A case of Drug Eruptions of Soyangin patient mistreated with Taeumin (- 음인(太陰人) 오치(誤治)로 인하여 약진(藥疹)이 발생한 소양인(少陽人) 환자(患者) 1례(例))

  • In this study, it is experienced that Drug eruptions of Soyangin patient mistreated with Taeumin, The patient was 61 years old, female. She entered our hospital for Stroke and sicked Drug eruptions by mistreatment of Taeumin, After we prescribed Yanggyuksanhwatang(凉膈散火湯), had the general improvement of drug eruptions. Drug eruptions is related to Yangdokbalban(陽毒發斑) in Oriental medicine. The Yangdokbalban(陽毒發斑) of Soyangin is classed as Wisuyulliyulbyung(胃受熱裏熱病). This is a kind of dermatitis caused by Wiyul(胃熱). We classified this as Soyangin-Liyuuljeung(小陽人裏熱證) and prescribed Yangdokbaekhotang(陽毒白虎湯) and Yanggyuksanhwatang(凉膈散火湯). In the result, we had the general improvement of Yangdokbalban(陽毒發斑).

A Case of Subcorneal Pustular Dermatitis treated with Oriental Medicine (각질하 농포성 피부병 한방 치험 1례)

  • Objective : Subcorneal pustular dermatosis is a rare, chronic neutrophilic dermatoses that was first described in 1956. The eruption is characterized by flaccid pustules grouped in a circinate pattern on normal or erythematous skin. We treated a case of subcorneal pustular dermatosis in one patient; there have been no former reports of this disease treated by oriental medicine. We present this case because we considered it to be useful as a possible Oriental medicine treatment for subcorneal pustular dermatosis, and something that canbe used for further study of neutrophilic dermatoses. Method : We used herbal medication, acupuncture, herbal wet dressing, herbal ointment, cupping therapy and phototherapy during treatment. We assessed symptoms during treatment and also carried out blood tests through out. Result : We observed and treated a case of subcorneal pustular dermatosis patient and after treatment the patient's objective and subjective symptoms showed noticeable improvement. Conclusion : We suggest that subcorneal pustualr dermatosis can be treated by Oriental medicine therapy effectively. We expect these study results will help provide useful evidence of Oriental medicine as an effective treatment for subcorneal pustualr dermatosis.

The Enhancing Effect of Atopic Dermatitis by cosmetics containing estern medical herbs (한방소재 화장품의 아토피 피부염 개선 효과)

  • Sixteen moderate atopic dermatitis patients were treated with applying toner and cream using estern medical herbs[Estern medical complex(Radix Glycyrrhizae, Radix Angelicae Dahuricae, Rhizoma Ligustici Chuanxiong, Fructus ponciri Seu Aurantii Immaturus, Radix Rehmanniae Praeparata, Radix Puerariae, Rhizoma Corydalis), Cypress, Green tea, Radix Astragali, Red ginseng, Syrup ferment, Apple ferment filtrate] on the atopic area 2-3 times per day for 6 weeks. After 6 weeks of study, the result was a decrease in SCORAD index($26.9{\pm}11.8$ to $14.9{\pm}9.7$(p<0.05)), IgE($641.9{\pm}1294.6$ to $565.8{\pm}1076.8$), Eosinophil count($246.7{\pm}203.5$ to $203.3{\pm}130.7$(p<0.05)) and TEWL($16.7{\pm}5.1$ to $15.4{\pm}8.7$(p<0.05)). And skin hydration was increased($36.6{\pm}10.0$ to $44.0{\pm}10.3$(p<0.01)) maintaining skin pH level. The patient;s and physician's global assessment also improved. Therefore, estern medical herbs may play a role in treatment of atopic dermatitis.

A Case of Adult Otitis Media with Effusion Accompanied by Atopic Dermatitis (아토피피부염을 동반한 성인 삼출성 중이염 치험례)

  • Objectives : This study is to report a case of otitis media with effusion accompanied by atopic dermatitis improved by Korean medicine monotherapy. Methods : A 31-year-old woman patient visited our clinic for aural fullness, hearing loss, and tinnitus of the left ear on August 14th, 2021. She also had abdominal pain, diarrhea, and general weakness because of side effects of antibiotics. Besides, she had nasal congestion, rhinorrhea, and dry skin. Through otoscopy, her tympanum was fully filled with yellow effusion. She only took Korean medicine therapy including acupuncture and herbal medicine weekly. The treatment continued for about 4 months(total 14 visits) until December 4th, 2021. Otoscopy, numerical rating scale(NRS), scoring atopic dermatitis(SCORAD) index were used to assess the symptoms. Results : On August 30, 2021, her intestinal symptoms improved. The left tympanum was recovered to normal condition as the treatment continued. The effusion disappeared on October 16th, 2021. On November 6th, 2021, peritympanic blood congestion also almost alleviated. Subjective symptoms including aural fullness, hearing loss, and tinnitus disappeared on October 16, 2021. Nasal symptoms and dry skin also improved. The SCORAD index dropped from 15 to 4. Conclusions : These results suggests that otitis media with effusion can be sufficiently managed by Korean medicine therapy. Further, active application of Korean medicine therapy to common otolaryngological diseases is necessary.

  • The Schwachman-Diamond syndrome is an autosomal recessive syndrome(1/20,000 births), consisting of pancreatic insufficiency, neutopenia, which may be intermittent, neutrophil chemotaxis defects, metaphyseal dysostosis, failure to thrive and short stature. Patients present in infancy with poor growth and grease, foul-smelling stools that are characteristic of malabsorption. These children can be readily differentiated from those with cystic fibrosis by their normal sweat chloride levels, lack of the cystic fibrosis gene, and characteristic metaphyseal lesions. Pathologically, the pancreatic acini are replaced by fat with little fibrosis. The neutropenia may be cyclic. Recurrent pyogenic infections otitis media, pneumonia, dermatitis(fig 1), sepsis are common and a frequent cause of death. In dental examination, these patients had a poor oral hygine and moderate generalized marginal gingivitis, also show delayed primary tooth exfoliation and oral development. This report illustrates a case that pancreatic agenesis 6 yeas-old boy with various esthetic dental problems has been served the esthetic dental restoration of 6 years.

Clinical Application of PCR-RFLP for the Differentiation of Trichophyton mentagrophytes var. erinacei in the Facial Dermatitis of Household African Pygmy Hedgehog (Erinaceus albiventris)

  • This report describes a case of severe and prolonged dermatophytosis in a hedgehog that was diagnosed by PCR-RFLP, a rapid and usefulness technique for identification of many causative agents and hereditary characters. A 5-month-old female hedgehog was presented with grade 2 facial pruritus, scaling, encrustation and hemorrhage. Cytology of exudates on the face showed a suspected fungal infection. A culture and tape imprint test of the cultured colony showed many hyphae and microcornidia, suspected to belong to the Trichophyton species. In the PCR-RFLP with MvaI and Hinf I, Trichophyton mentagrophytes var. erinacei was finally identified as a causative agent. The patient completely recovered after application of nystatin cream for 17 days.

  • Nummular eczema is well-shaped, coin-like lesion. Usually it is surrounded by the papules and vesicles, and is likely to be the impetigo. It's said that nummular eczema is combined with atopic dermititis, contact dermatitis, bacterial infection and so on. Recently, we experienced a nummular eczema with impetigo and that was successfully treated by the herb-medication and the ceramic resonance therapy. The medications taken by the patient were yunwhasagnatang and gamisopungsan which is used to treat allergic disease and make immune system be strong. The former effects the liver to be cool down and healthy and it also has anti-imflamatory reactions like the corticosteroid hormone. Ceramic resonance therapy utilises irradiation within the frequency range of the radiation emitted by the human body. So we report this case with a brief review of the oriental medical and medical literatures.

  • Nicolau syndrome is a rare complication of intramuscular injection consisting of ischemic necrosis of skin, soft tissue, and muscular tissue that arises locoregionally. The characteristic pattern is pain around the injection site, developing into erythema, a livedoid dermatitis patch, and necrosis of the skin, subcutaneous fat, and muscle tissue. Three patients were injected with drugs (diclofenac sodium, ketoprofen, meperidine) for pain relief. Three patients complained of pain, and a skin lesion was observed, after which necrosis developed on their buttocks. Each patient underwent debridement and coverage. The wound healed uneventfully. We report three cases of Nicolau syndrome in the buttocks following diclofenac intramuscular injection.

  • Dyshidrotic eczema is a chronic vesicular skin disease due to stress, sweating and nickel dermatitis. It has the characteristic symptoms of a cluster of vesicles and frequent recurrence. We diagnosed a 63-year old male patient case with hyperkeratosis, itching and vesicles at his right foot as dyshidrotic eczema and treated him with application of herbal medicine(bo-jung-ik-ki-tang) and acupunture at Dong-su Oriental-medicine hospital in 2003. We had a good result, so we report this case.

  • In clinical practice, Psoriasis is commomly difficult to be completely cured that only repeats improvement and to be worsen. We experienced one case of Psoriasis patient who had had generalized expoliative Psoriasis for more than one year. After hospitalization treatment of ten days and outpatient treatment of three months, he's chief complain improved continuously and there was not worsen sign any more. So, He could return to his usual life, We still did't know the mechanism that Saengkankunbi-Tang brought his improvement by, but we think that more case studies are necessary focusing on damp-heat because of much drinking for long time and years of duration of skin disease.
