Molecular Markers for Sulfadoxine/Pyrimethamine and Chloroquine Resistance in Plasmodium falciparum in Thailand

  • Drug resistance is an important problem hindering malaria elimination in tropical areas. Point mutations in Plasmodium falciparum dihydrofolate reductase (Pfdhfr) and dihydropteroate synthase (Pfdhps) genes confer resistance to antifolate drug, sulfadoxine-pyrimethamine (SP) while P. falciparum chloroquine-resistant transporter (Pfcrt) genes caused resistance to chloroquine (CQ). Decline in Pfdhfr/Pfdhps and Pfcrt mutations after withdrawal of SP and CQ has been reported. The aim of present study was to investigate the prevalence of Pfdhfr, Pfdhps, and Pfcrt mutation from 2 endemic areas of Thailand. All of 200 blood samples collected from western area (Thai-Myanmar) and southern area (Thai-Malaysian) contained multiple mutations in Pfdhfr and Pfdhps genes. The most prevalent haplotypes for Pfdhfr and Pfdhps were quadruple and double mutations, respectively. The quadruple and triple mutations of Pfdhfr and Pfdhps were common in western samples, whereas low frequency of triple and double mutations was found in southern samples, respectively. The Pfcrt 76T mutation was present in all samples examined. Malaria isolated from 2 different endemic regions of Thailand had high mutation rates in the Pfdhfr, Pfdhps, and Pfcrt genes. These findings highlighted the fixation of mutant alleles causing resistance of SP and CQ in this area. It is necessary to monitor the re-emergence of SP and CQ sensitive parasites in this area.

Genetic Polymorphisms in Plasmodium vivax Dihydrofolate Reductase and Dihydropteroate Synthase in Isolates from the Philippines, Bangladesh, and Nepal

  • Genetic polymorphisms of pvdhfr and pvdhps genes of Plasmodium vivax were investigated in 83 blood samples collected from patients in the Philippines, Bangladesh, and Nepal. The SNP-haplotypes of the pvdhfr gene at the amino acid positions 13, 33, 57, 58, 61, 117, and 173, and that of the pvdhps gene at the positions 383 and 553 were analyzed by nested PCR-RFLP. Results suggest diverse polymorphic patterns of pvdhfr alone as well as the combination patterns with pvdhps mutant alleles in P. vivax isolates collected from the 3 endemic countries in Asia. All samples carried mutant combination alleles of pvdhfr and pvdhps. The most prevalent combination alleles found in samples from the Philippines and Bangladesh were triple mutant pvdhfr combined with single mutant pvdhps allele and triple mutant pvdhfr combined with double wild-type pvdhps alleles, respectively. Those collected from Nepal were quadruple mutant pvdhfr combined with double wild-type pvdhps alleles. New alternative antifolate drugs which are effective against sulfadoxine-pyrimethamine (SP)-resistant P. vivax are required.

국내 수도용 제초제의 작용기작별 분류 (Classification According to Site of Action of Paddy Herbicides Registered in Korea)

  • 국내에 등록된 제초제의 효율적 사용을 위해서 제초제저항성관리위원회에서 제시한 제초제 작용기작별 분류를 기초로 23그룹으로 분류하였다. 세부그룹으로는 acetyl CoA carboxylase 억제제, acetolactate synthase 억제제, photosystem과 억제제, protoporphyrinogen oxidase 억제제, carotenoid biosynthesis 억제제, enolpyruvyl shikimate-3-phosphate synthase 억제제, glutamine synthetase 억제제, dihydropteroate synthetase 억제제, 세포분열 저해제(mitosis inhibitors), cellulose 생합성억제제, oxidative phosphorylation uncouplers, 지방산 및 지질생합성 억제제, synthetic auxins, auxin transport 억제제, potential nucleic acid Inhibitors 혹은 non-descript mode of action으로 나누었다. 이와 같은 그룹화 기준을 토대로 국내 등록되어 사용중인 논 제초제 성분들을 그룹화 하였다. 따라서, 이러한 약제 작용기작 관련 정보를 농약사용자에게 제공함으로써 특정약제의 연용과 중복사용을 방지하여 국내에서 제초제 저항성잡초 발생 문제를 줄일 수 있을 것으로 기대된다.

돼지유래 Salmonella Schwarzengrund의 약제내성 유전자에 관한 연구 (Investigation on antimicrobial resistance genes of Salmonella Schwarzengrund isolated from pigs)

  • To detect the virulence genes (invA and spvC) and antimicrobial resistance genes, polymerase chain reaction (PCR) was carried out using total 67 strains of S. Schwarzengrund isolated from pigs. As results, invA was detected from all 67 strains of S. Schwarzengrund, however, spvC was not at all. All 12 strains with ampicillin resistance, 15 strains with chloramphenicol resistance, 9 strains with kanamycin resistance, 1 strain with sulfamethoxazole/trimethoprim resistance, and 66 (98.5%) of 67 strains with tetracycline resistance carried TEM (${\beta}$-lactamase $bla_{TEM}$), cmlA (nonenzymatic chloramphenicol resistance), aphA1-Iab (aminoglycoside phosphotransferase), sulII (dihydropteroate synthase), and tetA (class A tetracycline resistance), respectively. All 63 strains with streptomycin resistance carried 3 aminoglycoside resistance genes, including aadA (aminoglycoside adenyltransferase), strA, and strB (streptomycin phosphotransferase). With respect to prevalence of antibiotic resistance genes occurred in S. Schwarzengrund, genes for strB (46.0%); strA and strB (30.2%); aadA, strA, and strB (9.5%); strA (7.9%); aadA and strB (3.2%); and aadA (3.2%) were detected by PCR.

A Case of Pneumonia Caused by Pneumocystis jirovecii Resistant to Trimethoprim-Sulfamethoxazole

  • A 50-year-old male visited the outpatient clinic and complained of fever, poor oral intake, and weight loss. A chest X-ray demonstrated streaky and fibrotic lesions in both lungs, and chest CT revealed multifocal peribronchial patchy ground-glass opacities with septated cystic lesions in both lungs. Cell counts in the bronchoalveolar lavage fluid revealed lymphocyte-dominant leukocytosis, and further analysis of lymphocyte subsets showed a predominance of cytotoxic T cells and few T helper cells. Video-assisted wedge resection of the left upper lobe was performed, and the histologic examination was indicative of a Pneumocystis jirovecii infection. Trimethoprim-sulfamethoxazole (TMP-SMX) was orally administered for 3 weeks; however, the patient complained of cough, and the pneumonia was aggravated in the follow-up chest X-ray and chest CT. Molecular studies demonstrated mutations at codons 55 and 57 of the dihydropteroate synthase (DHPS) gene, which is associated with the resistance to TMP-SMX. Clindamycin-primaquine was subsequently administered for 3 weeks replacing the TMP-SMX. A follow-up chest X-ray showed that the pneumonia was resolving, and the cough was also alleviated. A positive result of HIV immunoassay and elevated titer of HCV RNA indicated HIV infection as an underlying condition. This case highlights the importance of careful monitoring of patients with P. jirovecii pneumonia (PCP) during the course of treatment, and the molecular study of DHPS mutations. Additionally, altering the anti-PCP drug utilized as treatment must be considered when infection with drug-resistant P. jirovecii is suspected. To the best of our knowledge, this is the first case of TMP-SMX-resistant PCP described in Korea.
