Purpose: The purpose of this study was to identify relationships between quality of sleep, symptom cluster, depression, environmental disorder, and quality of life among hospitalized cancer patients. Methods: The subjects were 114 patients who underwent chemotherapy for colon cancer, gastric cancer, gynecologic cancer and breast cancer. They were recruited from the cancer center of a university hospital. Data were collected from August 4th to 30th, 2011. The questionnaires included the Korean sleep scale A (quality of sleep), MDASI-K (symptom cluster), the environmental sleep disturbing scale, Zung's depression scale, and the Korean version of EORTC QLQ-C30. The collected data was analyzed by t-test, ANOVA, multiple regression analysis using the SPSS 19.0 program. Results: Functional QOL was negatively associated with symptom QOL (r=-.798, p<.001). Symptom cluster, depression, & spouse (46.3%) were the most powerful predictors for functional QOL (46.3%) and symptom QOL (53.4%). Conclusion: It is evident that oncology nurses need to evaluate two dimensions of quality of life for cancer patients, for example, functional and symptom QOL. We recommend nurses develop specific protocols for relieving physical symptoms and alleviating depression, and furthermore test the effectiveness of them.
This study aimed to determine the relationship between the C825T polymorphism in the G-protein ${\beta}$3-subunit (GNB3) gene and the response to citalopram in a Korean population with major depressive disorder (MDD). Citalopram was administered for 8 weeks to the 84 MDD patients who completed this study. All subjects were examined using the Structured Clinical Interview for DSM-IV, and the severity of depression was assessed using the 21-item Hamilton Depression Rating (HAMD-21) scale. A main effect of an interaction of genotype with time on the decrease in the HAMD-21 score during the 8-week study period was not found. ANOVA revealed no significant effects of the GNB3 C825T polymorphism on the decrease in the HAMD-21 score at each time period. Although the C825T polymorphism of the GNB3 gene may affect the pathogenesis of MDD, our results do not support the hypothesis that this polymorphism is involved in the therapeutic response to citalopram in Korean patients with MDD.
Objective : The purpose of this study was to investigate effects of depression and anxiety on treatment response of social skills training in adolescents with autism spectrum disorders (ASD). Methods : A total of 107 teenagers that participated in the Program for the Education and Enrichment of Relational Skills (PEERS$^{(R)}$) between 2011 and 2015 were analyzed using linear regression. Outcome measures used were the Korean version of Social Responsiveness Scale (SRS), the Test of Adolescent Social Skills Knowledge-Revised (TASSK-R), and the Korean version of Autism Diagnostic Observation Schedule (K-ADOS). Depression and anxiety were assessed by the Korean version of Child Depression Inventory (CDI), the Korean version of State and Trait Anxiety Inventory for Children (STAIC), and the Korean version of Child Behavior Checklist (K-CBCL). Results : As a result, depressive symptoms measured by CDI (p<0.05) and anxiety symptoms measured by STAIC (p<0.05) had significant influence on score change of social interaction domain of ADOS. It remained significant even after the baseline score was adjusted as covariates (p<0.05). Conclusion : We observed that pre-treatment anxiety and depressive symptoms of teenagers had significant effects on the treatment outcome of PEERS$^{(R)}$, especially in terms of face-to-face social interaction.
Whenever a clinician manages the patients with depression, he may meet various problems that make it difficult to treat them. Even though he has good skills and knowledge about depression, some barriers will be appear during his practice. In general, the difficulties in treating depression are treatment-resistance, adverse effects of antidepressants, pregnancy in female patients, comorbid medical conditions, poor compliance, drug-drug interactions, and so on, which are related with pharmacological treatments. Here, only the two of them, the treatment-resistant depression and difficult problems concerned with pregnancy, were discussed. Some level of treatment resistance is the norm rather than the exception. As the treatment failure stems from inadequate treatment, it is important that the clinician should prescribe medications with sufficient doseage and adequate duration. And to overcome the treatment resistant depression the polypharmacy is necessary, in that case, the side effects and toxicities should be explored and managed immediately. So the clinician have to learn more about the pharmacokinetic and pharmacodynamic mechanisms of each drugs used in treatment of depression. When the risk of the fetus by the exposure is higher than the risk of untreated maternal psychiatric disorder, psychotropic medications should be used during pregnancy. Women who are maintained on psychotropics and become pregnant, as well as women with the new onset of psychiatric symptoms during pregnancy, should be carefully reassessed. However, data concerning the potential risk of long-term behavioral changes following prenatal exposure to psychotropics is rare, so further longitudinal follow-up studies are needed.

성인의 우울증과는 달리 아동기 우울증의 존재와 정의에 대해서는 많은 논란이 있어왔다. DSM-III(1980)에 이르러서야 공식적인 병명으로 인정되었으나, 아직도 아동기 우울증은 다른 정신적인 장애에 비해 진단 준거 자체가 명확하게 정의되어 있지 못할 뿐만 아니라 발달 단계에 따라 우울증상은 다른 양상으로 표현되므로, 아동기 우울증의 정확한 진단을 위해서는 다양한 평가 방법을 사용하여 여러 사람들로부터 정보를 얻는 것이 매우 중요하다. 따라서 본 연구에서는 자기보고형 질문지, 면접, 행동 평정 척도, 구조화된 검사, 투사법 검사 등 주요 평가 방법들을 고찰해보았고, 앞으로 아동기 우울증 평가 방법에서 해결되어야 할 신뢰도 및 타당도에 관한 몇가지 문제점들에 대해 논의하였다.
Purpose: The purpose of this study was to identify family stress, somatization, social support, depression and its influencing factor among middle-aged workers. Methods: The research was cross-sectional descriptive study. The subjects were 212 middle-aged workers living in Seoul and Gyeonggi-do. Data collection was done from May 21 to 31, 2015 using self-reported structured questionnaires asking about general characteristics, family stress, somatization, social support and depression. Data were analyzed using descriptive statistics, independent sample t-test, one way ANOVA, Pearson's correlation coefficient, stepwise multiple regression with SPSS/WIN 19.0 program. Results: Mean score of family stress was 41.67 (range: 25~125), somatization 17.42 (range: 12~60), social support 69.79 (range: 12~84) and depression 13.01 (range: 0~60) and reported as depression in 34.0%. Social support (${\beta}=-.36$, p<.001), somatization (${\beta}=.28$, p<.001), family stress (${\beta}=.15$, p=.014) had significant association with depression and the most important variable was social support. Conclusion: It is suggested to check social support system in middle aged workers and needed to reinforce social support of community based on the relation of occupational category. Also, it is necessary to legalize the institutional devices to prevent and control depression to ensure industrial safety and disaster prevention.
Objectives : This study aimed to investigate frequently used herbal materials among herbal prescription for depression focusing on randomized controlled trial. Methods : Every article relevant to depression was initially obtained from China National Infrastructure(CNKI), Korean database and book hand-searching. Searching keywords were 'depression', 'herbal medicine' and 'randomized controllled trial(RCT)'. Results : Among comorbidity with depression, the most accompanied disease was that of circulatory system. Among sixty-five articles, depression with cerebral vascular disease was twenty-eight. Article about mood disorder was twenty-four. High frequently used herbal materials were Bupleuri Radix(41times), Curcumae Radix(34 times), Acori Graminei Rhizoma and Cnidii Rhizoma(24 times). Conclusions : According to this study, we could know select frequent-used herbal medicine. In a clinical treatment, herbal materials can be added herbal prescription related to depression. As these results, it can be helpful to develop new drugs.
Objectives: The present study aims to review the antidepressant effect of herbal medicines reported in Korean local journals. Methods: We searched in electronic databases (Koreantk, KISS, OASIS, NDSL) for studies, published in Korean national journals, that assessed herbal medicine effect of depression model. The search term was 'depression' in the abstract or whole text. Depression model, herbal material, experimental results, mechanisms were extracted. Results: We included 43 articles in which 38 studies were in vitro experiments, and the rest 5 were in vivo experiments. The most common experiment subject model was a rat and the most widely used method to induce depression was Despair behavior test. 21 studies used simple herbal medicines, and 22 studies used complex herbal medication. Glycyrrhizae Radix was the most commonly used herbal material to improve depression model. The most common mechanisms of herbal medicine with antidepressant effect were inhibition of Monoamine activation mechanism and depression related neurohormone secretion. Conclusions: Herbal medicines may be a promising resource for treating depression.
Purpose : This study examined the effects of an integrated management program on physical function, cognitive function, and depression in patients with subacute stroke. Methods : A nonequivalent control group design was adopted. The participants were assigned to either the experimental group (n=20) or control group (n=23). The experimental group received an 8-week integrated management program and standard rehabilitation service (i.e., physical therapy and occupational therapy), while the control group received the standard rehabilitation service only. Physical function was measured as gait speed and balance ability using the Berg Balance Scale (BBS). Cognitive function was measured with neuro-behavioral cognitive status examination (NCSE), and depression was measured using the Beck Depression Inventory-II (BDI-II). Repeated measure ANOVA was used to determine changes in physical function, cognitive function, and depression over 8-weeks. Results : The interaction between group and time was significant, indicating that the experimental group showed improvement in gait speed, balance ability, cognitive function (linguistic ability, linguistic memory, reasoning), and a decrease in depression compared to the control group. Conclusion : These results indicate that the integrated management program developed herein was beneficial in restoring physical function, cognitive function, and depression in subacute stroke patients.
본 연구에서는 불안과 우울감과 같은 정서상태가 만성적인 통증 인지와 상관관계가 있는지를 알아보고자 PTSD 환자군의 압통에 대한 역치와 불안증상, 우울증상의 정도를 측정하였고, 불안, 우울증상의 정도와 압통역치의 상관관계를 대조군과 비교하였다. 연구대상 환자군은 1997년 10월부터 1998년 3월까지 중앙대학교 의과대학 부속필동병원 및 용산병원 신경정신과에 입원한 환자와 외래환자들 중에서 PTSD에 진단된, 20-60세에 해당되는 성인 남자환자 23명을 대상으로 하였다. 결과는 다음과 같다. 1) PTSD 환자군에서 통계적으로 의미있게 높은 우울증상과 불안증상을 보였다(p<.05). 2) 압통에 대한 역치는 환자군에서 유의하게 높은 결과를 보였다(p<.05). 이런 결과를 보인 요인으로는, 환자군의 불안증상보다는 만성적인 우울증상이 통증역치의 증가에 부분적으로나마 영향을 주었을 것으로 추정되며 그외에 환자군에서의 opiate system의 이상, 환자군의 질병역할, 환자군이 처한 여러 사회환경적인 상황의 영향 등과 같은 여러 요인이 관련되었을 것이라고 생각한다. 결론적으로 PTSD의 만성 통증은 감정, 특히 우울증과 관련이 있을 것으로 보였으나, 그 인과관계에 대한 결론을 내리기에는 미흡한 점들이 있다. 앞서 언급한 여러 제한점들을 보완한 향후 연구를 통해 정서적 증상과 동통과의 보다 특이적인 연관성을 규명하는 연구가 필요할 것이라 생각한다.
