The fluoroscopy-guided fine needle aspiration biopsy has been gaining widespread acceptance as a rapid and effective method to make a pre-operative diagnosis of mediastinal tumors including thymoma, malignant lymphoma, and metastatic carcinoma. Although thymoma is a most common tumor of the superior mediastinum, most cytopathologists are not experted in cytologic diagnosis of this tumor because of limited experience. In order to define the diagnostic cytologic features of thymoma, we have retrospectively reviewed imprinting smears and corresponding tissue sections from four cases of this tumor. All cases revealed an apparent biphasic pattern of epithelial cell clusters and lymphocytes with occasional branching capillary fronds extending from three dimensional epithelial cell clusters. Epithelial cell clusters predominated in one case and lymphocytes in two cases. Mixed epithelial cell and lymphocyte type represented in one of four cases. In the lymphocyte predominant type, the presence of epithelial cell clusters and small mature lymphocytes are helpful features to differentiate from a malignant lymphoma.

In this study, we propose a novel method to detect and enhance the feature of ST-segment which offers the crucial information for the diagnosis of myocardial infarction and ischemia. With this aim, PQRST features of Electrocardiogram initially are detected and subsequently ST-segment are estimated. And Dynamic Time Warping(DTW) transformation is applied recursively to minimize the difference in time between ST-segments and calculate the minimum cumulative distance that decides the degree of similarity among ST-segments. As of the results, the inherent characteristic of ST-segment can be emphasized in terms of time parameter and thus the diagnostic features of a ST-segment can be revealed further.
Incidental pancreatic cystic lesions are a common challenge encountered by diagnostic radiologists. Specifically, given the prevalence of benign pancreatic cystic lesions, determining when to recommend aggressive actions such as surgical resection or endoscopic ultrasound with sampling is difficult. In this article, we review the common types of cystic pancreatic lesions including serous cystadenoma, intraductal papillary mucinous neoplasm, and mucinous cystic neoplasm with imaging examples of each. We also discuss high-risk or worrisome imaging features that warrant a referral to a surgeon or endoscopist and provid several examples of these features. These imaging features adhere to the latest guidelines from the International Consensus Guidelines, American Gastroenterological Association (2015), American College of Gastroenterology (2018), American College of Radiology (2010, 2017), and European Guidelines (2013, 2018). Our focused article addresses the imaging dilemma of managing incidental cystic pancreatic lesions, weighing the options between imaging follow-up and aggressive interventions.
For the past decade, the full-field digital mammography has been widely used for early diagnosis of breast cancer, and computer aided diagnosis has been developed to assist physicians as a second opinion. In this study, we try to predict the breast cancer using both mediolateral oblique(MLO) view and craniocaudal(CC) view together. A skilled radiologist selected 35 pairs of ROIs from both MLO view and CC view of digital mammogram. We extracted textural features using Spatial Grey Level Dependence matrix from each mammogram and evaluated the generalization performance of the classifier using Support Vector Machine. We compared the multi-view based classifier to single-view based classifier that is built from each mammogram view. The results represent that the multi-view based computer aided diagnosis in digital mammogram could improve the diagnostic performance and have good possibility for clinical use to assist physicians as a second opinion.

Background: In Pakistan thyroid cancer is responsible for 1.2% cases of all malignant tumors. Ultrasonography (US) is helpful in detecting cancerous thyroid nodules on basis of different features like echogenicity, margins, microcalcifications, size, shape and abnormal neck lymph nodes. We therefore aimed to calculate diagnostic accuracy of ultrasound in detection of carcinoma in thyroid nodules taking fine needle aspiration cytology as the reference standard. Materials and Methods: A cross-sectional analytical study was designed to prospectively collect data from December 2010 till December 2012 from the Department of Radiology in Aga Khan University Hospital, Karachi, Pakistan. A total of 100 patients of both genders were enrolled after informed consent via applying non-probability consecutive sampling technique. Patients referred to Radiology department of Aga Khan University to perform thyroid ultrasound followed by fine-needle aspiration cytology of thyroid nodules were included. They were excluded if proven for thyroid malignancy or if their US or FNAC was conducted outside our institution. Results: The subjects comprised 76 (76%) females and 24 males. Mean age was $41.8{\pm}SD$ 12.3 years. Sensitivity and specificity with 95%CI of ultrasound in differentiating malignant thyroid nodule from benign thyroid nodule calculated to be 91.7% (95%CI, 0.72-0.98) and 78.94% (0.68-0.87) respectively. Reported positive predictive value and negative PV were 57.9% (0.41-0.73) and 96.8% (0.88-0.99) and overall accuracy was 82%. Likelihood ratio (LR) positive was computed to be 4.3 and LR negative was 0.1. Conclusions: Ultrasonography has a high diagnostic accuracy in detecting malignancy in thyroid nodules on the basis of features like echogenicity, margins, micro calcifications and shape.

The purpose of this study was to estimate the diagnostic availability of the common periapical lesions by using computer. The author used a domestic personal computer and rearranged the applied program appropriately with RF (Rapid File), a program to answer the purpose of this study, and then input the consequence made out through collection, analysis and classification of the clinical and radiological features about the common periapical lesions as a basic data. The 256 cases (Cyst 91, Periapical granuloma 74, Periapical abscess 91) were obtained from the chart recordings and radiographs of the patients diagnosed or treated under the common periapical lesions during the past 8 years (1983-1990) at the infirmary of Dental School, Chosun University. Next, the clinical and radiographic features of the 256 cases were applied to RF program for diagnosis, and the diagnosis by using computer was compared with the hidden final diagnosis by clinical and histopathological examination. The obtained results were as follows: 1. In cases of the cyst, diagnosis through the computer program was shown rather lower accuracy (80.22%) as compared with accuracy (90.1 %) by the radiologists. In cases of the granuloma, diagnosis through the computer program was shown rather higher accuracy (75.7%) as compared with the accuracy (70.3%) by the radiologists. 2. In cases of periapical abscess, the diagnostic accuracy was shown 88% in both diagnoses. 4. The average diagnostic accuracy of 256 cases through the computer program was shown rather lower accuracy (81.2%) as compared with the accuracy (82.8%) by the radiologists. 5. The applied basic data for radiographic diagnosis of common periapical lesions by using computer was estimated to be available.

In this study, the authors develop a methodology for a diagnostic system with a vibration parameter that is influenced by environmental factors. The data tends to have a varying average over time. Often, these features are found in statistical data retrieved from a production line. If we utilize existing statistical techniques for these features, we could derive an incorrect diagnostic conclusion based on the different average values. To overcome the limitations of previous methods, the authors apply a function analyzed through regression analysis to predict the mean value and corresponding upper and lower limits at each stage. This technique also provides corresponding statistical parameters in varying dynamic means. To validate the proposed methods, we retrieve data from the engine assembly line of H Motors and verify the results.
Background/Objectives: The aim of this study was to investigate the diagnostic value of i-scan in the differential diagnosis of oral cavity leukoplakia based on visualization of abnormal vascular features. Materials & Methods: Thirty- one patients with oral cavity leukoplakia were enrolled in the study. Images of their oral cavity obtained using conventional white light endoscopy and an i-scan-enhanced endoscopy (Pentax DEFINA EPK-3000 Video Processors, with Pentax VNLJ10) were reviewed. The microvascular features of the lesions and vascular changes were analyzed and the results were compared with the histopathologic diagnosis. Results: Among the 31 oral cavity leukoplakia patients, 8 (25.8%) patients revealed hyperkeratosis, 10 (31.2%) low-grade dysplasia, 5 (16.2%) high-grade dysplasia and 8 (25.8%) invasive squamous cell carcinoma on histopathologic examination. Using i-scan-enhanced endoscopy, we could found abnormal vascular change with neoplastic neoangiogenesis in most high-grade dysplasia or invasive cancer in oral cavity. (high-grade dysplasia: 4/5 [80.0%], and invasive squamous cell carcinoma: 7/8 [87.5%]). Conclusion: i-scan-enhanced endoscopy could be a useful optical technique for the diagnosis of oral cavity leukoplakia. Our results suggest that i-scan may be a promising diagnostic tool in the early detection of suspected oral mucosal lesion.
Objective: To evaluate the feasibility of texture analysis on non-contrast-enhanced T1 maps of cardiac magnetic resonance (CMR) imaging for the diagnosis of myocardial injury in acute myocardial infarction (MI). Materials and Methods: This study included 68 patients (57 males and 11 females; mean age, 55.7 ± 10.5 years) with acute ST-segment-elevation MI who had undergone 3T CMR after a percutaneous coronary intervention. Forty patients of them also underwent a 6-month follow-up CMR. The CMR protocol included T2-weighted imaging, T1 mapping, rest first-pass perfusion, and late gadolinium enhancement. Radiomics features were extracted from the T1 maps using open-source software. Radiomics signatures were constructed with the selected strongest features to evaluate the myocardial injury severity and predict the recovery of left ventricular (LV) longitudinal systolic myocardial contractility. Results: A total of 1088 segments of the acute CMR images were analyzed; 103 (9.5%) segments showed microvascular obstruction (MVO), and 557 (51.2%) segments showed MI. A total of 640 segments were included in the 6-month follow-up analysis, of which 160 (25.0%) segments showed favorable recovery of LV longitudinal systolic myocardial contractility. Combined radiomics signature and T1 values resulted in a higher diagnostic performance for MVO compared to T1 values alone (area under the curve [AUC] in the training set; 0.88, 0.72, p = 0.031: AUC in the test set; 0.86, 0.71, p = 0.002). Combined radiomics signature and T1 values also provided a higher predictive value for LV longitudinal systolic myocardial contractility recovery compared to T1 values (AUC in the training set; 0.76, 0.55, p < 0.001: AUC in the test set; 0.77, 0.60, p < 0.001). Conclusion: The combination of radiomics of non-contrast-enhanced T1 mapping and T1 values could provide higher diagnostic accuracy for MVO. Radiomics also provides incremental value in the prediction of LV longitudinal systolic myocardial contractility at six months.
According to living environment, the number of deaths due to thyroid diseases increased. In this paper, we proposed an algorithm for recognizing a thyroid detection using texture analysis based on shape, gray level co-occurrence matrix and gray level run length matrix. First of all, we segmented the region of interest (ROI) using active contour model algorithm. Then, we applied a total of 18 features (5 first order descriptors, 10 Gray level co-occurrence matrix features(GLCM), 2 Gray level run length matrix features and shape feature) to each thyroid region of interest. The extracted features are used as statistical analysis. Our results show that first order statistics (Skewness, Entropy, Energy, Smoothness), GLCM (Correlation, Contrast, Energy, Entropy, Difference variance, Difference Entropy, Homogeneity, Maximum Probability, Sum average, Sum entropy), GLRLM features and shape feature helped to distinguish thyroid benign and malignant. This algorithm will be helpful to diagnose of thyroid nodule on ultrasound images.
