  • Construction companies economic to lighten the restrictions on safety education and training due to the domestic recession as long as the global downturn. With this, the Korean government restrictions, but the incidents are being sharply increased nowadays. The construction industry hazardous than any others, so, they insist to reinforce the training for incidents prevention, to be disregarded at the subcontractors of the bigger construction companies problems. According to these matters, this study hereby surveyed regarding their suggestions for the improvements of the training concepts through questionnaires below.

Analysis on Management Status and Issues for Near Miss Reporting in Nuclear Power Industry (원전 사고근접사례의 보고체계 현황 및 현안분석)

  • When an event is occurred in a nuclear power plant (NPP), the NPP operator reports it referred by the regulation on reporting and public announcement of accidents and incidents. Some of the events do not need to be reported because they are not included in the reporting criteria of the regulation. However, it is necessary that they should be managed effectively because the accident can be occurred by the recurrence of a lot of them as precursors. Among the events not included in the reporting criteria of the regulation, near miss is the event that is not occurred but can generate a significant consequence. This can provide the cause of the event which does not result an accident. So, it is able to offer insightful knowledges to prevent higher level events about the function and process of NPP. The objective of this study is to analyze the issues of near miss events, prepare the defence against the risk, and improve the management process of NPP. To achieve it, this study performed to analyze the management structure and status of near miss events as well as the accident reporting system of the domestic and foreign regulation bodies. In case of Korea, the status was analyzed by quantitative data, licensee event reports and procedures. Based on these, we could find the causes that near miss events were not managed effectively. Then, systematic alternatives that reflected the perspective of man, technology and organization were drawn.

  • We develop the database program for accident cause analysis which can help to increase domestic safety custom and prevent recurrence of gas accident and analyze accidents easily The program developed in this study consists of two parts. one part uses accident case database applied if than rule, so it finds root causes by inference of some input values. The other uses Root Cause Analysis Map which divided human errors and equipment difficulties and so we get general root cause by reply some proper questions.

  • When the Fukushima nuclear power plant accident occurred in March of 2011, a hydrogen explosion in the reactor building at the 4th unit of Fukushima plants led to a big surprise because the full core of the unit 4 reactor had been moved and stored underwater at the spent nuclear fuel storage pool for periodic maintenance. It was because the possible criticality in the fuel storage pool by coolant loss may yield more severe situation than the similar accident happened inside the reactor vessel. Fortunately, it was assured to be evitable to an anxious situation by a look of water filled in the storage pool later. In the paper, the safety state of the spent fuel storage pool and rack structures of the domestic nuclear plants would be roughly reviewed and compared with the Fukushima plant case by engineering viewpoint of potential severe accidents.

  • Accident dangerousness of domestic hydro power generators which are operated in Korea is on the increase because of use at the long term. On this, the Off-line diagnosis techniques developed to the On-line by continued domestic technical development since 2000. Especially, On-line insulation diagnosis of domestic hydro power generator is possible by localization of partial electric discharge sensor and On-line insulation diagnosis system. This paper shows the diagnosis result that is applied localized Ceramic Coupler and PDMS-HG(Partial Discharge Monitoring System for Hydro Generator) at four Chung-ju hydro Generator used for 25 years. Particularly, the confidence of insulation diagnosis is improved by using high frequency filter and sampling the partial discharge signals which occur in site. For reviewing the confidence of On-Line insulation diagnosis system, we measured the outside noises and partial discharge signals during practical operation by using the partial discharge diagnosis system of the Full A/D process. And we reviewed the confidence of the On-Line insulation diagnosis system by comparing and analyzing these data.

  • Korea successfully achieved energy independence in the shortest period of time from being the poorest country in terms of energy 50 years ago through steady development of nuclear technology. In the past, the nuclear industry has been driven through government-centered policy development, public institution-based research, and industrial facility and infrastructure construction. Consequently, South Korea became a nuclear energy powerhouse exporting nuclear power plants to the UAE, surpassing the level of domestic technological independence. However, in recent years, the nuclear industry in Korea has experienced a decline in new plant construction since the Fukushima accident in Japan, which caused changes in public perspectives regarding nuclear power plant operation, more stringent safety standards on the operation of nuclear power plants, and a shift in governmental energy policy. These changes are expected to change the domestic nuclear industry ecosystem. Therefore, in this study, we investigate the priority of technology development investment from the perspective of experts in private nuclear power companies, shifting the focus from government-led nuclear R&D policies. To establish a direction in nuclear technology development, a survey was conducted by applying an analytic hierarchy analysis to experts who have worked in nuclear power plants for more than 15 years. The analysis items of focus were the 3 attributes of strategic importance, urgency, and business feasibility of four major fields related to nuclear energy: nuclear safety, decommissioning, radioactive waste management, and strengthening industrial competitiveness.

Review of the regulatory periodic inspection system from the viewpoint of defense-in-depth in nuclear safety

  • The regulatory periodic safety inspection system is one of the most important methods for confirming the safety of nuclear power plants and the defense in depth in nuclear safety is the most important basic means for accident prevention and mitigation. Recently, a new regulatory technology based on risk-informed and safety performance has been developed and used in advanced countries. However, since the domestic periodic inspection system is being used in the same way over 30 years, it is necessary to know how the inspection contributes to the safety confirmation of the nuclear power plants. In this study, the domestic periodic inspection system currently in use was analyzed from the perspective of defense in depth in nuclear safety. In addition, the analysis results were compared to the U.S. NRC's safety inspection system to obtain consistency and lessons in this study. As a result of analysis, the NRC's safety inspections were distributed almost evenly at the all levels of defense in depth, while in the case of domestic inspection, they were heavily focused on the level 1 of defense in depth. Therefore, it appeared urgent to improve the inspection system to strengthen the other levels of defense in depth in nuclear safety.

System Safety Application for the Weapon System (무기체계 시스템안전 적용방안)

  • In the early 1960s, US Air Force lost missile launch bases during ICBM development by a defect in the missile design and operation plan. U.S. DoD realized the limitation of the existing accident prevention method. Therefore, the weapon development required system safety activity, and procurement projects of U.S. DoD applied MIL-STD-882(System Safety). Development projects of U.S. DoD more emphasized the importance of system safety after the space shuttle Challenger exploded in 1986. Currently, Airworthiness certification for military aircraft uses system safety to minimize accidents. The domestic defense aviation R&D projects also use the system safety for the airworthiness certification. However, non-aviation weapon R&D projects rarely applied system safety. This paper presents a system safety application method for domestic weapon R&D projects by studying the U.S. military standards/organizations and domestic defense aviation projects.

Finding Hazard Factors by New Risks on Maritime Safety in Korea

  • The key features of maritime accidents are the change of their attributes by new risks from time to time. To prevent maritime accidents in Korea, the impacts by new risks on domestic safety environments should be identified or predicted. The purpose of this paper is to find the hazard factors by new risks on maritime safety in Korea. The meaning of new risks is the elements of accident hazard which is compiled from new or rare or unprecedented events in the worldwide maritime transportations. The problems of new risks are the lacks of optimum countermeasures to mitigate accident risks. Using the questionnaires with 152 event scenarios classified by 20 accident causes, the hazard identification and risk analysis of new risks was performed based on the Formal Safety Assessment (FSA) by IMO. A total of 22 Influence Diagrams, which is to depict the transit flows between accident causes to consequences, is used in the construction of 152 event scenarios. A total of 20 accidents causes is the same contents as the causation factors represented in Statistical Year Book for Maritime Accidents of Korean Maritime Safety Tribunals. After defining the evaluation equations to the response results of questionnaires by 46 experts, the work for risk analysis is carried out. As results from the analysis of 152 scenarios, it is known that the root cause to affect on maritime safety in Korea is the pressure of business competition and it led to the lacks of well experienced crews, the overload of vessel operations and crew's fatigue. In addition, as results from the analysis of 20 accident causes, the three accident causes are to be candidate as main issues in Korea such as the inadequate preparedness of departure, the neglecting of watch keeping in bridge and the inadequate management of ship operations. All of the results are thought to be as basic hazard factors to safety impediments. It is thus found that the optimum Risk Control Options to remove the hazard factors and to mitigate consequences required are the following two factors: business competition and crewing problems.
