담결석(膽結石)의 한의학적(韓醫學的) 문헌적고찰(文獻的考察) (A Oriental Medical study on the Gallstone therapeutics)

  • The disease of cholelith is common world widely. As life expectancy gets extended and life environment and dietary life change, increased to gallstone in Korea. On the case that elimination surgery is inapplicable and for the aged patients. recently the dissolution therapy of cholelith is studied world widely. From the view of oriental medicine, the causes of gallstone, treatment methods and the effects of therapy are as follows ; 1. The attack of gallstone is by the stagnation and disturbance of qi in the body. 2. The causes of gallstone are classified into three types. 1).The type of stagnation of qi. 2).The type of damp-heat. 3).The type of noxious heat. 3. The treatment effect is high in the damp-heat type. 4. Medical treatment of gallstone, considering the function of the intestines, are lithodialysis and removol of gallstone, soothing the liver and regulating the cerculation of qi, clearing away heat and elininating dampness. 5. The dissolution therapy of gallstone effected to the size within the diameter of 1.5cm 6. Through the oriental medicine therapy, besides excretion and dissolution of gallstone, the interval extention and elimination of spasm, the prevention of relapse and aftere effect are to be expected. The study which can improve the treatment rate of cholelith through the combination therapy of oriental and western is needed and deep study on oriental medicine diagnostic and classification according to the observation based on symptoms is necessary.

울산 지역 당뇨 질환 노인의 항산화 영양소 섭취실태 및 혈액 항산화 영양상태 (Dietary Antioxidant Vitamins Intakes and Plasma Antioxidant Levels in Korean Elderly with Diabetes Living in Ulsan)

  • Diabetic mellitus in an older population is associated with increased basal oxidative stress and free radical accentuated by hyperglycemic challenge. Enhanced free radical in diabetic elderly can cause the oxidative damage and such damage can be protected by antioxidant defense system. It is believed that vitamin C, A and E are the most abundant and effective antioxidants in human plasma. The purpose of this study was to determine the antioxidant status in Korean diabetic elderly using the case-control study. The antioxidant status was examined by determining plasma levels of antioxidant vitamins (vitamin C, A, E, ${\beta}$-carotene), total antioxidant status (TAS) and thiobarbituric acid reactive substance (TBARS) and intakes of vitamin C, A, ${\beta}$-carotene and retiol. Fasting glucose and HbA1c levels and serum lipid profiles (triglyceride (TG), total cholesterol, HDL-cholesterol and LDL-cholesterol) were also determined. Diabetic subjects were 122 elderly persons over 60 years old, visiting public health center, and control subjects were 96 healthy elderly persons living in Ulsan, Korea and they were matched by age, gender, smoking and drinking status. The diabetic and control subjects were divided into sub-groups according to the status of using diet therapy and vitamin supplement. The subjects were interviewed to collect data on their general characteristics, disease history, vitamin supplement, diet therapy and health-related habits by questionnaires. Their dietary intakes were obtained by means of semi-quantitative food frequency questionnaires (SQFFQ). Fasting plasma glucose and HbA1c levels were significantly higher in diabetes than in control subjects, and plasma total cholesterol level of diabetes was not significantly different from that of control subjects. However serum HDL cholesterol level of diabetes was significantly lower and serum TG level of diabetes was significantly higher than those of control group. The average vitamin A and ${\beta}$-carotene intakes of diabetes were significantly higher than those of control subjects. There was no significant difference in plasma vitamin C, ${\beta}$-carotene, and TBARS levels between two groups, but plasma vitamin A, E and TAS levels were significantly higher in diabetes than those in control group. Plasma vitamin A and TAS levels of diabetic subjects using diet therapy were higher than those of control using diet therapy, and plasma vitamin E, ${\beta}$-carotene and TAS levels of diabetic subjects using vitamin supplements were significantly higher than those of controls using vitamin supplements. These results suggested that diabetic mellitus could enhance antioxidant defences against reactive oxygen species and interest in healthy eating such as consumption of more antioxidant nutrients.

당뇨의 식·의약 치료를 위한 약선 미숫가루의 동, 서 식품학적 효능 분석 (Oriental and Western Food Effects Analysis of Misutgaru for Fusion Remedy in Diabetes Mellitus)

  • 본 연구는 쥐눈이콩을 주재료로 하는 당뇨의 식이요법에 활용할 수 있는 간편 편이식을 개발하고자 계획하였고 그 첫 단계로 고처방을 기초로 하여 미숫가루를 고안하였다. 동양의 기미론을 기본으로 그 효능을 분석하고 동시에 영양성분을 분석한 후 동 서 식품영양학적으로 그 효능을 비교, 고찰하여 당뇨의 식 의약 치료에 있어 활용가치를 판단해 보았다. 본 연구는 식품을 기미론(氣味論)적으로 분석하여 약선식료학 연구를 위한 기초자료를 확보하고 동시에 식품영양분야에서 식품효능 접근 방법과 원활한 접목을 위한 자료를 제시할 수 있을 것이라 사료된다. 또한 향후 식품의 효능이나 섭취방안에 대한 올바른 개념을 정립하여 식품 효능에 대한 인식 전환의 계기가 되어 동양에서 바라보는 식품의 섭취효능을 이해하는데 기초자료가 될 수 있으리라 판단된다. 한편, 본 연구자들은 본 연구에서 고안한 미숫가루의 보간익신(補肝益腎) 효능을 객관적으로 판단해 보고자 임상실험을 진행하여 당뇨의 식 의약치료에 활용할 수 있는 가치를 계속적으로 연구 보고하고자 한다.

Clinical Factors Related with Helicobacter Pylori Infection - Is there an Association with Gastric Cancer History in First-Degree Family Members?

  • Background: The aim of this study was to assess clinical factors associated with Helicobacter pylori positivity and to evaluate the incidence of gastric carcinoma in first-degree family members of infected patients. A total of 580 patients (mean age:$38{\pm}17$) with gastrointestinal complaints underwent C-14 urea breath test (UBT). Patients were grouped as: Group-1, untreated patients (n:384); and Group-2, patients who previously treated with eradication triple therapy (n:196). C-14 UBT was performed 1-2 months after the completion of eradication therapy. Associations of H pylori positivity with age, gender, ABO and Rhesus groups, smoking, dietary habits, and history of gastric cancer in first-degree family members were evaluated. The frequency of H pylori positivity was significantly higher in group-1 (58%) compared to group-2 (20%), p=0.001. There were no correlations between H pylori positivity and age, gender, ABO groups, Rhesus subgroups, smoking and dietary habits in both patient groups. The frequency of gastric cancer in family members was significantly higher in patients with H pylori infection among group-1, compared to infected patients among group-2 (56% vs. 28.6% respectively, p=0.03). We observed a significant association between H pylori positivity and the presence of gastric cancer in first-degree relatives of group-1 patients. Our results provide some confirmation of the presence of a link between gastric cancer development and H pylori. C-14 UBT is a sensitive, reliable and a widely recommended test for the detection of H pylori infection and recurrence. We suggest that detection and eradication of H pylori may contribute to a reduced risk of gastric cancer in the family members of infected patients.

비만 여성을 대상으로 한 단기간의 저열량 식사요법에서 체구성 성분의 변화 (Changes of body composition in obese women with short-term low calory diets)

  • Background: A reasonable weight reduction is to reduce fat-body mass while preserving free-fat mass. Recently, many people in Korea have been trying reckless diet therapy for weight reduction by low calory and protein diets, which gave rise to many side effects consequently. For reasonable weight reduction, this study was undertaken to investigate the influential diet factors that change body composition during short-term low calory diet. Methods: 26 healthy obese women aged 23-27 years whose BMI was greater than 24volunteered for the 6 week diet therapy from January to April in 2003. All subjects were instructed to eat approximately 1200kca1/day and write dietary diary. In the beginning of the study, the 3rd week and the 6th week, subjects' body weight and fat body weight were measured by BlA. In the 6th week, the analysis of dietary diary was conducted. Results : There was significant positive correlation between protein intake per ideal bodyweight in the first 3 weeks and free fat mass increase in the later 3 weeks (P<0.05). At the same time, fat intake in the first 3 weeks had positive correlation with free fat mass in the second 3 weeks, too(P<0.05). But, we could not find any significant values that had effect on free fat mass increase in multiple regression analysis. Conclusion: The protein intake in the first 3 weeks helps preserve fat free mass in the later 3 weeks, and has positive effects.

실증성(實證性) 비만을 위한 약선식 개발에 관한 연구 (1) 이습(利濕) 작용이 있는 한약재 혼합 추출물이 체중과 혈청 포도당, 인슐린 농도 및 지질 조성에 미치는 효과 (Development of Yak-Sun for Excess Syndrome Obesity (1) Effects of Weight, Serum Glucose, Insulin and Lipid Profiles of Oriental Medicinal Herbs with Removal of Dampness through Diuresis)

  • We are concerned about how to apply the theories of oriental medicine appropriately, which direction should it be taken, and how it should be developed. And it is believed to be essential for the government to make efforts to set a standard and laws to validate the medicinal effects and process of the assessment so that the systematic development can be encouraged, and to prepare guidance for the food development for national health improvement Ihis study was peformed to provide basic data that predict the usefulness of oriental medicinal herbs to remove the dampness through diuresis for excess syndrome obesity with oriental diet therapy cuisine(Yak-Sun). Five oriental medicininal herbs, Coix lachryma-jobi L, Atractylodes lancea DC., Ligusticum wallichii F., Angelica sinensis D., Zingiber officinale R. were collected and made into mixing extracts(OMCE). And we examined the effects of OMCE on body weight serum glucose, insulin and lipid profile improvement in rats fed high fat diets. Sprague-Dawley rae(230-250 g) were randomly divided into five groups : basal diet(normal diet control group, NCG), only high fat diet(High fat control group, HCG), high fat diet and supplemented with 25mg/100g body weight 50mg/100g body weight 75mg/100g body weight by OMCE(HLG, HMG, HHG). These experimental diets were fed for 6 weeks. The OME fed groups decreased more significantly in weight serum glucose insulin and lipids than the high fat control group did. These results imply that the OMCE can be used as a safe and clinically applicable ingredients for diet called Yaksun of excess syndrome obesity in human.

고지혈증 성인 여성의 혈청 지질, 호모시스테인 농도 및 산화 스트레스에 미치는 약선차의 효과 (Effects of Yaksuncha, a Combination of Oriental Medicinal Herbs on Serum Lipids, Homocystein Levels, and Oxidative Stress in Hyperlipidemic Women)

  • This research was designed and implemented to evaluate how the composition of yaksuncha can affect the health of individuals suffering from diet-related diseases such as obesity and hyperlipidemia by prescribing Yaksun as a nutritional supplement with daily meals. The yaksunucha was prepared as Koekac, Sansa, Heshouwu and Woolong tea and the tea's effects on serum lipids and oxidative stress were evaluated by clinical procedures. The yaksuncha significantly increased HDL-cholesterol and decrease of LDL-cholesterol concentrations in serum. It also had significant effects on decreasing oxidative stress and homocystein levels. The physical characteristics of the yaksunucha were also examined, showing. Brix, pH and titratable acidity values of 1.4, 5.50 and 0.05% respectively. It is thought that scientific and objective evaluations were completed on the components of the yaksuncha prescription. Thus one can concluded that the components could be applied not only in the form of tea, but also various food. The data derived from this study provides basic information that will aid in the application of oriental medicinal resources to other foods as well as facilitate the study of medicinal herbs within the field of functional food research, which already draws sizable attention worldwide.

Beyond the clinical walls: registered dietitian nutritionists providing medical nutrition therapy in the home setting

  • BACKGROUD/OBJECTIVES: Registered dietitian nutritionists (RDN) are providers of medical nutrition therapy (MNT) to address health and chronic disease. Traditionally, RDNs have provided care in healthcare facilities including hospitals and private care facilities. The purpose of this study was to determine how RDN individualized MNT in the home impacted nutrition, physical activity, and food security. SUBJECTS/METHODS: This is a secondary data analysis. The mean age of the participants (n = 1,007) was 51.6 years old with a mean body mass index (BMI) of 34.1 kg/m2. Individualized MNT visits were delivered by an RDN in the home setting from January to December 2019. Participants were referred by healthcare professionals or self-referred. Participants had MNT benefits covered by their health insurance plan (43.3% Medicaid; 39.8% private insurance; 7.9% Medicare, 9% other). Health outcomes related to nutrition care were measured. Outcomes included self-reported consumption of nutrition factors and physical activity. Our secondary outcome focused on food security. The changes in weight, BMI, physical activity, and nutrition factors were analyzed by a linear regression model or linear mixed model, adjusting for age, sex, baseline value, and number of appointments. Food security was summarized in a 2 by 2 contingency table. RESULTS: Baseline values had significantly negative impacts for all changes and number of appointments was significant in the changes for weight and BMI. Increases in physical activity were significant for both female and male participants, 10.4 and 12.6 minutes per day, respectively, while the changes in weight and BMI were not. Regarding dietary factors, the consumption total servings per day of vegetables (0.13) and water (3.35) significantly increased, while the consumption of total servings of whole grain (-0.27), fruit (-0.32), dairy (-0.80) and fish (-0.81) significantly decreased. About 24% (of overall population) and 45% (of Medicaid population) reported improvements in food security. CONCLUSIONS: This study found that home visits were a useful setting for MNT delivered by RDNs. There is a strong need for individualized counseling to meet the participants' needs and personal goals.

유방암 환자의 식행동 수정을 위한 영양중재 효과판정 (Nutritional Intervention for Dietary Behavior Modification in Breast Cancer Patients)

  • 본 연구는 유방암 환자의 식행동 수정을 위한 영양중재 효과판정을 위해 전문가 설문을 통한 통계분석으로 진행하였다. 연구의 대상은 2015년 12월부터 2016년 5월까지 강남세브란스병원에서 유방절제수술을 받은 30명의 여성 환자였다. 연구방법은 숙련된 임상영양사가 영양중재 전후 식행동 수정 및 체중 변화를 측정하였고, 운동, 음주 및 건강보조식품 복용여부에 대해서도 조사하였다. 연구결과 본 연구대상자의 평균 연령은 $54.3{\pm}9.3$세이었고, 월평균 체중감소율은 $1.4{\pm}3.5%$이었다. 영양중재 전후 식행동 수정변화 점수는 3점 척도로 측정 시 $2.00{\pm}0.45$ 점에서 $2.76{\pm}0.18$ 점으로 유의하게 상승하는 결과를 보였다(p<0.001). 음주를 하는 환자의 비율은 20%에서 영양중재 후 전원 금주하는 효과를 보였고, 운동수행 비율도 53%에서 영양중재 후 97%로 증가하였다. 건강보조식품의 복용 비율은 40%에서 영양중재 후 20%로 감소하는 결과를 보였다. 이상의 결과를 고려해 볼 때, 유방암환자의 영양중재 후 식행동 수정은 추후 유방암의 재발을 방지하는데 매우 중요하다고 사료된다.

Dietary Habits Contributing to Breast Cancer Risk Among Iranian Women

  • Background: The aim of this study was to investigate demographic features, dietary habits, and some possible risk factors for being susceptible to breast cancer in Iranian women. Materials and Methods: A study of dietary habits and breast cancer was conducted among 53 Iranian women with histological confirmed disease and 40 matched controls. A dietary habits questionnaire was used to evaluate the pattern of selected food intakes. The risk of cancer was analyzed after adjustment for confounding factors. Age, weight, body mass index (BMI), waist circumference, educational status, parity, lactation, marital status, menopause, history of estrogen therapy, and family history of breast disease or cancer were assessed among participants. Special attention was given to the relationship between consumption of high fat meat, milk, yogurt and cheese as well use of frying oils for frying foods, use of olive/liquid oils for cooking, removing fat from meat and poultry, removing chicken skin and not use of mayonnaise as salad dressing and the risk of breast cancer. Moreover, salad, vegetable and fruit consumption, and eating outdoors owere investigated. Results: Our results revealed significant lower education and higher BMI and waist circumference levels in patients with breast cancer. There was significantly increased breast cancer risk in overweight women in comparison with normal weight (OR=2.91, 95%CI 1.24 to 6.82). High intake of fat dairy products including milk and cheese was found to be a statistically significant factor for increasing breast cancer risk in models adjusting for age, BMI and education. Use of olive/liquid oils for cooking and avoidance of mayonnaise as salad dressing are related to lower risk of breast cancer. The frequency of vegetable and fruit consumption was significantly lower in patients with breast cancer compared to healthy women. Conclusions: Dietary habits might be risk factors for breast cancer among Iranian women. Adoption of a prudent diet could be an appropriate strategy for preventing breast cancer.
