Traumatic Bilateral Diaphragmatic Ruptures in a 6-Year-Old Boy

  • Traumatic diaphragmatic rupture is an unusual finding that may occur after blunt trauma. In addition, diaphragmatic rupture occurring bilaterally is extremely rare. We experienced a 6-year-old boy with bilateral diaphragmatic rupture, whom survived after surgical treatment by open thoracotomy but, complicated with spinal cord injury discovered after surgery.

  • 외상성 횡격막 파열은 방사선 검사로 진단하기가 어려운 경우가 빈번하다. 다발성 손상을 동반한 37세 남자 환자에서 횡격막 파열이 의심되었으나, 흉부 CT 검사에서는 횡격막 파열을 확인할 수는 없었고 진단을 위해 흉강경을 이용한 수술을 결정하였다. 횡격막 신경이 횡격막으로 들어가는 부위로부터 8 cm정도의 횡격막 파열이 있었고, 횡격막 마비를 동반하였다. 작업창을 5 cm크기로 추가하여 만든 후, 파열된 횡격막을 연속 봉합하였으며, 동시에 횡격막 주름성형술을 시행하였다. 수술 3개월후 시행한 흉부 방사선 사진에서 만족할 만한 결과를 보였다.

  • A one-year-old female sika deer died suddenly with no preliminary signs during exhibition at a zoo. At necropsy, the carcass was emaciated and had dried fur. Examination of the thoracic cavity revealed a diaphragmatic rupture measuring 2 cm in diameter and a fracture in the middle of the right eighth rib. The liver and lungs had irregular circular discolorations caused by diaphragmatic rupture and subsequent herniation. Dark-brown-colored ascitic fluid, hydrothorax, and yellowish hydropericardium were also observed. The cause of death was determined to be diaphragmatic rupture caused by a rib fracture, which led to respiratory imbalance and circulatory disorders.

  • Blunt diaphragmatic rupture (BDR) is a relatively rare injury and occurs in 0.8% to 7% of all thorocoabdominal blunt trauma. Especially right diaphragmatic rupture after blunt abdominal trauma is a rarer than left. The diagnosis of BDR can be missed while evaluating the multiple trauma patient. Other severe injuries may mask BDR during the primary resuscitation and survey. We experienced two cases of traumatic rupture of right diaphragm, one diagnosed immediately and the other diagnosed delayed. In this paper we present two cases of traumatic diaphragmatic rupture.

  • Traumatic diaphragmatic rupture is quite uncommon and rarely lethal injury. However, delayed presentation between the injury and the diagnosis can cause a life-threatening condition with various complications such as intestinal hernia, obstruction, strangulation, respiratory distress. Here, we present a case of delayed presentation of traumatic diaphragmatic rupture in a 51-year-old man, and then discuss about the clinical implication of delayed presentation of diaphragmatic rupture with a review.

  • Spontaneous rupture of the eventrated diaphragm is not common. The authors report a case of spontaneous rupture of the congenital diaphragmatic eventration. An 8 year-old girl with right congenital diaphragmatic eventration and nephrotic syndrome was seen in emergency room because of severe abdominal pain and vomiting. She had intermittent abdominal pain for 1 year. Plain chest X-ray and ultrasonography showed entrapped bowels in the right thoracic area. Exploratory laparotomy revealed a ruptured right eventration. THE displaced abdominal viscera were repositioned into the abdominal cavity and the ruptured diaphragm was trimmed and plicated. The postoperative course was uneventful. Only one case of spontaneous rupture of eventrated diaphragmatic has been reported in the English literature.

토끼의 횡격막 파열 진단에 있어서 자기공명영상, 나선형전산화단층촬영 및 초음파의 가치 비교 (Comparative Evaluations of Magnetic Resonance Image, Spiral Computed Tomography and Ultrasound in the Diasnosis of Experimental Diaphragmatic Rupture in the Rabbit)

  • 목적: 횡격막 파열은 그 진단이 어렵고 지연되는 경우가 많으며, 진단이 늦어지면 유병율과 사망율이 높아지므로 조기진다는 매우 중요하다. 저자들은 최근 많이 이용되는 영상진단법인 나선형 CT와 자기공명영상, 그리고 초음파건사중 어떤 방법이 가장 예민하고 정확한 진단방법인지 동물실험 모델을 통하여 평가하고자 하였다. 대상 및 방법: 토끼에서 인위적으로 횡격막파열을 대, 중, 소로 만든 후 1일, 3일, 7일째 나선형 CT와 자기공명영상, 그리고 초음파검사를 동시에 시행하였다. 결과: 횡격막파열의 진단 민감도와 특이도는 초음파검사가 94.4%와 92.9%, 자기 공명영상이 54.0%와 85.75, 나선형 CT가 46.0%와 78.6%이었다. 횡격막 파열의 진단은 초음파검사, 자기공명영상, 나선형 CT순으로 정확하였으며 초음파 검사가 자기공명영상이나 나선형 CT보다 유의하게 우월하였고(P<0.05), 자기공명영상과 나선형 CT는 유의한 차이가 없었다. 초음파검사는 파열의 크기와 상관없이 횡격막파열의 구별이 용이하였고 자기공명영상과 나선형 CT는 파열의 크기가 클수록 횡격막파열의 구별이 용이하였으나 통계적으로 유의한 차이는 없었다(P>0.05). 수술후 1일에는 전예에서 늑막삼출액이나 혈흉을 동반하였다. 이때에는 초음파나 자기공명영상이 나선형 CT에 비해 우월하였다. 시간경과에 따랄 초음파검사와 자기공명영상은 진단율이 상대적으로 저하되었으나 통게적으로 유의한 차이는 없었다(P>0.05) 탈장을 동반한 경우에는 진단의 민감도가 자기공명영상, 나선형 CT, 초음파검사순으로 높았으며 초음파검사가 상대적으로 낮았다. 그러나 통계적으로 유의한 차이는 없었다(P>0.05). 결론: 초음파검사가 횡격막파열의 진단에 가장 유용한 것으로 밝혀졌으며, 이러한 결과는 앞으로 임상에 적용하여 검증되어야 할 필요가 있다고 생각된다.

Massive Spontaneous Diaphragmatic Rupture Induced by a Squatting Position

  • While a diaphragmatic rupture commonly results from trauma to the abdomen and chest, a spontaneous diaphragmatic rupture is very rare. A 68-year-old male presented with chest pain that had originated while doing farm work in a squatting position. Images revealed a 5 cm defect of the left diaphragmatic dome, and the entire stomach was displaced into the thorax. The diaphragmatic defect was round and half had a well-demarcated margin. The remaining fragile tissue was completely excised and was closed primarily. The patient was uneventfully discharged and resumed with a normal diet 10 days after the operation.

Incidental traumatic right diaphragmatic rupture: a missed case after trauma

  • Traumatic diaphragmatic hernia is among the most uncommon conditions after severe trauma, and it is associated with high morbidity and mortality. The diagnosis is difficult and might be missed, but a multimodal investigation might help in terms of diagnostic yield. In this case report, we present a missed right diaphragmatic rupture 14 years after the trauma.

외상성 횡경막 탈장: 5례 수술 보고 (Traumatic Diaphragmatic Hernia: A Report of 5 Cases)

  • Five cases of traumatic diaphragmatic hernia were repaired in the Department of Thoracic Surgery, Seoul National University Hospital, during the period from 1967 to 1974. The first case, a 14-year aid girl, was diagnosed as diaphragmatic hernia during laparotomy because of jejunal perforation 3 days after traffic accident. Herniated stomach, transverse colon, spleen and left lobe of the liver were repositioned and the diaphragmatic rupture on left posterolateral portion was repaired with two layers of nonabsorbable sutures by transthoracic approach. The second case, a 26-year old man,was diagnosed immediately after traffic accident at a local clinic and transferred to this hospital 24 hours later. Herniated stomach, transverse colon and jejunum were repositioned amd diaphragmatic rupture,about 9 cm in length,from the posterolat.edge to the base of pericardium was sutured in two layers. The third case, a 26-year old man who had stab wound on the left lower lateral chest two years ago,was admitted with sudden abdominal pain and vomiting. Upper gastrointestinal series with barium meal revealed diaphragmatic hernia. The herniated stomach and transverse colon through the defect,about 3.5cm in diameter, at anterolateral portion on the left side,were repositioned and repaired with two layers of nonabsorbable sutures. The forth case, a 26-year old man, sustained blunt trauma to the chest by a roller and was transferred to the emergency room complaining of dyspnea 40 minutes after the accident. The diaphragmatic rupture extended from left midaxillary line to contralateral anterior axillary line,about 20cm long, at anterior portion of diaphragm, which was repaired with two layers, of nonabsorbable sutures. The fifth case, a 4-year old girl, had two separate diaphragmatic ruptures on both sides, which were caused by traffic accident. Immediate upper gastrointestinal series after injury showed herniated stomach, colon and spleen into left Chest cavity. Another small rupture with anterior edge of right lobe of the liver in chest cavity was noted. These were repaired with non-absorbable sutures via thoracotomy.
