Effect of Verapamil on Renal Function in Dog (Verapamil이 개의 신장기능에 미치는 영향)

  • Verapamil, $Ca^{2+}$-channel blocker, when given into vein or into carotid artery, produced the decrease of urine flow accompanied with the decreased amounts of Na$^{+}$ and $K^{+}$ excreted in urine ($E_{Na}, E_{K}$) and with the decreased clearances of free water (C$_{H_2O}$) and osmolar substance (C$_{osm}$), and then increased reabsorption of Na$^{+}$ and $K^{+}$ in renal tubules (R$_{Na}$, R$_{N}$), glomeruler filtration rate (GFR) and renal plasma flow (RPF) were inhibited when verapamil was given into carotid artery, but were only tendency of reduction when given intravenously. Verapamil, when infused into a renal artery, exhibited diuresis accompanied with the increased GER, RPF, E$_{Na}$ and E$_{K}$, with the decreased filtration fraction (FF) in only infused kidney. At the same time, $C_{H_2O}$ was not changed, R$_{Na}$ and R$_{K}$ were reduced. Antidiuretic action by verapamil administered into vein or into carotid artery in normal kidney was reversed to diuretic action in denervated kidney. At this time, parameters of renal function exhibited the opposite phenomena compared to that elicited by verapamil in normal kidney, wherease renal denervation did not influence the action of verapamil infused into a renal artery. Above results suggest that verapamil produce both antidiuresis through nervous system centrally, not endogenous substances and diuresis by direct action in the kidney. Diurectic action are caused by hemodynamic improvement through dilatioon of vas efferense and by greatly inhibited reabsorption of electrolytes in distal tubules.

Effect of Acetazolamide on the Diuretic Action of Furosemide in Rabbits (Acetazolamide가 Furosemide의 이뇨작용에 미치는 영향)

  • Effect of acetazolamide on the diuretic action of furosemide was investigated in rabbits. The rates of urine flow and excretion of salts were significantly reduced when furosemide (0.5 mg/kg) was administered with acetazolamide (10 mg/kg) compared to the diuretic response of the single furosemide (0. 5 mg/kg) administration. Reduction in the fractional excretion rate of urine volume was more pronounced than the fractional excretion rate of salts. The results suggest that reduction of diuretic action on furosemide by combined administration of acetazolamide is probably due elevated urinary pH and interference in the mechanism of inhibition of chloride transport in the ascending Henle's limb.

Studies on Diuretic Action of Bumetanide (Bumetanide의 이뇨작용에 관한 연구)

  • Bumetanide, when given intravenously in dogs, induced a potent diuresis with an increased amounts of sodium and potassium excreted in urine due to inhibition of reabsorbing them in renal tubule. Furthermore, clearances of osmolar substance and para-aminohippuric acid were increased, but clearace of free water diminished without any change of creatinine clearance. Bumetanide, administered directly into a renal artery, elicited diuresis only in the infused(experimental) kidney by the same mode of action as in the intravenous cases in renal function of the dog. Renal effects of intravenous bumetanide after pretreatment with the small dose of indomethacin (5.0mg/kg) revealed reduction only in clearance of paraaminohippuric acid. However the much dose of indomethacin (5.0mg/kg+5.0mg/kg/hr) or arachidonic acid showed a significant inhibition in the change rates of all renal function by bumetanide. Morover, pretreatment of probenecid also made a marked reduction in renal effects induced by bumetanide. From the above results, it is thought that bumetanide causes diuretic action due to dual mechanism inhibiting reabsorption of electrolytes in loop of Henle and increasing blood flow in kindney, that are provoked through the mediation of prostaglandins.

Diuretic action of hoelen in the dog (복령의 이뇨작용)

  • Intravenously administered total water extract of Hoelen, (Pachymae fungus) in a dose of 10mg/kg and its methanol lextract, in a smaller dose than total water extract, produced significantly increase on urinary volume, sodium nad potassium excretion, and osmolar nad free water clearances. Increasing the doses produced more pronounced renal responses. Glomerular filtration rate and renal plasma flow changed littl with both extract. On the contrary, water extract from residue obtained on extraction with organic solvents exhibited no significant changes on the parameters of the renal function. In experiments, in which the total water extract was infused directly into a renal artery and urines from both ureters were collected separetely, a small dose of 0.3mg/kg/min showed no diuresis, but a large dose of 1.0mg/kg/min elicited diuretic action even on the contralateral kindneys. It is, therfore, concluded that Hoelen induces diuresis, mainly by inhibiting reabsorption of electrolytes in the renal tubules, and that the renotropic action may be mediated by some endogenous humoral substances.

  • Attempts were to investigate the effects of 'Bojungikgi-tang' on the digestive system, blood pressure and diuresis. The results showed that relaxing action was shown on the isolated ileum in mice and that strong antagonistic actions were seen on $BaCl_2$, acetylcholine and histamine-induced contraction of the ileum in mice and guinea-pigs that the relaxing effect of the intestinal smooth muscle was recognized. Inhibitory effects on transport rate in the small intestine of mice and castor oil-induced catharsis in mice were noted. Inhibitory action on the secretion of gastric juice, pH ascending effect and decreasing effect of the free acidity and total acidity were recognized. Continuous hypotensive action was seen, but when the vagus nerve was cut, hypotensive action was remarkably decreased. The diuretic effect was also recognized.

Influence of Clonidine on Renal Function of Dogs (Clonidine이 개의 신장기능에 미치는 영향)

  • This study is an attempt to study the influence of clonidine, which has a central sympatholytic action, on the renal function in dogs and to elucidate its mechanism of action. Clonidine ($15\mu$g/kg) injected into a cephalic vein of the dog produced a marked increase in urine flow and in amounts of $Na^{+}$ and $K^{+}$ excreted in urine, and clearances of free water and osmolar substance, the reabsorption rates of $Na^{+}$ and $K^{+}$ in renal tubules were significantly decreased. Clonidine ($50.0]mu$g/kg) administered intravenouly elicited a transient reduction in urine flow, along with inhibition of all renal functions. Intravenous clonidine-induced diuretic effect was completely blocked by pretreatment with reserpine, and was lessened by water diuresis. Clonidine ($3.0\mu$g/kg) injected tnto a carotid artery revealed a transient diuresis with a increase in clearance of free water. Clonidine injected into a renal artery showed a significant antidiuretic effect and all functions of an experimental kidney were reduced. Antidiuretic action induced by clonidine given into a renal artery markedly suppressed by pretreatment with reserpine. The above results suggest that clonidine has dual mechanisms: 1) diuretic effect due to the central sympatholytic action and inhibition of release of antidiuretic hormone, and 2) antidiutetic effect indued by indirect symptheic stimulation in the periphery.

The Influence of guanethidine on the renal function of the rabbit (Guanethidine이 가토의 신장기능에 미치는 영향)

  • The influence of guanethidine on the renal function was investigated in the rabbit. Guanethidine, 1-10mg/kg, i.v., produced no marked change in the renal function, while second and successive doses of guanethidine elicited a significant increase in urine flow and electrolyte excretion as well as renal plasma flow and glomerular filtration rate. It was suggested that the diuretic action was brought about by improvement of hemodynamic state in the kidney ; increased filtration as a result of increased renal perfusion. Atropine alone did not significantly influence the renal function but pretreatment of animals with atropine, 4 mg/kg i.v., completely abolished the diuretic action of guanethidine. It is suggested that guanethidine influences the renal function by activating parasympathetic nervous system or some cholinergic mechanism in the kidney.

  • A study on the diuretic action of Siegesbeckiae herba in dogs was made using a clearance technique with ethanol extract. Siegesbeckiae herba ethanol extract (SGEE) administered intravenously in doses 6.0 and 60.0mg/kg elicited diuresis and an increase in sodium and potassium excretion, and it simultaneously produced a decrease in the reabsorption rate of sodium and potassium in renal tubules. Neither renal plasma flow nor glomerular filtration rate changed significantly. SGEE, when infused directly into a renal artery in doses of 0.5 and 1.5mg/kg min, exhibited identical results to the intravenous responses confined only to the infused kidney. The above observations lead to the suggestion that SGEE elicits diuresis in dogs by decreasing the reabsorption rates of sodium and potassium in renal tubles.

  • Phellinus linteus (Hymenocaetaceae) is a member of Basidiomycotinae, which has been used for the treatment of gastric cancer, non-insulin-dependent diabetes mellitus, diarrhea, and menstrual irregularity. To investigate the diuretic action of Phellinus linteus, urinary volume, serum parameters and urinary electrolytes in HgCl$_2$-induced acute renal failure rats in vivo were measured. The results showed that its MeOH extract produced significant increases on urinary volume, sodium and potassium excretion, and revealed lowering effects of the abnormally elevated BUN, creatinine and uric acid value in acute renal failure rats.
