Standard and regulations for functional food evaluation cases form overseas (1) Japan For food function indication, Food Nutrition Improvement Act was amended in September 1991 and they managed functional food after setting specific health food in one of classification of special functional foods. For manification of raw material usage, the classification of health functional foods was performed by their application on: the control of internal organ status, cholesterol, blood pressure, mineral absorption, and prevention of dental caries. 치과위생사의 감염관리 실천도를 측정하는 척도를 개발하고 타당화하기 위해 254명의 치과위생사를 대상으로 설문조사를 실시하였다. 예비척도는 리커트 4점 척도로서 21개의 문항으로 구성되었으며, 신뢰도 분석, 탐색요인분석, 확인요인분석을 통해 척도의 차원과 항목을 축소하고 신뢰도와 타당도를 검증하였다. 1. 탐색요인분석과 신뢰도 분석을 통해 4개 요인과 12개의 항목으로 척도를 수정하였다. 수정된 척도의 요인은 '예방접종 및 정기검진'(2항목), '기구 소독 및 멸균'(3항목), '손위생 관리'(4항목), '개인 보호'(3항목)가 추출되었으며, 전체분산 중 58.296%를 설명하고 있다. 예비척도 중 '환경 관리' 요인이 제외되었다. 2. 확인요인분석을 통해 한 문항을 제외하였다. 최종적인 측정모형은 4개 요인과 11개 항목으로 구성되었으며, 모형의 적합도는 $x^2$= 79.593(df = 38, 0 = 0.000), RMR= 0.045, GFI = 0.940, CFI = 0.904, AGFI = 0.896, NFI = 0.837, TLI = 0.861, RMSEA = 0.67로 나타나 대체로 기준을 충족하였다. 모든 측정변수의 요인부하량은 유의하였으나(p < 0.001), 세 변수만이 0.7 이상으로 나타났다. 평균분산추출값과 구성개념신뢰도는 대체로 기준을 충족하지 못하였으나, 모든 요인의 평균분산추출값은 각 요인간 상관관계제곱값 보다 크게 나타났다. 3. 요인분석을 통해 추출한 네 개의 요인이 치과위생사의 '감염관리 실천률 인식'에 미치는 영향을 알아보기 위해 네 요인을 독립변수로 선정하고 실천률 인식을 종속변수로 선정하여 회귀분석을 실시하였다. $R^2$는 0.304, 수정된 $R^2$은 0.431, F = 25.813, p = 0.000이며, '손위생 관리'를 제외한 세 변수의 회귀계수는 통계적으로 유의하게 나타났다.
Purpose: Deproteinized bovine bone or synthetic hydroxyapatite are 2 prevalent bone grafting materials used in the clinical treatment of peri-implant bone defects. However, the differences in bone formation among these materials remain unclear. This study evaluated osteogenesis kinetics in peri-implant defects using 2 types of deproteinized bovine bone (Bio-Oss® and Bio-Oss/Collagen®) and 2 types of synthetic hydroxyapatite (Apaceram-AX® and Refit®). We considered factors including newly generated bone volume; bone, osteoid, and material occupancy; and bone-to-implant contact. Methods: A beagle model with a mandibular defect was created by extracting the bilateral mandibular third and fourth premolars. Simultaneously, an implant was inserted into the defect, and the space between the implant and the surrounding bone walls was filled with Bio-Oss, Bio-Oss/Collagen, Apaceram-AX, Refit, or autologous bone. Micro-computed tomography and histological analyses were conducted at 3 and 6 months postoperatively (Refit and autologous bone were not included at the 6-month time point due to their rapid absorption). Results: All materials demonstrated excellent biocompatibility and osteoconductivity. At 3 months, Bio-Oss and Apaceram-AX exhibited significantly greater volumes of formation than the other materials, with Bio-Oss having a marginally higher amount. However, this outcome was reversed at 6 months, with no significant difference between the 2 materials at either time point. Apaceram-AX displayed notably slower bioresorption and the largest quantity of residual material at both time points. In contrast, Refit had significantly greater bioresorption, with complete resorption and rapid maturation involving cortical bone formation at the crest at 3 months, Refit demonstrated the highest mineralized tissue and osteoid occupancy after 3 months, albeit without statistical significance. Conclusions: Overall, the materials demonstrated varying post-implantation behaviors in vivo. Thus, in a clinical setting, both the properties of these materials and the specific conditions of the defects needing reinforcement should be considered to identify the most suitable material.
Mineral Trioxide Aggregate(MTA) has been used in Endodontic treatment successfully for more than 10 years. But the high cost of MTA limits its use in endodontics in Korea. Recently many studies have been done to compare MTA and Portland cements. To investigate the chemical constitutions of MTA (Proroot MTA, Tulsa Dental), Gray Portland cement (Lafarge Halla cement), White Portland cement(Union corp), and fast setting cement (SSangyong cement), we performed SEM(scanning electron microscope)(S4700, Hitachi) examination and EDS(Energy dispersive spectrometry)(emax, Horiba) analysis. SEM examination and EDS analysis were committed to and performed in SNU DRI (Seoul National University Dental Research Institute). We found that particles of MTA were relatively round, uniform in size, and compactly packed compared to Portland cements. Chemical constitutions of MTA, GPC, WPC and FSC were similar. It was shown that MTA contains much BiO2 . MTA and WPC showed less heavy metals such as Fe and Mg compared to GPC and FSC. FSC showed remarkably high aluminum content.
Altered consciousness may be the first clinical sign of a serious medical problem that requires immediate and intensive therapy to maintain life. There are many causes of the loss of consciousness in the dental office setting, such as, vasodepressor syncope, drug administration or ingestion, orthostatic hypotension, epilepsy, hypoglycemic reaction, acute adrenal insufficiency, cerebrovascular accident, hyperglycemic reaction, acute myocardial infarction, acute allergic reaction and hyperventilation. This is a case report of syncope and coma during endodontic treatment of a maxillary third molar under local infiltration anesthesia in multiple medically compromised patient. The main cause was thought to be hypoglycemic reaction. The patient was transferred to the medical emergency room and cared properly by the emergency medical physicians. The prognosis was good.

Background: Emergencies in outpatient clinics are rare. However, potentially catastrophic events can be challenging to manage due to a variety of factors, including limited equipment and staff. The purpose of this quality improvement project was to improve the staff knowledge and familiarity with critical performance elements for emergencies encountered in the setting of a periodontics clinic. Methods: Emergency cognitive aids tailored to the clinic's resources were created for anaphylaxis, airway obstruction, and sublingual hemorrhage. The project pre-post-test repeated measures design evaluated the effectiveness of cognitive aids using a combination of hands-on simulation, written knowledge assessments, and self-efficacy surveys. Training sessions and simulations were provided to the clinic's existing care teams made up of a periodontist and two dental assistants with an anesthetist who was present for simulations involving sedation. Due to the small sample size (N = 14) and non-normal distribution, all metrics were evaluated using non-parametric statistics. Results: Significant improvements were found in knowledge assessment (-2.310, P = 0.021) and self-efficacy (-2.486, P = 0.013) scores when retention after a training session before and after the introduction of cognitive aid was compared. The mean simulation scores and times improved steadily or reached maximum scores during the project progression. Conclusion: Training sessions before and after cognitive aid introduction were effective in improving knowledge, self-efficacy, and simulation performance. Future projects should focus on validating the process for creating contextualized cognitive aids and evaluating the effectiveness of these cognitive aids in larger samples.
Background: Reconstructive surgery is often required for tumors of the oral and maxillofacial region, irrespective of whether they are benign or malignant, the area involved, and the tumor size. Recently, three-dimensional (3D) models are increasingly used in reconstructive surgery. However, these models have rarely been adapted for the fabrication of custom-made reconstruction materials. In this report, we present a case of maxillary reconstruction using a laboratory-engineered, custom-made mesh plate from a 3D model. Case presentation: The patient was a 56-year-old female, who had undergone maxillary resection in 2011 for intraoral squamous cell carcinoma that presented as a swelling of the anterior maxillary gingiva. Five years later, there was no recurrence of the malignant tumor and a maxillary reconstruction was planned. Computed tomography (CT) revealed a large bony defect in the dental-alveolar area of the anterior maxilla. Using the CT data, a 3D model of the maxilla was prepared, and the site of reconstruction determined. A custom-made mesh plate was fabricated using the 3D model (Okada Medical Supply, Tokyo, Japan). We performed the reconstruction using the custom-made titanium mesh plate and the particulate cancellous bone and marrow graft from her iliac bone. We employed the tunneling flap technique without alveolar crest incision, to prevent surgical wound dehiscence, mesh exposure, and alveolar bone loss. Ten months later, three dental implants were inserted in the graft. Before the final crown setting, we performed a gingivoplasty with palate mucosal graft. The patient has expressed total satisfaction with both the functional and esthetic outcomes of the procedure. Conclusion: We have successfully performed a maxillary and dental reconstruction using a custom-made, pre-bent titanium mesh plate.
Background: In dental intravenous sedation, continuous intravenous infusion of a low-dose drug requires an infusion pump such as a syringe pump. To develop a new syringe pump for clinical use, the functions of the pump must meet certain international standards. Various safety and efficacy tests must be performed on the syringe pump, as stipulated by these standards, and an approval must be received from the approving agency based on such test results. Methods: The authors of the present study developed a novel syringe pump and performed efficacy evaluation by testing its infusion speed at 1 and 25 ml/h, and infusion performance testing at 2 and 24 h. Moreover, performance evaluation was conducted by comparing the novel pump to an existing pump with the infusion speed varied from 1 to 5 ml/h. Results: In the efficacy testing on the newly developed syringe pump, infusion with the infusion speed initially set to 1 ml/h resulted in infusion speeds of 1.00 and 0.99 ml/h in the 2- and 24-h assessment, respectively. Changing the infusion speed setting to 25 ml/h resulted in an infusion speed of 25.09 and 23.92 ml/h in the 2- and 24-h assessment, respectively. These results show no significant differences when compared with other commercially available pumps. Conclusions: The efficacy testing of the newly developed syringe pump showed the accuracy to be within tolerance. Based on these findings, we believe that the newly developed syringe pump is suitable for clinical use.
Objectives : The purpose of this study was to examine the educational effects of team learning by having learners keep a journal to record, check and reflect on their own learning. It's basically meant to investigate the effects of team learning on learning motivation and academic achievements to provide some information on teaching-learning strategy setting tailored to learner characteristics. Methods : The subjects in this study were 49 college sophomores. After they were divided into an experimental group and a control group, the experimental group of 24 students engaged in team learning from March to June, 2012, and that group was asked to keep a reflective journal every week to determine whether writing a reflective journal in every session served to boost their learning motivation and academic achievement or not. Results : 1. After the experimental group kept a reflective journal, that group underwent a significant change in learning motivation from a mean of 3.34 in the pretest to a mean of 4.01 in the posttest(p<0.05). The experimental group showed a more positive improvement than the control group. 2. After the experimental group kept a reflective journal, they underwent a significant change in academic achievement from 22.7 in the pretest to 23.5 in the posttest(p<0.05). The control group showed no significant improvement in academic achievement. Conclusions : The findings of the study illustrated that keeping a reflective journal had a positive impact on the learning motivation of the learners, and the findings of the study are expected to lay the foundation for future research in new teaching methods for the department of dental hygiene.
Objectives : The purpose of this study was to examine students' difficulties in the process of oral radiography practice, to raise awareness of the importance and necessity of oral radiography and decipher, and to provide some information on effective ways of oral radiography practice. Methods : The subjects in this study were 285 dental hygiene students at K college, who included 153 sophomores and 132 graduates-to-be from June to November, 2010. Results : 1. The parts of the anatomy structure that they found it most difficult to decipher were maxillary molar(25.3%) and lower molar(22.1%). 2. They made during oral radiography was an improper film positioning(35.1%). 3. The part of bisecting technique was adjusting vertical and horizontal angles(53.0%). 4. The part of paralleling technique was positioning XCP in the oral cavity(44.2%). 5. The part of bite-wing technique was adjusting vertical and horizontal angles(38.2%). 6. The part of occlusion technique was positioning film and tube head(36.5%). 7. The part of panorama technique was finding out program setting(42.5%). Conclusions : The findings of the study indicated that in terms of anatomy structure decipher, it's especially difficult to decipher maxillary molar and lower molar, and that film positioning was difficult to do in the process of oral radiography. What difficulties they faced in applying each kind of oral radiography techniques and which part of the oral cavity they found it hard to radiograph were analyzed as well in this study. Given the findings of the study, more intensive practice is required to help students to acquire accurate oral radiography techniques to ensure their successful job performance in the future.
