A comparative study of effects using physical treatmental apparatus and diagnosis of shoulder pain on cerebrovascular attack patients (뇌졸중환자중(腦卒中患者中) 견관절동통(肩關節疼痛)에 관(關)한 진단(診斷) 및 치료기기효과(治療器機效果)에 대(對)한 비교(比較) 연구(硏究))

  • This study was to investigate diagnosis of shoulder pain on cerebrovascular attack patients and treatment effects by using physical treatment and orienatal diagnosis aparatus, as oriental diagnosis equipments, these were used thermography, maegzin and yangdorag machine. as physical treatment equipments, these were used Hot pack, TENS (Transcutaneous electrical nerve stimulation), ICT(Interferential current therapy) the results were followed 1. In Sex and Age distribution on cerebrovascular attack patients with shoulder pain, male(51.7%)had many than female(48.3%) and 60s were the top, next 70s 50s 40s. 2. In the reason of cerebrovascular attack and the type of motor paresis, cerebrovascular infarction(73.3%) had many than hemorrhage(26.7%) 3. In hospitalizied period of cerebrovascular attack patients with shoulder pain, the period from 11 to 60 days had almost occupied. 4. Oriental medical diagnositic propriety of shoulder pain by using maegzin and yangdorag machine were not recognized. 5. In measuring passive range of motion of pre and post treatment, improvement effect on Hot pack was not showed, but effects on TENS, ICT were showed. 6. In measuring temperature of painful shouler joint of pre and post treatment by using thermography, the effect of Hot pack was not recognized but the effects of TENS, ICT were recognized.

Oral Potentially Malignant Disorders among Dental Patients: a Pilot Study in Jordan

  • Background: To determine the prevalence, types, and risk factors of oral potentially malignant disorders (OPMDs) among a group of Arab Jordanian dental patients, and to evaluate their awareness and attitudes toward early diagnosis and treatment. Materials and Methods: A total of 1,041 patients attending a University Hospital for dental care were examined for the presence of OPMDs. Histopathological examination was performed on all cases clinically diagnosed and patients were directly interviewed to evaluate their knowledge and attitudes toward early detection and treatment of oral cancer. Results: The prevalence of OPMDs overall was 2.8%. Lichen planus/lichenoid lesions were the most common lesions (1.8%) followed by leukoplakias (0.48%), chronic hyperplastic candidiosis (0.38%), and erythroplakia (0.096%). Smoking, alcohol, and age (>40 years) were the main identifiable risk factors. Patients with OPMDs displayed a general lack of awareness and negative attitudes towards early diagnosis and treatment. Conclusions: OPMDs among Arab dental patients are relatively uncommon and awareness about oral cancer among Jordanian dental patients is low. Interventions to improve public knowledge about oral cancer and attitudes toward early diagnosis and treatment are urgently indicated.

  • SBR is one of the most general sewage/wastewater treatment processes and, particularly, has an advantage in high concentration wastewater treatment like sewage wastewater. A Kernel PCA based fault diagnosis system for biological reaction in full-scale wastewater treatment plant was proposed using only common bio-chemical sensors such as ORP(Oxidation-Reduction Potential) and DO(Dissolved Oxygen). During the SBR operation, the operation status could be divided into normal status and abnormal status such as controller malfunction, influent disturbance and instrumental trouble. For the classification and diagnosis of these statuses, a series of preprocessing, dimension reduction using PCA, LDA, K-PCA and feature reduction was performed. Also, the diagnosis result using differential data was superior to that of raw data, and the fusion data show better results than other data. Also, the results of combination of K-PCA and LDA were better than those of LDA or (PCA+LDA). Finally, the fault recognition rate in case of using only ORP or DO was around maximum 97.03% and the fusion method showed better result of maximum 98.02%.

Diagnosis, Clinical Course and Treatment of Hypersensitivity Pneumonitis (과민성폐렴의 진단, 임상 경과 및 치료)

  • Hypersensitivity pneumonitis (HP) is an inflammatory and fibrosing interstitial lung disease caused by inhaled antigens in susceptible individuals. Various environmental materials can serve as inciting agents of HP. Recent guidelines have defined typical radiologic and histopathologic findings of HP, classifying it into nonfibrotic and fibrotic HP categories, as this classification better correlates with clinical outcomes. Diagnosing HP poses challenges, necessitating multidisciplinary discussions based on clinical assessments and radiologic or histopathological features. The cornerstone of treatment lies in avoiding the causative antigen. Oral glucocorticoids serve as the initial treatment for symptomatic HP. Immunosuppressants may be employed in patients who do not respond to initial treatment, and antifibrotics could be considered for patients displaying progressive fibrosis. This review outlines the clinical features, diagnostic evaluation, and treatment of HP.

Treatment of Superficial Peroneal Nerve Entrapment Syndrome under Local Anesthesia using Ultrasonogram (초음파를 이용한 국소마취하 표재비골신경 포착증후군의 치료)

  • Purpose: To review the outcomes of surgical treatment for superficial peroneal nerve entrapment. Materials and Methods: Ultrasonogram was used for diagnosis and surgical treatment. Seven superficial peroneal nerve entrapment were surgically treated with follow up of average 16 months (range, 6~29 months). Three patients were male and four patients were female with mean age 36.7 years (range, 19~51 years). Four cases developed after repetitive ankle sprain and three cases had no etiology. Results: Operation was performed mini-open and subcutaneous fasciotomy under local anesthesia. The results were excellent in two cases, good in four cases, fair in one case. Conclusion: Ultrasonogram was useful for diagnosis and surgical treatment of superficial peroneal nerve entrapment syndrome.

Facet joint disorders: from diagnosis to treatment

  • One of the most common sources of spinal pain syndromes is the facet joints. Cervical, thoracic, and lumbar facet joint pain syndromes comprise 55%, 42%, and 31% of chronic spinal pain syndromes, respectively. Common facet joint disorders are degenerative disorders, such as osteoarthritis, hypertrophied superior articular process, and facet joint cysts; septic arthritis; systemic and metabolic disorders, such as ankylosing spondylitis or gout; and traumatic dislocations. The facet pain syndrome from osteoarthritis is suspected from a patient's history (referred pain pattern) and physical examination (tenderness). Other facet joint disorders may cause radicular pain if mass effect from a facet joint cyst, hypertrophied superior articular process, or tumors compress the dorsal root ganglion. However, a high degree of morphological change does not always provoke pain. The superiority of innervating nerve block or direct joint injection for diagnosis and treatment is still a controversy. Treatment includes facet joint injection in facet joint osteoarthritis or whiplash injury provoking referred pain or decompression in mass effect in cases of hypertrophied superior articular process or facet joint cyst eliciting radicular pain. In addition, septic arthritis is treated using a proper antibiotic, based on infected tissue or blood culture. This review describes the diagnosis and treatment of common facet joint disorders.

  • Inherited muscle diseases are heterogeneous with varying genetic etiologies and present with common symptoms and signs, including weakness, motor developmental delay, and hypotonia. To diagnose these various diseases, a meticulous family and clinical history, physical and neurological examinations, laboratory findings with electromyography, muscle biopsy, and genetic testing are needed. Here, I review several inherited muscle diseases, with a focus on muscular dystrophy in children and its genetics and general management.

Downregulated microRNAs in the colorectal cancer: diagnostic and therapeutic perspectives

  • Colorectal cancer (CRC), the third most common cancer in the world, has no specific biomarkers that facilitate its diagnosis and subsequent treatment. The miRNAs, small single-stranded RNAs that repress the mRNA translation and trigger the mRNA degradation, show aberrant levels in the CRC, by which these molecules have been related with the initiation, progression, and drug-resistance of this cancer type. Numerous studies show the microRNAs influence the cellular mechanisms related to the cell cycle, differentiation, apoptosis, and migration of the cancer cells through the post-transcriptionally regulated gene expression. Specific patterns of the upregulated and down-regulated miRNA have been associated with the CRC diagnosis, prognosis, and therapeutic response. Concretely, the downregulated miRNAs represent attractive candidates, not only for the CRC diagnosis, but for the targeted therapies via the tumor-suppressing microRNA replacement. This review shows a general overview of the potential uses of the miRNAs in the CRC diagnosis, prognosis, and treatment with a special focus on the downregulated ones.

The Patients' Experiences of the Diagnosis and Pre-Treatment Period of Breast Cancer (유방암 환자의 치료 전 경험)

  • Purpose: To date most research related to patients with breast cancer has discriminately investigated the status within or after the treatment although the patients demand holistic nursing care from the time of diagnosis. Thus, the purpose of this study was to investigate the trajectory of breast cancer diagnosis and patients' experiences in the pre-treatment period. Method: This qualitative study used qualitative thematic analysis. Nineteen Korean women who were diagnosed with breast cancer within the last 6 month participated in the study. Individualized interviews were conducted with each participant in a cancer center in K city. The interviews were tape-recorded, transcribed, and analyzed using the thematic analysis process. Results: The overriding theme was "the scattered life in an unforeseen swirl", which illustrates the participants' unexpected crisis with confusion and emotional distress. Two subthemes included "falling into an unavoidable journey", and "staggering in a muddle with urgency". The categories were "unexpected probability", "nagging nodularity", "ominous presentiment", "emotional upheaval", "bad thought intrusion", and "a sense of urgency". Conclusion: Patients in the pre-treatment period encountered utter emotional distress and a sense of urgency after being diagnosed breast cancer. Strategies to develop nursing care for patients in this period and nursing implications are discussed.

Alveolar Soft Part Sarcoma (포상 연부 육종)

  • Alveolar soft part sarcoma, named by Christopherson in 1952, is a very rare soft tissue sarcoma. It reportedly accounts for 0.5 to 1.0% of all soft tissue sarcoma and no established treatment principle assists it. The relative lack of symptoms makes it easily overlooked and consequently early diagnosis is frequently impossible. Often, the metastasis to the lung or the brain is the first manifestation of the disease. In some cases wide excision is not performed because it is misdiagnosed as a benign tumor. Authors experienced 7 cases of alveolar soft part sarcoma in 20 years and found the ultimate prognosis grave and the treatment not promising. Therefore, the authors recommend the following; 1. Early diagnosis and early treatment are important for successful management of alveolar soft part sarcoma. 2. Preoperative incisional biopsy is necessary for pathologic diagnosis. 3. The recommended treatment modality is a combination of wide excision, chemotherapy and radiotherapy.
