The fifty-year history of dental hygiene education in South Korea

  • The aim of this study was to review the history of dental hygiene education in Korea on its $50^{th}$ anniversary in 2015. Internal resources and documents from the Korean Dental Hygienists' Association and other accessible resources were examined to verify the historical facts of dental hygiene education in Korea. From 1965 to 1967, the first class of four dental hygiene students graduated, and the legal basis for dental hygiene as a profession was established, which led to the start of the profession Registered Dental Hygienist in 1971. From 1977 to 1993, 13 higher educational institutions started dental hygiene programs. The Korean Dental Hygienists' Association (1977) and Korean Association of Dental Hygiene College Professors (1987) were established in this period. From 1994 to 2006, the duration of major-intensive course programs in dental hygiene was extended to three from the two years of 1994, and the first bachelor's degree program in dental hygiene started in 2002. In this period, studies on dental hygiene became significantly active owing to vigorous activities by academic societies. The master's degree program in dental hygiene was established in 2007. Academic dental hygiene journals, the Journal of Dental Hygiene Science, and Journal of Korean Academy of Dental Hygiene Education were promoted as the registered journals of the Korean Research Foundation. From 1965 to 2015, dental hygiene education in South Korea expanded both quantitatively and qualitatively and is now ready for further progress in the future.

연세대학교 치위생학과 구강위생교육실을 방문한 대상자의 구강위생관리 서비스체계에 대한 만족도 조사 (Clients Satisfaction with Oral Hygiene Care Services System Provided by the Dental Hygiene Clinics in the Department of Dental Hygiene, Yonsei University)

  • The setting of dental hygiene clinics is very important to dental hygiene education, which is the place not only to educate students but also to care clients. The purpose of this study is to provide basic research material for improvement of dental hygiene care system in dental hygiene clinics by analyzing the client satisfaction. A questionnaire survey by means of self-entry method was conducted to find out satisfaction of the client, who was visited to the dental hygiene clinics in the department of dental hygiene, Yonsei University. An analysis of frequency, one way ANOVA and T-Test were performed through SPSS 12.0K program. 1. Most clients were mainly composed of students in Wonju College of Medicine. 2. The clients visited for scaling(85.8%) and oral examination(9.73%) were much than treatment(4.42%). 3. Clients aged 21 to 25 were relatively lower in satisfaction with the facilities, system, attitude than any other ages. 4. The dental hygiene students are the lowest group in satisfaction with the facility, system, attitude than medical and nursing students. 5. The clients satisfaction with dental hygiene clinics was decreased in reverse proportion to visiting frequency. 6. Most of the clients pointed out the problems of appointment system(54.0%) and fee(23.0%), which should be improved than any other operation conditions. 7. Most of the clients were not satisfied with chair time and pain during care. 8. Most clients recognized to receive the better care service than other dental offices(81.3%). Especially, they paid attention to oral health education using phase-contrast microscope. 9. Many clients were dissatisfied with facilities of the dental hygiene clinics(71.7%). The problems of appointment system(54.0%) and chair time of dental hygiene care services(63.6%) had also inconvenienced to clients. The dental hygiene clinics in school play a crucial role in dental hygiene education to foster the student to be competent as a professional dental hygienist in the future. Therefore, well-organized dental hygiene care program based on dental hygiene process is essential. It is also required to improve the environment of dental hygiene clinics including facilities, appointment system and fee etc.

한국치위생학회지 게재논문(창간호~2015년)의 연구동향 분석 (Trend analysis of research in the journal of Korean society dental hygiene from 2001 to 2015)

  • Objectives: The purpose of study is to investigate trends in dental hygiene research published in the Journal of Korean Society Dental Hygiene. Methods:Total of 992 researches were published between 2001 to 2015 were reviewed using analysis criteria. Results: 23.7% of the researches were supported financially. The most frequent research topics were oral health (40.9%). 3 to 5 researchers per research (38.5%) was most common and 84.1% of principal researchers were professors. 29 researchers were approved by IRB (2011 to 2015). As for the type of research, 87.1% of quantitative studies adapted survey and 9.5% adapted experiment. Convenient sampling was used in 85.6% of the researches. 32.7% of studies searched information in school. Interventions of dental material were the most in pre experimental design (19.2%) and Interventions of activation of oral health program were the most in quasi experimental design (37.5%). Conclusions: The findings of this study suggested the recent trends and the direction in dental hygiene research.

치위생과정에 기반한 임상치위생학 교육과정 역량 개발을 위한 타당도 평가 (Validity assessment for competency development in clinical dental hygiene education based on dental hygiene process of care)

  • Objectives: Before implementing a competency-based clinical dental hygiene curriculum, it is essential to establish competency development as a foundational educational objective. Therefore, this study aimed to develop the competency of clinical dental hygiene with secured validity using the Delphi survey method. Methods: Dental hygiene competencies were categorized within the dental hygiene process stages, and questions were formulated accordingly. A Delphi survey involving ten qualified experts was conducted to refine the final items based on their review opinions. Results: The expert Delphi survey confirmed that all items met stability criteria, with CVI values of 0.80 or higher, CVR values of 0.60 or higher, and a CV coefficient of variation of 0.5 or less. In total, 42 items were derived. Conclusions: Clinical dental hygiene can contribute to developing specialized dental hygienists if the competencies derived from this study are well applied to the standardized curriculum and operations.

한국 치위생(학)과 표준교육과정안 개발 (Development of Korean standard dental hygiene curriculum proposal)

  • Objectives : In order to make a proposal of a Korean standard dental hygiene curriculum, the job description of the National Health Personnel Licensing Examination Board was analyzed to prepare a job analysis matrix of dental hygienist to select required core competency. Methods : Data were analyzed to test the job validity of dental hygienist and categorize the duties of dental hygienist. A proposal of a standard dental hygiene curriculum was mapped out by making a comparative analysis of the courses necessary for American authorization of dental hygiene curriculum, existing proposals of Korean standard dental hygiene curriculum and the courses of the departments of dental hygiene offered by domestic three-year colleges and four-year universities. Results : Basic medicine courses were reinforced or newly offered to provide an opportunity for students to transfer to a domestic four-year university or to a foreign university. In the field of clinical dental hygiene and practice, preventive dentistry, prophylaxis and practice were integrated, and the names of the related courses were unified to give a chance for students to engage in comprehensive dental hygiene practice. There were different courses for dental clinical science at present. In this proposal, students should earn 125 credits in total. Conclusions : Standard dental hygiene curriculum proposal was required by three-year and four-year colleges across the nation. To make it happen, the original names of dental hygiene courses should be used, and the dental hygiene certification and evaluation institute that is under the umbrella of the Korean Dental Hygienists Association should lay out the standard of the KSDHEP to facilitate the utilization of the standard dental hygiene curriculum proposal.

국내 대학 치위생(학)과의 포괄치위생관리(CDHC) 과정 적용 교육과정 현황 (Use of CDHC-applied educational curricula by dental hygiene departments in Korean universities)

  • Objectives: This study surveyed the application of Comprehensive Dental Hygiene Care (CDHC), based on non-surgical periodontal therapy within the educational curricula of Korean universities and examined whether they recognized the need for CDHC. Methods: This study analyzed data from professors of dental hygiene practicum related subjects in 75 Korean universities. The collected data were analyzed using a Chi-square test, a Mann-Whitney U-test, and a Kruskal Wallis test via SPSS (Ver. 21.0). Results: First, the application scope of CDHC was higher in four-year universities than three-year ones with the scope of several characteristics increasing along with enrollment quota. The application scope of dental hygiene plans and actions increased with the number of patient practices in terms of periodontal evaluations. Second, the needs of the dental hygiene courses were more than 4 points in all characteristics. Although four-year universities exhibited higher points than three-year ones, there was no significant difference found except other test. Finally, all universities based their dental hygiene courses on case histories, oral and maxillofacial information, hard tissue modules, periodontal issues, and other tests, with needs also being high. Meanwhile, the application scope of the dental hygiene course evaluations was lowest in all characteristics. Conclusions: There were no significant differences in dental hygiene curricula among the universities. However, highly qualified and standardized educational courses and lectures should be developed by the Korean evaluation center for dental hygiene curricula using various studies as the basis of theory and practical classes, the total number of terms, number of case studies used, number of students per class, ratio of students to professor, and so on.

임상치위생관리과정 수행 전·후에 따른 구강위생 상태 비교 (Comparison of oral hygiene status by clinical dental hygiene care performance)

  • Objectives : The purpose of this study was to investigate the oral hygiene status before and after the dental hygiene care performance to patients by the dental hygienists. Methods : Subjects were 55 adults visiting to dental clinics in Chungnam. The patients agreed to undergo a dental hygiene care performance. A skilled dental hygienist provided dental hygiene care performance including periodontal status, halitosis and oral hygiene status. Results : The periodontal pocket depth was compared before and after the performance. The depth dropped from 4.02 mm to 2.81 mm, and the value of halitosis dropped from 45.78 bbv to 35.76 bbv. The O'Leary index of the patients dropped form 49.37 to 32.84, and all the differences were statistically significant. Conclusions : Proper use of oral hygiene supplies and regular dental checkup can prevent periodontal diseases. This study will provide the useful information of the effective application of dental hygiene care performance.

임상구강위생 교육과정의 개발 방향 (Developing a Integrated Curriculum for a Clinical Dental Hygiene)

  • The objective of this research was to evaluate a curriculum for dental hygiene program, and to develop a clinical dental hygiene course. Learning objectives of dental hygiene program was used as the measure of courses related to clinical dental hygiene. Dental hygiene process is now recognized as a standard of education and practice of clinical dental hygiene. This study demonstrated that we have to integrate oral prophylaxis and preventive dentistry based on dental hygiene process for clinical dental hygiene education curriculum. And the results indicate that competency in comprehensive dental hygiene care is a priority for dental hygiene program.

Investigation of Dental Hygienists' Practice about Rules on Dental Disputes Prevention

  • Background: This study analyzed the practice of dental medical dispute prevention rules of dental hygienists to present an improvement plan for improving perceived importance and practice and provide data for the development of effective medical dispute prevention programs. Methods: A self-administered questionnaire survey was conducted targeting dental hygienists who were providing assistance at dental hospitals and dental clinics in Seoul and Gyeonggi-do regions from March 22 to April 28, 2022. The questionnaire collected from 273 dental hygienists consisted of eight questions on general characteristics, 30 questions on medical dispute experience, and 14 questions on medical dispute prevention. Results: Complaints showed a high experience rate in 'Consultation & reservation', medical disputes in 'Patient handling (unkind) related', and 'Prosthesis installation and cement removal'. In both the importance and practice of medical dispute prevention rules, 'Preservation of medical records and other medical-related data' was high, and 'Management of patients on standby for a long time' was low in terms of practice. 'Lack of time' and 'Lack of manpower' were cited as reasons for not resolving dental treatment disputes. The importance of dental dispute prevention rules was found to be significant according to age and position, and it was also found to affect the level of practice. Conclusion: Seventy-six-point six percent of the respondents said that education on the prevention of medical disputes was necessary, although they lacked recognition of prevention rules compared to their perceptions and experiences. This study suggested specifying prevention rules in dental hygiene subjects and expanding education, improvement of dental treatment system, revise the law on the range of work to improve the recognition and practice of prevention rules.

국내 표준 임상치위생실무지침의 개발 및 타당성 평가 (Development and evaluation of standards for clinical dental hygiene practice)

  • Objectives: The purpose of this study was to develop applicable standards for clinical dental hygiene practice in Korea and to evaluate their validity. Methods: Based on the standards for clinical dental hygiene practice developed in the United States and Canada, the standards were adapted to be applicable in Korea. The validity of the standards was evaluated by a self-writing questionnaire among 14 professors and 10 clinicians using a developed tool for evaluating the standards. A focus group interview was additionally conducted for clinicians to increase the validity of the standards. Descriptive statistics and Mann-Whitney test were performed using SPSS 25.0. To analyze the content of the focus group interviews, content analysis was conducted. Results: The standards for clinical dental hygiene practice consisted of five elements of professionalism for dental hygienists and a total of 28 items to perform the five stages of dental hygiene process of care (assessment, diagnosis, planning, implementation, and evaluation) and included conceptual meaning, clinical significance, and application methods with examples for each item. Conclusions: The standards for clinical dental hygiene practice developed in this study could contribute to standardizing dental clinical practices provided by dental hygienists. It is necessary to consistently improve the standards that are highly practical, to prevent oral diseases and maintain oral health of the public, based on the results of this validity evaluation.
