디지털영상의 치근이개부 골손실 판독효과에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE READABILITY OF THE DIGITAL IMAGES IN THE FURCAL BONE DEFECTS)

  • The aim of this study was to evaluate and compare observer performance between conventional radiographs and their digitized images for the detection of bone loss in the bifurcation of mandiblar first molar. One dried human mandible with minimal periodontal bone loss around the first molar was selected and serially enlarged 17 step defects were prepared in the bifurcation area. The mandible was radiographed with exposure time of 0.12, 0.20, 0.25, 0.32, 0.40, 0.64 seconds, after each successive step in the preperation and all radiographs were digitized with IBM-PC/32 bit-Dx compatible, video camera (VM-S8200, Hitachi Co., Japan), and color monitor(Multisync 3D, NEC, Japan). Sylvia Image Capture Board for the ADC(analog to digital converter) was used. The obtained results were as follows: 1. In the conventional radiographs, the mean score of the readability was higher at the condition of exposure time with 0.32 second. Also, as the size of artificial lesion was increased, the readability of radiographs was elevated (P<0.05). 2. In the digital images, the mean score of the readability was higher at the condition of exposure time with 0.40 second. Also, as the size of artificial lesion was increased, the readability of digital images was elevated(P<0.05). 3. At the same exposure time, the mean scores of readibility were mostly higher in the digitized images. As the exposure time was increased, the digital images were superior to radiographs in readability. 4. As the size of lesion was changed, the digital images were superior to radiographs in detecting small lesion. 5. The coefficient of variation of mean score has no significant difference between digital images and radiographs.

  • Recently, the digital image was introduced into radiological image. The digital image has the power of contrast enhancement, histogram control, and other digitally enhancement. At the point of the resolution, periapical radiograph is superior to the digital image, but enhanced digital procedure improves the diagnostic ability of the digital image. The purpose of this study was to evaluate the diagnostic ability of artificial proximal caries in conventional radiographs, digital radiographs and enhanced digital radiographs (histogram specification). ROC (Receiver Operating Characteristic) analysis and paired t-test were used for the evaluation of detectability, and following results were acquired: 1. The mean ROC area of conventional radiographs was 0.9274. 2. The mean ROC area of unenhanced digital image was 0.9168. 3. The mean ROC area of enhanced digital image was 0.9339. 4. The diagnostic ability of three imaging methods was not significant difference(p>0.05). So, the digital images had similar diagnostic ability of artificial proximal caries to conventional radiographs. If properly enhanced digital image, it may be superior to conventional radiographs.

  • The aim of this study was to evaluate clinician's detectability in the diagnosis of bone loss in the bifurcation of mandibular molars on periapical radiographs and Digital images. Periapical radiographs were obtained of the first molars in 2 dry mandibles after preparation of bony defects corresponding to degree I, degree II and degree III buccal furcation involvements. The radiographs were randomly presented to 39 clinicians(1 oral radiologist, 4 periodontist, 34 general dentists) who were asked to determine the presence or absence of bone loss. Periapical films were digitized with a TV camera. Digital images were assessed by 15 clinicians(1 oral radiologist, 4 periodontist, 10 general dentists). I. the overall diagnostic accuracy of Digital images for detection of bone loss in the bifurcation of mandibular molars was higher than that of the periapical radiographs. 2. the largest increase in diagnostic accuracy was found between lesion grade II and III on both radiographs and Digital images(P<0.05). 3. there was no significant difference between the standard state and the controlled contrast state on Digital images. 4. the overall diagnostic accuracy of I radiologist and 4 periodontists was better than that of the general dentists for detecting bifurcation involvements.

모의 인접면 치아우식병소의 진단을 위한 구내 표준방사선사진과 그 디지털 영상의 비교 (A COMPARISON OF PERIAPICAL RADIOGRAPHS AND THEIR DIGITAL IMAGES FOR THE DETECTION OF SIMULATED INTERPROXIMAL CARIOUS LESIONS)

  • The purpose of this study was to compare the diagnostic accuracy of periapical radiographs and their digitized images for the detection of simulated interproximal carious lesions. A total of 240 interproximal surfaces was used in this study. The case sample was composed of 80 anterior teeth, 80 bicuspids and 80 molars which were prepared in order to distribute the surfaces from carious free to those containing simulated carious lesions of varying depths (0.5㎜, 0.8㎜, and 1.2㎜). The periapical radiographs were taken by paralleling technique and film used was Kodak Ektaspeed(E group). All radiographs were evaluated by five dentist to recognize the true status of simulated carious lesion. They were asked to give a score of 0, 1, 2, or 3. Digitized images were obtained using a commercial video processor(FOTOVIX Ⅱ- XS). And the computer system was 486 DX PC with PC Vision and frame grabber. The 17' display monitor had a resolution of 1280×1024 pixels(0.26㎜ dot pitch). But the one frame of the intraoral radiograph has a resolution of 700×480 pixels and each pixel has a grey level value of 256. All the radiographs and digital images were viewed under uniform subdued lighting in the same reading room. After a week the second interpretation was performed in the same condition. The detection of lesions on the monitor was compared with the finding of simulated interproximal carious lesions on the film images. The results were as follows: 1. When the scoring criteria was dichotomous ; lesion present and not present 1) The overall sensitivity, specificity and diagnostic accuracy of periapical radiographs and their digital images showed no statistically significant difference. 2) The sensitivity and specificity according to the region of teeth and the grade of lesions showed no statistically significant difference between periapical radiographs and their digital images. 2. When estimate the grade of lesions ; score 0, 1, 2, 3 1) The overall diagnostic accuracy was 53.3% on the intraoral films and 52.9% on digital images. There was no significant difference. 2) The diagnostic accuracy according to the region of teeth showed no statistically significant difference between periapical radiographs and their digital images. 3. The degree of agreement and reliability 1) Using gamma value to show the degree of agreement, there was similarity between periapical films and digital images. 2) The reliability of each twice interpretation of periapical films and digital images showed no statistically significant difference. In all cases P value was greater than 0.05, showing that both techniques can be used to detect the incipient and moderate interproximal carious lesions with similar accuracy.

  • This investigation was performed to test the readability of the video based digital radiography, that can be applied clinically, compared with the periapical radiograph. The experiments were performed with IBM-PC/AT compatible, video camera and ADC (analog-digital converter). And spatial resolution was 512 X 480 with 256 (8 bit) gray levels. The radiographs obtained by using variable steps of exposure time were digitized. and then the digital images were analyzed. The obtained results were as follows: 1. There was no remarkable difference in readability between the radiographs and their digital images. However, under over exposure the digital images were superior to the radiographs in readability and vice versa. 2. As the exposure time was increased, the gray level of the digital image was decreased proportionally. 3. The correlation beween the regions of interest and the aluminum step wedges were relatively close; R=0.9965 (p <0.001).

Residual bone height measured by panoramic radiography in older edentulous Korean patients

  • PURPOSE. The aim of this study was to obtain statistical data on the residual bone height at different natural tooth positions by panoramic radiography in edentulous Korean patients aged 60-90 years. MATERIALS AND METHODS. The study included the diagnostic panoramic radiographs of 180 randomly selected edentulous patients without systemic diseases affecting bone. The radiographic selection criteria included absence of obvious facial asymmetry, clearly visible anatomic structures, and no surgical and fracture history. The panoramic radiographs of 79 patients met these criteria and were used in the analysis. The same researcher processed all the radiographs by using a standardized method. The height of the residual bone was measured at 18 predetermined sites (7 in the maxilla and 11 in the mandible) on digitized and printed radiographs by using a Digimatic caliper, triangle, and ruler. Gender- and age-related differences were statistically analyzed by using the t-test and rank-sum test (${\alpha}=0.05$). RESULTS. The maxillary residual bone height did not show significant gender-related differences, but male patients had significantly higher residual bone in the mandible(P<.05). No significant height differences at the measured sites were noted among the 60s, 70s, and 80s age groups. CONCLUSION. Dentists should pay greater attention to older female edentulous patients because they are more prone to rapid residual bone resorption. Residual bone resorption may not be affected by age.

  • The purpose of the present study was to use digital profile image features and digital image analysis of fixed-dimension bone regions, extracted from standardized periapical radiographs of the maxilla, to determine whether differences exist in alveolar bone of younger women(mean age: 59.23±7.34 years) and just menopaused women(mean age: 59.23±7.34). Periapical films were used from two groups of 20 randomly selected women. None of the subjects had a remarkable medical history. To simplify protocol, we chose one interproximal bone area between the maxillary right canine and lateral incisor for study. Ech film was digitized into a 1312 x 1024 pixel x 8 bit depth matrix by means of a Nikon 35 mm film scanner(LS-35lOAF, Japan) with fixed gain and internal dark current correction to maintain constant illumination. The scanner was interfaced to a Macintosh LC III computer(Apple Computer, Charlotte, N.C.). Area and profile orientation were selected with a NIMH Image 1.37(NIH Research Services Branch, Bethesda, Md.). Histogram features were extracted from each profile and area. The results of this study indicate that mean pixel intensities didn't differ significantly between two groups and there was a high correlarion-coefficient between digitized radiographic profile features and area features.

프랙탈 분석을 통한 임플란트 주변골 평가 (Evaluation of peri-implant bone using fractal analysis)

  • Purpose : The purpose of this study was to investigate whether the fractal dimension of successive panoramic radiographs of bone after implant placement is useful in the characterization of structural change in alveolar bone. Materials and Methods. Twelve subjects with thirty-five implants were retrospectively followed-up from one week to six months after implantation. Thirty-six panoramic radiographs from twelve patients were classified into 1 week, 1-2 months and 3-6 months after implantation and digitized. The windows of bone apical and mesial or distal to the implant were defined as periapical region of interest (ROI) and interdental ROI; the fractal dimension of the image was calculated. Results There was not a statistically significant difference in fractal dimensions during the period up to 6 months after implantation. The fractal dimensions were higher in 13 and 15mm than 10 and 11.5mm implant length at interdental ROIs in 3-6 months after implantation (P<0.01). Conclusion : Longer fixtures showed the higher fractal dimension of bone around implant. This investigation needs further exploration with large numbers of implants for longer follow-up periods.

Automatic detection of the lung orientation in digital PA chest radiographs

  • An image processing algorithm is presented that can identify the orientation as well as the left/right side (parity) of the digitized radiographs. The orientation was found by computing the mean square deviation between the sampled gray values along the center and their best-fit linear regression relations. The parity was determined by comparing the area difference between two thresholded images of the left and the right side around the heart, which is assumed to be around the center of the image. This method was tested with 86 images with their orientations intentionally rotated. The rotation was limited to multiples of 90 degrees, as this was the way the rotation is most likely to happen in the clinical environment. We obtained positive responses for 85 out of 86 images subject to the screening.

A reliable method for evaluating upper molar distalization: Superimposition of three-dimensional digital models

  • Objective: The aim of this study was to evaluate the reliability of measurements obtained after the superimposition of three-dimensional (3D) digital models by comparing them with those obtained from lateral cephalometric radiographs and photocopies of plaster models for the evaluation of upper molar distalization. Methods: Data were collected from plaster models and lateral cephalometric radiographs of 20 Class II patients whose maxillary first molars were distalized with an intraoral distalizer. The posterior movements of the maxillary first molars were evaluated using lateral cephalometric radiographs (group CP), photocopies of plaster models (group PH), and digitized 3D models (group TD). Additionally, distalization and expansion of the other teeth and the degrees of molar rotation were measured in group PH and group TD and compared between the two groups. Results: No significant difference was observed regarding the amount of molar distalization among the three groups. A comparison of the aforementioned parameters between group PH and group TD did not reveal any significant difference. Conclusions: 3D digital models are reliable to assess the results of upper molar distalization and can be considered a valid alternative to conventional measurement methods.
