Relationship between diet and prevalence of depression among Korean adults: Korea National Health and Nutrition Examination Survey 2010 (한국 성인에서 식이섭취와 우울증 유병의 관련성: 2010년 국민건강영양조사 자료를 이용하여)

  • Background: Many studies focus on the causes of depression, but few assess the dietary intake. This study investigated the relationship between diet and prevalence of depression. Methods: This study used the data from the Korea National Health and Nutrition Examination Survey 2010. A total of 734 persons aged 20-65 years old in whom the presence or absence of depression was confirmed by doctor were analyzed. We analyzed associations between prevalence of depression and diet based on carbohydrate, fiber, calcium, iron, sodium, potassium, vitamin A, thiamine, riboflavin, niacin and vitamin C intakes for male and female groups. Results: There was no association between diet and prevalence of depression in the male group. For the female group, as fiber intake was increased, the prevalence of depression significantly decreased (odds ratio=0.923, 95% confidence interval=0.850-0.986), but there was no association between the other dietary intakes and prevalence of depression. Conclusion: The fiber intake and prevalence of depression are negatively correlated in women aged 20-65 years. A diet therapy including fiber may decrease the prevalence of depression.

Evaluation of Nutritional Deficit Status in Hemodialysis Patients' Based on Usual Dietary Intake, Anthropometric and Biochemical Parameters (일상식이섭취상태와 신체계측 및 혈액화학적 영양지표에 의한 혈액투석 환자의 영양결핍상태 평가)

  • Long-term hemodialysis(HD) patients manifest various signs of protein and caloric malutrition due to poor intake of nutrients and other causes. Poor nutritional status increases the mortality and morbidity rates in HD patients. Thus, mataintnance of adequate nutritional status has been a major task in taking care of patients receiving HD. This study was to evaluate the nutritional status of HD patients and to clarify the degree of nutritional deficit based on usual dietary intake, anthropometric and biochemical indicators. Sixty HD patients comprised a HD group, while the control group consisted of 60 healthy adults whose age and sex matched those of the HD group. Nutritional status was evaluated by dietrary intake using instant nutritional scale, anthropometric measures, serum protein concentrations and the number of lymphocytes. The data were analyzed by using Chi-square test and unpaired t-test. The results are as follows. 1. Regarding usual dietary intake of HD group. 1) Estimated caloric intake was significantly lower than the recommended daily allowance(RDA) and among them, 35% were taking calories less than 85% of the RDA. 2) Estimated protein intake was significantly higher than the RDA and among them 40% were taking protein more than 115% of the RDA. 3) Estimated fat intake was lower than the RDA. 4) Vitamin A, B, $B_1,\;B_2$, C and niacin in take was lower than the RDA respectively. 5) Estimated ferrous intake was within the normal limit the RDA while estimated calcium intake was higher than the RDA. 6) Both calorie and protein intake were higher for the 10 patients who had been under continuous ambulatory peritoneal dialysis than for the patients under HD from the beginning. 2. Regarding anthropometric measures : 1) Body mass index(BMI), midarm circumference(MAC), and triceps skinfold thickness(TSF) were lower in the HD group than in the control group. 2) Among HD group, 47.1% were within the normal limit of BMI, while 86.7% were within the same limit in the control group. 3) Among HD group, 35.0% were within the normal limit of MAC, while 83.3% were within the same limit in the control group. 4) Among HD group, only 8.3% were normal, 30.3% were mild deficit status of TSF, while 50% were normal and 48.3% were mild deficit status in the control group. 3. Regarding biochemical laboratory tests 1) Albumin, transferrin concentrations and the number of lymphocytes were lower in HD group than in the control group. 2) Among HD group, 98.3% were within the normal limit of albumin concentration and all were within the same limit in the control group. 3) Among HD group, only 11.7% were within the normal limit of transferrin concentration, while 81.7% were within the same limit in the control group. 4) Among HD group, 25% were within the normal limit, while 93.3% were within the same limit in the control group. The above findings suggest that HD patients were in nutritional deficit status. Adequate diet therapy and periodical evaluation of the nutritional status in HD patients are needed. Accordingly, it turned out that anthropometric measures were very reliable parameters and easy to use to evaluate nutritional status. So nurses are encouraged to adopt anthropometric measures to examine nutritional deficit status of HD patients.

Evaluation and Categorization of Commercially Prepared Enteral Nutrition Formulas

  • In order to investigate the types of enteral nutrition formulas currently used in hospitals and evaluate and categorize the commercially prepared enteral nutrition formulas formulas available in the domestic market, we asked dietitians working in 6 hospitals in Seoul to complete the questionnaire and obtained compositional characteristics of 12 commercially prepared enteral nutrition formulas. The average proportion of patients receiving the commercially prepared enteral nutrition formulas(60.6%) was greater than that of patients receiving the in-hospital preparations(31.9%). In the group of patients receiving the in-hospital prepared formulas, the enteral feeding was mainly administered orally, whereas, in the group of patients receiving the commercially prepared formulas, tube feeding was the primary route of formula administration. In both groups, however, a greater proportion of patients received the formulas as total replacements of their meals and for the purpose of dietary supplementation. On the basis of major criteria for evaluation of the commercially prepared enteral nutrition formulas, the 6 products out of the 9 nutritionally complete products formulated for the purpose of dietary supplementation were grouped into the same category(standard protein, caloric density of 1kcal/ml, and tube/oral), so they were considered therapeutically comparable. However, the remaining 3 products were different in protein content(high protein) or route of administration(tube only). Of the 3 nutritionally complete products formulated specifically for the purpose of dietary therapy, 2 products were formulated for patients with renal disease, and the one product was formulated for diabetic patients. Therefore, the data in this study showed that the commercially prepared enteral nutriton formulas became an important part of the enteral nutrition for hospitalized patients in Korea, but the domestic market has not yet generated a wide variety of the formulas, not providing many choices for clinicians to manage the diets for their patients. The results of this study would be helpful for clinicians in choosing appropriate products for their patients, for manufactures in developing new products, and for regulatory authorities to establish the regulation for the broad group of heterogeneous products that are marketed and will be developed as medical foods. In addition, the process of maintaining the categories for evaluation of the commercially prepared enteral nutrition formulas should be dynamic because new products may not reasonably fit any of the existing categories.

Effects of Dietary Approaches to Stop Hypertension Program on Blood Pressure among the Pre-hypertension Group in the Community (지역사회 주민의 고혈압 예방 식이프로그램의 효과 평가)

  • Objectives: DASH (dietary approaches to stop hypertension) diet are recommended as first-line therapy for prevention of hypertension individuals with high normal blood pressure. We studied the effects of DASH program on blood pressure among the pre-hypertension group. Methods: To determine the impact of dietary patterns on the control of hypertension we studied the subgroup of 141 participants with systolic blood pressure of 120 to 139 mm Hg or diastolic BP of 80 to 89mmHg, body mass index(BMI) ${\geq}25kg/m^3$ or waist to hip ratio(WHR) ${\geq}0.95$(for man). 0.85(for woman) enrolled in DASH program. Participants were received of education and consulting about DASH every week for 8 weeks. Results: The level of diet to prevention of hypertension and compliance of DASH increased after education. Also, The DASH program significantly reduced systolic BP (from $136.03{\pm}12.40mmHg$ to $126.09{\pm}11.25mmHg$, p< .01) and diastolic BP (from $81.80{\pm}6.32mmHg$ to $76.44{\pm}10.61mmHg$, p< .01). Conclusion: The DASH program effectively lowers BP and may be useful in achieving prevention of hypertension.

The Effects of Uncooked Powdered Food on Nutrient Intake, Body Fat and Serum Lipid Compositions in Hyperlipidemic Patients (규칙적인 생식섭취가 고지혈증 환자의 영양소 섭취상태, 체지방 및 혈청의 지질조성에 미치는 영향)

  • This study was designed to investigate the effects of weight reduction and changes in serum lipid composition using a commercial uncooked powdered food (UPF) diet on 27 obese hyperlipidemic women over a period of 12 weeks. We replaced the standard breakfast and dinner of each of the subjects with UPF. Their dietary intake status was evaluated by 24-hour recall method. Their body compositions were measured using a body fat analyzer. Also, we conducted hematological, clinical and lipid profile analyses of their blood. Their intake of energy, lipid and protein significantly decreased as the subjects started to take UPF, but their intake of vitamins and minerals, other than iron, increased. Due to the energy intake decrease, the subjects' weight, body fat and waist circumference significantly decreased. Among the lost weight the percentage of the body fat was high. We judged that this process of losing weight was successful in obese hyperlipidemic women. Serum HDL-cholesterol gradually increased and serum total-cholesterol, LDL-cholesterol, triglyceride levels showed gradual decreases. When obese hyperlipidemic women replaced two of three meals with UPF for 3 months, we were able to see useful changes like decreased body fat and serum lipid. Complete judgment on the usefulness of UPF may be difficult but if people take a standard meal once a day and replenish their iron by taking UPF twice a day, we assume that replacing UPF with balanced regular meal not be a problem in nutritional status. Our results show that UPF is effective in the diet therapy of obese hyperlipidemic women.

Epidemiological and Genome-Wide Association Study of Gastritis or Gastric Ulcer in Korean Populations

  • Gastritis is a major disease that has the potential to grow as gastric cancer. Gastric cancer is a very common cancer, and it is related to a very high mortality rate in Korea. This disease is known to have various reasons, including infection with Helicobacter pylori, dietary habits, tobacco, and alcohol. The incidence rate of gastritis has reported to differ between age, population, and gender. However, unlike other factors, there has been no analysis based on gender. So, we examined the high risk factors of gastritis in each gender in the Korean population by focusing on sex. We performed an analysis of 120 clinical characteristics and genome-wide association studies (GWAS) using 349,184 single-nucleotide polymorphisms from the results of Anseong and Ansan cohort study in the Korea Association Resource (KARE) project. As the result, we could not prove a strong relation with these factors and gastritis or gastric ulcer in the GWAS. However, we confirmed several already-known risk factors and also found some differences of clinical characteristics in each gender using logistic regression. As a result of the logistic regression, a relation with hyperlipidemia, coronary artery disease, myocardial infarction, hyperlipidemia therapy, hypotensive or antihypotensive drug, diastolic blood pressure, and gastritis was seen in males; the results of this study suggest that vascular disease has a potential association with gastritis in males.

Update on Irritable Bowel Syndrome Program of Research

  • Purpose: This article provides an update and overview of a nursing research program focused on understanding the pathophysiology and management of irritable bowel syndrome (IBS). Methods: This review includes English language papers from the United States, Europe, and Asia (e.g., South Korea) from 1999 to 2013. We addressed IBS as a health problem, emerging etiologies, diagnostic and treatment approaches and the importance of a biopsychosocial model. Results: IBS is a chronic, functional gastrointestinal disorder characterized by recurrent episodes of abdominal pain and alterations in bowel habit (diarrhea, constipation, mixed). It is a condition for which adults, particularly women ages 20-45, seek health care services in both the United States and South Korea. Clinically, nurses play key roles in symptom prevention and management including designing and implementing approaches to enhance the patients' self-management strategies. Multiple mechanisms are believed to participate in the development and maintenance of IBS symptoms including autonomic nervous system dysregulation, intestinal inflammation, intestinal dysbiosis, dietary intolerances, alterations in emotion regulation, heightened visceral pain sensitivity, hypothalamic-pituitary-adrenal dysregulation, and dysmotility. Because IBS tends to occur in families, genetic factors may also contribute to the pathophysiology. Patients with IBS often report a number of co-morbid disorders and/or symptoms including poor sleep. Conclusion: The key to planning effective management strategies is to understand the heterogeneity of this disorder. Interventions for IBS include non-pharmacological strategies such as cognitive behavior therapy, relaxation strategies, and exclusion diets.

A Literature Review on Obesity in Postmenopausal Women (폐경기 비만의 임상적 의의에 대한 문헌적 고찰)

  • Objective This study was designed to collect latest papers and to find more effective approach for Postmenopausal Women Methods We searched 85 papers in English from Pubmed(www.ncbi.nlm.nih.gov) and selected 40 in favor of our study. In addition to that, we searched 24 papers in Korean from Kiss(www.kiss.kstudy.com) and Medric(www.medric.or.kr). Results are as follows. Results and conclusion Many studies showed that postmenopausal obesity causes cardiovascular disease, osteoporosis, breast cancer and decrease in immunity. To prevent and treat obesity, many studies suggested that dietary and exercise are most effective methods. If needed, medicine and hormone therapy may be recommended as next steps. But, Herb medication and acupuncture are not used yet as postmenopausal obesity treatment around the world. Postmenopausal obesity is significant, resulting in many medical problems. We hope more effective treatment based on these studies and combined with Korean traditional medicine could be developed after.

Effects of Protein and Protein Hydrolysate on Nitrogen Metabolism in Rats with Cysteamine-induced Duodenal Ulcer (Cysteamine에 의해 유도된 십이지장 궤양 흰쥐의 체내질소대사에 대한 단백질과 단백질 가수분해물의 섭취효과)

  • This study aimed to verify the nutritional and curative effects of protein hydrolysate in rats with cysteamine-induced duodenal uncer. Duodenal ulcer rat model was established by intraperitoneal injections of cysteamine. Sprague-Dawley, female rats weighing approximately 200g were intraperitoneally injected twice cysteamine(13mg/100g BW) at intervals of 3h per day. This procedure was repeated 3$\times$at intervals of 3d. Animals fed on 10% casein diet for infection periods. After last injection, 4 kinds of diets(10% casein, 20% casein, 10% casein hydrolysate, 20% casein hydrolysate) were given. Gastric montility, trypsin activity in gastrointestinal content, retention rate of nitrogen, plasma total protein, albumin, amino-N, urinary urea nitrogen, creatinine and hydroxyproline were analyzed for nutritional effects of dietary nitrogen levels(10%, 20%) and sources(casein, casein hydrolysate). In duodenal ulcer rat model, there was no differences between 20% casein diet and 20% casein hydrolysate in the view of severeness of ulcer, gastric emptying rate, serum total protein, serum albumin, plasma $\alpha$-amino-N, UUN, creatinine excretion, GFR, nitrogen retention. On the other hand, rats on 10% casein hydrolysate diet group had more curative effect of the ulcer, higher plasma albumin concentration and nitrogen retention than 10% casein diet group. The casein hydrolysate diet group was lower trypsin activity in small intestinal content than the casein diet group, at both nitrogen levels(10%, 20%). The results suggest that protein hydrolysate be applied in diet therapy for the patients with gastrointestinal ulcer.

A Study on Dietary Therapy for Patient with Peptic Ulcer (식이요법에 의한 위궤양의 치료 연구)

  • A serious case of the stomach ulcer was cured completely through a dietery treatment. The patient was listed as one of worse cases by the doctors. The patient refused to accept doctors' advises to have al surgical operation and sticked to his idea of dietetics. The doctors openly expressed their scepticism to a successful out come of a dietery treatment. However, it has been a success. A special ulcer diet menu prepared for the patient was based on those principles as follow: 1. Ulcer diet contained high protein (70g/day) and high energy (2,000cal/day). 2. To maintain a minimum degree of pressure on digestive process of the stomach, several small meals more than three times a day were served. Fat was avoided except that contained in milk and small amount of butter on toast. 3. To help to neutralize acid of the stomach, ample amount of milk was served. 4. Irritating food and beverage were prohibited. It was concluded that milk was one of the best diet for a peptic ulcer patient, for it could neutralize acid of the stomach as well as could provide ample amount of high protein which expedite healing craters formed by the stomach or duodenal ulcer. It was reported that most of unsucessful peptic ulcer cases of the local patients could be attributed to their failure in observance of an ulcer diet during their dietery treatment period, particularly their unfriendly attitude toward a milk diet.
