  • Urinary incontinence is the most common urinary symptom in children and causes considerable anxiety in children and parents. In most cases, there is no underlying organic pathology and the longterm prognosis is excellent. Despite these reassuring facts, children with wetting problems can be a challenge for primary pediatrician. This is probably because the problem is poorly understood, there is no readily identifiable medical pathology, and because treatment is usually time consuming and arduous. There is a great demand for treatment because wetting is an unpleasant symptom that can cause a stress and anxiety in the family. There may also be other coexisting problems such as urinary tract infection, constipation, soiling, and behavioral or emotional difficulties. Despite the frequency and vexing nature of voiding dysfunction, physicians may not always obtain a careful history to identify and to treat children properly with this condition. This article addresses the comprehensive diagnostic and therapeutic approach to such children.

Spinal Joint Pain Syndrome (척추관절통증증후군)

  • Spinal joint pain syndrome is composed of atlanto-occipital, atlanto-axial, facet, and sacro-iliac joints pain. The syndrome is characterized as referred pain which is originated from deep somatic tissues, which is quietly different from radicular pain with dermatomal distribution originated from nerve root ganglion. The prevalence of facet joint pain in patients with chronic spinal pain of cervical, thoracic, and lumbar regions has been known 56%, 42%, and 31% as in order. It is generally accepted in clinical practice that diagnostic blocks are the most reliable means for diagnosing spinal joints as pain generators. The sacroiliac joint has been shown to be a source of 10% to 27% of suspected cases with chronic low back pain utilizing controlled comparative local anesthetic blocks. The treatment of spinal joints ideally consists of a multimodal approach comprising conservative therapy, medical management, procedural interventions, and if indicated.

  • The equipment and techniques associated with magnetic resonance imaging (MRI) have rapidly evolved. The development of 3.0 Tesla MRI has enabled high-resolution imaging of the intracranial vessel wall. High-resolution MRI (HRMRI) can yield excellent visualization of both the arterial wall and lumen, thus facilitating the detection of the primary and secondary features of intracranial arterial dissection. In the present report, we describe the manner in which HRMRI affected our endovascular treatment planning strategy in 2 cases with unruptured intracranial vertebral artery dissection aneurysm. HRMRI provides further information about the vessel wall and the lumen of the unruptured intracranial vertebral artery dissecting aneurysm, which was treated by an endovascular approach in the 2 current cases.

A study on fault diagnosis for chemical processes using hybrid approach of quantitative and qualitative method (정성적, 정량적 기법의 혼합 전략을 통한 화학공정의 이상진단에 관한 연구)

  • This paper presents a fault detection and diagnosis methodologies based on weighted symptom model and pattern matching between the coming fault propagation trend and the simulated one. At the first step, backward chaining is used to find the possible cause candidates for the faults. The weighted symptom model(WSM) is used to generate those candidates. The weight is determined from dynamic simulation. Using WSMs, the methodology can generate the cause candidates and rank them according to the probability. Secondly, the fault propagation trends identified from the partial or complete sequence of measurements are compared to the standard fault propagation trends stored a priori. A pattern matching algorithm based on a number of triangular episodes is used to effectively match those trends. The standard trends have been generated using dynamic simulation and stored a priori. The proposed methodology has been illustrated using two case studies and showed satisfactory diagnostic resolution.

  • 지난 수십 년 간 연구자들은 효과적인 진료지원시스템을 개발하기 위해 다양한 도구와 방법론들을 제안하였고 지금도 새로운 방법론과 도구들을 계속적으로 개발하고 있다. 그 중에서 흉통으로 응급실에 내원한 노인환자에 대한 정확한 진단은 중요한 이슈 중의 하나였다. 따라서 많은 연구자들이 의사의 진단 능력을 향상시키기 위한 지능적인 의료의사결정과 시스템 개발에 투신하고 있지만 전통적인 의료시스템에 따른 대부분의 진료의사결정이 단일 분류기(classifier)에 기반하고 있어 만족스런 성능을 보여주지 못하고 있는 것이 현실이다. 따라서 이 논문은 앙상블 전략을 활용하여 의사들이 노인환자들의 흉통을 더 정확하고 빠르게 진단하는데 있어 도움을 줄 수 있게 하였다. 의사결정나무, 인공신경망, SVM 모델을 결합한 앙상블 기법을 실제 응급실에서 수집한 응급실 자료에 적용하였고, 그 결과 단일 분류기를 사용하는 것에 비해 월등히 향상된 진단 성과를 보이는 것을 관찰 할 수 있었다.

LINE EMISSION FROM THE MAGNETOSPHERE OF MAGNETIC CATACLYSMIC VARIABLES (MCV 자기구에서의 선방출)

  • A magnetic cataclysmic variable has a rotating magnetic white dwarf which accretes matter from its late type companion. Kim & Beuermann (1995) presented a phenomenological model of the accretion from its surrounding structure e.g., a disk into the magnetosphere of the white dwarf, and presented results for the spin modulated X-ray spectrum and light curves. Using this model, we calculate the optical continuum and line emission which result from reprocessing of X-rays in the accretion stream within the magnetosphere. Penning (1985) suggested the observed spin-modulated radial-velocity variations might result from reprocession of X-rays in the disk. We, however, find the radiation can be originated from the magnetosphere accretion stream. We use the same geometrical model to calculate the optical and the X-ray behaviour. The results from the two wavelength bands are internally consistent. We conclude that this approach will increase the diagnostic accuracies of the results.

  • Recently, more and more clinical cases of peri-implantitis for which surgical therapy is indicated have been reported. Ideal diagnostic modalities, prevelance, indicated therapy, and prognosis for peri-implantitis are still investigated. Scientific evidence of surgical therapy for peri-implantitis are not enough for now, and should be obtained from well-designed, long-term clinical studies in the future. When the surgical approach is applied for peri-implantitis, long-term prognosis of the surgical therapy should be kept in mind and importance of maintenance therapy also have to be emphasized.

  • Recently, the demand for fuel efficient electric vehicles (EVs) and hybrid electric vehicles (HEVs) has been growing globally. Due to the increased number of switching devices in the electrified vehicles, the probability of the semiconductor device failure is much higher than in other application areas. A sudden failure in one of the power switches and insufficient power management ability in the systems not only decreases system performance, but also leads to critical safety problems. In this paper, novel switch open circuit fault detection method is proposed, and the proposed approach is verified by experiments.

  • Diffuse alveolar hemorrhage (DAH) is a life-threatening and medical emergency that can be caused by numerous disorders and presents with hemoptysis, anemia, and diffuse alveolar infiltrates. Early bronchoscopy with bronchoalveolar lavage is usually required to confirm the diagnosis and rule out infection. Most cases of DAH are caused by capillaritis associated with systemic autoimmune diseases such as anti-neutrophil cytoplasmic antibody-associated vasculitis, anti-glomerular basement membrane disease, and systemic lupus erythematosus, but DAH may also result from coagulation disorders, drugs, inhaled toxins, or transplantation. The diagnosis of DAH relies on clinical suspicion combined with laboratory, radiologic, and pathologic findings. Early recognition is crucial, because prompt diagnosis and treatment is necessary for survival. Corticosteroids and immunosuppressive agents remain the gold standard. In patients with DAH, biopsy of involved sites can help to identify the cause and to direct therapy. This article aims to provide a general review of the causes and clinical presentation of DAH and to recommend a diagnostic approach and a management plan for the most common causes.

The developmental biology of birth defect (선천성 기형의 발달생리학)

  • Knowledge of developmental biology is essential for clinicians who seek to develop a rational approach to the diagnostic evaluation of patients with birth defects. After an accurate diagnosis, a clinician can make predictions about prognosis, recommend management options, and provide an indication of recurrence risk for the parents and relatives. In this paper, we first review the basic mechanisms of embryological development and clinical dysmorphology. We then review cellular and molecular mechanisms in development and related congenital anomalies. Developmental anomalies have a major impact on public health. Genetic counseling and prenatal diagnosis, with the option to continue or to terminate a pregnancy, are important for helping families faced with the risk of a serious congenital anomaly in their offspring. Moreover, primary prevention of birth defects, for example, supplementation of prenatal folic acid and prevention of consumption of alcohol which has teratogenic effects, can be accomplished using developmental biology knowledge.
