Surgical Treatment of Peripheral Arterial Disease -Clinical Analysis Of 55 Cases- (말초동맥질환의 외과적치료 -55예의 임상분석-)

  • This report is a review of 55 cases of peripheral arterial disease, who were treated at the department of thoracic and cardiovascular surgery, Masan Koryo General Hospital from January, 1986 to December, 1990. The result are summerized as follows ; 1. The incidence of peripheral arterial disease were as follows that : Arterial injury was in 21 cases(38.2%), arteriosclerosis oblitrans 18cases(32.7%), thromboembolism 9cases(16.4%), Buerger's disease was in 7cases(12.7%). 2. Overall male to female ratio was 6.8 : 1, the prevalent age was 3rd and 4th decade in arterial injury, 7th and 8th decade in atherosclerosis and thromboembolism and 5th and 6th decade in Buerger's disease. 3. The farmer was the first ranked occupation of these patients with chronic occlusive arterial disease, which was composed of 17 cases (68%). 4. 23 cases of patients with chronic occlusive disease has been smoking and most of them have been smoking over 10 years. 5. The clinical symptoms in acute and chronic arterial obstruction were pain, claudication, gangrene and coldness in order. 6. The duration of symptom of chronic arterial occlusive disease was less 1 years in 15 cases(60%). 7. The lower extremity were more affected than upper extremity in peripheral arterial disease. 8. The cause of arterial injury was traffic accident 9 cases(42.9%) stab wound 8 cases (38.1%), postangiography 2 cases(9.5%) and belt injury 1 case. 9. The etiologic factors of acute arterial occlusion was arterial fibrillation myocardial ischemia and postangiography in order. 10. Lumber sympathectomy in Buerger's disease, artificial bypass graft in atherosclerosis and thromboembolctomy in thromboembolism, end to end with vein graft in arterial injury were performed frequently. 11. Conclusively overall result was satis factory but 3 cases was below knee amputated after operation of chronic arterial occlusive disease.

Simulation of Grape Downy Mildew Development Across Geographic Areas Based on Mesoscale Weather Data Using Supercomputer

  • Weather data for disease forecasts are usually derived from automated weather stations (AWS) that may be dispersed across a region in an irregular pattern. We have developed an alternative method to simulate local scale, high-resolution weather and plant disease in a grid pattern. The system incorporates a simplified mesoscale boundary layer model, LAWSS, for estimating local conditions such as air temperature and relative humidity. It also integrates special models for estimating of surface wetness duration and disease forecasts, such as the grapevine downy mildew forecast model, DMCast. The system can recreate weather forecasts utilizing the NCEP/NCAR reanalysis database, which contains over 57 years of archived and corrected global upper air conditions. The highest horizontal resolution of 0.150 km was achieved by running 5-step nested child grids inside coarse mother grids. Over the Finger Lakes and Chautauqua Lake regions of New York State, the system simulated three growing seasons for estimating the risk of grape downy mildew with 1 km resolution. Outputs were represented as regional maps or as site-specific graphs. The highest resolutions were achieved over North America, but the system is functional for any global location. The system is expected to be a powerful tool for site selection and reanalysis of historical plant disease epidemics.

Measuring the Burden of Hypertension using DALY in Korea (장애보정생존년수(DALY)를 활용한 우리 나라 고혈압의 질병부담 측정)

  • This study estimated the burden of disease due to hypertension in Korea in disability-adjusted life years(DALYs) using vital registration data and the National Health Examination Survey data. Firstly, we estimated the years life lost due to premature death (YLLs) of hypertension using the vital registration data. Secondly, to calculate the years lived with disability (YLDs), we estimated the average age at onset and disease duration using the National Health Examination Survey data. The disability weights for hypertension were estimated by person trade off method. Finally, the burden of hypertension was calculated in DALYs, which are the sum of YLLs and YLDs. The burden of hypertension for males was attributed mainly to YLD(97.9%). DALYs for females were also attributed mainly to YLD(96.7%). DALYS for males were 993,950 person-years and for females were 743,282 person-years. Results of this study provide a rational basis to plan a national health policy regarding the disease burden of hypertension in Korea. We will need accurate epidemiological study results and other study results of national burden of disease in Korea to get more accurate results of this burden of disease study.

The Effect of wool in patients with chronic obstructive pulmonary disease

  • The purpose of this study was to analyze the effect wool in patients with chronic obstructive pulmonary disease. The study was conducted experimentally on 53 patients with chronic obstructive pulmonary disease attending the chest diseases polyclinic of a hospital located in Erzurum. A randomized selection method was used to categorize patients into two groups; a treatment (n = 27), and a control group (n = 26). Patients in the treatment group (pre-test measurements were taken) wore wool vests for a period of three months, while patients in the control group wore cotton vests (placebo) for the same duration; post-test measurements were taken for both groups at the end of the three-month period. The Medical Outcomes Study Short Form 36, the St. George's Respiratory Questionnaire, and pulmonary function tests were used to collect data. Results concluded that there was a significant decrease (p < 0.05) in the overall score of the St. George's Respiratory Questionnaire, and the symptom, effect, and activity mean scores, while there was a significant increase (p < 0.05) in the SF-36 physical functioning, general health, bodily pain, role physical, vitality, role emotional, social functioning, mental health, physical and mental component summary mean scores of patients in the treatment group. Results of the study concluded that the symptoms, activity, disease effectiveness, and quality of life improved in patients that wore wool vests.

Retrospective Observational Study for Effectiveness of Inmok-tang on Dry Eye Syndrome (건성안 증후군 환자에서 인목탕의 유효성 평가를 위한 후향적 관찰연구)

  • Objectives : The purpose of this study is to analyze effectiveness of herbal medicine treatment on Dry Eye Syndrome. Methods : We analyze medical records of 114 patients who visited Indara oriental medical clinic for dry eye syndrome. Patients were treated with acupuncture and Inmok-tang, and the ocular surface disease index was evaluated before and after treatment. Results : Analysis of 114 medical records showed a significant decrease in the pre-treatment and post-treatment ocular surface disease index, and a statistically significant positive correlation between the duration of Inmok-tang administration and the change in the ocular surface disease index. Conclusions : Herbal medicine treatment with Inmok-tang reduces the Ocular Surface Disease Index, an indicator of subjective symptoms of dry eye syndrome.

Ultrasound-guided epidural block in axial spondyloarthritis patients with limited spine mobility: a randomized controlled trial

  • Background: Evaluation of the effectiveness of caudal epidural injection on pain, spine mobility, disease activity, and activity of daily living in axial spondyloarthritis (SpA) patients. Methods: A total sample of 47 patients were registered in this study. They were randomly assigned into 2 groups; Group I received caudal epidural injections, ultrasound-guided, with 1% lidocaine hydrochloride mixed with triamcinolone, whereas Group II did not receive any injections. All participants fulfilled the ASAS criteria for axial SpA. Outcome measures were as follows: visual analogue scale, Oswestry disability index (ODI), modified Schober test, lateral lumbar flexion, and Ankylosing Spondylitis Disease Activity Score (ASDAS) with assessment at baseline, 2 weeks, and 8 weeks post-treatment. This clinical trial was registered on clinicaltrials.gov under the number NCT04143165. Results: There was a significant difference between both groups regarding pain, ODI, spine mobility and ASDAS scores in favor of group I. This effect was at its maximum after 2 weeks. Despite the decline of this effect after 2 months, the difference between the groups remained significant. Higher disease activity, younger age, and shorter disease duration were associated with better outcomes. Conclusions: Epidural injection of lidocaine and triamcinolone is a cost effective and a practical technique for controlling pain, as well as improving the function of the spine and disease activity scores in axial SpA patients with acceptable complications and relatively sustained effect.

Impact of postoperative duration of Aspirin use on longevity of bioprosthetic pulmonary valve in patients who underwent congenital heart disease repair

  • Purpose: Generally, aspirin is used as a protective agent against thrombogenic phenomenon after pulmonary valve replacement (PVR) using a bioprosthetic valve. However, the appropriate duration of aspirin use is unclear. We analyzed the impact of postoperative duration of aspirin use on the longevity of bioprosthetic pulmonary valves in patients who underwent repair for congenital heart diseases. Methods: We retrospectively reviewed the clinical data of 137 patients who underwent PVR using a bioprosthetic valve between January 2000 and December 2003. Among these patients, 89 were included in our study and divided into groups I (${\leq}12$ months) and II (>12 months) according to duration of aspirin use. We analyzed echocardiographic data from 9 to 11 years after PVR. Pulmonary vale stenosis and regurgitation were classified as mild, moderate, or severe. Results: The 89 patients consisted of 53 males and 36 females. Their mean age was $14.3{\pm}8.9$ years (range, 2.6-48 years) and body weight was $37.6{\pm}14.7kg$ (range, 14-72 kg). The postoperative duration of aspirin use was $7.3{\pm}2.9$ months in group I and $32.8{\pm}28.4$ months in group II. However, no significant difference in sex ratio, age, body weight, type of bioprosthetic valve, and number of early redo-PVRs. In the comparison of echocardiographic data about 10 years later, no significant difference in pulmonary valve function was found. The overall freedom rate from redo-PVR at 10 years showed no significant difference (P=0.498). Conclusion: Our results indicated no benefit from long-term aspirin medication (>6 months) in patients who underwent PVR with a bioprosthetic valve.

  • Objective : It was well knownth at electro-acupuncture(EA) of ST36 in the hindimb affected a gastrointestinal disease. This study was designed to compare the factors affected EA`s effect such as stimulation duration, operating time and inserted needle`s length. Meyhods and Results : The lightly anaesthetized rat was used and serum gastrin level was observed in this study. Standard stimulation of ST36 consisted of a 5 mA at 2 Hz for a duration of 30 min, applied at 5 times the threshold to evoke muscle twitch. In the study of comparing stimulation duration, 60 min of EA produced no effect, whereas both of 10 and 30 min of EA increased gastrin level. When operating time was altered, both of 5 and 10 days only produced effects of increasing gastrin level. When comparing the depth of inserting needle, stimulation of the outer layer increased gastrin serum level as well as stimulation of full length. Conclusion : These data suggest that the difference of factors affected EA`s effect such as stimulation duration, operating time and inserted needle`s length produced difference effects.

Current Trends in Diagnosis and Management of Laryngopharyngeal Reflux Disease; A Survey of Korean Otolaryngologists (인후두역류질환의 진단 및 치료 경향; 이비인후과 전문의 설문조사 결과)

  • Background and Objectives: Although laryngopharyngeal reflux disease (LPRD) is a widely recognized disease in the field of otolaryngology, controversies still exist regarding the diagnosis and treatment for it. The purpose of this survey was to find out current trends in diagnosis and management of LPRD among Korean otolaryngologists. Materials and Method: Questionnaires were obtained from 26 Korean otolaryngology specialists by electronic balloting, and 46 members of the Korean Society of Head and Neck Surgery (KSHNS) bye-mail. Survey participants were asked about their preferences in diagnostic policy, duration/dosage of antireflux treatment, and their familiarity to the reflux symptom index (RST) and the reflux finding scores (RFS). Results: The majority of the survey participants (64%) preferred empiric therapy trial with proton pump inhibitors (PPIs) based on patients' symptoms and their laryngeal findings. PPIs alone (27%) or PPIs with prokinetics (32%) were usually prescribed as a first line treatment mostly with once daily schedule in the morning before meal and for less than 4 months of duration (86%). More than 90% of participants were aware of the RSI and RFS, but less than 50% of them were actually using those items in the clinics. Conclusion: Most of Korean otolaryngologists who participated in this survey answered that they prefer starting empirical treatments with proton pump inhibitors mainly based on the patient's symptoms and their laryngeal findings.

The relationships among birth season, sunlight exposure during infancy, and allergic disease

  • Purpose: The recent increase in the prevalence of allergic diseases is hypothetically attributed to immune dysregulation in turn caused by a reduction in exposure to sunlight. We explored relationships between birth season, sunlight exposure, exercise duration, and an allergic disease. Methods: We performed a questionnaire-based survey on allergic diseases among elementary school students. Birth time was categorized according to the season (summer and winter). Results: The prevalence of atopic dermatitis (AD) "symptoms ever" was higher in the children born in winter than in those born in summer (adjusted odds ratio [aOR], 1.24; 95% confidence interval [CI], 1.03-1.49; P=0.024). Birth in winter was associated with an increase in the "symptoms in the past 12 months" prevalence of food allergy (FA) (aOR, 1.56; 95% CI, 1.09-2.24; P=0.015). The lifetime prevalence of allergic diseases except FA was higher in the children whose parents considered their sunlight exposure prior to 24 months of ageas inadequate than those who considered their exposure as adequate ("diagnosis ever" asthma: aOR, 1.4; 95% CI, 1.17-1.67; P<0.001; allergic rhinitis [AR]: aOR, 1.4; 95% CI, 1.17-1.67; P<0.001; AD: aOR, 1.26; 95% CI, 1.06-1.51; P=0.01). Neither recent sunlight exposure nor exercise duration was associated with the prevalence of an allergic disease. Conclusion: Birth in winter may be associated with development of AD and FA. Inadequate sunlight exposure before the age of 24 months might possibly increase the risks of development of asthma, AR, and AD.
