A Clinical Study on Characteristics of Pediatric Patients with Halitosis (소아.청소년 구취 환자의 임상적 특징에 대한 연구)

  • Objectives Halitosis affects a large proportion of the population, and it may cause a significant social or psychological problems but pathophysiological mechanism of halitosis is unclear. The purpose of this study is to investigate pediatric halitosis patients' characteristics and their related symptoms. Methods 133 patients were participated at the Pediatric Clinic of Kyanhee Oriental Medical Center from May 29th, 2010 to July 10th, 2010. We measured sulfide level and reviewed medical charts and asked some questions. Patients of sex, age, respiratory symptoms, digestive symptoms, systemic symptoms and sulfide level were analyzed. Results Stuffy nose, cough after sleep, sputum, snoring, indigestion, stomachache, motion sickness, and Bi-We-Soo diagnosis grades were significantly associated with halitosis Conclusions Based on data, we found that halitosis is related to the respiratory and digestive symptoms. Also, we can refer this study when diagnosing patients or giving treatments to patients.

A Case of Bilateral Sudden Sensorineural Hearing Loss with Incomplete Recovery by Oriental and Western Medical therapy (한양방 동시 치료를 통하여 회복되지 않은 양측성 돌발성 난청 환자 1례)

  • Back ground & Objectives : Bilateral sudden sensorineural hearing loss(SSNHL) is very rare disease. This is the first case report in the oriental medical literature. This study describes a case of bilateral SSNHL with incomplete recovery. Methods : We treated 1 patient who had SSNHL sequentially in both ears. He received oriental and western medical treatments for more than 2 weeks and measured the hearing recovery. For diagnosis on this case, pure tone audiometry was used. The degree of improvement was evaluated by Siegel's Criteria. Results & Conclusions : This patient discharged without hearing improvement. For bilateral SSNHL patients, the result of treatment is important for their qualities of life. But the recovery rate of hearing in bilateral SSNHL is lower than unilateral SSNHL. The oriental medical therapy had some theraputic effects on SSNHL and the theraputic effects on bilateral SSNHL requires further study.

A Clinical Analysis on Outpatients with Ear Diseases of Korean Medicine Ophthalmology, Otolaryngology & Dermatology Department at Daejeon Korean Medicine Hospital - From March, 2013 to February, 2019- (대전한방병원 한방안이비인후피부과로 내원한 귀 질환 환자 특성 분석 -2012년 3월부터 2019년 2월까지-)

  • Objectives : This study is designed to analyze the outpatients who received medical treatments for ear diseases in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean Medical Hospital from March, 2013 to February, 2019. Methods : We classified the otology outpatients who received medical treatments in ophthalmology & otolaryngology & dermatology clinic of Daejeon Korean Medical Hospital from March, 2012 to February, 2019 according to gender, year, age, season, anatomical parts, and main diagnosis. The statistical analysis was performed using SPSS 25.0. Results : 1. The number of outpatients with ear diseases were 880; 505 female and 375 male. The number of female patients were approximately 1.7 times higher. 2. The average age of the outpatients with ear diseases were 50.0. The number of middle-aged (40-50s) patients were 439, which is almost half the number(49.9%) of the whole patients. 3. By analyzing the number of patients classified by year, we found out that the number of the outpatients with ear diseases has increased gradually for the past 7 years. 4. In the analysis of the number of patients classified by the anatomical parts of the ear, the inner ear diseases group were the largest, amounting to 79.3% of the whole patients. 5. In the analysis of the number of patients classified by main diagnosis, Tinnitus turns out to be the largest group with 338 patients, followed by sudden sensorineural hearing loss, dizziness, otalgia, Meniere's syndrome, otitis, auditory tube dysfunction, BPPV. 6. No statistical-significant difference were shown in the analysis of the number of patients classified by season. Conclusions : It turns out that patients with ear diseases had increased by the years, and middle aged patients (40-50s) were the largest among the age groups. Most of the patients came for the inner-ear diseases, and tinnitus, sudden sensorineural hearing loss took up the largest proportions.

A Study on the Change of Digital Infrared Thermographic Imaging and Skin Resistance Variability before and after Treatment of Dysmenorrhea (월경통 치료 전과 후의 적외선체열영상 및 피부저항변이도 변화에 대한 연구)

  • Purpose: The aim of this study was to examine the effect of oriental diagnosis and treatment for dysmenorrhea patients on lower abdominal temperature and Skin Resistance Variability(SRV). Methods: We studied twenty-five dysmenorrhea patients, who visited Oriental Medicine Hospital of ${\bigcirc}{\bigcirc}$ University for 10 months(Sep. 2011~Jun. 2012). Patients with organic disease such as endometriosis, uterine myoma and pelvic inflammatory disease were excluded from this study. We prescribed 3 types of treatments, herbal medicine, acupuncture, and moxibustion, for one menstrual cycle. Digital Infrared Thermographic Imaging(DITI), 7-Zone-Diagnostic System(CP-6000A), VAS(Visual analog scale) and MVRS(Multidimensional verbal rating scale) were conducted before and after the treatment. The result of DITI, 7-Zone-Diagnostic System, VAS and MVRS before and after treatment were compared in order to objectively assess the therapeutic effect. Results: When we compared the results of DITI, CV4 temperature after treatment was increased compared with that of before, and temperature difference between CV12 and CV4 after treatment was decreased compared with that of before. But there was no significant difference. When we compared the results of 7-Zone-Diagnostic System at 1st and 2nd trial, the height of 1~5 area was lower than that of before. And at 1st and 2nd trial, the height of 6, 7 area was higher than that of before, but there were no significant difference in most. When we compared the results of VAS and MVRS, VAS and MVRS after treatment was significantly decreased compared with that of before(p<0.01). Conclusions: This study shows that oriental diagnosis and treatment for dysmenorrhea patients affects the results of DITI. Further study will be needed.

Four Cases of Bell's Palsy Patients during Pregnancy Treated with Sasang Constitutional Medicine (사상방을 활용한 임신 중 구안괘사(口眼喎斜) 치험 4례)

  • Purpose: This study was aimed to report the effect of oriental treatment on Bell's palsy during pregnancy with Sasang Constitutional method. Methods: 4 patients who had Bell's palsy during pregnancy were treated with Sasang constitutional prescriptions. After treatments we evaluated the effects by House-Brackmann facial nerve grading system. Results: As a result, the clinical symptoms of Bell's palsy during pregnancy were improved. Conclusions: This case studies show that Sasang constitutional method can be effectively used on Bell's palsy during pregnancy. Especially the diagnostic results by using Diagnosis System of Oriental Medicine (DSOM) revealed that some of patients were in a very weak condition. We could apply these results to choose prescription and get good therapeutic effects.

The Trend in Clinical Study on Atopic Dermatitis Over the Last 3 Years (아토피 피부염 임상 연구의 최근 3년간 동향)

  • Objective : To observe the trend in clinical study on atopic dermatitis(AD) over the last 3 years in order to develop a study methodology of AD in Oriental Medicine. Methods : Using the Pub med on line site; search terms: atopic dermatitis, field as title/abstract, limitation as published in the last 3 years, only items with links to full text, Humans, Clinical Trial, English. I observed the study contents of all this research and focused on the classification of treatments. I also observed of AD clinical trials registered on a clinical trial site(www.clinicaltrial.gov) on 23rd of June, 2007: study contents, localization and study designs. Results : Through the Pub Med site, I found 169 articles. Classified according to study subject, the studies related to treatment were 114(67.5%); physiology, pathology and prevention 12(7.1%); Diagnosis and Evaluation(7.1%); psychological aspects including quality of life 10(5.9%); diet and management 10(5.9%); epidemiology 7(4.1%); and others 7(4.1%). However, the treatment study of herb-medicine as intervention showed only 1 article. Futhermore, it wasn't based on Oriental Medicine philosophy. In the clinical trial site, there were 31 studies in which patients were recruited or planned for the future. The study on efficacy and safety of the medicine produced 14 trials, 5 comparative trials, 2 phototherapy, 2 diagnosis, 6 physiology and pathology, and 2 epidemiology. The trial institutions were concentrated in U.S.A. Conclusion : I suggest we'd better try to make a good clinical guideline and standardization of diagnosis and herb-medicine in order to develop a clinical study methodology of AD as soon as possible in the future. Even though it 's very hard to find the study methodology, we should aim to achieve positive results and show the evidence of the efficacy and safety of herb-medicine treatment for AD using Oriental Medicine.

A Review of Clinical Shoulder Assessment Scales (견관절의 임상적 평가도구에 관한 고찰)

  • Objectives : A number of instruments have been developed to measure the quality of life in patients with various shoulder disorders. Much progress has been made in this area, and currently an appropriate instrument exists for each shoulder state. The purpose of this study is to review the instruments that are currently in use for assessing the shoulder joint. Methods : A literature research was performed to choose appropriate scales for assessment of function and the disability of the shoulder. Theoretically based scales were selected for review. Therefore, 11 scales were reviewed. The status of scales involved in shoulder treatment of acupuncture throughout several countries was evaluated. Results : 11 scales: The American shoulder and elbow surgeons evaluation form(ASES), Constant Shoulder Score, The disabilities of the arm, shoulder and hand(DASH), Shoulder Disability Questionnaire (SDQ), The Shoulder Pain And Disability index(SPADI), The simple shoulder test(SST), Oxford Shoulder Questionnaire(OSQ), The Rotator Cuff quality-of-life Measure(RC-QoL), Western Ontario Shoulder Instability Index(WOSI), Western Ontario Osteoarthritis of the Shoulder Index(WOOS), Western Ontario Rotator Cuff Index(WORC), wereevaluated. Each measurement has its own composition and characteristics. Their validity, reliability, responsiveness and practical characteristics were already evaluated. We found 3 domestic and 10 overseas papers about shoulder treatments using acupuncture assessed with shoulder scales. Conclusions : In clinical research, the selection of the measurement scale should take account of the condition of disease, the patient's traits and the characteristics of the research. Moreover, appropriate scales, which havevalidity, reliability, responsiveness and practical characteristics, are needed to enhance the quality of research.

  • Objects : This study propose to compare on general characteristics of acute stroke patients between Fire-Heat group and Deficiency of Yin group. Method : We recruited acute stroke patient within 4 weeks after stroke admitted to Department of Internal Medicine of Kyunghee University Oriental Medicial Center, Dongguk University Ilsan Oriental Medical Center, Kyungwon University Songpa Oriental Medical Centar and Kyungwon University Incheon Oriental Medical Center from April 2007 to August 2009. We investigate age, sex, type of stroke, past history, general characteristics, smoking, drinking alcohol, marriage sasang constitution, etc. Result : It was significant different in sex, smoking, dringking alcohol and marriage between Fire-Heat group and Deficiency Yin group. We need more cases for another result that could associate with diagnosis of stroke. Conclusion : We found that Fire heat group was more associated with lifestyle than deficiency yin group. Further study will be needed to analysis for the diagnosis and the treatments of stroke.

  • Background: Liu-Yuan-Lei(陸淵雷) said that a medical record is both the marks of treatments and arts made by a excellent practitioner and the essence of TCM(Traditional Chinese Medicine). Jiang-Guan(江瓘) also said that reading medical records is one of the best way to develop one’s abilities If curing a disease without perfect clinical practice. Objectives: study on the special treatment about metrostaxis(崩漏) based on the Yi-Da-Gan(易大艮)’s medical records. and study on the differentiation of abnormal symptoms and signs about cold syndrome with pesudo-heat(眞寒假熱) based on the Yu-Chang(喩昌)'s medical records. Methods: First, read and study the medical records on metrostaxis(崩漏) of that made a profound study by Yi-Da-Gan(易大艮) and cold syndrome with pesudo-heat(眞寒假熱) of that made a profound study by Yu-Chang(喩昌). The next, write a paper on results and conclusions. Results and Conclusions: First, Yi-Da-Gan(易大艮) insist that must control the Qi under the blood disease conditions, taking the case of metrostaxis(崩漏). Secondly, we must study more on estimating the changing condition of Qi and the blood as time goes by, also study on the pulse and pulse condition in the four seasons(四時脈). Thirdly, Yu-Chang(喩昌) insist that be more careful in differentiation of symptoms and signs, taking the case of cold syndrome with pesudo-heat(眞寒假熱). Fourthly, Yu-Chang(喩昌) give an example that in condition of cold syndrome with pesudo-heat(眞寒假熱), sometimes, the pulse and pulse condition can be strong.

The Effect of a Cooperative System of Oriental and Western Medicine in the Treatment of Allergic Rhinitis

  • Background and Purpose : Allergic rhinitis is a well-known, relatively controllable chronic disease. Although a number of methods for treating allergic rhinitis have been tried, many patients have not been satisfied with their treatment. Therefore, this study tried to evaluate the effect of a cooperative system of Oriental and Western medicine and to develop a new diagnosis protocol for treatment of allergic rhinitis. Methods : We measured improvement rate and acoustic rhinometry after the allergeninduction test and performed a filter paper test as a nonspecific hypersensitivity test with 60 patients who are allergic to house dust mite. Patients were divided into two groups, one treated with Western medicine only and one treated with both Western and Oriental herbal medicine. For the group with Western medicine only, antihistamine for one week and local steroid medicine for two weeks were prescribed. For the group with combined medicine, Oriental herbal medicine was prescribed according to the patient s constitution, along with Western medicine. After all treatments, the above tests were re-performed and the improvement rate was compared. Results and Conclusion : We observed better results in the group treated with both Western and Oriental herbal medicines, comparing improvement rate and the alteration of total nasal volume through acoustic rhinometry after the allergen induction test. In the filter paper test, there was no significant difference between the two groups. In conclusion, we showed the additive effect of Oriental herbal medicine without any severe side effects compared with treatment with Western medicine only. In this study, we set only two patient groups, but further study is required to create various experimental groups and compare among them. We suggest that it might enhance understanding of the improved effect of Oriental herbal medicine in the therapy of allergic rhinitis.
