This study examined guange (關格) in Yixuerumen (醫學入門) from a new perspective and evaluated its value in terms of emergency medicine. We compared and analyzed the definition, symptoms, and treatment methods of guange in Yixuerumen with related clinical presentations in modern medicine. Guange includes nausea and vomiting, dysphagia, dysuria, and constipation without diarrhea. A representative disease related to dysuria is uremia, while one related to constipation, nausea, and vomiting is enterocleisis, and and a third related to dysphagia is chronic esophagitis and esophagus cancer. Li Chan, the author of Yixuerumen, recognized guange as an emergency of serious eating and elimination disorder as various symptoms of diseases (病症) gradually get worse from the perspective of external diseases (外感病) and miscellaneous diseases (雜病) and handled it in various ways including yin-tonifying medicinals (補陰藥), qi-tonifying medicinals (補氣藥), qi-regulating formulas (理氣劑), phlegm-dispelling formulas (去痰劑), and "water rising, fire falling" formulas (水升火降) in addition to emetic formulas (催吐劑), diuretic formulas (滲利劑), and interior-attacking formula (攻裏劑). Concepts like guange in Yixuerumen were somewhat useful when handling the applicable emergencies at the level of then medical treatment.
Since the discovery that Mycobacterium aviumsubsp. paratuberculosis (Map) is the causative agent of Johne's disease (JD) in cattle at the end of the nineteenth century, movement of livestock latently infected with Map has led to the spread of JD throughout the world. A new form of enteritis with clinical features of JD in cattle appeared in humans concurrent with the appearance of Map as a disease problem in livestock. The demonstration that Map is a zoonotic pathogen and the causative agent of the new form of enteritis in humans, however, wasn't recognized until late in the twentieth century when methods were developed to detect the presence Map in tissues from patients with the new form of clinical enteritis. The objective of this short review is to provide a brief history explaining how Map has become a major disease problem in livestock and humans and then provide a review of the progress that has been made in treating patients with an enteritis caused by Map and the strategies underway to develop a vaccine to control infection in livestock.
Background: Patients with long-standing inflammatory bowel disease (IBD) have an increased risk of developing colorectal cancer (CRC). Colon cancer risk in IBD increases with longer duration and greater anatomic extent of colitis, the presence of primary sclerosing cholangitis, family history of CRC and degree of inflammation of the bowel. This study aimed to characterize the histopathological pattern of benign colorectal diseases among Saudi patients and to highlight age and gender variations of lesions as base line data for future studies to investigate the link between benign/IBD and colorectal cancers in the local population. Materials and Methods: The materials consisted of 684 biopsies, reported as benign (excluding malignancies and polyps) at the Department of Pathology, King Fahad Hospital, Madinah, Saudi Arabia from January 2006 to December 2013. Data collected and entered in MS-Excel and were analyzed using SPSS-20. Results: Of 684 colorectal tissues reviewed, 408 specimens (59.6%) were from male patients and 276 specimens (40.4%) were from females giving a male: female ratio of 1.5:1. Age of the patients ranged from 4 to 75 years with a mean of 39.6 years. The most frequent histologic diagnosis was a chronic non specific proctocolitis followed by ulcerative colitis, accounting respectively for 52.6% and 31.7% of all cases. These were followed by Crohn's disease 22 (3.2%), ischemic bowel disease 20 (2.9%), diverticular disease 14 (2%), eosinophilic colitis 12 (1.7%) and solitary rectal ulcer 12 (1.7%). A minority of 21 patients (3.1%) were cases of acute nonspecific proctocolitis, schistosomiasis, tuberculosis, volvulus and pseudomembranous colitis. Conclusions: These data show that although chronic non specific proctocolitis and ulcerative colitis were the dominant diagnoses, Crohn's disease, ischemic bowel disease and diverticular disease also existed to a lesser extent and should be considered in the differential diagnosis of benign colorectal diseases. This study provides a base line data for future studies which would be taken up to investigate the link between benign/IBD and colorectal cancers in the local population.

Primary renal tumors are uncommon in dogs with prevalence rate of approximately 1%. Renal carcinoma originating from epithelium of proximal convoluted tubules are more likely to be affected to Middle-aged dogs (average age, 8y), males about twice as often as bitches. A 10-year-old, female, German Shepherd dog with history of anorexia, vomitting and hematuria was referred to the Animal Disease Diagnostic Division in Animal and Plant Quarantine Agency. The dog was necropsied and several organs were collected, fixed in 10% phosphate-buffered formalin, embedded in paraffin wax and sectioned for histopathology. Grossly, the kidneys were bilaterally enlarged ($18{\times}12{\times}8cm$; left, $18{\times}10{\times}8cm$; right). The numerous cysts varying sizes from 3 to 6 cm in diameter were protruding from the surface of both kidney. A large nodule ($10{\times}6{\times}6cm$) was discovered between cardiac and diaphragmatic lobe in the right lung. Immunohistochemical examination revealed strong positive reaction to cytokeratin and ki-67 in the nuclei of the epithelial tumor cells. But showed negative reactions to vimentin and CD10. Based on the pathological and immunohistochemical examination, we diagnosed as the bilateral renal cystadenocarcinoma in German shepherd dog.

The reading medical records is one of the best way to develop one's abilities of curing a disease without clinical practice. so we can't help dealing with medical records, because it is one of important method of understanding Oriental Medicine. On this study, we investigate a objective method on understanding medical records in "Chimgudaeseong(鍼灸大成)".
Since the onset of modern psychosomatic medicine decades ago. it has made significant at contribution to general medicine in terms of understanding the patient as a whole person suffering from an illness rather than as a disease per se. However, initial expectation of delineating specific psychogenesis and developing specific treatment modality has not been fulfilled. Consequently, it has raised substantial concern about the continuing utility of psychosomatic medicine in modem medicine. The author attempted to review critically the evolution of psychosomatic medicine from the perspective of the present and to make projections into the near future. It is concluded that psychosomatic medicine in the future will continue to contribute to development of modem medicine not only as the scientific principle but also as the practical methodology, working as creative interface among disciplines and fusing ideas from rapidly developing molecular biology and neuroscience with psychosomatic concepts. Finally, suggestions for further developing psychosomatic medicine in Korea are made.

Gas forming brain abscess is a rare disease caused by Klebsiella pneumoniae occurring in patients with impaired host defense mechanism such as diabetes mellitus or liver cirrhosis. A 59-year-old man with 2-year history of diabetes mellitus and 20-year history of liver cirrhosis presented to the hospital with headache. On the day after admission, severe headache was developed and he deteriorated rapidly. Brain CT showed a non-enhanced mass including multiple air density as well as surrounding edema seen in the right occipital lobe, and isodensity air-fluid level seen in the right lateral ventricle. Despite emergent ventricular drainage and intraventricular and intravenous administration of antibiotics, his condition progressively worsened to sepsis and to death after 5 days. Bacterial culture of blood and ventricular fluids disclosed a Gram (-) rod, Klebsiella pneumoniae. In this report we review the pathogenic mechanism and its management.

Necrotizing sialometaplasia (NS) which mimics malignancy both clinically and histopathologically is an uncommon benign, self-limiting inflammatory disease of the mucus-secreting minor salivary glands. The lesion is believed to be the result of vascular ischemia that may be initiated by trauma. Till date, the diagnosis of NS remains a challenge. This report demonstrates a case of NS in a 73-year-old male patient who presented with an ulcerative lesion in his palate. He had a history of local trauma and was long-term user of salbutamol inhaler. An incisional biopsy was carried out and the diagnosis was established through history, clinical examination, histopathology using Hematoxylin and Eosin stain. The patient was given symptomatic treatment and the lesion healed in about 7 weeks.
