A study on the marginal fit of CAD/CAM 3-unit bridges (CAD/CAM 3-unit bridges의 변연 적합도에 관한 연구)

  • The purpose of this study was to assess the marginal fit of three-unit bridges produced using LAVA CAD/CAM (computer-aided design/computer-aided manufacturing) system and conventional PFG in vitro. Materials and methods: #11, 13 resin teeth were prepared on dentiform, then duplicated. Twenty resin models were fabricated, ten for PFG 3-unit bridges and ten for LAVA 3-unit bridges. Each bridge was cemented on the resin model. Marginal discrepancy was measured with stereoscopic microscope (Nikon DS-Fi 1, Nikon, Japan) at a magnification of ${\times}75$. Independent t-test was done for the statistical analysis. Results: The mean marginal discrepancy values and standard deviations of the PFG bridges was $97.1{\pm}18.7\;{\mu}m$ for incisors, $76.6{\pm}21.8\;{\mu}m$ for canines; that of the LAVA bridges was $90.4{\pm}26.7\;{\mu}m$ for incisor, $110.2{\pm}30.2\;{\mu}m$ for canines. The mean marginal discrepancy between PFG and LAVA for incisor did not show significant difference (P<.05). But for canine, the mean marginal discrepancy of PFG bridges was smaller than that of LAVA bridges (P<.05). Conclusion: The LAVA CAD/CAM 3-unit bridges and the PFG 3-unit bridges showed clinically acceptable marginal discrepancy.

A Renal Size Discrepancy among the Findings of Renal Sonogram in Children with Their First Episode of Pyelonephritis is One of the Useful Parameters to Predict the Presence of Cortical Defects on the Acute DMSA Renal Scan

  • Purpose: We investigated whether a renal size discrepancy on a renal sonogram (US) in children with febrile urinary tract infection (UTI) was correlated with the presence of cortical defects on their dimercaptosuccinic acid (DMSA) renal scan. Methods: We examined 911 children who were admitted consecutively to our hospital with their first episode of febrile UTI from March 2001 to September 2014. All enrolled children underwent a US and DMSA scan during admission. According to the US findings, including the renal size discrepancy, data were compared between children with positive and negative DMSA scan results. A positive DMSA scan result was defined as reduced or absent tracer localization and indistinct margins that did not deform the renal contour. Results: Mean renal lengths of the right and left kidneys were larger in children with positive DMSA scan results than in children with negative DMSA scan results ($63.2{\pm}11.3mm$ vs. $58.4{\pm}7.8mm$, P<0.001; $64.9{\pm}11.2mm$ vs. $59.9{\pm}7.9mm$, P<0.001; respectively). A significant difference was observed in both renal lengths between children with positive and negative DMSA scan results ($4.6{\pm}3.8mm$ vs. $3.3{\pm}2.6mm$, P<0.001). A multiple logistic regression analysis, revealed that a small kidney, cortical thinning, and a renal length discrepancy on US findings were significant factors for predicting the presence of cortical defects on an acute DMSA scan [P=0.028, 95% confidence interval (CI) 1.054-2.547; P= 0.004, 95% CI 1.354- 4.810; P<0.001, 95% CI 1.077-1.190, respectively]. Conclusion: In conclusion, a renal size discrepancy on US findings in children with their first episode of febrile UTI was a helpful tool for predicting the presence of cortical defects on an acute DMSA scan.

The Relationship between External Contingencies of Self-Worth and Materialism of Undergraduates: Self-Discrepancy and the Stability of Self-Esteem as Mediators (대학생의 외적 자기가치 수반성과 물질주의의 관계: 자기 불일치, 자존감 안정성의 매개효과)

  • The present study aimed to emprically verify the materialistic pursuits process of undergraduates by applying the escape theory of materialism(Donnelly et al., 2016). Among the six steps of Donnelly's model, the first step is percieving that the fall short of high standards of life, as measured by external contingencies of self-worth. The second step is blaming the self for falling short of standards, as measured by self-discrepancy. The third step of high aversive self-awareness is measured by low stability of self-esteem. A total of 465(203 male and 262 female) college students participated in the online survey and the model was tested using structural equation model. We found that the relationship of external contingencies of self-worth and materialism was mediated by self-discrepancy and there was a sequential mediation effect of self-discrepancy and the stability of self-esteem. The direct effect of external contingencies of self-worth on materialism was also significant. This study is meaningful in that it used the escape theory to emprically verify the materialistic pursuit process of Korean undergraduates living in a meritocratic society.

Discrepancy between Parent and Child Report on Quality of Life and Behavioral Problems in Child and Adolescent cancer survivors and Healthy Control Group (소아암 생존자 집단과 건강통제 집단의 삶의 질과 문제행동 비교 및 부모보고와 자기보고의 차이 연구)

  • Since the introductionof modern therapies, more children with cancer have survived their illness. As a result, an interest in the degree of adjustment achieved by cancer survivors has increased. In this study, quality of life and behavior problemsof child and adolescent cancer survivors were compared with those of healthy controls. Also, the patternsof the discrepancy between parent-report and self-report were compared. Childhood cancer survivors aged 8-18 and their mothers were participated in the study. Parent and self report versions of PedQL were administered to assess the quality of life. K-CBCL and K-YSR were further completed to assess internalizing and externalizing behavior problem. Results suggested that cancer survivors generally showed lower quality of life in physical and social domains and more internalizing behavior problem than healthy control group. However, the results were affected by the domain, age group, and informant. The pattern of discrepancy between parent and self report was similar. All parents reported higher levelsof quality of life and lower levels of behavior problems compared to the self reports of their siblings. In assessing internalizing problem behaviors, cancer survivor group showed greater discrepancy than the healthy control group.The implications, limitations, and directions for future research were also discussed.

The Role of Relational Agency in a Need-reality Colliding Situation (욕구-현실 충돌 상황에서의 주체성의 역할)

  • The goal of this study was to explain the phenomenon of making efforts to overcome the need-reality collision as a cultural characteristic of Koreans. Specifically, we examined whether the behavior varies depending on the degree of relational agency in the situation where conflicts between one's needs and reality have occurred. To this end, a total of 217 participants participated in the online experiment, and the data of 156 participants were finally analyzed. After responding to the relational agency scale, the participants were exposed to a decision-making scenario in which conflicting factors existed. The scenario were about buying a house and making a wedding hall contract, and in each scenario, two important values were set to conflict with each other in the market. Participants read the scenario and entered the level they wanted for each value. After that, they encounter a situation in which he or she has not found the candidate site corresponding to the level he or she wants. Then, the participants responded to their willingness to make additional efforts themselves. As a result of the study, the degree of relational agency of the participants showed a positive relationship with the degree of additional effort. In addition, the degree of the desired level beyond the reality (expectancy discrepancy) showed a nonlinear (reverse U-shape) influence on the additional effort while controlling for individual difference. Furthermore, the interaction effect between relational agency and expectancy discrepancy was significant. Specifically, individuals with low agency did not have a significant relationship between the degree of expectancy discrepancy and the dependent variable, but individuals with high relational agency had a significant non-linear relationship between the degree of expectancy discrepancy and the dependent variable. Based on the results of the study, the role and function of Koreans' psychological characteristics (relational agency) in the scene of managing needs-reality collision were discussed.

Analysis of the Rate of Discrepancy between Preliminary Reports by Radiology Residents and Final Reports by Certified Radiologists for Emergency Radiology: Studies in a University Hospital (대학병원 응급 영상검사에서 영상의학과 전공의 가판독과 전문의 최종 판독 간의 불일치 발생률 분석)

  • Purpose In the adult emergency department of a university hospital, we investigated the frequency of major discrepancies between the preliminary reports by radiology residents and the final reports by certified radiologists. Materials and Methods Based on CT and MRI scans obtained between December 2016 and November 2019, we selected cases with diagnoses or treatment plans that could be changed due to discrepancies between preliminary and final reports and classified them by the type of discrepancy. We also examined the distributions of the major discrepancies and stratified them by residents' working time zone, experience, and subspecialty. Results Based on the 72137 preliminary reports evaluated, 1348 tests (1.9%) showed major discrepancies. Most of the major discrepancies were false negatives (72.0%), followed by misdiagnosis (26.3%) and false positives (1.7%). Acute findings (87.2%) were more common than non-acute findings (12.8%). The major discrepancy rate increased toward the second half of the 24-hour shift, with the highest rate of 2.9% occurring between 2 am and 4 am. The major discrepancy rate did not vary with experience, and it varied from 0.6% to 4.5% for each subspecialty. Conclusion The major discrepancy rate was less than 2%, and it increased with longer working hours during a 24-hour shift.

  • An overall reduction in the horizontal and vertical dimensions occurs following tooth extraction and the resorption of the buccal part of the ridge is more pronounced than the lingual part. Thus, the resulting morphology of the healed alveolar ridge is often presenting with a discrepancy in bone height between the buccal and lingual aspects of the ridge. The implant with a sloped marginal configuration that is designed to match the sloped contour of the alveolar ridge provides the opportunity to maintain the buccal-lingual bone discrepancy and soft tissue around the implant. This paper introduces the OsseoSpeed TX Profile implants with sloped marginal configuration and explains the characteristics and clinical meanings of those implants.

  • Oligodontia is defined as a congenital tooth agenesis with the absence of six or more permanent teeth. This clinical report describes a patient with non-syndromic partial oligodontia, with retained deciduous teeth and the absence of 16 permanent teeth. Anterior esthetic problems were caused by interarch tooth size discrepancy, interdental space, aberrant tooth dimensions, and the absence of centric contacts of the anterior teeth. Prosthetic restoration after orthodontic and implant treatment was performed with a multi-disciplinary team approach. Favorable functional and esthetic results were obtained using a definitive prosthesis.

  • A model selection approach is used to find out whether the mean and the variance of a unique sample are different from the pre-specified values. Normal distribution is selected as an approximating model. Kullback-Leibler discrepancy comes out as a natural measure of discrepancy between the operating model and the approximating model. Several estimates of selection criterion are computed including AIC, TIC, and a coupleof bootstrap estimator of the selection criterion are considered according to the way of resampling. It is shown that a closed form expression is available for the parametric bootstrap estimated cirterion. A Monte Carlo study is provided to give a formal comparison when the operating family itself is normally distributed.
