Drug Use in the Elderly (노인의 약물 사용 실태 조사)

  • Purpose: This study was conducted to identify drug use by elders. Method: There were 304 participants age 65 or older included in this study. Data were collected using a questionnaire about drug use in the past 4 weeks. The questionnaire, a modification of Uhm's (2005), Lee's (2001), and Ellor and Kurz's (1982) tools, consisted of 17 questions on general and health characteristics, 17 on drug usage and 9 on behaviors related to drug misuse. Results: 85.5% of participants reported laking at least one type of prescription or non-prescription drug. 26.0% of participants reported taking only prescription drugs, 3.9% reported taking only non-prescription drugs, and 55.6% reported taking both prescription and non-prescription drugs. 17.1% of participants reported side effects from the drugs. The mean score for behaviors related to drug misuse was 7.53. Elders taking only non-prescription drugs showed more drug misuse than elders taking only prescription drugs or both. Women used more prescription, non-prescription drugs or both than man. Elders in rural areas used more non-prescription drugs than those in urban areas. Conclusion: Even though pharmacies were separated from medical practices in 2000, most older adults continue to use and misuse prescription and non-prescription drugs.

  • Objective: A case report on the Leukopenia Patients Using Ortho-Cellular Nutrition Therapy (OCNT) Methods: A Korean female in her 50s suffering from the rare side effect of leukopenia after taking athlete's foot medicine (Terbinafine HCl) Results: The results revealed that the white blood cell count, which had dropped before the OCNT, had improved to a normal level. Conclusion: The application of the OCNT can help change health-related figures in patients with relevant problems.

  • Sibutramine is a serotonin-norepinephrine reuptake inhibitor indicated for the management of obesity in conjunction with a reduced calorie diet. The oral administration of sibutramine is followed by its dose-related side effects. In this study, sibutramine was formulated into drug in adhesive (DIA) patches in an attempt to overcome these problems. The effects of different formulation variables including pressure-sensitive adhesive (PSA), loading amount of drug, thickness of matrix and enhancer on the skin permeation of the drug were evaluated using excised hairless mouse skin. In the acrylic adhesive with carboxyl functional group, low release of sibutramine was observed due to the strong interaction between carboxyl group of adhesive and amine group of sibutramine. The acrylic adhesive without functional group provided good adhesion force and allowed high drug loading. Changing drug load as well as thickness of the matrix was found to alter permeation rate. $Crovol^{(R)}$ PK40 and $Crovol^{(R)}$ A40, were found to be effective enhancers for sibutramine. The optimized patch contained 20% sibutramine, and 5% $Crovol^{(R)}$ A40 as permeation enhancer, in $80\;{\mu}m$ thick Duro-$Tak^{(R)}$ 87-9301 matrix.

Analysis of Research Trends on Interactions between Herbal Formula and Conventional Drugs Using Papers from PubMed (PubMed 수록 논문을 활용한 한약 처방과 양약 상호작용에 관한 연구 동향 분석)

  • Objectives : Herbal formula consist of multiple herbs, which can potentially interact with conventional drugs. If these interactions are not properly understood, they may reduce treatment efficacy or cause unexpected side effects. Thus, this study collected and analyzed papers on herbal formula and conventional drug interactions from PubMed to analyze various research trends. Methods : To analyze research trends on herbal formula and drug interactions, we first created search queries using a dictionary of herbal formula terms and collected related papers from PubMed using the Entrez API. The PubTator API was applied to identify compound names in the abstracts, recognizing compounds registered in the DrugBank as conventional drugs. Sentences describing interactions between herbal formulas and drugs were extracted using pattern matching, and relevant papers were selected. Trends were then analyzed by year, country, major formulas, major drugs, and interaction networks. Results : Yearly analysis showed a gradual increase in paper counts with a significant rise after 2010. Country analysis revealed that China published the most papers (53), followed by Japan (19) and South Korea (8). formula analysis identified 'sosiho-tang' and 'siryung-tang' as the most frequently mentioned (7 times each). Drug analysis showed '5-fluorouracil', 'acetaminophen', 'entecavir', and 'streptozotocin' were the most frequently mentioned (4 times each). Network analysis revealed 'sosiho-tang and tolbutamide' and 'siryung-tang and prednisolone' as the most frequently, mentioned interactions (3 times each). Disease analysis indicated 'urogenital diseases' were the most discussed (32 mentions), Followed by 'pathological conditions, signs, and symptoms' and 'digestive system diseases' (25 mentions each). Conclusions : Analyzing research trends on herbal formula and conventional drug interactions provides basic data for subsequent research, aiming to reduce side effects and enhance treatment efficacy in clinical settings.

A Study on the Status of Drug Misuse and Abuse among Community-dwelling Elders (일개 지역사회 거주 노인의 의약품 오남용 실태)

  • Purpose: This study was done to investigate the status of drug misuse and abuse in community-dwelling elders. Methods: The participants in this study were elders who were 65 years or over, and lived in ChungNam province. Descriptive statistics were used to analyze the data. Results: All of the participants reported taking both prescription and non-prescription drugs, and 78.4% used two or more kinds of drugs. Of the elders, 74.5% reported that they did not receive any education about drug misuse and abuse. The mean score for behaviors related to drug misuse was 8.76. Conclusions: Results indicate that many elders take medicine frequently, but they do not have any knowledge about drugs and possible side effects. This lack of knowledge might mean that they continue to use and misuse prescription and non-prescription drugs. It is important that elders be provided with precise information about medicines.

Nurses' Monitoring Practice for Adverse Drug Reactions (약물 유해 반응에 대한 간호사의 모니터링 실천에 관한 연구)

  • Purpose: The aim of this study was to identify the relationship between nurses' knowledge of adverse drug reactions (ADR), attitudes, and monitoring practices and to establish an effective ADR reporting system and provide baseline data for its activation. Methods: The participants in this study were chosen by convenience sampling and included 182 nurses working at major general hospitals that operate a Regional Drug Safety Center. Data were collected from June 1 to 12, 2015 and analyzed using Cronbach's ${\alpha}$, descriptive statistics, independent t-test, one way ANOVA, Pearson correlation coefficient and stepwise regression with the SPSS program. Results: The nurses' average score for knowledge was 7.62 points, for attitude, 41.04 points and for monitoring practices, 34.22 points. ADR monitoring practices positively correlated with knowledge (r=.19, p=.011), attitude (r=.41, p<.001), drug performance competency (r=.54, p<.001), and drug education satisfaction (r=.54, p<.001). Drug performance competency, drug education satisfaction, and attitudes explained 42.0% of the ADR monitoring practices (Adj $R^2=.42$, F=43.95, p<.001). Conclusion: In order to facilitate and encourage nurses' voluntary monitoring practice of ADR, efforts must be made to create positive attitudes toward ADR, and to increase drug performance competency and drug education satisfaction.

Adverse drug reactions reported over 5 years in a regional university hospital (일개 대학병원에서 경험한 5년간의 약물이상반응)

  • Adverse drug reactions (ADRs) are closely associated with increased morbidity and mortality rates, prolonged hospitalization durations, and higher healthcare costs. This study aimed to estimate the incidence, clinical features, and reporting status of ADRs to improve the current ADR reporting system and prevent recurrent ADRs in hospitals. This retrospective study was conducted at a regional referral hospital. Patients diagnosed with ADRs over a 5-year period (2009-2014) were recruited for this study. An ADR was identified as an ADR-related diagnosis in a patient's medical record or an ADR registered through the in-hospital ADR reporting system. The incidence, culprit drug, clinical manifestations, reporting source, severity, related management, and recurrence rate were assessed. Among 1,112 patients, 1,375 ADR events were collected, an estimated 0.06% of the total number of patient visits. Diagnostic contrast agents (46.4%) were the most common culprit drugs, followed by antibiotics (22.0%), nonsteroidal anti-inflammatory drugs (9.9%), and opioids (4.5%). Skin reactions (67.5%) such as rashes and hives were the most frequent manifestations. Additional ADR-related medical attention was necessary in two thirds of cases. One hundred eighty ADR events (13.1%) were categorized as severe, and 19 patients (1.4%) experienced re-exposure to the culprit drugs. Four patients (0.3%) experienced fatal ADRs. Physicians were the most frequent ADR reporters in the in-hospital ADR reporting system. In conclusion, many ADR events may be overlooked, and re-exposure to causative drugs commonly occurs. Continuous education and maintenance of a reporting system may be important for preventing recurrent ADRs.

Awareness of Adverse Drug Reaction Reporting System in General Population (일반인에서의 의약품 부작용보고제도 인식도)

  • Background: Safety of drugs has become a major issue in public healthcare. Spontaneous reporting of adverse drug reaction (ADR) is the cornerstone in management of drug safety. We aimed to investigate the awareness and knowledge of spontaneous ADR reporting in general public of Korea. Methods: A total of 1,500 study subjects aged 19-69 years were interviewed with a questionnaire for their awareness and knowledge related to spontaneous ADR reporting. Computer assisted telephone interview was performed from 27th February 2013 to 4th March 2013. Target population was selected with quota sampling, using age, sex, and residence area. Healthcare professionals such as physicians, pharmacists, and nurses were excluded. The survey questions included awareness of spontaneous ADR reporting, opinions on ways to activate ADR reporting, and sociodemographic characteristics. Results: Overall awareness of spontaneous ADR reporting system was 8.3% (${\pm}2.53%$) among general population of Korea. Major source from which people got the information regarding ADR reporting was television/radio (69.9%), followed by internet (19.3%), and poster/brochure (6.1%). Awareness level differed between age groups (p<0.0001) and education levels (p<0.0001). Upon learning about the ADR reporting system, 88.5% of study subjects agreed on the necessity of ADR reporting system, while 46.6% thought promotion through internet and mass media as an effective way to activate ADR reporting. Conclusion: The overall awareness of spontaneous ADR reporting should be enhanced in order to establish a firm national system for drug safety. Adequate promotions should be performed targeting lower awareness groups, as well as various publicity activities via effective channels for the general population.

Aripiprazole-Related Oculogyric Dystonia

  • Objectives : Aripiprazole is unique drug among the SGA (Second generation antipsychotics) in its pharmacology and pharmacokinetics,but is similar in clinical efficacy. Aripiprazole acts as a partial agonist at dopamine D2 receptors, activating the receptor but eliciting a reduced response compared to the natural neurotransmitter. There are some side effects of aripiprazole, the most common side effects of aripiprazole are headache, nausea, vomiting, insomnia, tremor, constipation and EPS. Difficulty in opening eyes is not defined EPS yet, but it is a rare but important side-effect symptom of aripiprazole. Methods : This article is about a case of side-effect symptom of aripiprazole, 26-year-old single female suffering from schizophrenia had difficulty in opening eyes while she was taking antipsychotics. During the hospitalization, the relaxation therapy is helpful not only to reduce tension in the eyelids but also to headache. Results : It is important that early recognition of aripiprazole-induced oculogyric dystonia can prevent life-threatening complications. Education medical staff to this easily treatable reaction will improve overall quality of health care. Conclusions : This case notifies the need for awareness of the risk of acute oculogyric dystonia in adolescent female patients receiving aripiprazole.

Review of Regulatory Management System on Side Effects for Veterinary Medical Devices in Korea (국내 동물용의료기기 부작용 관리시스템에 대한 고찰)

  • This study reviewed the regulatory safety management system for prevention of side effects of veterinary medical devices in Korea based on data obtained through analyses of those medical devices. In order to ensure the safety of medical devices, Ministry of Food and Drug Safety has established the management system include licensing, test inspection (134 designated clinical trial agencies) and post management. In addition, it was introduce the good manufacture practice (GMP) certification system and device tracking requirements. In contrast, it is necessary to establish regulations for side effects report and awareness on veterinary medical devices in Korea. The center for veterinary medicine of FDA in the United States has been operated side effects reporting system of veterinary medical devices from user, manufactures and distributor. Due to increase the use of various kinds of veterinary medical devices for animals, it should be required to improve regulation related to the licensing procedures, GMP system, and the side effects reporting system in our country.
