당뇨와 지역사회치주지수의 관련성: 제5기 국민건강영양조사 (The association between diabetes mellitus and community periodontal index: The 5th Korean National Health and Nutrition examination survey)

  • Objectives: The purpose of the study was to examine the association between diabetes mellitus and community periodontal index in Korean adults. Methods: The study populations were recruited by the Fifth Korea National Health and Nutrition Examination Survey. Study subjects were 10,411 who were examined oral examination, blood test, and aged over 19 years. Using multiple logistic regression analyses, the variables were adjusted for gender, age, household income, family history of diabetes, body mess index, smoking habit, and frequency of tooth brushing. Periodontal tissue examination of the subjects was performed and scored by Community periodontal index(CPI). Using probe, six teeth were examined for hemorrhage, plaque, and pocket depth and classified into $CPI_0$, $CPI_1$, $CPI_2$, $CPI_3$ and $CPI_4$. Healthy periodontal groups($CPI_{0-2}$) and periodontal disease groups($CPI_{3-4}$) were divided by the periodontal disease status. The definition of diabetes mellitus(DM) was decided by the diagnosis by the doctors and fasting blood sugar level. Those who were diagnosed as DM were included in DM group. The DM variables included normal blood sugar level, increased fasting blood sugar level, and DM blood sugar level. The DM variables were compared to periodontal disease blood sugar level and analyzed. Results: The periodontitis prevalence rate was 23.2%. Those who had diabetes mellitus accounted for 5.5% of the subjects. Those who had impaired fasting glucose accounted for 17.7% and 7.9% of subjects were diabetes mellitus by blood test. In the confirmed diabetes group by doctor, the periodontitis prevalence rate was significantly higher than the non-diabetic group. Diabetic group by blood test had the highest prevalence rate of periodontitis than those who had impaired fasting glucose group or normal group. After adjusting for gender, age, household income, family history of diabetes, body mass index, smoking habit, and frequency of tooth brushing, the risk of periodontitis in diagnosed diabetes mellitus was 1.57 times(95% CI; 1.27-1.94) higher than the normal group. In impaired fasting glucose group and diabetes mellitus group by blood test, the risk of periodontitis was 1.11 times(95% CI; 0.95-1.30) and 1.45 times(95% CI; 1.45-2.12) higher, respectively. Conclusions: There was a significant relationship between diabetes mellitus and periodontitis in Korean adults. These results suggest that diabetes mellitus is a risk factor for periodontitis.

  • The purpose of this study was to collect basic data on the prevention of and education about diabetes mellitus for the nutritional management of a diabetes mellitus risk group. The study which took place in Kangbukgu, Seoul, involved a diabetes mellitus risk group (DMR $\geq$ 110 mg/dL, 61), of males and females, aged 36 to 68 years, and a group of healthy people as a control group ( < 110 mg/dL, 183), using luting blood sugar (FBS) levels. The proportion of people in the abnormal range was higher in the DMR than that of control group for total cholesterol, high-density lipoproteins-cholesterol (HDL-C), total protein, glutamic oxaloacetic transaminase (GOT), glutamic pyruvic transaminase (GPT) and creatinine. Particularly with respect to serum protein the proportion in the DMR in abnormal range (p < 0.05) was significantly higher than that of the control group. The proportion in the DMR with a family history of disease was significantly higher than that of the control group (p < 0.01). Using body mass index (BMI), waist-hip ratio (WHR) and relative body weight (RBW), the obesity indices in the DMR was higher than that of the control group. Comparing the DMR and the control group with respect to dietary habits, it seems that the DMR had more undesirable dietary habits than the control group. When the intake of each nutrient for the DMR and the control group was compared to the Korean recommended dietary allowances (RDA), the proportion of excess intake and deficient intake in the DMR was higher than that of the control group. The DMR showed a greater undesirable dietary intake pattern as compared to that of the control group, based on the RDA. With respect to the dietary diversity score (DDS) and the mean adequacy ratio (MAR) for quality estimation of the overall flood intake, the DMR showed a feater undesirable pattern than the control group. According to the above results, the DMR tended to have more undesirable eating habits when compared to the control group. Therefore, to provide a more efficient nutritional education program for the DMR we must conduct lurker studies on eating habits, so as to provide systematic nutritional management based on theme differences between the DMR and the control group.

한국 성인의 비타민 D와 당뇨병의 관련성 (Vitamin D intake and Diabetes Mellitus in Korean Adults)

  • Objectives: This study aimed to investigate the association between vitamin D deficiency and the risk of diabetes mellitus, and also examined whether their association is differed by sex and age. Methods: We analyzed the data from 28,135 subjects ${\geq}20years$ old who were registered for the Korea National Health and Nutrition Examination Survey (KNHANES) 2008-2014. Vitamin D was measured using serum 25-hydroxyvitamin D (25(OH)D) concentrations. Results: After adjusting for potential confounders (age, sex, education, income, total energy intake, smoking, physical activity, body mass index, and waist circumference), lower vitamin D was dose-dependently associated with a higher risk of diabetes mellitus (p-trend=0.004). When compared with the sufficient group (25(OH)D ${\geq}30ng/mL$), the severe deficient group (25(OH)D <10 ng/mL) had 1.46 (95% CI 1.11-1.94) odds ratio for the risk of diabetes mellitus. This association was markedly strong in men and adults aged ${\geq}40$, whereas no significant association was observed in women and adults aged <40. Conclusions: We found that vitamin D deficiency may be associated with an increased risk for diabetes mellitus in Korean adults. Our findings suggest that intake of higher vitamin D may help reduce the risk of diabetes mellitus.

Prevalence and Risk Factors of Diabetic Retinopathy in Diabetes People using Korean National Health and Nutrition Examination Survey VII

  • Purpose: This study aimed to investigate the prevalence and risk factors of diabetic retinopathy (DR) in people with diabetes mellitus (DM) using Korean National Health and Nutrition Examination Survey VII (2017~2018). Methods: DM was defined as in two ways; 1) doctor's diagnosis (Group 1, n=549), 2) one of doctor's diagnosis, medication, or hyperglycemia (Group 2, n=849). The DR prevalence was measured as the prevalence proportion (%). Risk factors for developing DR were analyzed using multiple logistic regression, and odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Results: The prevalence of DR was 25.87% in Group 1 and 20.14% in Group 2. Risk factors for DR were identified as insulin therapy (Group 1: OR=5.31, Group 2: OR=5.27), DM duration ≥10 years (Group 1: OR=2.20, Group 2: OR=3.10), and systolic blood pressure ≥140 mmHg (Group 1: OR=2.26, Group 2: OR=2.23) for both groups. Conclusion: Considering the DR prevalence, eye examinations education is highly recommended as part of a diabetes management programs in the community. It is also proposed to shorten the eye examination cycle for people with risk factors and establish a referral system to link between screening to treatment.

거제지역 당뇨병 및 고혈압 환자의 복합질병 위험요인 (Risk factors of a Complex Disease for the Diabetes Mellitus Group and the Hypertension Group in the Geoje Community)

  • The purpose of this study is to compare the correlation of clinical characteristics and patterns of disease. Subjects of the study were the adults(207) living in Geoje City, the diabetes mellitus and the hypertension patients(166) and the normal people(41). In the diabetes mellitus group and the hypertension group, blood pressure, blood glucose, total cholestero LDL-cholesterol and atherogenic index(AI) were significantly high. As the obesity index was getting higher, the blood pressure of the diabetes mellitus group was high, and the HDL-cholesterol of the hypertension group was low, but AI of it was significantly high. The AI was significantly high as serum lipid index were getting higher in both groups. The rate of the prevalence was very high in the diabetes mellitus group(74.3%) and the hypertension group(73.7%). The pattern in the diabetes mellitus group was in order of the hypertension, the hyperlipidemia, and the obesity but, in the hypertension group was the hyperlipidemia, and the obesity. The obesity index and serum lipid index of complex patient group were higher than single patient group.

당뇨병 환자와 건강인의 맥상과 사상체질에 관한 연구 (Comparative Study on the Pulse Wave Variables and Sasang Constitution in Diabetes Mellitus Patients and Healthy Subjects)

  • The purpose of this study was to determine whether a pulse analyzer was useful 1) to characterize the variables of pulse wave of diabetes mellitus group, compared with those of healthy subjects, and 2) to determine Sasang Constitution in diabetes mellitus group and healthy subjects. 1. The sum of pulse pressure (energy) and the ratio of systolic period area (As%), called pulse pressure-related variables, were higher in diabetes mellitus group than healthy group, while the height of dicrotic wave (h5) and the ratio of height of dicrotic wave to height of percussion wave (h5/h1), correlated with arterial compliance, were lower in diabetes mellitus group than healthy group. 2. Taeumin of diabetes mellitus group showed higher pressure-related variables than that of healthy group. 3. Soumin of diabetes mellitus group had shorter the time to dicrotic wave (t5) than that of healthy group. 4. Soyangin of diabetes mellitus group showed higher heart rates and lower values in pulse wave time-related variables, including time to dicrotic wave(t5), time to incisura (t4), total time minus time to incisura (t-t4), total time (t), width of percussion wave (w), and the ratio of width of percussion wave to total time (w/t), than that of healthy group. 5. Contact pressure (CP), sum of pulse pressure (energy), height of pre-incisura (h2), height of incisura (h4), width of percussion wave (w), time to incisura (t4), time to percussion wave (t1), variance of total time (Vt), variance of height of percussion wave(Vp) and the ratio of height of incisura to height of percussion wave (h4/h1) were used to develop the rules of Sasang Constitution Classification with about seventy five percents accuracy. These suggested that the pulse analyzer was useful to evaluate the risk degree of diabetes mellitus and to determine Sasang Constitution among either diabetes mellitus group or healthy group.

당뇨병 유발에 영향을 미치는 식이 요인 (The Influence of Dietary Factors on the Incidence of Non-Insulin-Dependent Diabetes Mellitus)

  • The purpose of this study was to identify dietary factors related to the incidence of diabetes mellitus in Korea. The study consisted of 165 diabetic patients, male and female, aged 30 to 70 years and 198 healthy persons as controls. Diabetic patients who had been diagnosed with diabetes mellitus for less than five years before the study period were recruited from eight different hospitals located in Seoul, Korea. Socioeconomic status, state of illness, physical activity, food habits and food intake were assessed. Food intake was assessed by food frequency questionnaire method using a 105-food frequency questionnaire developed for diabetic patients. The stress and activity indices of diabetic patients were not significantly different from control, but alcohol consumption and smoking levels were higher in diabetics than controls. Other diseases of male diabetic patients included liver diseases, digestive system diseases, and hypertension, while those of female diabetics were hypertension, neuralgia, arthritis and digestive system diseases. These disease patterns are different from Western countries whose most common complications are hypertension and hyperlipidemia. More irregular and less varied meals were found in the diabetic group compared to the control group, suggesting that diabetic patients have generally undesirable food habits. Otherwise, food and nutrient intake of diabetics did not differ greatly from the control group. It was found that diabetic patients consumed more cereals and less fruit than the control group, and also that male diabetics consumed more alcohol. The carbohydrate : protein : fat energy distribution ratio was 61.7 : 15.8 : 22.5 in male patients and 65.1 : 14.9 : 19.7 in female patients. Discriminant analysis showed that diabetes risk factors differed with sex. In male patients, the important factors were body mass index(BMI), vitamin C intake, family diabetic history and vegetable intake, while in female patients they were BMI, cereals intake, carbohydrate intake, vitamin C intake, stress, food habits and Ca intake. These results show that excessive intake of energy and fat are not the major causal factors in Korean NIDDM. Therefore, the diabetes risk factors of Western countries may not directly apply to Koreans. Mors study is needed to clarify the risk factors of Korean NIDDM.

  • The purpose of this study to examine lipid profiles and lipoprotein (a) concentrations and identify the risk factors of type 2 diabetes mellitus patients with macrovascular complications. The subjects were consisted of 618 out patients with type 2 diabetes mellitus from beginning of March through the end of April in 2001, who visited at the endocrinology department at Kangnam St. Mary's Hospital of Catholic University in Seoul. The patient's clinical laboratory data and the occurrence of chronic complications of diabetes were assessed at medical record review. The data were analyzed using for t-test. chi-square test and logistic regression. The results were as follows : 1. There were no significant differences in age. duration of diabetes. body mass index, sex, fasting blood glucose, 2-hour postprandial blood glucose and $HbA_{1}c$ level between macrovascular complication group and non macrovascular complication group. 2. There were significant differences in the level of total cholesterol. triglyceride. HDL(High density lipoprotein) cholesterol, LDL(Low density lipoprotein) cholesterol and lipoprotein(a) between macrovascular complication group and non macrovascular complication group. 3. Significant factor associated with macrovascular complication in the logistic regression best gut model was HDL cholesterol.

단일 족지 절단 환자의 상처 치유 위험인자에 대한 통계적 분석 (Statistical Analysis of the Risk Factors for Single Toe Amputation Patients in Wound Healing)

  • Purpose: This study compared and analyzed the risk factors that affect a wound healing group and healing failure group. Materials and Methods: From 2010 to 2018, 39 patients who had suffered a single toe amputation were evaluated retrospectively. The patients were divided into two groups (wound healing group and healing failure group - within at least 3 months following the amputation). Regarding the possible risk factors, age, gender, Wagner and Brodsky classifications, duration of diabetes mellitus, whether the patient had peripheral arterial occlusive disease (PAOD) or cardiovascular disease, body mass index, HbA1c, total cholesterol, estimated glomerular filtration rate (eGFR), blood urea nitrogen (BUN), smoking, and alcohol were investigated. Results: The mean duration of diabetes mellitus was 140 months in the healing group and 227 months in the healing failure group, and the duration of diabetes was significantly longer in the failure group (p=0.009). A significant difference in eGFR was observed between the two groups ($59.17mL/min/1.73m^2$ in the healing group and $31.1mL/min/1.73m^2$ in the failure group) (p=0.022). Sixteen patients with PAOD were found, all 10 patients in the healing failure group were PAOD patients. Conclusion: To reduce the additional complications in single toe amputation patients, the underlying disease and appropriate treatment are the most important factors. In addition, a more proximal level of amputation also should be considered in cases of patients with PAOD, high BUN and low eGFR, and patients with long-term diabetes.

심장외막지방두께와 당뇨와의 연관성 연구 (The Relationship Between Epicardial Adipose Tissue and Diabetes Mellitus)

  • The incidence of cardiovascular disease increases rapidly after 40's. The thickness of the epicardial adipose tissue was measured to analyze the risk factors affecting the thickness change. We present the cut off value for the epicardial adipose tissue thickness for high impacted variables. For the study, 547 patients underwent echocardiography, epicardial adipose tissue thickness, body mass index, abdominal subcutaneous fat thickness and diabetes mellitus were used as analytical variables. As a result, multiple regression analysis of age group showed that diabetes mellitus was highly influential in all age group, so we could predict the cut off value for the epicardial adipose tissue thickness for diabetes mellitus. As a result of ROC curve analysis, cut off value of 7.44 mm was obtained with sensitivity of 79.6% and specificity of 60.6% for diab etes variab le. Logistic regression analysis b ased on this value showed that the risk for diab etes increased 6 times with each increase in the thickness of the epicardial adipose tissue. Among the various obesity indexes, epicardial adipose tissue is used as an important index especially to prevent the risk of cardiovascular disease. Therefore, we suggest that the use of echocardiography as a routine screening method for the prevention of cardiovascular disease and metabolic syndrome, which is rapidly increasing in patients over 40's age, will enable more effective monitoring.
