난소낭종에 대한 한의 임상 연구의 종설 (A Review of Ovarian Cyst Management in Oriental Medicine)

  • Purpose: Ovarian cysts are fluid-filled sacs or pockets within or on the surface of an ovary. Most cysts present little or no discomfort and are noncancerous and harmless. So non-surgical treatments with herbal medicine or acupuncture and moxa therapy are important and suitable in this pathology. But there are few RCT articles about ovarian cyst in Korean Medicine(KM) clinical and research field. Methods: To review treatment method and other research tendency about ovarian cyst in journals related with KM and Traditional Chinese Medicine(TCM) through web-searching. Searching was done through web site #http://omis.ksucc.ac.kr# and pub-med(http://www.ncbi.nlm.nih.gov/sites/entrez) with key words #ovarian cyst#, #TCM#, #herb#, #CAM#, and #acupuncture#. Results: Seven articles were searched in journals related with KM, then six of them were simple case report and the other one was about diagnosis for ovarian cyst and another gynecologic disease such as uterine myoma and infertility through the iris diagnosis. And two articles were searched in journals related with TCM, then one of them was simple case report but the other is RCT article. Conclusion: KM and TCM therapies have some beneficial effect to resolve cyst, but objective power as a evidence is weak in the viewpoint of EBM. So continuous clinical trials such as RCT and multi center trials are needed.

교통사고 환자 47례에 대한 임상적 고찰 (The clinical study of the 47 traffic accident victims)

  • A retrospective study has been carried out for 47 cases of traffic accident victims which was entered to the Dongguk Bundang O. M. Hosp. from Jan 1, 1999 to May 4, 2000. This study was focused on finding out the distribution, pattern of the traffic accidents, clinical characteristics of the patients, motives of their choosing oriental medical treatments, the kinds of oriental medical treatment. This results were as follows : 1. The highest incidence was shown in the age of twenties to thirties as much as 59.6% and the ratio of male to female was 17:30. 2. 80.9% of all patients was injured by car accidents. 68.2% of all patients was injured as drivers and passengers and 19.2% was injured as pedestrians. The highest type in the collision between cars, was rear impact. 3. The number of patient(61.7%) via other hosp. was more than that of the first visitor(38.3%) to our hosp. 4. In admission motives, The patients who had been treated by western medical treatment, expressed the dissatisfaction of that treatment, so hoped to be treated by oriental medical styles. Also The others wanted to be treated by oriental medical methods in the reason of no deep surgical, orthopaedical lesions 5. C-spine sprain(80.9%) was the most frequent out of all patients's diagnosis and then L-spine sprain(51.1%), contusion(46.8%), etc. 6. Head Cervix(80.9%) was the commonest pain region of all patients. and then shoulder back(70.2%), lumbar region(61.7%), etc. 7. In general, the pedestrian injuries were more serious than those sustained in the car as passengers. 8. After discharge, Duration of OPD treatment was long as much as that of admission treatment. 9. At first stage of admission period, Herb medication was frequently prescribed for hwalhyultonglakgige(活血通絡之劑). but As going to end stage, was frequently used for bogi(補氣) bohyul(補血), gudam(祛痰), ansin(安神).

소아(小兒) stress에 관한 문헌적(文獻的) 고찰(考察) (A study on stress in Children)

  • With the progress of civilization, the disorders due to the stress, which derived from the social-structural complexity and diversity, are on an increasing trend in our times. Accordingly, the accurate diagnosis and appropriate treatment for them are required. Especially in the current years, children's disorders delivered by the emotional problems keep increasing. In this research, the researcher tried to figure out the cause of the children's stress and its treatment, studied the theories of the stress in the modem medicine and the sever emotions in oriental medicine, and came to the conclusion as follows: 1. The stress can be defined as the combination of the reaction to noxious stimuli and its defense mechanism of the body, In oriental medicine, it is considered as pathological notions which includes seven emotions as the internal factor, six evils as the external factor and other foods, expectoration, ecchymoma as the non-internal/external factors. 2. Children usually get stressed by various reasons in a growth process such as schooling, relationship with friends, the opposite sex of family, or change of surroundings, and these can cause the various disorders. 3. In the study of the children's stress symptoms, it is found that the silent reaction is uncommon. It usually appeared in both reactions: firs, physical reactions such as stomachache, vomiting, headache, neural frequent urination, bronchial asthma or excessive respiration and/or, second, behavioral reactions such as a decline of performance, alimentary disorder, e.g. anorexia nervosa or bulimia, sleep disorder, e.g. nightmare or panic in sleep, anthrophobia, refusal to a school attendance or hyperactiveness. Besides, the peculiar mental disorder such as paroxysm of anger, tic, autism, nocturnal enuresis, lack of attentiveness, impediment in linguistic development, learning difficulty, intellectual decline, etc. can be appeared, and the heavy stress during the babyhood can cause the regression of behavior or the immaturity of formation of character. 4. The appropriate treatments for the children's stress are Osteopathy, Manpulation, Aroma Therapy, Alexander Technique, Autonomic Never Control Treatment, Biofeedback, Chiropractic, Dance Therapy, Feldenkrasis Technique, Gravity Therapy, Homepathy, Aquatherapy, Hypnotherapy, Naturopathy and Meditation.

음악치료(音樂治療)의 원리(原理)와 체질의학적(體質醫學的) 접근을 위한 검토 (A Review on Principles and Access Methods to Sasang Constitutional Medicine of Music Therapy)

  • 1. Objectives The present study purposed to examine the contents and the principles of music therapy according to Oriental medicine theories in order to prove that music therapy is not a new research area but its principle is found in the long tradition of Oriental medicine. 2. Methods We investigate the possibility of music therapy based on Oriental medicine theories and examine the meanings of music therapy from the viewpoint of Oriental medicine. 3. Conclusions and discussions (1) The principles of music therapy are the principle of homogeneity, catharsis and balance. (2) When one’s mind changes, there are naturally occurred sounds, which are called Oseong (五聲: the oriental five voices exhalation, laughing, singing, wailing and groaning), and the notes defined by arranging the Oseong according to the principle of Ohaeng (五行: the oriental five phases wood, fire, earth, metal, water) are Oheum (五音: the oriental five musical notes Gakeum, Chieum, Gungeum, Sangeum and Wooeum.). If Eum (musical notes) is classified into Ohaeng, it can be divided into Gakeum, Chieum, Gungeum, Sangeum and Wooeum. (3) Change of Sinji (神志: consciousness) induces change of Gigi (氣機: function of Gi), which can change the character of voices. Oseong controls the functions of Ojang (五臟: the oriental five viscera) by ruling one’s Jeongji (情志: emotion). It can reduce the damage of the viscera caused by excessive vent of emotion resulted from unconscious expression of Oseong - Hoseong (呼聲: exhalation), Soseong (笑聲: laughing), Gaseong (歌聲: singing), Gokseong (哭聲: wailing) and Sinseong (呻聲: groaning). (4) Yijeongseungjeong (以情勝情: Control emotion with emotion) therapies, which suppresses an emotion by stimulating another, include Noseungsabeop (怒勝思法: Control anxiety with anger), Heeseungbibeop (喜勝悲法: Control sorrow with joyfulness), (思勝恐法: Control fear with anxiety), Biseungnobeop (悲勝愁法: Control anger with sorrow) and Gongseungheebeop (恐勝喜法: Control joyfulness with fear). (5) Seongeum (聲音: voices and musical notes) can be applied to a stimulation method that not only harmonizes the rhythm of living organs but also controls the occurrence of diseases caused by mutual Pyeonseongpyeonsoi (偏盛偏衰: relative preponderance and weakness) through direct induction of the strength and weakness of Gi function of the oriental five viscera in a human body according to the individual character. Sounds preferred by the patient, the material of an instrument selected by the patient, the character of rhythm and music expressed by the patient and the sound or voice uttered frequently by the patient can be considered in diagnosis and treatments for the patient’s body and mind.

Changes in Blood Lipid Profile and Hepatic Enzyme Levels after Oriental Medical Treatment to Metabolic Syndrome Patients with Abnormal Liver Function

  • Among patients who are receiving treatments at an oriental medical hospital for various symptoms and diseases, patients whose main disease is accompanied by metabolic syndrome with abnormal liver function. This research was performed in order to observe the progression of changes in the liver function and serum lipid profile after the oriental medical treatments to patients who have been receiving oriental medical treatment for various other diseases and have been diagnosed as having metabolic syndrome at their first visit to the hospital based on NCEP ATP III diagnosis criteria and WHO Asia Pacific region criteria. Total number of subject patients were 39cases(mean age:55.58${\pm}$2.09 years) which had 20 male and 19 female. For the references for hepatic enzyme levels and blood lipid profile were measured in before treatment and four times after treatments(every 2.31${\pm}$06.17 weeks). Serum AST was 48.86${\pm}$7.46 IU/L before oriental medical treatment. After the treatment, 40.63${\pm}$4.69, 43.12${\pm}$5.46, 37.82${\pm}$4.52 IU/L were measured where although the level decreased to the normal level compared to pre-treatment, the value was not significant statistically(P>0.05). ALT was 66.26${\pm}$11.01 IU/L before oriental medical treatment. After the treatment 62.10${\pm}$8.20, 61.10${\pm}$8.76, 43.79${\pm}$5.68 were measured where although the level decreased, abnormally high level was maintained. The last result was significant statistically(P<0.05) compared to pre-treatment. ALP was 193.06${\pm}$14.20 IU/L before oriental medical treatment. After the treatment, 176.80${\pm}$6.48, 177.46${\pm}$11.81, 162.41${\pm}$9.06 where although compared to pre-treatment the last result was significant statistically(P<0.05), the change was within the normal range. ${\gamma}$-GGT was 87.83${\pm}$12.59 IU/L before oriental medical treatment. After the treatment, progressively near normal level was achieved with 118.73${\pm}$46.45, 85.03${\pm}$17.12, 70.64${\pm}$10.93 and the last result was statistically significant compared to pre-treatment (P<0.05). Blood triglyceride was 217.63${\pm}$32.18 mg/dL before oriental medical treatment. After treatment 215.09${\pm}$22.18, 189.93${\pm}$22.44, 191.22${\pm}$18.51 where abnormal values continued even after treatment although results was not statistically significant compared to pre-treatment(P>0.05). Total-cholesterol was 197.28${\pm}$9.24 mg/dL before oriental medical treatment, after treatment 201.55${\pm}$11.13, 186.87${\pm}$8.77 and 186.68${\pm}$7.61 were measured that results were not statistically significant compared to pre-treatment(P>0.05). HDL-cholesterol was 41.88${\pm}$2.38 mg/dL before oriental medical treatment, after treatment 48.75${\pm}$4.22, 44.10${\pm}$1.91, 48.00${\pm}$2.06 the results were not statistically significant compared to pre-treatment(P>0.05). LDL-cholesterol was 111.66${\pm}$13.08 mg/dL before oriental medical treatment, after treatment 109.94${\pm}$10.18, 101.79${\pm}$8.63, 104.00${\pm}$6.98 the results were not statistically significant compared to pre-treatment(P>0.05). With such results, even if common oriental medical treatments were given to metabolic syndrome patients with abnormal liver function, the liver function was confirmed not to be aggravated, and the concentration of lipids in the blood was confirmed not to be affected in most patients.

위암(胃癌)의 동서의학적(東西醫學的) 진치(診治) 개황(槪況) (An Outlook of the Oriental and Western Medical Diagnosis and Treatment on Gastric Cancer)

  • Gastric cancer shows the most high friquency in cancers that occurs in Korea. The western medicine treatment for gastric cancer has radiation and surgery, chemical treatment. While, oriental medicine cures the gastric cancer by herb-drugs, acupunture , moxa and srigma. With just one way of treating gastric cancer can't be effective remedy. Because each medicine has a strength and weakness. Thus, it is effective treatment when two medicine combins and supplement each other. We got the following result about a trend of oriental and westernal combination treatment for gastric cancer through studing records. 1. The western medicine treats gastric cancer patient with surgery first and right after surgery. They need on assembly treatent such as chemical and immune treatment. In oriental medicine, they treats gastric cancer patients with differentiation of symptone and signs and treatment(辨證施治)[for example:incoordination between liver and stomach(肝胃不和), insufficiency of spleen and stomach(脾胃虛弱), stagnation of blood stasis and toxic agent(瘀毒內阻), deficiency of yin by stomach heat(胃熱傷陰), reinforcing both qi and blood(氣血雙虧), stagnation of damp-phlegm(痰濕凝結)] and cure for them by acupuncture and stigma, too. 2. In combination with oriental and western medical treatment principle of gastric cancer by each stage is as follows. First stage and second stage gastric cancer is cured with radical surgery mainly. After operation, the herb of invigoration of the spleen(健脾), coordination of the stomach(和胃), and smoothing the liver and regurating the circulation of qi(疏肝理氣), is used for good gastroenteric condition. The second stage patients can be concidered using in combination with chimical treatment. The third stage gastric cancer is treated with radical surgery or with temporizing surgery. After those surgery, herb-drugs treatment is used jointly. The fourth stage patients who have no extensively metastasis or local contraindication can undergo temporizing and curcuit surgical operation. Herb-drugs and chemical treatments are used together for patients after operating. If he has operating contraindication, he would be treated with herb-drugs and chemical treatment. 3. In case of using in combination with oriental and western medical treatment as follows. As for herb-drugs with chemical treatment, reinforcing both qi and blood(補益氣血), invigorate the spleen and the stomach(健脾和胃), reinforcing liver and kidney(滋補脾腎), clear out the heat and relieve the toxic agent(淸熱解毒), can be used and with radiation treatment, clear out the heat and relieve the toxic agent(淸熱解毒), promoting the production body fluid and moisturizing the vicera(生津潤燥), reinforcing both qi and blood(補益氣血), invigorate the spleen and the stomach (健脾和胃), reinforcing liver and kidney(滋補肝腎) etc, can be used. 4. According to the research of oriental and western medical combination treatment are the 5-year-survival degree with oriental and western medicine combination treatment was for better than that just with oriental or western medical treatment. Especially, it has good effect on the third, fourth stage gastric cancer. That is, the middle and the end of stage gastric cancer. 5. The merits of oriental and western medicine combination treatment are lengthers one's life and diminish the bad effect of chemical treatment and radiation treatment be near completion, prevent from relapsing, maintain the balance in their eveirenment of body and improve immunity.

치매 환자(患者) 84례(例)에 대한 유형별(類型別) 임상고찰(臨床考察) (The Clinical Analysis on 84 Cases of Dementia)

  • Objective : In western medicine, many medication therapies and non-medication therapies have been treated for dementia. But these methods did no more than symptomatic therapies, not basic treatment, which just can better subordinate symptoms. In fact, care or control became the very point of treatment of dementia. So, to try to find oriental medical treatments for Dementia, a clinical analysis was carried out for 84 patients who were diagnosed as Dementia through MMSE-K and treated in Dong-shin Oriental Medicine Kwangju Hospital and Karitaas Sanatorium from 10. Jan. 1998 to 20. May. 2000. Methods : Regardless of patterns of Dementia, the basic treatment(本治) is to supplement encephalon deficiency(腦髓不足). And the external treatment(標治) must be accompanied by through diagnosis according to symptoms(辨證). All patients were given Yukmi-jihwanghwan gami and acupuncture treatment. Results : Dementia of cerebral vascular type shows a high distribution(51 patients, 60.7%). Women also are distributed more widely(62명, 73.8%). In Age distribition, 70-79 years-32 patients(38.1%), over 80 years-21 patients(25%), 60-69 years-17 patients(20.2%), 50-59 years-13 patients(15.5%) and under 49 years-1 patient(1.2%). From this, Dementia occurs well over 60 years and the rate of occurrence of Dementia is high as people grow older. In past history of people with Dementia, hypertension is associated much(50 patients, 64.1%). Distribution of mental state by MMSE-K examination declined in orientaion, mathmatical faculty, memory faculty and composition faculty. In therapeutic effect by treatment duration, 14 patients(93.3%) out of 15 in sanatorium show a significant effect. And 52 patients(75.4%) out of 69 in the hospital show the same result. Especially, 11 patients with Alzheimer type in sanatorium showed an good effect when treated over 4 months. 40 patients(83.3%) with cerebral vascular type out of 48 in the hospital showed the stage of betterment. Conclusions : The basic treatment(本治) for Dementia is to supplement encephalon deficiency(腦髓不足). For promoting encephalon through supplementing kindney(補腎健腦), All patients were given Yukmi-jihwanghwan gami. And the external treatment(標治) through diagnosis according to symptoms(辨證) were be accompanied by. The result is quite effective. We consider there must be more research based on this study.

고혈압(高血壓) 환자(患者)의 혈압관리실태(血壓管理實態)와 한방이용(韓方利用) 현황(現況) (The Condition of Controlling Blood-pressure and the Use of Oriental Medical Services in Hypertensive Patients)

  • This Study aims at looking into the use of oriental medical services in treating hypertension. The first objective to be explored through this study is the morbidity caused by the disease, classifying them by age, gender, and occupation. The second is to determine the regular use of anti-hypertensive medicine and their efficacy in controlling blood-pressure. The third is to investigate the use-rate and satisfaction of oriental medical service. 838 households across the country, were asked to answer questionnaires for the period of time from Apr. to Jun. 2005. The conclusion from the survey can be summarized as following. The age of which the first medical diagnosis of hypertension were made showed lower in males, those with higher education and income. The study showed females were more active in blood-pressure control, with their frequent monitoring of blood-pressure for the past year. With age, people in under 50 age group proved to be less active. The management of high blood pressure was more effective in group with regular dosage compared to group with irregular use of anti-hypertensive medication, but their difference was minimal. Also, group with no medication showed signs of blood-pressure control. The use of oriental medical service for the past year, were more frequent in groups with lower education, either low or high income, old age, females and occupations in agriculture, forestry and fishing industry. Results were similar in both general population and high blood-pressure patient group. Females, people over 51 years old and with lower education showed more intentions in utilizing oriental medical services in the future. Results were similar in both general population and high blood-pressure patient group. It is necessary to offer a more accurate information on oriental medical treatments. Also, a systematic reform to reduce the patient's share of the treatment cost, as well as, heightening public awareness on the infirmity of present blood-pressure management system is crucial.

한방안이비인후피부과에 내원한 피부질환 환자의 임상적 고찰 (A Clinical Analysis of Out-Patient with Skin Disease)

  • Objective : This study was performed to examine the patterns of skin disease in patients that had visited Dept. of Dermatology Hospital of Oriental Medicine, Dongguk University. We also compared on disease, age and treatment. Methods : We analysed statistic study in 505 patients, who had visited to our hospital from July, 2005 to February, 2007. Results : The results were as follows; 1. Distribution of dermatology classification was 40.6%, males were 41 % and females were 59%. 2. Distribution of aging was 18.2% in 0-9 years old and 18.8% in 10-19 years old and 17.4% in 20-29 years old. 3. Common disease group were Atopic dermatitis (24.55%); Acne (10.89%); Eczema (10.89%); Urticaria (10.30%). 4. Distribution of treatment according to diagnosis were external preparation in atopic dermatitis, acne, eczema, dermatitis seborrheica ; herbal-medication in urticaria ; herbal-medication and external preparation in facial redness. 5. Distribution of treatment according to aging were external preparation in all group except 50-59 years old; acupuncture treatment in 20-29 years old; herbal-medication in 10-19 years old; consultation in 70-79 years old. Conclusion : This study suggests that oriental medical treatments is popular in dermatologic diseases, especially atopic dermatitis and acne. We have to take steps for effective management and treatment for special diseases and ages.

Characteristics of Vibration Response Imaging in Healthy Koreans

  • Background: Vibration response imaging (VRI) is a new technology that records energy generated by airflow during the respiration cycle. Analysis of lung sound using VRI may overcome the limitations of auscultation. Objectives: To set a VRI standard for healthy Koreans, we conducted a clinical assessment to evaluate breath sound images and quantification in healthy subjects and compared the findings with reported breath sound characteristics. Methods: Recordings were performed using the VRIxp. Eighty subjects took a deep breath four times during a 12-second interval while sitting upright. The quantitative aspect was analyzed using the VRI quantitative lung data (QLD) for total left lung, total right lung and for six lung regions: left upper lung (LUL), left middle lung (LML), left lower lung (LLL), right upper lung (RUL), right middle lung (RML), right lower lung (RLL). The qualitative aspect was provided through image assessments by three reviewers. Results: In all regions the left lung had significantly higher QLD than the right lung (P<0.005, paired t-test). The inter-rater agreement was 0.78. 84% of the images were found normal by the final assessment. Among the 16% (n=13) of images with abnormal final assessment, the most common flawed features were dynamic image (77%, n=10) and maximum energy frame (MEF) shape (77%, n=10). No significant differences were found between males and females for QLD but there were significant differences in qualitative aspects including dynamic images, MEF shape, and missing LLL. Conclusion: The characteristics of healthy Koreans are similar to those of Western subjects reported previously. VRI is easy to use and objective, and so is helpful to diagnose patients with respiratory diseases and to monitor the progress of diseases after medical treatments.
