Drug-Induced Gastrointestinal Dysfunction in Parkinson's Disease: Treatment with Korean Medicine

  • Parkinson's disease (PD) is a neurodegenerative disease, where treatment with medication may lead to gastrointestinal (GI) symptoms. The objective of this case study was to investigate the effectiveness of Korean medicine (KM) in treating PD with drug-induced GI dysfunction. A 70-year-old female participant was diagnosed with PD in 2010 and drug-induced gastritis in 2016. Her major symptoms were related to GI, PD, and overall feeling of weakness. She was treated with KM including pharmacopuncture, acupuncture, moxibustion, and herbal medicines, in combination with Western medicines during 46 days of in-patient care. This study showed an improvement in symptoms and scores on the GI symptom scale, Unified Parkinson's disease rating scale, Hoehn and Yahr staging, Berg balance scale, PD quality of life, and stress index at discharge. This case demonstrated that the symptoms of drug-induced GI dysfunctions in PD was improved by treatment with KM.

EFFICACY EVALUATION OF THE WHITENING COSMETICS USING MELANOGENESIS INHIBITION ASSAY COSMETICS IN B-16 MELANOMA CELL

  • We investigated the inhibitory effect of whitening materials with growth factor or alone on melanomas derived from Human (B-16) and mouse (SK-MEL-31) using melanin content. Melanin content was determined by the absorbance value at 470nm per cells. we used the growth factors known as activators of Adenylate cyclase, Protein kinase C and tyrosine kinase pathway separately. In addition, we compared the action of UV-induced with non-biological growth factor with whitening materials in melanomas derived from Human and mouse. The results showed that the aspect of inhibitory effect of whitening materials on B16 and SK-MEL-31 was not different. And, the action of each growth factor involved in the differentiation and proliferation of melanoma on the inhibition of melanogenesis in B-16 and SK-MEL-31 using whitening agents showed no difference. Also, The action of UV -induced and non-biological growth factors didn't exhibit different pattern on the effect of whitening agent in B-16 and SK-MEL-31.

복진정제 및 수면제 유발 사건수면 (Sedative Hypnotics Induced Parasomnias)

  • Parasomnias induced by hypnosedatives are rare but serious side effect. Such parasomnias have not been reported with all hypnosedatives. However, frequent use of hypnosedatives, particularly nonbenzodiazepine receptor agonists is associated with parasomnias. Associated symptoms are sleep eating, sleepwalking with object manipulation, sleep conversations, sleep driving, sleep sex and sleep shopping etc. Mechanisms include high affinity for $GABA_A$ receptor, interruption of the consolidation phase of memory formation by drug, pharmacokinetic or pharmacodynamic drug-drug interaction and concomitant administration with alcohol. Managements for parasomnias induced by hypnosedatives involve stopping medication, switch to other medications or nonpharmacological treatment, lowest effective dose of NBRAs (Non-Benzodiazepine Receptor Agonists), taking into consideration drug-drug interactions, identification and treatment of underlying disease states.

Drug eruption by antihistamine mistaken for chronic urticaria in a child

  • Although rare, antihistamines can cause adverse effects, including drug-induced eruptions or anaphylaxis. A 4-year-old child visited the pediatric department of a hospital for skin eruptions after administration of antihistamines, (e.g., ucerax [hydroxyzine] or leptizine [levocetirizine]), for cholinergic rashes; he did not have pruritus. Skin prick, intradermal, and drug provocation tests were performed to determine the relationship between the antihistamines and eruptions. Levocetirizine induced wheals in the skin prick test and a rash in the oral drug provocation test. In contrast, ketotifen induced no reaction in the skin prick test but showed a positive reaction in the oral provocation test. Our case report highlights that children can experience the same types of adverse reactions as seen in adults, and cross-reactivity between various antihistamines can occur.

조영제로 유발된 약진(藥疹)환자 치험 1례 (A Clinical Case Report of Contrast-induced drug eruption patient Treated with Traditional Oriental Medicine)

  • Drug eruption is a cutaneous reaction caused by various drugs. It is a very common drug induced adverse reaction. Contrast media induced drug eruption is rare. But approximately 10% of people injected with contrast media experience adverse reaction, and drug eruption accounts for more than 40% of all adverse reactions. We report a case of mild drug eruption and its treatment process based on korean traditional medicine. The patient is a 50 years old female diagnosed with functional dyspepsia. The patient had gastric discomfort and alternation between diarrhea and constipation. All symptoms showed nearly complete remission with continued korean traditional medical treatment. The patient had been injected with contrast media for Computed tomography(CT) evaluation of her breast cancer history. Rashes appeared on back and abdomen, and urticaria and pruritus appeared on the patient's finger 1 day after injection. We prescribed Goreisan(TSUMURA CO &, TJ-17) and acupuncture on Quchi(LI11), Zusanli(ST36), Yangxi(LI5), Yanggu(SI5). Consequently, the drug eruption showed remarkable improvement. So the author reports korean traditional medicine is effective complementary treatment for drug eruption.

Objectives: The purpose of this case is to report that a Tae-eumin patient diagnosed as drug-induced liver injury (DILI) caused by Galgeunhaegi-tang (Gegenjieji-tang) treated with Taeumjowe-tang reduced ephedra to 2g (Taiyintiaowei-tang) and then liver function test Results: improved. 2. Methods: We diagnosed him as Tae-eumin Exterior cold disease induced from the esophagus affected by cold. We made him stop taking Galgeunhaegi-tang (Gegenjieji-tang) and take Taeumjowe-tang reduced ephedra to 2g. (Taiyintiaowei-tang) 3. Results: After our treatment, the symptom and the liver function test Results: improved. 4. Conclusions: The wrong diagnosis of constitution and pathological syndromes can be caused drug-induced liver injury. Taeumjowe-tang reduced ephedra to 2g (Taiyintiaowei-tang) may have an effect on treatment of drug-induced liver injury of Tae- eumin.

  • 1. Objectives The primary purpose of this case is to report that a Soeumin patient diagnosed as drug-induced hepatitis was treated with 'K wokhyangchungki-san added Perennial artemisia' and then her symptom and liver function test results improved. 2. Methods We diagnosed her as yin-jaundice on Taeum Syndrome. So we treated her with 'Kwokhyangchungki-san added Perennial artemisia'. 3. Results After we treated het with 'K wokhyangchungki-san added Perennial artemiJia' her symptom and liver funaion test results was improved rapidly. 4. Conculusions 'Kwokhyangchungki-san added Perennial artemisia' may have an added on Treatment of Drug-induced hepatitis of Soeumin.

Apoptosis of Rat Embryonic Midbrain Cells in Ochratoxin A-induced Microcephaly

  • Induction of DNA fragmentation of rat embryonic midbrain cells was studied to see whether apoptosis plays a role in OTA-induced microcephaly observed in cultured rat whole embryos during embryogenesis. We first cultured whole embryos (prepared from day 9.5 gestation rats) for 48 hrs with OTA and found that OTA induced microcephaly in cultured rat whole embryos. We also examined whether the microcephaly seen in cultured whole embryos is partially related to the increase of apoptosis of undifferentiated embryonic midbrain cells. Embryonic midbrain cells were prepared from day 12 gestation rat embryos, and cultured in the mixture media of Dulbecco's modified eagle's medium nutrient and Ham's F12 (1:1) containing 10% Nuserum, 100 $\mu\textrm{g}$/ml of streptomycin and 100 units/ml of penicillin for 96 hrs. Induction of DNA fragmentation was increased by 0.25-1 $\mu\textrm{g}$/ml OTA in a dose dependent manner in the embryonic midbrain cells. We also tested whether increase of apoptosis by OTA would be associated with change of apoptosis-related proteins (TNF-$\alpha$ and P$^{53}$ ) level in embryonic midbrain cells. OTA also increased TNF-$\alpha$ and P$^{53}$ levels. These results show that OTA induced microcephaly in cultured whole embryos and this effect may be at least a part due to the induction of apoptosis and apoptosis-related protein levels of undifferentiated embryonic midbrain cells.

Increase of Membrane Potential by Ginsenosides in Prostate Cancer and Glioma cells

  • Ginseng has an anti-cancer effect in several cancer models. As a mechanism study of ginsenoside-induced growth inhibition in cancer cells, we measured change of membrane potential in prostate cancer and glioma cells by ginsenosides, active constituents of ginseng. Membrane potential was estimated by measuring fluorescence change of DiBAC-Ioaded cells. Among 11 ginsenosides tested, ginsenosides $Rb_2$, $Rg_3$, and $Rh_2$ increased significantly and robustly the membrane potential in a concentration-dependent manner in prostate cancer and glioma cells. Ginsenosides Rc, Ro, and $Rb_1$ slightly increased membrane potential. The ginsenoside-induced membrane potential increase was not affected by treatment with pertussis toxin or U73122. The ginsenoside-induced membrane potential increase was not diminished in $Na^+$-free or $HCO_3^-$-free media. Furthermore, the ginsenoside-induced increase of membrane potential was not changed by EIPA (5-(N-ethyl-N-isopropyl)-amiloride), SITS (4-acetoamido-4'-isothiocyanostilbene-2,2'-disulfonic acid), and omeprazole. In summary, ginsenosides $Rb_2$, $Rg_3$, and $Rh_2$ increased membrane potential in prostate cancer and glioma cells in a GPCR-independent and $Na^+$ independent manner.
