Touch Imprint Cytology of Adenomatoid Tumor of the Tunica Albuginea - A Case Report - (고환의 백색막에 발생한 샘모양종양이 압착도말 세포소견 - 1예 보고 -)

  • Adenomatoid tumor is a benign neoplasm of a mesothelial origin, and it usually occurs in the reproductive organs, especially in the epididymis. The author experienced a case of adenomatoid tumor involving the tunica albuginea and testicular parenchyme without any evidence of epididymis involvement. The patient was a 36-year-old man with a painless scrotal mass that he had experienced for 2 months, and this mimicked testicular neoplasia, including metastatic carcinoma, or other benign lesions. The imprint cytology of the tumor showed a hypocellular smear with mainly arranged cells in cohesive monolayered clusters along with occasional singly dispersed cells and naked nuclei in a clean background. The cellular clusters formed vague glandular and cord-like structures. The tumor cells were large polygonal to columnar cells with a relatively monomorphic appearance. The nuclei were oval to round shape and they showed vesicular, fine chromatin and inconspicuous nucleoli. The cytoplasm was moderate to abundant, and it contained fine vacuoles in some tumor cells. Mitoses and cellular pleomorphism were not present. Awareness of the cytologic finding of this lesion is necessary to screen or differentiate a testicular or paratesticular mass before and/or during surgery because the cytology may be useful as a diagnostic tool. Pathologists should be aware of the cytologic features of common lesions in this anatomic region so as to avoid performing aggressive and unnecessary surgical procedures.

Dumbbell-Shaped Trigeminal Schwannoma in a Child (소아에서 발생한 삼차신경초종)

  • Trigeminal schwannoma is an uncommon intracranial tumor and constitutes less than 0.08-8% of all intracranial schwannoma. Trigeminal shchwannoma is even rarer in childhood and only 11 cases of trigeminal schwannoma under the age of 14 have been reported including our case. We experienced a case of trigeminal schwannoma in a 9 year-old girl without the stigmata of neurofibromatosis. She presented with a 2 week-history of right abducens nerve palsy. Magnetic resonance image demonstrated a dumbbell-shaped tumor extending into right middle and posterior fossas with a size of $60{\times}45{\times}35mm$. The tumor was successfully removed by right transzygomatic subtemporal extradural approach. Pathologic examination of the mass revealed typical features of schwannoma.

Cervical Spondylotic Myelopathy due to the Ochronotic Arthropathy of the Cervical Spine

  • Ochronosis is a musculoskeletal manifestation of alkaptonuria, a rare hereditary metabolic disorder occurs due to the absence of homogentisic acid oxidase and leading to various systemic abnormalities related to deposition of homogentisic acid pigmentation (ochronotic pigmentation). The present case reports the clinical features, radiographic findings, treatments and results of a cervical spondylotic myelopathy woman patient due to the ochronotic arthropathy of the cervical spine. The patient aged 62 years was presented with gait disturbance and hand clumsiness. Physical examination, X-rays, computed tomography and lab results of the urine sample confirmed the presence of ochronosis with the involvement of the cervical spine. The patient underwent a modified cervical laminoplasty due to multi-segment spinal cord compression. The postoperative follow-up showed a good functional outcome with patient satisfaction. The present study concludes the conditions and important diagnostic and surgical aspects of a patient. It is necessary to identify the condition clinically and if cord compression is observed, appropriate surgical interventions needs to be instituted.

  • Even experienced clinicians have difficulties in diagnosing posttraumatic stress disorder (PTSD) exactly, due to its diverse clinical features, which vary according to individuals, traumas, and various comorbid psychopathologies, and its related compensation issues. It is usually mandatory for clinicians and researchers to use screening and assessment tools when diagnosing and evaluating PTSD. To date, research has developed numerous PTSD screening and assessment tools ; therefore one of the cardinal issues is to select the best of the various tools, the one most suitable for the clinician's or researcher's purposes. This article reviews several currently-available subjective and objective instruments for the diagnosis and evaluation of PTSD and groups them according to whether they are Diagnostic and Statistical Manual for Mental Disorders-Correspondent Measures ; PTSD-Focused, Non-DSM-Correspondent Measures ; or Empirically Derived Measures. We present the instruments' psychometric properties and scoring methods and describe their merits and weak points, focusing on their practical usage.

Diagnostic Imaging Features of Concomitant Flexor Enthesopathy in a Dog

  • Flexor enthesopathy is an important cause of elbow lameness in dogs. Flexor enthesopathy is divided into primary and concomitant forms deciding the treatment. The imaging characteristics in affected dogs are irregular medial humeral epicondyles, spur, calcified bodies adjacent to medial humeral epicondyle, thickened and contrast enhancement of the affected muscles. In this report, the radiography, computed tomography (CT), and magnetic resonance imaging (MRI) were performed for right forelimb lameness of a 3-year-old dog. The irregular sclerotic changes and spur of the medial humeral epicondyles were shown with calcified bodies on radiography. Thickened flexor muscles in right forelimb and fragmented coronoid processes of both elbows were observed on CT. On MRI, high signal intensity of the bilaterally thickened flexor muscles with contrast enhancement was detected. Based on these results, concomitant flexor enthesopathy with fragmented medial coronoid process of bilateral elbows was diagnosed in this dog.

Spectral Reflectivity on Geological Materials in Yangsan-Dongrae Fault Area (양산-동래 단층 지역의 암석에 대한 분광학적 연구)

  • The study was performed to recognize the most preferable spectral chennels for discriminating geological materials using the portable radiometer. The portable radiometer covers the visible and short infrared regions from approximately 0.4 to 2.5 microns which are coincided with Landsat TM, and the rock samples used for the study are pyrophylites, andesites, granite, granodiorite and silicified sedimentary rocks which are collected in Yangsan-Dongrae fault area. The analysis of the rock sample provides a preliminary basis for determining the wavelength regions showing diagnostic spectral features and for discriminating hydrothermal altered rocks from the unaltered rocks. The measurement of spectral of spectral reflectance for the rock samples was carried out in the laboratory which environment condition such as temperature, light sources, and humidity are constant. The analysis of the measured data was based on correlation between the reflectance value of the rock samples, and the follow discriptions are output of the study. 1) Pyrophyllite shows absorption at 0.83 $\mu\textrm{m}$ due to the oxidation of pyrite, and absorption at 2.22 $\mu\textrm{m}$ due to OH. 2) The altered rocks have generally higher reflectance than the unaltered rocks. 3) The ratio mesurement of pyrophyllites shows strong absorption at band 5/6 and band 6/4(in Landsat TM 5/7, 7/4). The ratio 1/5(Landsat TM 1/5) may be useful to discriminate andesite from the granite.

Comparison of nano/micro lead, bismuth and tungsten on the gamma shielding properties of the flexible composites against photon in wide energy range (40 keV-662 keV)

  • In the radiation protection application, the metal-polymer composites have been developed for their radiation shielding properties. In this research, the elastomer composites doped by 10 ㎛ and 100nm size of lead, bismuth and tungsten particles as filler with 30 and 60 wt percentages were prepared. To survey the shielding properties of the polymer composites using gamma-ray emitted from 152Eu and 137Cs sources, the gamma flux was measured by using NaI(Tl) detector, then the linear attenuation coefficient was calculated. Also, the Monte Carlo simulation (MCs) method was used. The results showed a direct relationship between the linear attenuation coefficients of the absorbent and filler ratio. Also, the decrease in the particle size of the shielding material in each weight percentage improved the radiation shielding features. When the dimension of the particles was in the order of nano-size, more attenuation was achieved. At low energies used for medical diagnostic X-ray applications due to the predominance of the photoelectric effect, bismuth and lead were suitable selection as filler.

Mechanistic Target of Rapamycin Pathway in Epileptic Disorders

  • The mechanistic target of rapamycin (mTOR) pathway coordinates the metabolic activity of eukaryotic cells through environmental signals, including nutrients, energy, growth factors, and oxygen. In the nervous system, the mTOR pathway regulates fundamental biological processes associated with neural development and neurodegeneration. Intriguingly, genes that constitute the mTOR pathway have been found to be germline and somatic mutation from patients with various epileptic disorders. Hyperactivation of the mTOR pathway due to said mutations has garnered increasing attention as culprits of these conditions : somatic mutations, in particular, in epileptic foci have recently been identified as a major genetic cause of intractable focal epilepsy, such as focal cortical dysplasia. Meanwhile, epilepsy models with aberrant activation of the mTOR pathway have helped elucidate the role of the mTOR pathway in epileptogenesis, and evidence from epilepsy models of human mutations recapitulating the features of epileptic patients has indicated that mTOR inhibitors may be of use in treating epilepsy associated with mutations in mTOR pathway genes. Here, we review recent advances in the molecular and genetic understanding of mTOR signaling in epileptic disorders. In particular, we focus on the development of and limitations to therapies targeting the mTOR pathway to treat epileptic seizures. We also discuss future perspectives on mTOR inhibition therapies and special diagnostic methods for intractable epilepsies caused by brain somatic mutations.

Fault Diagnosis Method based on Feature Residual Values for Industrial Rotor Machines

  • Downtime and malfunction of industrial rotor machines represents a crucial cost burden and productivity loss. Fault diagnosis of this equipment has recently been carried out to detect their fault(s) and cause(s) by using fault classification methods. However, these methods are of limited use in detecting rotor faults because of their hypersensitivity to unexpected and different equipment conditions individually. These limitations tend to affect the accuracy of fault classification since fault-related features calculated from vibration signal are moved to other regions or changed. To improve the limited diagnosis accuracy of existing methods, we propose a new approach for fault diagnosis of rotor machines based on the model generated by supervised learning. Our work is based on feature residual values from vibration signals as fault indices. Our diagnostic model is a robust and flexible process that, once learned from historical data only one time, allows it to apply to different target systems without optimization of algorithms. The performance of the proposed method was evaluated by comparing its results with conventional methods for fault diagnosis of rotor machines. The experimental results show that the proposed method can be used to achieve better fault diagnosis, even when applied to systems with different normal-state signals, scales, and structures, without tuning or the use of a complementary algorithm. The effectiveness of the method was assessed by simulation using various rotor machine models.

First record of Heterorhabdus papilliger(Calanoida, Heterorhabdidae) from Korean waters based on morphological and molecular features

  • Heterorhabdus papilliger (Claus, 1863) is newly reported from the Tsushima Warm Current realm of the southern Korean waters. Its morphological diagnostic characteristics generally agreed well with the original description and the previous records of H. papilliger. The female of H. papilliger can be recognized by the genital somite, which in lateral view has a more or less rounded genital prominence and an uninflated posterior ventral margin; the second exopodal segment of male right leg 5 with the medial projection with a large, rounded, plumose proximal lobe, and a poorly developed distal lobe. The genetic difference for the partial mtCOI gene between Korean specimens and H. papilliger from Spain and Japan of the same clade is 0.4%, while the difference between Korean specimens is 0.5%. However, the interspecific difference for the mtCOI gene between H. papilliger from the Korean waters and the other Heterorhabdus species is in the range of 14.7-20.8%, suggesting that the former is a valid species.
