ankylosis. Infection, rheumatoid arthritis and neoplasm are another, significant cases of TMJ ankylosis. The necessity for using an interpositional material to prevent TMJ reankylosis has been widely discussed and many interpositional materials have been used, including temporal muscle and fascia, dermis, auricular cartilage, fascia lata, fat, Lyo-dura, Silastic and various metals. The temporal muscle and fascia have been widely used pedicled flap for head and neck reconstruction. The use of a temporal muscle and fascia for reconstruction of the TMJ, particularly in cases of ankylosis is a very reasonable option. Its principle advantages are its autogenous nature, resilience, and proximity to the joint, allowing for a pedicled transfer of vascularized tissue into the joint area. However, the viability of temporal muscle and fascia is a critical question. We treated 2 cases of TMJ ankylosis with temporal muscle and fascia transfer and one case with temporal fascia. We obtained satisfactory results as to functional aspects.

Morphometric Study of the Nerve Roots Around the Lateral Mass for Posterior Foraminotomy

  • Objective : Morphometric data on dorsal cervical anatomy were examined in an effort to protect the nerve root near the lateral mass during posterior foraminotomy. Methods : Using 25 adult formalin-fixed cadaveric cervical spines, measurements were taken at the lateral mass from C3 to C7 via a total laminectomy and a medial one-half facetectomy. The morphometric relationship between the nerve roots and structures of the lateral mass was investigated. Results from both genders were compared. Results : Following the total laminectomy, from C3 to C7, the mean of the vertical distance from the medial point of the facet (MPF) of the lateral mass to the axilla of the root origin was 3.2-4.7 mm. The whole length of the exposed root had a mean of 4.2-5.8 mm. Following a medial onehalf facetectomy, from C3 to C7, the mean of the vertical distance to the axilla of the root origin was 2.1-3.4 mm, based on the MPF. Mean vertical distances from the MPF to the medial point of the root that crossed the inferior margin of the intervertebral disc were 1.2-2.7 mm. The mean distance of the exposed root was 8.2-9.0 mm, and the mean angle between the dura and the nerve root was significantly different between males and females, at $53.4-68.4^{\circ}$. Conclusion : These data will aid in reducing root injuries during posterior cervical foraminotomy.

  • Hypophosphatemic rickets is lack of reponse to physiologic doses of vitamin D, different from the vitamin D-dependent rickets. It is inherited in an X-linked dominant fashion. The prime features of this disorder are lowered serum phosphate levels, elevated serum alkaline phosphatase and normal serum calcium levels. The dental manifestation often include apical radiolucencies, abscess and fistulas of clinically sound teeth. Dental radiographs show ricketic bone trabeculations, abscent or abnormal lamina dura and abnormal cementum. This case which was diagnosed to hypophosphatemic rickets, showed multiple spontaneous periapical abscess and gingival fistula enlarged pulp chambers, extension of the pulp horns into the cusp tips and delayed eruption.

  • Wegener's granulomatosis (WG) is an autoimmune disease of unknown etiology characterized by the triad of necrotizing granulomatous lesion in the upper and lower respiratory tracts or both, disseminated vasculitis involving both small arteries and veins, and necrotizing glomerulonephritis. The most common oral lesions associated with WG are ulceration and strawberry gingivitis. A 47-years old man in medical care associated WG was consulted our Division of Oral and Maxillofacial Surgery for the chief complaint of toothaches. Pre-operative panorama showed the alveolar radiolucency and the loss of lamina dura regarding the left upper teeth. An oropharyngeal magnetic resonance imaging also revealed the increased bone marrow signal intensity on the left maxilla. Under the impression of maxillary osteonecrosis due to WG, maxillary saucerization with removal of involved teeth was performed. We obtained good results and report the first case of WG in Korea, with the review of literatures regarding oral and general systemic features.
