A Case of Paradoxical Flare of Pustular Psoriasis after Ustekinumab Therapy (Ustekinumab 치료 후 발생한 고름물집건선의 Paradoxical Flare 1예)

  • Biologics are the most advanced treatment for psoriasis. Ustekinumab, one of the biologics for psoriasis, is a human monoclonal antibody that binds to the p40 subunit of interleukin-12 and interleukin-23. A 41-year-old woman with a 17-year history of plaque psoriasis and psoriatic arthritis presented with worsening lesions. The patient had previously been treated with a number of topical and systemic medications and narrow band ultraviolet B. However, none of the treatments consistently controlled her disease. Thus, treatment with ustekinumab 45 mg via subcutaneous injection was initiated. Approximately 7 days after the first treatment, she experienced a flare with generalized pustules in her whole body. The condition was controlled with systemic steroid treatment. The patient was subsequently treated with adalimumab, and improvement in her plaque and pustular lesions was noted. Herein, we report a case of psoriasis that flared up after ustekinumab therapy, which was accompanied by a morphological change from plaque to pustular lesions.

A Suspected Case of Hyperkeratotic Head and Neck Malassezia Dermatosis Presenting with Acanthosis Nigricans

  • A 19-year-old woman presented with an asymptomatic cutaneous discoloration on her nape. Dermatological examination revealed localized dark brown to dark pigmented velvety macules and patches. She denied a past medical history, except for a 7-kg weight gain over a 6-months period. Histopathologically, the specimen showed hyperkeratosis, epidermal hyperplasia, mild acanthosis, and papillomatosis. In addition, keratin material and fungal spore were noted between the papilloma ridges. Periodic acid-Schiff special stain showed a positive result for the fungal spore in the horny layer. She was treated with a daily oral dose of terbinafine and topical flutrimazole. After 12 weeks of treatment, she is currently under observation without pigmentation and adverse effect. We considered the initial diagnosis as acanthosis nigricans because of weight gain, similar clinical features, and histological findings. However, the final diagnosis was hyperkeratotic head and neck Malassezia dermatosis considering the presence of yeast and the dramatic improvement after antifungal treatment. To the best of our knowledge, hyperkeratotic head and neck Malassezia dermatosis is not well characterized in the literature. It is an unusual variant of pityriasis versicolor. The skin lesion appears brown to dark, scaly, and hyperkeratotic macules and patches located on the face and neck. Here, we reported an unusual case of hyperkeratotic head and neck Malassezia dermatosis.

A Study of Clinical Aspects of Scabies in the Jeonbuk Province (전북지역 옴 감염의 임상양상에 대한 연구)

  • Background: Scabies is a contagious skin infestation primarily observed in poor or overcrowded environments. However, an individual may be affected regardless of hygiene and/or socioeconomic status, and/or age. Objective: To investigate the characteristics and epidemiology of scabies in patients visiting a single tertiary hospital in Jeonbuk province. Methods: We retrospectively reviewed the medical records of 194 patients with scabies who visited the Dermatology clinic at Wonkwang University Hospital between June 2015 and May 2018. Results: Of the 194 patients investigated, 82 (42.3%) were men and 112 (57.7%) were women. The mean age of men was 46.3 years and that of women was 60.1 years. Scabies was diagnosed in patients most commonly in autumn. The most common symptom of scabies was the occurrence of papules (83.5%) followed by the presence of burrows (23.2%). The most common route of infection was a nursing hospital (26.3%), except unknown (29.4%). Medications used to treat scabies included 5% permethrin cream, 10% crotamiton ointment, and 1% gamma benzene hexachloride lotion. The mean duration of treatment was 33.9 days using 5% permethrin cream, 34.8 days using 10% crotamiton ointment, and 34.6 days using 1% gamma benzene hexachloride lotion. Conclusion: This study could help in the prevention and management of scabies, by guiding clinicians in choosing optimal therapeutic agents based on patients' condition.

Periocular Allergic Contact Dermatitis Associated with the Use of Ofloxacin Ophthalmic Ointment: A Case Report (Ofloxacin 안연고에 의해 유발된 눈주위 알레르기접촉피부염)

  • Allergic contact dermatitis is an inflammatory condition associated with periorbital erythema, edema, and pruritus. The periorbital skin is relatively thin compared with the skin over other facial areas; therefore, it is vulnerable to allergen penetration and may show a variety of cutaneous manifestations. Recently, vision enhancement surgery is a widely performed procedure, and the prevalence of senile cataract and glaucoma is increasing. The prevalence of periocular allergic contact dermatitis is increasing secondary to the growing use of topical ophthalmic medications. Several studies in Korea have reported periocular allergic contact dermatitis secondary to the use of topical ophthalmic medications including latanoprost ($Latano^{(R)}$), fluorometholone ($Tolon^{(R)}$), polymyxin B ($Terramycin^{(R)}$), atropine sulfate ($Atropine^{(R)}$), neomycin sulfate ($Cambison^{(R)}$), and befunolol hydrochloride ($Bentos^{(R)}$), among others. However, ofloxacin ($Effexin^{(R)})$-induced allergic contact dermatitis has not been reported in the domestic and/or foreign literature. We report a case of periocular allergic contact dermatitis secondary to the use of ofloxacin ophthalmic ointment.

Apoptosis and Autophagy Induced by Methanol Extract of Kochia scoparia in Human Mucoepidermoid Carcinoma Cell Line (점액표피양암종 세포주에서 Kochia scoparia 추출물의 세포자멸과 자가포식 유도 효과)

  • Natural products are vastly utilized as a source of chemotherapeutic agents for human cancers. Kochia scopraia is traditionally used for the cure of urological and dermatological diseases. Recently, methanol extract of Kochia scoparia (MEKS) has been shown to have anti-cancer activity to various human cancers. However, there is no report demonstrating the anti-cancer activity of MEKS in human mucoepidermoid carcinoma (MEC) cells. In this study, the authors studied the effects of MEKS on the cell proliferation and underlying mechanism in YD15 human MEC cells. MEKS decreased YD15 cell proliferation proven by trypan blue exclusion assay and induced apoptosis, evidenced by cell cycle analysis and western blotting. Autophagy induction by MEKS was verified by western blotting. In addition, MEKS regulated the expression of phosphorylated Akt, phosphorylated p38 and Nrf2 protein. This results can imply that MEKS might be a potential candidate for the treatment of human MEC cells.

Spectroscopic Evaluation on the Chemical Damage of Hair by Hydrogen Peroxide (과산화수소에 의한 모발의 화학적 손상에 관한 분광학적 평가)

  • Spectroscopic evaluation of hair chemical damage was performed by SEM/EDS, CLSM, and FT-IR spectroscopy. In SEM/EDS, hydrogen peroxide treated hair showed the loose packing of surface scales, lower ratio of sulfur element and higher ratio of oxygen atom. In the optical single section by using CLSM, high fluorescent intensity appeared in untreated hair. However, in case of treated hair, low fluorescent intensity appeared. This results the aromatic amino acids which can be autofluorescent were more abundant than bleached hair. FT-IR spectra showed that cysteic acid band intensity was increased by performing the bleaching treatments. These results indicate that the oxidative damage cleaves the S-S bond and results in the lower working force of hair fiber.

  • Acne vulgaris is a common dermatological condition affecting the pilosebaceous units and having a multifactorial etiology. In Unani terminology, acne vulgaris is referred as Busoor Labaniya, characterized by white lesions on the face, nose, and cheeks; on squeezing, release cheesy material. In conventional medicine, mild cases are best addressed with topical regimens, but more severe cases require systemic medications. Retinoids (retinoic acid, adapalene, isotretinoin, tazarotene), benzoyl peroxide, clindamycin, erythromycin, and azelaic acid are a few examples of topical medications, whereas systemic drugs include antibiotics (Doxycycline, minocycline, erythromycin, azithromycin). In the Unani System of Medicine, numerous single and compound drugs have been used to treat Busoor Labaniya. These drugs are considered harmless and do not have any major side effects. In Unani system of medicine, acne vulgaris is effectively managed with natural medicines as well as therapeutic regimens with minimal side effects even after long-term usage. Acne vulgaris is usually treated with systemic blood purifiers along with topical Unani drugs. Hence, clinical studies with proper scientific parameters are needed to be conducted to establish and validate their efficacy in the prevention and control of acne vulgaris.

Cerebral fat embolism syndrome: diagnostic challenges and catastrophic outcomes: a case series

  • Fat embolism syndrome is a rare but alarming, life-threatening clinical condition attributed to fat emboli entering the circulation. It usually occurs as a complication of long-bone fractures and joint reconstruction surgery. Neurological manifestations usually occur 12 to 72 hours after the initial insult. These neurological complications include cerebral infarction, spinal cord ischemia, hemorrhagic stroke, seizures, and coma. Other features include an acute confusional state, autonomic dysfunction, and retinal ischemia. In this case series, we describe three patients with fat embolism syndrome who presented with atypical symptoms and signs and with unusual neuroimaging findings. Cerebral fat embolism may occur without any respiratory or dermatological signs. In these cases, diagnosis was established after excluding other differential diagnoses. Neuroimaging using brain magnetic resonance imaging is of paramount importance in establishing a diagnosis. Aggressive hemodynamic and respiratory support from the beginning and consideration of orthopedic surgical intervention within the first 24 hours after trauma are critical to decreased morbidity and mortality.

  • 최근 피부산업은 미용적인 관점뿐만 아니라 피부질환치료에 대한 관심까지 폭넓게 성장하고 있어 피부의 건강을 개선하고 치료할 수 있는 새로운 신기술 개발이 다각적으로 이루어지고 있다. 특히 기술의 고도화와 체계화를 통한 피부과학기술의 진보가 화학, 화학공학, 재료공학을 기반으로 하는 전통학문분야와 조직공학, 바이오나노공학, 감성공학 등을 기반으로 하는 신학문분야가 융복합되어 이루어지고 있다. 따라서, 본 고에서는 피부산업에서 전략적인 응용이 가능한 피부 나노바이오 콜로이드의 연구개발에 대한 최근 현황을 소개하고자 한다.

A Case of Parotid Mucoepidermoid Carcinoma Recurring along the Mask Strap Line (마스크 끈자국을 따라 재발한 이하선 점액표피양암종 1예)

  • Mucoepidermoid carcinoma is one of the most common malignant tumors that occur in the salivary glands. While several cases of distant metastasis have been reported, instances of metastasis to the skin are rare. We present the case of a 72-year-old man diagnosed with parotid mucoepidermoid carcinoma who experienced temporary remission following surgical resection and adjuvant chemoradiotherapy. However, the carcinoma recurred along the skin at the periauricular area along the mask line. Given the current prevalence of COVID-19, the practice of wearing masks has become more widespread. This case report highlights the recurrence of mucoepidermoid carcinoma along the mask strap line, emphasizing the importance of distinguishing it from benign dermatological conditions.
