Bone regeneration and graft material resorption in extraction sockets grafted with bioactive silica-calcium phosphate composite (SCPC) versus non-grafted sockets: clinical, radiographic, and histological findings

  • Purpose: The purpose of the present study was to evaluate the effect of silica-calcium phosphate composite (SCPC) granules on bone regeneration in extraction sockets. Methods: Ten patients were selected for a split-model study. In each patient, bone healing in SCPC-grafted and control ungrafted sockets was analyzed through clinical, radiographic, histomorphometric, and immunohistochemical assessments 6 months postoperatively. Results: A radiographic assessment using cone-beam computed tomography showed minimal ridge dimension changes in SCPC-grafted sockets, with 0.39 mm and 1.79 mm decreases in height and width, respectively. Core bone biopsy samples were obtained 6 months post-extraction during implant placement and analyzed. The average percent areas occupied by mature bone, woven bone, and remnant particles in the SCPC-grafted sockets were 41.3%±12%, 20.1%±9.5%, and 5.3%±4.4%, respectively. The percent areas of mature bone and woven bone formed in the control ungrafted sockets at the same time point were 31%±14% and 24.1%±9.4%, respectively. Histochemical and immunohistochemical analyses showed dense mineralized bundles of type I collagen with high osteopontin expression intensity in the grafted sockets. The newly formed bone was well vascularized, with numerous active osteoblasts, Haversian systems, and osteocytes indicating maturation. In contrast, the new bone in the control ungrafted sockets was immature, rich in type III collagen, and had a low osteocyte density. Conclusions: The resorption of SCPC granules in 6 months was coordinated with better new bone formation than was observed in untreated sockets. SCPC is a resorbable bone graft material that enhances bone formation and maturation through its stimulatory effect on bone cell function.

  • Domestic maintenance strategies are shifting from safety assessment to performance assessment, and related systems and laws are being restructured to meet those criteria. In order to introduce asset management based on performance evaluation, related evaluation methods such as performance measures and level of service should be made to evaluate performance according to the characteristics of the structure, but these are not well-prepared in Korea. In this study, we present a computerized model and system for implementing asset management that introduces techniques such as performance evaluation, life cycle cost analysis, performance measures, and level of service in conjunction with existing maintenance and safety diagnosis procedures. The features of this system consist of three modules to enable separate operations of existing maintenance, safety management and asset management. The system is designed to be used as a reference for public institutions to introduce asset management in the future.

Comparison of radiographic and computed tomographic acetabular index in small-breed dogs: a preliminary study using Maltese and Shih Tzu

  • Background: The morphometry of the acetabulum is one source of information that assists in the clinical diagnosis of the hip and influences the proper selection of a prosthesis, reducing post-operative complications such as those seen in total hip replacement (THR). However, acetabular parameters in small-breed dogs are rarely reported. Objectives: To investigate acetabular parameters using radiography and computed tomography (CT) in small-breed dogs with Maltese and Shih Tzu dogs used as model breeds. Methods: Standard calibrated, extended hip radiographs and CT images were obtained. Subsequently, acetabular width (AW) in various directions was measured using radiography and CT, whereas acetabular depth (AD) was obtained by CT. Acetabular index (AI) is a ratio calculated from AD and AW. Results: The values of AW and AD were much higher in Shih Tzu than in Maltese dogs. Male Shih Tzus showed higher values of these parameters than females, while sex-based differences in most of the parameters could not be detected in Maltese. Body weight, but not age, influenced AWs and ADs. While AWs and ADs were influenced by several factors, AI was comparable among the assessed factors and between Maltese and Shih Tzu dogs (p = 0.172; 31.42 ± 1.35 and 32.60 ± 1.80, respectively). Also, AI did not vary with breed, sex, or body size. Conclusions: The obtained radiographic and CT acetabular parameters could be useful as guidelines for evaluating the acetabulum of small-breed dogs in clinical practice.

Treatment Costs and Factors Associated with Glycemic Control among Patients with Diabetes in the United Arab Emirates

  • Background: We aimed to estimate the proportion of patients with diabetes who achieved target glycemic control, to estimate diabetes-related costs attributable to poor control, and to identify factors associated with them in the United Arab Emirates. Methods: This retrospective cohort study used administrative claims data handled by Abu Dhabi Health Authority (January 2010 to June 2012) to determine glycemic control and diabetes-related treatment costs. A total of 4,058 patients were matched using propensity scores to eliminate selection bias between patients with glycosylated hemoglobin (HbA1c) <7% and HbA1c ${\geq}7%$. Diabetes-related costs attributable to poor control were estimated using a recycled prediction method. Factors associated with glycemic control were investigated using logistic regression and factors associated with these costs were identified using a generalized linear model. Results: During the 1-year follow-up period, 46.6% of the patients achieved HbA1c <7%. Older age, female sex, better insurance coverage, non-use of insulin in the index diagnosis month, and non-use of antidiabetic medications during the follow-up period were significantly associated with improved glycemic control. The mean diabetes-related annual costs were $2,282 and $2,667 for patients with and without glycemic control, respectively, and the cost attributable to poor glycemic control was $172 (95% confidence interval [CI], $164-180). The diabetes-related costs were lower with mean HbA1c levels <7% (cost ratio, 0.94; 95% CI, 0.88-0.99). The costs were significantly higher in patients aged ${\geq}65$ years than those aged ${\leq}44$ years (cost ratio, 1.45; 95% CI, 1.25-1.70). Conclusion: More than 50% of patients with diabetes had poorly controlled HbA1c. Poor glycemic control may increase diabetes-related costs.

Factors Affecting Breast Self-examination Behavior Among Female Healthcare Workers in Iran: The Role of Social Support Theory

  • Objectives: In women, breast cancer is the most common cancer and the leading cause of cancer death. Screening tests are the basis for early diagnosis. In Iranian women, the mortality rate of breast cancer is high due to insufficient screening examinations and delayed visits for care. Therefore, this study aimed to determine the factors affecting breast self-examinations among Iranian women employed in medical careers. Methods: This cross-sectional study included 501 women working in the medical professions at Hamadan University of Medical Sciences in western Iran in 2018. The subjects were selected by stratified random sampling. Data were collected using a researcher-developed, self-report questionnaire that contained demographic information and questions based on protection motivation theory and social support theory. Descriptive data analysis was conducted using SPSS version 23 and model fitting with PLS version 2. Results: The mean age of the participants was $37.1{\pm}8.3years$, and most of the women (80.4%) were married. Most women had a bachelor's degree (67.5%). The findings of this study showed that the coping appraisal construct was a predictor of protection motivation (${\beta}=0.380$, p<0.05), and protection motivation (${\beta}=0.604$, p<0.05) was a predictor of breast self-examination behavior. Additionally, social support theory (${\beta}=0.202$, p<0.05) had a significant positive effect on breast self-examination behavior. Conclusions: The frequency of practicing self-examinations among women employees in the medical sector was low; considering the influence of social support as a factor promoting screening, it is necessary to pay attention to influential people in women's lives when designing educational interventions.

Endothelial dysfunction induces atherosclerosis: increased aggrecan expression promotes apoptosis in vascular smooth muscle cells

  • Endothelial dysfunction-induced lipid retention is an early feature of atherosclerotic lesion formation. Apoptosis of vascular smooth muscle cells (VSMCs) is one of the major modulating factors of atherogenesis, which accelerates atherosclerosis progression by causing plaque destabilization and rupture. However, the mechanism underlying VSMC apoptosis mediated by endothelial dysfunction in relation to atherosclerosis remains elusive. In this study, we reveal differential expression of several genes related to lipid retention and apoptosis, in conjunction with atherosclerosis, by utilizing a genetic mouse model of endothelial nitric oxide synthase (eNOS) deficiency manifesting endothelial dysfunction. Moreover, eNOS deficiency led to the enhanced susceptibility against pro-apoptotic insult in VSMCs. In particular, the expression of aggrecan, a major proteoglycan, was elevated in aortic tissue of eNOS deficient mice compared to wild type mice, and administration of aggrecan induced apoptosis in VSMCs. This suggests that eNOS deficiency may elevate aggrecan expression, which promotes apoptosis in VSMC, thereby contributing to atherosclerosis progression. These results may facilitate the development of novel approaches for improving the diagnosis or treatment of atherosclerosis.

  • During the last two decades, much joint research regarding vibration based methods has been done, leading to developing various algorithms and techniques. These algorithms and techniques can be divided into modal methods and signal methods. Although modal methods have been widely used for health monitoring and damage detection, signal methods due to higher efficiency have received considerable attention in various fields, including aerospace, mechanical and civil engineering. Signal-based methods are derived directly from the recorded responses through signal processing algorithms to detect damage. According to different signal processing techniques, signal-based methods can be divided into three categories including time domain methods, frequency domain methods, and time-frequency domain methods. The frequency domain methods are well-known and interest in using them has increased in recent years. To determine dynamic behaviours, to identify systems and to detect damages of bridges, different methods and algorithms have been proposed by researchers. In this study, a new algorithm to detect seismic damage in the bridge's piers is suggested. To evaluate the algorithm, an analytical model of a bridge with simple spans is used. Based on the algorithm, before and after damage, the bridge is excited by a sine force, and the piers' responses are measured. The dynamic specifications of the bridge are extracted by Power Spectral Density function. In addition, the Least Square Method is used to detect damage in the bridge's piers. The results indicate that the proposed algorithm can identify the seismic damage effectively. The algorithm is output-only method and measuring the excitation force is not needed. Moreover, the proposed approach does not need numerical models.

Association between Korean Medicine Hospital Utilization and Cardiovascular Risks in Patients with Hypertension: a National Korean Cohort Study (고혈압 환자에서 한방의료기관 이용과 심혈관 위험 요소와의 관계: 국민건강보험공단 표본코호트 DB)

  • Objectives: This study aims to investigate the effects of Korean Medicine Hospital Utilization (KMHU) on major adverse cardiovascular events (MACE), myocardial infarction (MI), stroke, and death in hypertensive patients taking antihypertensives. Methods: Using the Korean National Health Insurance Service-National Sample Cohort database, this study identified and diagnosed 68,457 hypertensive patients taking antihypertensives between 2003 and 2006. They were divided into KMHU and non-KMHU groups. The follow-up period ended with the diagnosis of myocardial infarction, stroke, or death. After propensity score matching (PSM), there were 18,242 patients each in the non-KMHU and KMHU groups. We calculated the incidence rate, hazard ratio (HR), and 95% confidence interval (CI) for MACE, myocardial infarction, stroke, and death in patients with hypertension using a stratified Cox proportional hazard model. In addition, secondary outcome analyses for stroke and cardiovascular mortality were performed. Results: After PSM, the HRs for MACE (HR: 0.84, 95% CI: 0.81-0.87), all-cause mortality (HR: 0.75, 95% CI: 0.72-0.79), and myocardial infarction (HR: 0.90, 95% CI: 0.83-0.97) were significantly lower in the KMHU group than in the non-KMHU group. Moreover, the HRs for stroke-related mortality, haemorrhage and ischaemic stroke-related mortality, and ischaemic heart disease-related and circulatory system disease-related mortality were significantly lower in the KMHU group than in the non-KMHU group. Conclusions: On long-term follow-up observation, this study supported the effect of KMHU for managing hypertension and reducing the burden of cardiovascular diseases.

Decision-Tree Analysis to Predict Blood Pressure Control Status Among Hypertension Patients Taking Antihypertensive Medications (약물복용 중인 고혈압 환자의 혈압관리양상 예측을 위한 의사결정나무분석)

  • Purpose: This study was performed to analyze the levels of blood pressure and to identify good or poor blood pressure control (BPC) groups among hypertension patients. The study was based on the Korea National Health and Nutrition Examination Survey (KNHANES VI and VII) conducted from 2013 to 2016. Methods: The sociodemographic and clinical data of 4,151 Korean hypertension patients aged 20-79 years and who were taking antihypertensive medications was extracted from the KNHANES VI and VII database. Descriptive statistics for complex samples and a decision-tree analysis were performed using the SPSS WIN 24.0 program. Results: The mean age was $62.46{\pm}0.21years$. The mean systolic blood pressure (SBP) was $128.07{\pm}0.28mmHg$, and the diastolic blood pressure (DBP) was $76.99{\pm}0.21mmHg$. 71.9% of participants showed normal blood pressure (SBP < 140mmHg and DBP < 90mmHg). From the decisiontrees analysis, the characteristics of participants related to good BPC group were presented with 9 different pathways same as those from the poor BPC group. Good or poor BPC groups were classified according to the patients' characteristics such as age, living status, occupation, education, hypertension diagnosis period, numbers of comorbidity, perceived health status, total cholesterol, high density lipoprotein-cholesterol, alcohol drinking per month, and depressive mood. Total cholesterol level (< 201mg/dL or ${\geq}201mg/dL$ cutoff point) was the most significant predictor of the participants' BPC group. Conclusion: This decision-tree model with the 18 different pathways can form a basis for the screening of hypertension patients with good or poor BPC in either clinical or community settings.

CD166 promotes the cancer stem-like properties of primary epithelial ovarian cancer cells

  • Cancer stem cells (CSCs) or tumor-initiating cells are thought to play critical roles in tumorigenesis, metastasis, drug resistance, and tumor recurrence. For the diagnosis and targeted therapy of CSCs, the molecular identity of biomarkers or therapeutic targets for CSCs needs to be clarified. In this study, we identified CD166 as a novel marker expressed in the sphere-forming CSC population of A2780 epithelial ovarian cancer cells and primary ovarian cancer cells. The CD166+ cells isolated from A2780 cells and primary ovarian cancer cells highly expressed CSC markers, including ALDH1a1, OCT4, and SOX2, and ABC transporters, which are implicated in the drug resistance of CSCs. The CD166+ cells exhibited enhanced CSC-like properties, such as increased sphere-forming ability, cell migration and adhesion abilities, resistance to conventional anticancer drugs, and high tumorigenic potential in a xenograft mouse model. Knockdown of CD166 expression in the sphere-forming ovarian CSCs abrogated their CSC-like properties. Moreover, silencing of CD166 expression in the sphere-forming CSCs suppressed the phosphorylation of focal adhesion kinase, paxillin, and SRC. These results suggest that CD166 plays a key role in the regulation of CSC-like properties and focal adhesion kinase signaling in ovarian cancer.
