The Association between Obesity and Periodontal Disease on Convergence Study (비만과 치주질환간의 관련성에 관한 융합연구)

  • The aim of this study was to evaluate the association between obesity and periodontal diseases in Korea adults. The date from the 2015 Korean National Health and Nutrition Survey were used, and 4,827 subjects over 19 years were included in the analysis. Periodontal disease was assessed using the Community Periodontal Index. Obesity was measured according to body mass index. Odd ratios (ORs) were estimated using binary logistic regression analysis model. The stratification variables were smoking and diabetes, and also adjusted for sex, age, and education level. The adjusting variables included sex, age, and education level. The ORs of obesity for periodontal disease were 0.78 (95% CI = 0.51-1.22) for the category of <18.5 of BMI and 1.29 (95% CI = 1.11-1.49) for the ${\geq}25$ BMI category (both compared to the 18.5 to 24.9 category). In a subgroup analysis, the OR of BMI among those who were both non-smoking and no diabetes was 1.20 (95% CI = 1.02-1.48) for those with BMI levels ${\geq}25$. This study implies that obesity intolerance may be an independent risk factor for periodontal diseases. Obesity should also be considered when managing periodontal disease to improve oral health.

Electrophysiological Characterization of AMPA and NMDA Receptors in Rat Dorsal Striatum

  • The striatum receives glutamatergic afferents from the cortex and thalamus, and these synaptic transmissions are mediated by ${\alpha}$-amino-3-hydroxy-5-methyl-4-isoxazolepropionate (AMPA) and N-methyl D-aspartate (NMDA) receptors. The purpose of this study was to characterize glutamate receptors by analyzing NMDA/AMPA ratio and rectification of AMPA and NMDA excitatory postsynaptic currents (EPSCs) using a whole-cell voltage-clamp method in the dorsal striatum. Receptor antagonists were used to isolate receptor or subunit specific EPSC, such as (DL)-2-amino-5-phosphonovaleric acid (APV), an NMDA receptor antagonist, ifenprodil, an NR2B antagonist, CNQX, an AMPA receptor antagonist and IEM-1460, a GluR2-lacking AMPA receptor blocker. AMPA and NMDA EPSCs were recorded at - 70 and + 40 mV, respectively. Rectification index was calculated by current ratio of EPSCs between + 50 and - 50 mV. NMDA/AMPA ratio was 0.20${\pm}$0.05, AMPA receptor ratio of GluR2-lacking/GluR2-containing subunit was 0.26${\pm}$0.05 and NMDA receptor ratio of NR2B/NR2A subunit was 0.32${\pm}$0.03. The rectification index (control 2.39${\pm}$0.27) was decreased in the presence of both APV and combination of APV and IEM-1460 (1.02${\pm}$0.11 and 0.93${\pm}$0.09, respectively). These results suggest that the major components of the striatal glutamate receptors are GluR2-containing AMPA receptors and NR2A-containing NMDA receptors. Our results may provide useful information for corticostriatal synaptic transmission and plasticity studies.

Increased Prevalence of Chronic Disease in Back Pain Patients Living in Car-dependent Neighbourhoods in Canada: A Cross-sectional Analysis

  • Objectives: Chronic diseases, including back pain, result in significant patient morbidity and societal burden. Overall improvement in physical fitness is recommended for prevention and treatment. Walking is a convenient modality for achieving initial gains. Our objective was to determine whether neighbourhood walkability, acting as a surrogate measure of physical fitness, was associated with the presence of chronic disease. Methods: We conducted a cross-sectional study of prospectively collected data from a prior randomized cohort study of 227 patients referred for tertiary assessment of chronic back pain in Ottawa, ON, Canada. The Charlson Comorbidity Index (CCI) was calculated from patient-completed questionnaires and medical record review. Using patients' postal codes, neighbourhood walkability was determined using the Walk Score, which awards points based on the distance to the closest amenities, yielding a score from 0 to 100 (0-50: car-dependent; 50-100: walkable). Results: Based on the Walk Score, 134 patients lived in car-dependent neighborhoods and 93 lived in walkable neighborhoods. A multivariate logistic regression model, adjusted for age, gender, rural postal code, body mass index, smoking, median household income, percent employment, pain, and disability, demonstrated an adjusted odds ratio of 2.75 (95% confidence interval, 1.16 to 6.53) times higher prevalence for having a chronic disease for patients living in a car-dependent neighborhood. There was also a significant dose-related association (p=0.01; Mantel-Haenszel chi-square=6.4) between living in car-dependent neighbourhoods and more severe CCI scores. Conclusions: Our findings suggest that advocating for improved neighbourhood planning to permit greater walkability may help offset the burden of chronic disease.

  • Objective : The prostatic disease is characterized as relatively incurable, chronic and recurrent illness. Because of this property, we have the difficulty in treating this illness. Besides, the drug selectivity is restricted to the specific prostatic membrane, the barrier of prostate. This study was to evaluate the efficency of the activating blood flow and removing blood stasis Method to Prostatic disease. Methods : We investigated 51 prostatic patiens with NIH chronic prostatitis symptom index, digital rectal examination(DRE), expressed prostatic secretion(EPS). After investigation we treated the patients with herb which was aimed to activate the blood flow and remove the blood stasis method to prostatic disease. Results : After the treatment, symptom index score of the prostatic patients was reduced from 11.7 to 7.4. The cure rate of 51 prostatic patients was 76.5%. Also the cure rate of the patients who was treated over 4 weeks was higher than the patients treated below 4 weeks, 87.5% to 66.7% respectively. conclusions : These results indicate that oriental medical theraphy is useful enough to treat the prostatic patients. Therefore, the more research about the herb which has activating blood flow and removing blood stasis effect is necessary.

To Predict Body Composition of Children and Adolescents by BIA in China

  • Objective : The paper aims to provide predictive coefficients via BIA for the assessment of body composition in children and adolescents to serve clinical as well as research purposes. Methods : Body composition via dual-energy x­ray absorptiometry (DXA) and bioelectric impedance as well as other anthropometric index were derived from meaurements on 1026 children and adolescents aged from 6 to 18 years from Beijing City. The best subset regression and principle component analysis were adopted to build the predictive coefficients with the logarithm of body composition via DXA as response variable. Results : Condition index ${\varphi}$ of fat-free mass multiple linear regression achieves 113.49 and 91.18 for males and females respectively, demonstrating severe multicollinearity among anthropometric indexes in children and adolescents. BIA predictive coefficients base on the best subset regression and principle component analysis boast a content predictive value for lean mass ($r^2$ = 0.9697 and 0.9664 for boys and girls respectively, p < 0.0001) and for Fat$\%$ ($r^2$ = 0.7705 and 0.6959 for boys and girls respectively, p < 0.0001). Conclusions : BIA method is applicable for the prediction of body composition for children and adolescents.

The Effect of Dual-task Gait Training on Balance, Gait, and Activities of Daily Living for Patients with Parkinson's Disease -A Single-subject Experimental Design- (이중과제 보행훈련이 파킨슨병 환자의 균형, 보행능력 및 일상생활동작에 미치는 효과 -단일사례연구-)

  • Purpose: This study identified the effects of dual-task gait training on balance, gait function, and activity of daily living in patients with Parkinson's disease. Methods: This study used a single-subject design. Two patients with Parkinson's disease participated in this study. Dual-task gait training was performed 1 hour per day 8 times during intervention phase. The subjects were measured 8 times in the baseline phase, 8 times in the intervention phase, and 8 times in the follow-up phase. The outcome measurements included a timed up and go test (TUG), a Berg balance scale (BBS), a 10 meter walk test (10MWT), a 6 minute walk test (6MWT), a dynamic gait index (DGI) and a Korean modified Barthel index (K-MBI). Results: When compared to the average of the baseline process, the data collected during the intervention period showed that the TUG and 10MWT results improved and the tendency line was above the baseline. In addition, BBS, 6MWT, DGI, and K-MBI values for both patients increased remarkably after the training. Conclusion: The results of this study revealed that dual-task gait training may be helpful to improve balance, walking function, and activity of daily living for patients with Parkinson's disease. Further studies need to confirm our findings.

Association of Nutritional Status with Obesity by Body Mass Index and Waist Circumference among Hypertensive Elderly Patients (노년기 고혈압 관리 대상자의 체질량지수, 허리둘레에 의한 비만정도와 영양소 섭취 상태 비교 연구)

  • Hypertension and obesity are important modifiable risk factors for cardiovascular disease, the leading cause of death in Korea. Therefore, we assessed the association between dietary pattern and obesity in hypertensive patients to formulate health promotion strategies for the older population. Dietary information was collected from hypertensive patients visiting community health education and information center by using 24 hour recall method. The 2005 DRIs for Koreans was used to evaluate the dietary adequacy. When subjects were categorized by body mass index (BMI) as normal, overweight and obese, no significant difference in energy intake was found among groups. Dietary intakes of folate, and vitamin C in obese hypertensive patients were significantly lower than in normal weight patients (p < 0.05). When we compare the nutritional status by waist circumferences, dietary intakes of zinc, vitamin A, thiamin, vitamin C and folate were significantly lower in the obese group. Vegetable intake was significantly lower in the obese group according to BMI as well as waist circumference. Energy intake from carbohydrate was significantly higher in obese hypertensive patients (p < 0.05). Obese hypertensive patients had a higher risk of nutritional inadequacy compared to normal weight patients. Our results indicated the need for developing interventions that encourage greater consumption of vegetables while cutting down salt intake with wise selection of staple foods, for obese hypertensive patients.

Effect of dietary quality on periodontal diseases in Korean adults: the Korea National Health and Nutrition Examination Survey (2016-2018) (한국 성인의 식생활의 질이 치주질환에 미치는 영향: 국민건강영양조사 제7기(2016-2018) 자료 활용)

  • Objectives: This study investigated the relationship between the Korean healthy eating index (KHEI) and periodontal disease in the Korean adult population. Methods: The data used in the analyses were obtained from the seventh Korean National Health and Nutrition Examination Survey (2016-2018). Data were analyzed by chi-square tests and t-test. Multiple regression analysis was also performed to assess the association between KHEI and periodontal disease. Statistical significance was set at p<0.05. Results: Multiple logistic regression analysis adjusted for socioeconomic variables showed that medical and health behavior variables were significantly related to the KHEI 1 (<63.7, odds ratio [OR]: 1.23, 95% confidence interval [CI]: 1.03-1.46), KHEI 2 (63.7-79.9, OR: 1.14, 95% CI: 0.97-1.34), and risk for periodontal disease. Conclusions: The results showed a significant association between the KHEI and periodontal disease in the Korean adult population.

Clinical Study on Nonalcoholic Fatty Liver Disease Related Factors: Tumor Marker, Heart Rate Variability, Sasang Constitution (비알코올성지방간의 유관 인자 임상 연구: 종양표지자, 심박변이도, 사상체질)

  • Objectives The aim of this study is to investigate the related factors of nonalcoholic fatty liver disease (NAFLD). Methods The subjects were 187 persons diagnosed as fatty liver by abdominal ultrasonography. They were divided into three groups according to the severity of fatty liver: control, mild, moderate or severe. The three groups' general characteristics, laboratory results, liver function indexes, metabolic syndrome indexes, tumor markers, heart rate variability values and Sasang constitution distribution were compared and analyzed. Results Male ratio, height, weight, body mass index, red blood cell count, hemoglobin level and creatinine level were higher in NAFLD groups than in control group. The levels of sodium and amylase were higher in control than in NAFLD. In liver function, the levels of aspartate transaminase, alanine transaminase and gamma-glutamyl transpepsidase of NAFLD were higher. In metabolic syndrome index, systolic blood pressure, diastolic blood pressure, waist circumference, total cholesterol, triglyceride and low density lipoprotein cholesterol levels were higher in NAFLD, while high density lipoprotein cholesterol level was higher in control. The alpha-feto protein level was higher in NAFLD, and the heart rate variability was not different between NAFLD and control groups. In Sasang constitution, Taeeumin ratio of NAFLD was higher than of control. Conclusions The results suggest that nonalcoholic fatty liver is clinically related to liver dysfunction, metabolic syndrome, tumor markers, and Sasang constitution. Further studies are needed to control nonalcoholic fatty liver disease and prevent severe disease such as cirrhosis and cancer caused by fatty liver.

A Case Report of Nonalcoholic Fatty Liver Disease with Obesity and Dyslipidemia (비만과 이상지질혈증을 동반한 비알코올 지방간질환 환자 치험 1례)

  • Objectives: This study aims to report the clinical efficacy of Korean medical treatment of a patient with nonalcoholic fatty liver disease accompanied by obesity and dyslipidemia. Methods: A 52-year-old man with nonalcoholic fatty liver disease, obesity, and dyslipidemia was treated with Saenggangunbi-tang extract from November 3, 2021 to January 8, 2022. During the treatment period, the patient also undertook exercise and maintained a hypocaloric diet to reduce body weight. Changes were observed in the patient's symptoms, laboratory findings, such as liver enzymes and lipid profiles, and bioelectrical impedance analyzer results. Results: After taking Saenggangunbi-tang extract for approximately two months, with concomitant adjustments to exercise and diet, the serum levels of liver enzymes and triglyceride were decreased and the patient's body weight, body mass index, hepatic steatosis index, and visceral fat area were also reduced. In addition, symptoms including fatigue and dyspepsia improved. Conclusion: This study suggests that Saenggangunbi-tang extract with exercise and a regulated diet could be a helpful treatment strategy for managing nonalcoholic fatty liver disease with metabolic disorders.
