  • The ulcerative colitis is a chronic inflammatory bowel disease with an unknown etiology. The major symptoms of ulcerative colitis are diarrhea, abdominal pain and hematochezia. However, arthritis, skin disorders, hepatobiliary inflammation and uveitis are occasionally recognized as systemic complications. Although there are few reports of coexistent pulmonary and inflammatory bowel disease, the lung is not generally considered to be a target organ in ulcerative colitis. We report a patient with ulcerative colitis-related bronchilolitis obliterans organizing pneumonia confirmed by video-assisted thoracoscopic surgery, who responded to corticosteroid therapy.

Analysis of herbal formulation about a series of Chijasi-tang in Dongeuibogam (동의보감(東醫寶鑑)에 수록된 치자시탕(梔子豉湯) 가미방(加味方) 분석(分析))

  • In this study, we selected some herbal formulation about a series of Chijasi-tang in Dongeuibogam by using web-based open program;Prescription Lineage Graph (http://164.125.206.43/PrescriptionLineageGraph.aspx). And we compared and analyzed the changes of efficacy, major target symptoms of each herbal formulation according to compositional variation of each herbal formulation. Chijasi-tang, first appeared in Sanghanlun, consists of Capejasmine and Fermented soybean, and it is mainly used to treat insomnia due to vexation, heartburn and yellow greasy tongue fur. Capejasmine can clear away irritable feverish sensation in chest by flowing downward the heat, and Fermented soybean can disperse stagnated heat throughout the chest by expelling stagnated heat from the exterior In the case of the heat stagnation caused by relapsing of disease due to overstain, Chisijisil-tang can be used. And if symptom appear more on the upper or exterior than a case of Chisijisil-tang, Seosisi-tang could be more suitable, if half exterior and half interior, Omae-tang could be for it. In addition, if symptom caused by relapsing due to improper diet, Chisijihwang-tang could be proper formulation. In the case of the heat stagnation body inside and jaundice, if it is caused by alcohol, Chijadaehwang-tang could be used for the purpose of urgent purgation, Galchul-tang would be suitable for helping the function of the spleen and the stomach and for treatment of damp-heat of the spleen and the stomach. And if it is caused by pandemic infection, Jangdal-hwan would be good formulation for it. Samhwangseokgo-tang and Yangdokchija-tang could be appropriate formulation for the raging of noxious heat and pathogenic fire caused by febrile disease with toxic yang. Daehwangeum-ja is for severe constipation due to heat-dryness with stagnated fever, Haebaek-tang is appropriate for severe diarrhea due to heat type change of Soeum. According to the result of our investigation, although there are various target causes and symptoms of each herbal formulations, whatever pathogenetic cause is, the stagnated heat in interior side is the basis of symptoms. Therefore, the purpose of including Chijasi-tang in composition of each herbal formulation is treatment of the stagnated heat. For such reason, on the fundamental or ancillary basis of Chijasi-tang plus some herbs for each therapeutic purpose.

  • This thesis is a study composed of eight chapters from 9. to 16. of the Maek Kyoung(脈經) Vol. VIII. ; the symptom & the pulse of Jaundice(黃疸), Malarial Disease(?疾). Carbuncle(癰腫) Intestines Carbuncle(腸癰), & c. It is as follows : Chapter 9 refers to the symptom, pulse, treatment and prognosis of Jaundice(黃疸) and Malarial Disease(?疾). Chapter 10 refers to the cause, symptom, pulse and treatment of Cardialgia(胸痺), Cardiagra(心痛) and Nephric Accumulation(賁豚). Chapter 11 refers to the symptom, pulse and treatment of Abdominal Fuliness(腹滿), Cold Mounting(寒疝) and Abiding Food(宿食). Chapter 12 refers to the symptom and pulse of Accumulation and Mass of the Five Viscera(五臟積聚). Chapter 13 refers to the cause, pathogenesis, symptom, pulse, treatment and prognosis of Terror and Palpitation due to Fright(驚悸), Hematemesis(吐血), Nasal Hemorrhage(?血), Metrorrhagia(下血) and Extravasated Blood(瘀血). Chapter 14 refers to the cause, pathogenesis, symptom, pulse and treatment of Vomiting(嘔吐), Hiccough(?) and Diarrhea(下利). Chapter 15 refers to the cause, pathogenesis, symptom, pulse and treatment of Atrophy of Lung(肺?), Pulmonary Abscess(肺癰), Lung-distention(咳逆上氣) and Phlegm(痰飮). Chapter 16 refers to the cause, pathogenesis, symptom, pulse. treatment and prognosis of Carbuncle(癰腫), Intestines Carbuncle(腸癰), Wound(金瘡) and Acute Eczema(侵淫瘡). There have been abundant investigations in China. But we couldn't find a clear result yet, and they were written in archaic texts and colloquial Chinese, therefore it is needed to be translated into Korean. And there was only one inaccurate translation with insufficient annotation. So I hope this study will be useful to develope Oriental Medical Diagnostics.

A Clinical study of Taeumin Chongsim Yonja Tang (청심연자탕(淸心蓮子湯) 활용(活用)에 대(對)한 임상적(臨床的) 연구(硏究))

  • 1. Background and Purpose : I intended to be helpful to understand constitutional symptoms of a disease and an application of the prescription through a clinical study of Taeumin Chongsim Yonja Tang. 2. Methods : I studied 125 patients(62 men and 63 women) treated with Taeumin Chongsim Yonja Tang in constitutional clinic of Dongguk Pundang Oriental Hospital. And then I investigated the characteristics of Chongsim Yonja Tang treated patients, remedial values and side effects. 3. Results : Taeumin Chongsim Yonja Tang was used in all age groups both men and women. There were 60 kinds of chief complaints including numbness of hands and feet, headache and fatigue etc. The distributions of remedial values were slightly improved(60.8%), much improved(15.2%), no improved(24.0%) and there was no aggravated patient. Using the prescription, mostly I prescribed only Taeumin Chongsim Yonja Tang without any changing, and added 2-3 herbs to the prescription according to the symptoms of a disease. Taeumin Chongsim Yonja Tang was affected in remedial values by the period of sufferring from illness, treatment and medication. Side effects were diarrhea, indigestion and so on.

Evaluation for Protective Effect of CPV-2 and CPV-2b Vaccines against a Korean CPV-2a Isolate in Pups (국내에서 유행하는 CPV-2a 분리주에 대한 CPV-2와 CPV-2b 백신의 방어효능 평가)

  • The aim of this study was to determine if vaccines containing CPV-2 or CPV-2b provided protection against challenge with a recent Korean CPV-2a isolate. Twenty mongrel pups aged 9 weeks old were used. The commercial CPV-2 or CPV-2b vaccines were administered to each of the 8 pups thrice every 3 weeks, respectively. Two weeks after the last vaccination, all pups were challenged with CPV-2a (VR00174 strain) $1{\times}10^6\;TCID_{50}$. Clinical signs, fecal excretion of challenged CPV, and serological response of pups were observed for 2 weeks after challenge. All vaccinated pups did not display any clinical signs of disease after challenge with Korean CPV-2a isolate, whereas all non-vaccinated pups exhibited mucoid or hemorrhagic diarrhea, vomiting and anorexia. In all non-vaccinated pups, the virus could be detected in feces from 4 days after challenge, whereas in vaccinated pups, no evidence of viral excretion could be detected. Two of 4 non-vaccinated pups died 6 days after the challenge. This study showed that the two commercial CPV-2 and CPV-2b vaccines were effective in preventing infection and/or disease caused by the Korean CPV-2a isolate.

Epidemiological investigation on the outbreak of foodborne and waterborne disease due to Norovirus with delayed notification (노로바이러스에 기인한 수인성·식품매개감염병 집단발생의 지연신고에 대한 역학조사)

  • Objectives: There was an outbreak of foodborne and waterborne disease among high school students at Okcheon in June, 2018. First attack occurred June $5^{th}$ but seven days later it was notified. The purpose of this investigation was to evaluate the pathogen of outbreak and cause of delayed notification. Methods: First, we did a questionnaire survey for 61 cases and 122 controls to find what symptoms they had and whether they ate foods or drank water from June $2^{nd}$ to June $12^{th}$. Second, we investigated the environment of cafeteria and drinking water. Third, we examined specimen of cases and environment to identify bacteria or virus. Results: Attack rate of this outbreak was 7.8%. Drinking water was strongly suspected as a source of infection in questionnaire survey but we could not find the exact time of exposure. Norovirus was identified in specimen of cases (2 students), drinking water (at main building and dormitory) and cafeteria (knife, dishtowel, hand of chef) Conclusions: We decided norovirus as the pathogen of this outbreak based on the clinical features of cases with diarrhea vomiting, abdominal pain and recovery within 2 or 3 days after onset, outbreak due to drinking water and microbiologic examination, And the cause of delayed notification might be the non-existence of the nurse teacher at that time and the lack of understanding of teachers on immediate notification under the outbreak. To prevent the delayed notification, notification system about outbreak of foodborne and waterborne disease in school is needed to be improved.

Diagnostic Efficacy of Anorectal Manometry for the Diagnosis of Hirschsprung's Disease (Hirschsprung병에서 항문직장 내압검사의 진단적 유용성)

  • Purpose: As diagnostic tools for Hirschsprung's disease (HD), barium enema and rectal biopsy have radiation exposure and invasiveness respectively; however anorectal manometry does not have these disadvantages. We therefore performed this study to evaluate the diagnostic efficacy of anorectal manometry. Methods: We reviewed medical records of infants with one or two symptoms of vomiting, abdominal distension, chronic diarrhea or constipation who had a anorectal manometry followed by barium enema and/or biopsy from July 1995 to May 2002. We evaluated the sensitivity, specificity and predictive value of anorectal manometry and barium enema for diagnosis of HD. We also measured sphincter length, median value of balloon volume at which rectoanal inhibitory reflex (RAIR) occurred. Results: All 61 patients received anorectal manometry, 33 of 61 received barium enema. 18 of 61 were diagnosed as HD according to histology and 43 of 61 were evaluated as a control. The sensitivity, specificity, positive predictive value, negative predictive value of anorectal manometry and barium enema for diagnosis of HD were 1.00, 0.91, 0.82, 1.00 and 0.93, 0.67, 0.70, 0.92 respectively. The mean value of sphincter length in control was $1.68{\pm}0.67$ cm and correlated with age, weight and significantly longitudinal length. The median value of balloon volume at which RAIR occurred was 10 mL and did not correlated with age, weight and longitudinal length. Conclusion: This study suggests that anorectal manometry is an excellent initial screening test for Hirschsprung's disease because of its safety and accuracy.

Experimental Pathogenesis of Pullorum Disease with the Local Isolate of Salmonella enterica serovar. enterica subspecies Pullorum in Pullets in Bangladesh

  • The research work was carried out to study the pathogenesis covering the clinical signs, gross and histopathological lesions in different organs, and reisolation and identification of the organisms after experimental infection with the local isolate of Salmonella enterica serovar. enterica subspecies (S.) Pullorum at different time interval of the experiment during the period February 2006 to December 2006. One hundred pullets (seronegative to S. Pullorum of 12 weeks age were purchased and divided into 5 (A, B, C, D and E) groups and each group consisted of 20 birds. Four groups (A, B, C and D) were infected orally with a dose of $10^6\;CFU$, $10^7\;CFU$, $2{\times}10^7\;CFU$, $10^8\;CFU$ of S. Pullorum, respectively, and one group (E) was treated as uninfected control. The used methods were necropsy and histopathology, culture of bacteria, staining and biochemical test of Salmonella. Five birds from each group were randomly selected and sacrificed $1^{st}$ week, $2^{nd}$, $3^{rd}$ and $4^{th}$ weeks of post infection (PI). From all the groups, the bacteriological samples (crop, liver, lung, heart, spleen, bile duodenum, ceca and blood) were collected with pre enriched in buffered peptone water in sterile poly bags. Liver, lungs, heart, spleen, intestine, etc. were collected in 10% buffered-formalin for histopathological examination. No clinical signs, gross and histopathological lesions were found in control group and no S. Pullorum was reisolated. Clinical sign of experimentally infected with S. Pullorum in pullets were loss of appetite (100%), slight depression (75%), ruffled feathers (85%), diarrhea (60%) and loss of weight (100%) in chickens. The feed intake and body weight at different weeks after PI differed significantly (p<0.01) among the groups. Grossly, the highest recorded lesion was button-like ulcer in the ceca (80%) and the lowest was white nodules in lungs (1.25%). S. Pullorum were reisolated from crop (91.25%), liver (91.25%), lung (83.75%), heart (71.25%), spleen (87.75%), bile (33.25%), duodenum (92.50%), ceca (97.50%) and from different group of infection (61.25%). The highest microscopic findings were intestinal and cecal mucosa and submucosa exhibited infiltration of mononuclear cells and congestion (96.25%), and the lowest finding was nodule formation in the lungs (3.75%). The pattern of the disease production by local isolate of S. Pullorum in Bangladesh is almost similar with other isolates in different countries.

Epidemiology and Clinical Characteristics of Clostridium difficile-associated Disease in Children: Comparison between Community- and Hospital-acquired Infections (소아에서 발생한 Clostridium difficile 관련 질환의 역학과 임상양상: 지역사회감염과 원내감염의 비교)

  • Purpose: Recent studies have reported an increase in the incidence of community-acquired Clostridium difficile-associated disease (CA-CDAD) among children. There is an overall lack of information on CA-CDAD in the pediatric population. The aim of our study was to compare the epidemiologic and clinical features between CA-CDAD and hospital-acquired C. difficile-associated disease (HA-CDAD) in children. Methods: We retrospectively reviewed the medical records of all patients who were diagnosed with C. difficile-associated disease (CDAD) at Gil Hospital between April 2008 and March 2009. The diagnosis of CDAD was made when patients with gastrointestinal symptoms had positive results for C. difficile toxins A and B assay or stool culture. Results: Sixty-one (male, 32 and female, 29) patients were included. The mean age was 3.79${\pm}$4.54 years. Of the 61 patients, 22 (36.1%) were <1 year of age. Twenty-three patients (37.7%) had a history of antibiotic exposure in the previous 3 months. Forty-one patients (67.2%) were diagnosed with CA-CDAD. There were no significant differences in age, gender, symptoms, laboratory findings, recovery period, complications, and recurrence between the CA-CDAD and HA-CDAD groups. On the other hand, exposure to antibiotics was significantly more frequent among patients in the HA-CDAD group (p=0.005). Conclusion: This study suggests that the occurrence of CA-CDAD is increasing in the pediatric population, especially in younger children with no history of exposure to antibiotics and in outpatients. Awareness of the increasing incidence of CA-CDAD and prompt investigation of C. difficile in susceptible patients is needed to avoid misdiagnosis and for appropriate therapy.

Effects of Feeding Eucommia ulmoides Leaves Substituted for Rice Straw on Growth Performance, Carcass Characteristics and Fatty Acid Composition of Muscle Tissues of Hanwoo Steers (볏짚을 두충잎으로 대체급여 시 거세한우의 생산성, 도체특성 및 육의 지방산 조성에 미치는 영향)

  • This study was conducted to examine the growth performance, carcass characteristics, physico-chemical properties and fatty acid composition of muscle tissues of Hanwoo steers when they were fed diets containing four levels of E. ulmoides leaves. Steers were allotted to one of four dietary treatments, which were designed to progressively substitute Eucommia ulmoides for 0, 3, 5 and 10% of the rice straw in the basal diet. Seventy two Hanwoo steers (321±13kg) were used. Average daily gain (0.81-0.86kg) and feed conversion (10.05-10.59) were not changed by feeding E. ulmoides leaves. Emission of fecal ammonia gas was decreased by increasing substitution levels of E. ulmoides leaves. Emission of ammonia gas in feces of steers were significantly (p<0.05) decreased in the 5% and 10% treatments at 40℃ compared with in control (30.6ppm) and 3% treatment (29.8ppm), respectively 20.3 and 21.6ppm. Back fat thickness was higher (p<0.05) in steers fed control diet (15.0mm) than 5 and 10% E. ulmoides diets (10.2 and 10.5mm respectively). The grade ‘A’ appearances of meat yield of steers were increased up to 20% by the 3% substitution of E. ulmoides leaves, and the grades ‘1+’ and ‘1’ appearances of meat quality were significantly improved (p<0.05) by the 5 and 10% Eucommia ulmoides feeding. There was no statistical difference in meat color of loin and top round among all treatments. It appeared that the 5% and 10% E. ulmoides leaves feeding affected (p<0.05) a increase in oleic acid concentration in loin and top round muscles. The concentration of serum cholesterol was lower (p<0.05) in steers fed 10% E. ulmoides (53.3mg/100g) than that of the other treatments (55.7-57.0mg/100g). The diarrhea incidence was about 20.26% in control, while it was lower (p<0.05) in the 5 and 10% E. ulmoides leaves treatments (10.4-12.5%) than control. There was no statistical difference in the occurrences of respiratory disease among all treatments. It is concluded that E. ulmoides leaves should be a prospective feed additive because it contains various functional substances. It is likely that air-dried E. ulmoides leaves can reduce the back fat thickness of Hanwoo steers and the occurrences of diarrhea. Therefore, the 5 and 10% substitution of E. ulmoides for roughage are highly recommended to be used in practice.
