Blood Eosinophil Counts in Chronic Obstructive Pulmonary Disease: A Biomarker of Inhaled Corticosteroid Effects

  • Blood eosinophil counts have emerged as a chronic obstructive pulmonary disease (COPD) biomarker that predict the effects of inhaled corticosteroids (ICS) in clinical practice. Post-hoc and prospective analysis of randomized control trials have shown that higher blood eosinophil counts at the start of the study predict a greater response to ICS. COPD patients with frequent exacerbations (2 or more moderate exacerbations/yr) or a history of hospitalization have a greater response to ICS. Ex-smokers also appear to have a greater ICS response. Blood eosinophil counts can be combined with clinical information such as exacerbation history and smoking status to enable a precision medicine approach to the use of ICS. Higher blood eosinophil counts are associated with increased eosinophilic lung inflammation, and other biological features that may contribute to the increased ICS response observed. Emerging data indicates that lower blood eosinophil counts are associated with an increased risk of bacterial infection, suggesting complex relationships between eosinophils, ICS response, and the airway microbiome.

  • The purpose of this study is to evaluate factors that have correlation with improvement of Cerebrovascular accidents(C.V.A) and to decide ranking of influence about improvement of C.V.A. This observation was made on 153 subjects of C.V.A. that were diagnosed through brain MRI or brain CT. They were hospitalized in the Semyung University Oriental Medicine Hospital from the January 1st 2006 to December 31th 2007. The subjects of this study are divided into two groups. The one group has slight motor disturbance, and the other group has severer motor disturbance. Based on medical treatment chart, we analyze differences of many factors like past history, family history, drinking, smoking, several symptoms with C.V.A., etc between two groups. As a result, The past history of cerebrovascular disease and past history of hypertension are the most influencing factors in improvement of C.V.A.

한국 성인에서의 당뇨병성 족부 질환의 신발 및 족부 질환 연구 ("Study for the orthotics & shoes of diabetic among Korean adult")

  • Purpose: To provide baseline data for prevention and education of diabetic foot in Korea. Materials and Methods: 216 diabetic patients are included in this survey. The average age is 58.64 year old with average 9.92 years of diabetes mellitus periods. Among them, 41% used insulin injection, 42% oral medication, 14% both and 3% diet were used for their blood sugar control. The diabetic foot survey was done. Results: The survey of patients symptom and disease of their feet results: 49% had sensory decrease in their feet. Associated disease in foot were callus(15%), heel pain(10%), bunion(4%), toe deformity(2%), Tinea pedis were 9%, tingle sensation were 17%, and swelling on feet were 2%, The survey of patients privious history of foot problems are: 8% had previous operation history on their feet and 17% had history of infection on their feet, 23% had bullae history, The survey of patients prevention and education are: Those who have had more than one diabetic foot education were 45.7%, and among them 24% had routine regular foot check up. Only 5% wear special diabetic shoe and 3% had custom orthotics. Conclusion: Interests to diabetic foot care is increasing in diabetic patients in Korea. However still we need more education, and efforts for prevention. Special education and diabetic shoe is still low, The survey data could be good baseline for diabetic prevention and education.

Gender-related Factors Associated with Upper Extremity Function in Workers

  • Objectives: This study aimed to find gender distinctions in terms of the sociology of the population; to determine work-related factors; to analyze gender differences in daily living, work, sports, and art performances; and to identify gender-related factors that limited performance of daily living and work activities. Methods: A questionnaire was designed that included disabilities of the arm, shoulder, and hand (DASH), accident history, disease history, work duration at current workplace, marital status, job satisfaction, job autonomy, and physical demands of the job. Out of 1,853 workers surveyed, 1,173 questionnaires (63.3%; 987 males, 186 females) included responses to DASH disability and DASH optional work and were judged acceptable for analysis. Results: Upper extremity functional limitation during work and daily living was higher for females than males. The limitations for males increased according to their household work time, accident history, work duration, job satisfaction, physical demand, and job autonomy. Meanwhile, female workers' upper extremity discomfort was influenced by their disease history, job satisfaction, and physical demands. In addition, the size of the company affected male workers' upper extremity function, while marriage and hobbies influenced that of female workers. Conclusion: This study addressed sociodemographic factors and work-related factors that affect each gender's upper extremity function during daily living and working activities. Each factor had a different influence. Further studies are needed to identify the effect that role changes, not being influenced by risks at work, have on musculoskeletal disorders.

『의방유취』의 도인도(導引圖)에 관한 연구 (The Study on Pictures of Daoist exercises of Euibangyoochi)

  • This article is about the study on pictures of Daoist exercises described in Euibangyoochi- which is the largest book in Joseon Dynasty. Daoist exercises are the active exercises to prevent disease by training their body. In Euibangyoochi, there are lots of information of Daoist exercises. Specially, pictures of Daoist exercises are from '$q{\acute{u}}xi{\bar{a}}nhu{\acute{o}}r{\acute{e}}$nxīnf$\bar{a}$ng'. There are movements of body, methods of respiration, tooth brushing, and hand clapping in Daoist exercises this book. On the other hand, the pictures of Daoist exercises were composed with simple explanations so that readers actually might follow the directions at ease. There are 8 pictures of Daoist exercises left in Euibangyoochi. Those are Daoist exercises tradition in the midium of Joseon Dynasty which led to Hwalinshimbang of Yi-hwang. Also with the trend of preventing disease in these days, the importance of Daoist exercises is emphasized and these pictures became a tool of teaching the old Daoist exercises.

정조의 격기(膈氣)에 대한 연구 - 『승정원일기』를 중심으로 - (A Study on Gyeok-ki(膈氣) Symptoms of King Jeongjo - Foucsed on The Daily Records of Royal Secretariat of Joseon Dynasty -)

  • Objectives : To examine the progression and presentation of King Jeongjo's Gyeok-ki(膈氣) symptoms during his reign. Methods : Based on recordings in the The Daily Records of Royal Secretariat of Joseon Dynasty related to Gyeok-ki(膈氣), characteristics of King Jeongjo's case was analyzed, then compared and examined against the political situation he was in. Results : During the 24 years of reign, King Jeongjo's Gyeok-ki(膈氣) developed in four stages; beginning stage with mild symptoms, intrinsic stage where it became the King's main disease, peak stage where symptoms intensified, and chronic stage where symptoms became rather manageable but the disease occurred regularly. Conclusions : From the year of accession to the year of death, King Jeongjo consistently complained of Gyeok-ki(膈氣), which seems to be strongly related to his family history and political situation.

Assessment of the correlation between various risk factors and orofacial cleft disorder spectrum: a retrospective case-control study

  • Background: Orofacial clefts (OFCs) comprise a wide range of malformations, including cleft lip, cleft palate, and cleft lip with cleft palate, which can vary in terms of etiology, severity, and disease burden. Objective(s): This study aimed to evaluate the correlation between various risk factors and orofacial cleft disorder spectrum in newborns. Study design: A total of 323 cases and 400 controls were enrolled in this study and evaluated in terms of the maternal history of abortion or miscarriage, child's sex, maternal and paternal age, maternal history of systemic disease, history of medication therapy during pregnancy, birth order, consanguineous marriage, and complications during pregnancy. Results: Analysis of the results suggested that consanguineous marriage, a maternal history of abortion/miscarriage, and complications during pregnancy could potentially increase the risk of OFCs in children (P < 0.05). However, the analyses revealed that the other variables could not potentially increase the risk of OFCs (P > 0.05). Conclusion(s): Multiple cofactors may simultaneously contribute to the formation of such abnormalities; therefore, a comprehensive, multidisciplinary care program is necessary to ensure a successful pregnancy period and the birth of a healthy newborn.

통규탕가미방(通竅湯加味方)의 비질환(鼻疾患) 치료에 대한 임상고찰 (A Clinical study on the Treatment of Nasal disease using TonggyutangGamibang)

  • Objective: For last 20 years our living environment has been changed rapidly. Accordingly. there have been many changes in prevalence of diseases; In nasal disease of the pediatrics, the number of children suffering from chronic rhinorrhea and nasal obstruction has been increased. And respiratory symptoms suggesting rhinitis and sinusitis also were increased. Allergic disease is considered as an important cause of this phenomenon while importance of infection as pathogen is fading today. The aim of this study is to investigate the effect of TonggyutangGarnibang on the nasal disease of children. Methods: Children with nasal disease answer the questionnaires on their chief complaints and medical past history on their first visit to our clinic. They were treated 'With TonggyutangGarnibang for 4weeks. After treatment they answered the questionnaire on improvement of their symptoms. Results: Among treated patients, 70 were male(69%) and 32 were female(31%); sex ratio was 2.2.3:l(male:female). And children from 3-8 years old took majority of patients(64%). Major symptoms of children with nasal disease were as follows; nasal obstruction 9lchildren(62%), rhinorrhea 86children(84%), sneezing 63children(62%), cough 63children(62%), snoring 48children (47%), poor appetite 44children(43%), itching 39children(38%), headache 32children(31%), stomache 25children(25%), epistaxis 23children(23%), constipation 17children(17%), diarrhea 12children(12%). Past history and impression of the disease of the patients were as follows; allergic rhinitis(66%), atopic dermatitis(42%), sinusitis(40%), asthma(25%), urticaria(12%). Severity of symptom improved after TonggyutangGamibang treatment, it showed high treatment efficacy as follows; sneezing 70%, cough 70%, rhinorrhea 63%, nasal obstruction 59% Conclusion: This study shows TonggyutangGamibang has distinctive treatment effect on children with nasal disease.

한국 한의학 질병사인분류 체계에 관한 연구 (The research on the disease classifications of the traditional medicine in Korea)

  • Korea follows the Korea standard classification of disease and causes of death according to the ICD(international classification of disease) Oriental medicine began to of officially follow the classification of disease for using the Korean classification of diseases in 1972. The classification of OM(oriental medicine) has changed in shape experiencing two amendments. The largest difficulty was to overcome the different names of diseases between OM and ICD. A one-to-one correspondence of the name of a disease between OM and ICD is impossible So in the primary stage one-to-one and one-to-many correspondence was made. During the first amendment the international disease names were re-classified on the oriental medicine disease name's basis and at the same time the classification of OM was corresponded on a one-to-one basis to the ICD . During the second amendment this changed to many-to-many correspondence . Analyzing the history of classification of OM during the first and second amendments, it was discovered that establishment of the standards of classification, the unification of oriental medical terms, and overcoming the difference of disease names between the OM and ICD is necessary Also th classification and standardazation of OM must not stop as a single round. It must go on for a long time. The hosts of this project Korean oriental medical society and AKOM(association of korean oriental medicine) need to build a independant department which will supervise the classification project and monitor any problems to come up. Also a route through which suggestions can be taken in and new solutions can be brought up needs to be secured and an atmosphere in which studies can take place about the basis of classifications needs to be developed.

생산직 근로자의 심뇌혈관질환 예방실천행위에 영향을 미치는 요인 (Factors Affecting the Preventive Behavior of Cardiocerebrovascular Disease in Blue Color Workers)

  • Purpose: The purpose of this study was to investigate blue color workers' knowledge and attitudes about cardiocerebrovascular disease, and these influence on their preventive health behavior. Methods: Data were collected from 235 blue color workers using questionnaire in 'H' city. Data were analyzed using hierarchical multiple regression analysis with SPSS/WIN 17.0 version. Results: Health behaviors for preventing cardiocerebrovascular disease were correlated positively with knowledge about cardiocerebrovascular disease. In addition, preventive behaviors for cardiocerebrovascular disease was influenced by marital status, monthly income, past history of cardiocerebrovascular disease, and knowledge about cardiovascular disease. Conclusion: Knowledge about cardiocerebrovascular disease has an effect on preventive behaviors regarding cardiocerebrovascular disease. For preventing cardiocerebrovascular disease, national campaign and health education program in each industry need to be built.
