Chronic non-bacterial osteomyelitis in the jaw

  • Chronic recurrent multifocal osteomyelitis (CRMO) is one of the most severe form of chronic non-bacterial osteomyelitis (CNO), which could result in bone and related tissue damage. This autoinflammatory bone disorder (ABD) is very difficult for its clinical diagnosis because of no diagnostic criteria or biomarkers. CRMO in the jaw must be suspected in the differential diagnosis of chronic and recurrent bone pain in the jaw, and a bone biopsy should be considered in chronic and relapsing bone pain with swelling that is unresponsive to treatment. The early diagnosis of CRMO in the jaw will prevent unnecessary and prolonged antibiotic usage or unnecessary surgical intervention. The updated researches for the identification of genetic and molecular alterations in CNO/CRMO should be studied more for its correct pathophysiological causes and proper treatment guidelines. Although our trial consisted of reporting items from Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA), there are very few articles of randomized controlled trials. This article was summarized based on the author's diverse clinical experiences. This paper reviews the clinical presentation of CNO/CRMO with its own pathogenesis, epidemiology, recent research studies, and general medications. Treatment and monitoring of the jaw are essential for the clear diagnosis and management of CNO/CRMO patients in the field of dentistry and maxillofacial surgery.

Gastric Cancer with Peritoneal Tuberculosis: Challenges in Diagnosis and Treatment

  • Herein, we report a 39-year-old female patient presenting with gastric cancer and tuberculous peritonitis. The differential diagnosis between advanced gastric cancer with peritoneal carcinomatosis and early gastric cancer with peritoneal tuberculosis (TB), and the treatment of these two diseases, were challenging in this case. Physicians should have a high index of suspicion for peritoneal TB if the patient has a history of this disease, especially in areas with a high incidence of TB, such as South Korea. An early diagnosis is critical for patient management and prognosis. A surgical approach including tissue biopsy or laparoscopic exploration is recommended to confirm the diagnosis.

  • This study interprets the records of Bianque (醫案) as a medicine practice rather than a simple legend or story, and analyzes its effect on the development of oriental medicine. It also compares aspects of the Bianque related to patient treatment as described in 『韓非子』, 『韓詩外傳』, 『史記』, and reinterpreted using other contemporary literature. The clairvoyant power of 『史記』 「扁鵲傳」 was created by Sama-Cheon(司馬遷) exaggerating the pulse diagnosis with clairvoyance. The contents of the treatment of Sogan-Ja(趙簡子)'s confusion affairs in the first agenda, show that Bianque master the pulse diagnosis. The contents of the treatment of Kwek-Seja(虢世子)'s disease in the second agenda is a record of knowledge of acupuncture using acupuncture points. The content of refusal of treatment in the third agenda, 腠理, 肌膚, 腸胃, 骨髓 and the progression of the disease and the corresponding treatment of 湯熨, 鍼石, 火齊 are specified in detail. From the content of Bianque, he was not a martial art, but a doctor who practiced medicine, and contributed to the development of the pulse diagnosis.

The Impact of Cancer Diagnosis and Its Treatment on Korean Women's Lives: A Meta-synthesis Study

  • Purpose: Korean women, who have come to the forefront at a risk of cancer, have been notable objects for qualitative nursing research in last a couple of decades. Given the imparity and varieties of those findings, this study was aimed to synthesize the impact of cancer diagnosis and its treatment on Korean women's lives using a qualitative meta-synthesis method. Methods: By searching five English-based databases and four Korean databases, 21 qualitative studies on Korean women's particular experiences of cancer diagnosis and treatment since 2000 were included. Using a meta-synthesis process by Sandelowski & Barroso (2007), the selected studies were synthesized for interpretive integration of the findings. Results: The meta-synthesis elicited three themes: detachment from the usualness, awareness of profound desires, and redefinition of every relation. With destructive experiences of a diagnosis and its treatments, Korean women felt apart from their everyday life, daily roles, and even from their own body. They then grasped a strong desire for life and for beauty, and reconfirmed the sense of mission for being a mother. Those changes made them to reconstruct all relations surrounded them. Conclusion: The findings yield a substantive portrait of the given issue, which could be helpful for health care professionals.

Fatal Case of Cerebral Aspergillosis : A Case Report and Literature Review

  • Cerebral aspergillosis is rare and usually misdiagnosed because its presentation is similar to that of a tumor. The correct diagnosis is usually made intra-operatively. Cerebral abscess with fungal infection is extremely rare and few cases have been reported, but it carries a poor prognosis. A 73 year-old man presented with decreased visual acuity and paresis of the right cranial nerve III. Magnetic resonance imaging (MRI) revealed a mass in the right cavernous sinus, extened to the anterior crainial fossa and the superior orbital fissure. During surgery, a well encapsulated pus pocket was found, and histopathological examination of the mass resulted in the diagnosis of aspergillosis. Despite appropriate anti-fungal treatment, the patient eventually died from fatal cerebral ischemic change and severe brain swelling. The correct diagnosis of cerebral aspergillosis can only be achieved by histopathological examination because clinical and radiological findings including MRI are not specific. Surgical intervention and antifungal therapy should be considered the optimal treatment. Early diagnosis and aggressive antifungal treatment provide good results.

  • Children with abnormal liver function can often be seen in outpatient clinics or inpatients wards. Most of them have respiratory disease, or gastroenteritis by virus infection, accompanying fever. Occasionally, hepatitis by the viruses causing systemic infection may occur, and screening tests are required. In patients with jaundice, the tests for differential diagnosis and appropriate treatment are important. In the case of a child with hepatitis B virus infection vertically from a hepatitis B surface antigen positive mother, the importance of the recognition of immune clearance can't be overstressed, for the decision of time to begin treatment. Early diagnosis changes the fate of a child with Wilson disease. So, screening test for the disease should not be omitted. Non-alcoholic fatty liver disease, which is mainly discovered in obese children, is a new strong candidate triggering abnormal liver function. Muscular dystrophy is a representative disease mimicking liver dysfunction. Although muscular dystrophy is a progressive disorder, and early diagnosis can't change the fate of patients, it will be better to avoid parent's blame for delayed diagnosis.

  • The diagnosis and treatment of early gastrointestinal cancers is the gastroenterologists' mission because of national cancer screening program in South Korea. The detection of early cancers is emphasized, because these were previously treated with surgical treatment can be currently cured with endoscopic treatment. Gastroenterologists who achieved at least on some level can make an exact diagnosis regardless of what type of endoscopy, but generally, there are some required conditions for an optimal diagnosis. First, clinically important lesions have to be detected easily. Second, the border and morphology of lesions have to be characterized easily. Third, lesions have to be diagnosed exactly. Precancers and early cancers are often subtle and can pose a challenge to gastroenterologists to visualize using standard white light endoscopy. The use of dye solutions aids the diagnosis of early gastrointestinal cancers, however, it is a quite cumbersome to use dye solutions all the time and the solution often bothers the exact observation by pooling into the depression or ulceration of the lesion. To overcome this weakness, newer endoscopes are now developed so called "image enhanced endoscopy" using optical and/or electronic methods such as narrow band imaging (NBI), autofluorescence imaging (AFI), i-scan, flexible spectral imaging color enhancement (FICE) and confocal endomicroscopy (CLE).

  • Objectives : The purpose of this study is to investigate the concept of the Chukbu(尺膚) diagnosis in Huangdineijing(黃帝內經) and explain the characteristic of Chukbu(尺膚) diagnosis and causes of its clinical applicational decline. It will help the application of the Chukbu(尺膚) diagnosis to clinical treatment. Methods : The Sikuquanshu(四庫全書) database and Traditional Chinese Medical(TCM) books web database were used. The related contents of the Chukbu(尺膚) diagnosis in Huangdineijing (黃帝內經) and its annotation books were analyzed. The mentions on the Chukbu(尺膚) diagnosis in other medical books were examined. Results & Conclusions : The Chukbu(尺膚) diagnosis is important in the diagnostic system of the Huangdineijing(黃帝內經). The Chukbu(尺膚) diagnosis is composed of inspection and palpation. Its characteristic is something different compared to that of the Chongumaek(寸口脈) diagnosis; it relatively diagnoses condition of exterior disease(表病). The causes of its clinical applicational decline are relative inconvenience, limits of feudal society, and development of Zangfubianzheng(臟腑辨證) in that era.

Advances in Optimal Detection of Cancer by Image Processing; Experience with Lung and Breast Cancers

  • Clinicians should looking for techniques that helps to early diagnosis of cancer, because early cancer detection is critical to increase survival and cost effectiveness of treatment, and as a result decrease mortality rate. Medical images are the most important tools to provide assistance. However, medical images have some limitations for optimal detection of some neoplasias, originating either from the imaging techniques themselves, or from human visual or intellectual capacity. Image processing techniques are allowing earlier detection of abnormalities and treatment monitoring. Because the time is a very important factor in cancer treatment, especially in cancers such as the lung and breast, imaging techniques are used to accelerate diagnosis more than with other cancers. In this paper, we outline experience in use of image processing techniques for lung and breast cancer diagnosis. Looking at the experience gained will help specialists to choose the appropriate technique for optimization of diagnosis through medical imaging.

Impact of the Interval between Previous Endoscopic Exam and Diagnosis on the Mortality and Treatment Modality of Undifferentiated-Type Gastric Cancer

  • Purpose: The impact of the interval between previous endoscopy and diagnosis on the treatment modality or mortality of undifferentiated (UD)-type gastric cancer is unclear. This study aimed to investigate the effect of endoscopic screening interval on the stage, cancer-related mortality, and treatment methods of UD-type gastric cancer. Materials and Methods: We reviewed the medical records of newly diagnosed patients with UD gastric cancer in 2013, in whom the interval between previous endoscopy and diagnosis could be determined. The patients were classified into different groups according to the period from the previous endoscopy to diagnosis (<12 months, 12-23 months, 24-35 months, ≥36 months, and no history of endoscopy), and the outcomes were compared between the groups. In addition, patients who underwent endoscopic and surgical treatment were reclassified based on the final treatment results. Results: The number of enrolled patients was 440, with males representing 64.1% of the study population; 11.8% of the participants reported that they had undergone endoscopy for the first time in their cancer diagnosis. The percentage of stage I cancer at diagnosis significantly decreased as the interval from the previous endoscopy to diagnosis increased (65.4%, 63.2%, 64.2%, 45.9%, and 35.2% for intervals of <12 months, 12-23 months, 24-35 months, ≥36 months, and no previous endoscopy, respectively, P<0.01). Cancer-related mortality was significantly lower for a 3-year interval of endoscopy (P<0.001). Conclusions: A 3-year interval of endoscopic screening reduces gastric-cancer-related mortality, particularly in cases of UD histology.
