치과병의원에 근무하는 치과위생사의 사회적 가치관과 보건의료체계에 대한 가치관의 관련성 (The relationship between social values and health system values in dental healthcare hygienist)

  • Objectives : This study was to examine the social value of dental hygienists, their values about the health system and the relationship of all the related variables. Methods : The subjects in this study were 205 dental hygienists who worked in dental clinics and hospitals on Seoul. A survey was conducted from August 12 to October 15, 2010. The questionnaire consisted of nine items about general characteristics, two items about social values and 11 about values of the health system. The items related to social values and values of the health system were prepared by translating the items used in David et al's study, and the Cronbach alpha coefficient of those items respectively 0.80 and 0.76. Results : The dental hygienists got 3.94 in social values, which was above the average. In terms of values about the health care system, their values of the treatment delivery system(3.92) rated highest, followed by values of patient rights(3.79) and values of institutional restrictions(3.25). Their socal values had a closest positive correlation to their values of the treatment delivery system, and their values of patient rights had a strong positive correlation to those of the treatment delivery system and was positively correlated to those of institutional regulations as well. And there was a positive correlation between their values of the treatment delivery system and institutional regulations(r=.276). Conclusions : The above-mentioned findings illustrated that the social values of the dental hygienists had a positive correlation to their values of the health system. Therefore the kinds of educational programs that help dental hygienists to build their social values and values of the health system should be developed to improve their job efficiency as oral health experts.

치과위생사의 자기결정성동기가 직무열의에 미치는 영향 및 보상 만족도의 조절효과 (Effect of Self-determination Motivation on Job Engagement and the Moderating Effect of Compensation Satisfaction in Dental Hygienists)

  • Purpose : The purpose of this study was two-fold: to identify the effect of dental hygienist's self-determination motivation on their job engagement and to analyze the moderating effect of compensation satisfaction that affect the relevance. Methods : Data were collected using a structured self-report questionnaire administered to 260 dental hygienists working in dental hospitals and clinics in Busan, South Korea. These were analyzed using IBM SPSS Statistics version 26.0 and SPSS Process Macro 3.5. A frequency analysis, including the respondents' general characteristics, frequency, percentages, and standard deviations, was performed. A regression analysis was also performed using SPSS Process Macro to verify the moderating effect of compensation satisfaction in the effect of self-determination motivation on job engagement. Results : The self-determination motivation of the dental hygienists had a statistically significant positive effect on their job engagement, which was the dependent variable. Higher intangible compensation satisfaction levels led to a stronger effect of intrinsic motivation but a weaker effect of extrinsic motivation on job engagement. Moreover, higher tangible compensation satisfaction levels strengthened the effect of intrinsic motivation on job engagement. Conclusion : Recently, the demand for oral health care has been increasing, Competition in the dental medical service market warrants, high-quality dental services based on accurate diagnosis and treatment. In this context, dental hygienists' job engagement must be improved. For this purpose, increasing the satisfaction of dental hygienists with self-determination motivation and appropriate compensation is effective. In addition, attention must be paid to the moderating effect of compensation satisfaction on the relationship between intrinsic motivation and job engagement. On the basis of the implications of this study, the results can be used as basic data for improving dental hygienists' welfare system and manpower management.

치과병원에서 발생하는 소음에 대한 치과위생사의 반응 (A Study on the Dental Hygienists' Reactions to Noise When Occurred in Dental Clinic)

  • 본 연구는 치과병원에서 발생하는 진료시(스케일링, 치아절삭) 및 비진료시(기기만 가동) 가동되는 치료기기의 소음이 치과위생사들에게 미치는 영향 정도를 파악하기 위하여 NR 평가방법으로 분석하였으며 치과위생사의 기기소음에 대한 반응을 설문조사를 통하여 조사하였다. 그 결과는 다음과 같다. 1. 치과병원 기기의 진료 시 및 비진료 시의 주파수 특성의 경우 고주파로 갈수록 소음도가 높게 나타났으며 발생소음도의 대부분이 고주파 성분(4 KHz이상)에 의하여 영향을 받고 있으며 발생하는 소음레벨 dB(A)의 범위는 67.7~78.3 dB(A)로 치과위생사의 소음피해가 예상되는 수준으로 소음 영향을 최소화하기 위한 대책이 시급하다고 사료된다. 2. 치과병원에서 발생하는 소음에 대한 반응을 조사한 결과 응답자의 57.8%가 "왠지 불안하게 된다"라고 응답하였으며 치과위생사 경력이 5년 이하이고 30세 이하인 경우에는 "왠지 불안하게 된다", 경력이 5년 이상이고 31세 이상인 경우에는 "아무렇지도 않다"라고 응답한 비율이 높게 나타난 것으로 조사되었다(p<0.05, p<0.01). 이는 발생소음레벨이 소음이 인체에 미치는 영향에서 "청력손실의 발생 시작" 수준으로 경력과 연령이 많을수록 그만 큼 소음에 만성적으로 노출된 결과로 판단된다. 3. NR곡선에 의한 평가 결과 스케일링 치료를 할 경우 NR-78, 치아 삭제 시 NR-77, 기기만 가동 되는 경우 NR-67로 나타나 작업장의 소음기준을 훨씬 초과하는 수준임을 알 수 있었다. 특히, 스케일링 치료시 소음피해를 최소화하기 위해서 4 kHz이상의 고주파대역을 제어할 수 있는 방음대책을 수립하여 치과 위생사에게 미치는 소음에 대한 영향을 줄일 필요가 있는 것으로 판단된다. 4. 소음으로 인하여 발생되는 제반 문제와 병원환경만족 등의 변수에 대한 상관관계 분석결과 기기에서 발생하는 소음으로 인하여 기분이 거슬리게 되면 환자에게 진료결과를 충분히 설명하지 못하는 경우의 상관계수가 0.677로 유의미한 정적 상관관계(p<0.01), 치과위생사의 "병원환경만족도"와 "환자들로부터 기기소음에 대한 불평을 듣는" 항목에서 -0.595, "기기발생 소음으로 피곤을 느낀다" 항목에서 -0.343으로 유의미한 부정적 상관관계(p<0.01)를 보이고 있었다. 5. 소음에 노출되어 기분 거슬림이나 피곤을 느끼게 되면 진료결과를 충분히 설명하지 못하게 되고 병원이 소란할수록 병원환경만족도가 낮은 것으로 조사되어 치과병원의 기기소음에 대한 적절한 대책 수립이 필요하며 이를 통하여 치과종사자의 쾌적한 근무여건을 제공, 치과병원의 경쟁력 제고 측면에서 긍정적인 영향을 미칠 것으로 판단된다 치과 병원 기기에서 발생하는 소음이 치과위생사들에게 미치는 영향정도를 파악하고자 소음특성을 측정 및 설문 조사를 통하여 분석한 결과 기기발생 소음특성이 치과위생사에게 기분이 거슬리거나 피로를 느끼게 하는 수준이며 이로 인하여 환자들에게 진료결과를 충분히 설명하려는데 지장을 받고 있으며 병원이 시끄럽다고 느낄수록 병원환경만족도가 낮은 것으로 조사되었다. 치과위생사에 대한 적절한 방음대책(방음보호구 제공, 저소음 저진동 장비의 선택, 마스킹 효과 등)을 수립하여 쾌적한 근무여건을 제공하므로써 치과위생사들의 병원환경 만족도를 향상시켜 치과의료 서비스의 질 및 경쟁력 향상이 가능할 것으로 사료된다.

치과위생사의 보수교육 참여동기와 교육만족도에 관한 연구 (Participation motivation and satisfaction of continuing education in the dental hygienists)

  • Objectives: The purpose of the study is to investigate the participation motivation and satisfaction of continuing education in the dental hygienists. Methods: A self-reported questionnaire was completed by 900 dental hygienists in Seoul and Gyeonggido in continuing education 2015. The questionnaire consisted of general characteristics of the subjects (7 items), continuing education requirements (6 items), and continuing education evaluation (9 items). Cronbach's alpha was 0.859 in participation motivation and 0.901 in satisfaction. Likert five point scale was used. Data were analyzed by SPSS 22.0 program. Results: The priorities of the contents were as follows; patient care and counseling(49.0%), the latest information and technology(43.9%), middle management roles(23.3%), health care management(17.6%), and lecture studies(4.4%) in order by multiple responses survey. The favorite instructors were specialized dental hygienist(52.3%). The ideal pay for education fee was 50 percent supported by the institution. The best official announcement of education was e-mail. The participation motivation of education was 4.45 points and was focused on the education completion issue. The lowest score was 2.77 of development of human relationships between participants. The choice for instructor was 3.43 and the lowest score was 2.49 of education fee. Overall satisfaction score was 3.04. Conclusions: Diversification of the continuing education is very important. It is necessary to develop and implement the education methods and to train the dental hygiene specialists for the education.

치과위생사의 전신질환과 관련된 응급처치 지식 측정도구 개발 (Dental hygienist's knowledge measurement tool for emergency treatment related to systemic disease)

  • Objectives: This study was conducted by clinical dentistry hygienists to develop knowledge measurement tools and evaluate knowledge necessary to cope with emergencies related to systemic diseases occurring in dentistry. Methods: Basic questions were developed on the six most frequently occurring diseases in the dental office: diabetes, syncope, respiratory disease, coronary artery disease, cerebrovascular disease, and epilepsy. Fifty-eight preliminary questions were composed in a survey format through expert validation. The survey was then conducted on 290 dental hygienists to verify the composition validity and reliability. Factor and reliability analyses were performed using the SPSS 23.0 program, and the correct answer rate for each question was calculated. Results: 49 items of a total of 13 factors were adopted as the final items for signs, symptoms, prevention and treatment of five diseases. The question with the highest percentage of correct answers for each question was "Slowly set up the unit chair when raising the patient"(97.9%) during the prevention of unconsciousness and question with the lowest correct answer rate was "soft substances such as cloth should be put in the mouth to prevent trauma while the seizure persists" (16.6%). Conclusions: A total of 49 questions have been developed as tools to measure the knowledge of dental hygienists' first aid related to systemic diseases and it is expected that related curriculum can be operated or evaluated by using them.

일부 치과위생사의 직무스트레스 및 사회심리적 스트레스와의 관련성 연구 : 대구지역 중심으로 (Analysis of Correlation between Dental Hygienist's Job Stress and Social Psychological Stress)

  • 일부지역 치과위생사를 대상으로 일반적 특성 및 근무특성, 직무스트레스, 사회심리 스트레스와의 관련성을 파악하기 위해 치과위생사 219명을 대상으로 최종분석결과 치과위생사의 직무스트레스 총점은 $47.41{\pm}7.46$이었고, 사회심리적 스트레스의 총점은 $23.05{\pm}7.49$이었다. 연령이 낮을수록 직위가 낮을수록 미혼일수록 물리환경, 직무자율성, 부상부적절에 대한 직무스트레스가 높았고, 수입, 학력이 낮을수록 직무경력기간이 짧을수록 직무자율에 대한 스트레스가 높게 나타났다. 일평균근무시간이 길수록 물리환경, 직무요구에 대한 스트레스가 높았다. 물리환경, 직무요구, 직무자율성, 직무불안정, 보상부적절, 직장문화에 대한 직무스트레스가 높을수록 사회심리 스트레스가 높아지는 양의 상관관계를 가졌다. 직무불안정, 조직체계, 보상부적절, 직급이 높을수록 물리환경 스트레스가 많을수록 사회심리 스트레스에 영향을 미치는 것으로 나타났다.

치과의료기관의 특성에 따른 감염관리실태 -대구, 경북지역을 중심으로- (Infection Control practices in accordance characteristic of Dental institutions -Daegu and North Gyeongsang Province around-)

  • 본 연구는 치과의료기관의 감염관리실태의 규명과 감염예방을 위해 대구, 경북지역 2012년, 2017년 2개 년도 동일한 114개의 치과의료기관의 특성에 따른 감염관리실태와 오폐수처리 및 기구소독을 설문조사하였다. 감염관리실태 8문항 평균 3.37이며 '환자의 질병병력 기록'은 95.5%로 높고 '오폐수시설이 있는 경우' 1.8%로 낮았다. 오폐수처리 및 기구소독의 9항목 평균 4.87이며 '메탈트레이 소독' 94.7%로 높고, '타구통 폐기물을 폐기물통에 보관 후 위탁처리' 17.5%, '로 낮았다. 일반적인 특징과 치과의료기관 특성에 따른 감염관리실태와 오폐수처리 및 기구소독은 2012년보다 2017년이 높았으며, 치과위생사, 대학졸업이상, 환자상담 및 관리, 병원급 이상, 치과의사 수(치과위생사 수, 체어 수, 환자 수)는 많은 경우 높았다. 총괄적으로 치과 특성을 고려하여 감염관리를 하여야 할 것이며 병원규모와 관계없이 지속적인 교육과 홍보를 필요할 것으로 사료된다.

디지털 구강스캐너 임상적용 경험에 따른 치과위생사의 인지도 및 선호도 변화에 관한 융합연구 (A Convergence Study on the Changes of Awareness and Preference according to the Clinical Application Experience of Digital Intraoral Scanners in Dental Hygienists)

  • 본 연구는 디지털 구강스캐너 사용 경험에 따른 치과위생사의 인지도 및 선호도에 대한 변화를 융합연구를 통해 파악하고자 하였으며, 부산광역시와 경상 남 북도 지역의 치과위생사를 대상으로 온라인 설문에 의해 수집된 자료를 SPSS 24.0 프로그램을 통해 분석하였다. 디지털 구강스캐너 사용경험이 있을수록 디지털 구강스캐너의 인지도와 선호도가 유의하게 높게 나타났으며, 디지털 구강스캐너 사용경험은 디지털 구강스캐너 임상적용 경험과 디지털 구강스캐너 임상적용 경험은 디지털 구강스캐너 인지도와 디지털 구강스캐너의 인지도는 디지털 구강스캐너의 선호도와 정적 상관이 나타났다. 결국 치과위생사의 디지털 구강스캐너 임상적용 경험이 있을수록 그 인지도 및 선호도가 높아져 치과위생사의 업무 능력이 향상될 것으로 기대됨에 따라 디지털 구강스캐너 관련 지속적인 교육과 학습이 필요하다고 판단된다.

치과위생사 직업인식과 직무역량 강화방안 (Dental hygienist's job recognition and vocational competency reinforcement)

  • Objectives: The aim of this study was to analyze the qualities and problems to be solved to strengthen the vocational competency of dental hygienists and to identify the work to be institutionally established. Methods: From April 1 to May 30, 2019, a total of 325 people participated (152 registered dental hygienists: RDH, 173 students of dental hygiene: SDH) in this study. The survey items were as follows: "job awareness" (6 items), "problem to be solved to strengthen vocational competency" (5 items), "qualities to be equipped to strengthen"competency" (9 items), and "practical work to be institutionally established" (4 items). Results: RDH and SDH had the highest perception of professionalism (3.68, 3.99 points), and low remuneration against work (4.21, 4.18 points) was perceived as the most important problem to be solved. The qualities for competency reinforcement showed the highest results in expertise knowledge (4.68, 4.64 points) and vocation for job (3.76, 4.09 points). Practical work to be established institutionally appeared as follows: X-ray film reading, dental hygiene diagnosis, periodontal pocket measurement, and periodontal pockets (<6 mm) treatment. Conclusions: This study is expected to be actively used to strengthen the vocational competency among dental hygienists, construct work autonomy, and rationalize and legalize practical work.

치과위생사의 긍정심리자본이 직무만족과 직무성과에 미치는 영향 (Impact of dental hygienist's positive psychological capital on job satisfaction and job performance)

  • Objectives: The purpose of this study was to understand the effect of positive psychological capital on job satisfaction and job performance for dental hygienists. Methods: A survey was conducted from August to October 2020 among dental hygienists from a dental clinic in Busan, South Korea, and a total of 162 valid responses were analyzed. The statistical tool used for analysis was SPSS 25.0 (IBM Crop, Armonk, USA) and the significance level was 0.05. Results: The results of this study showed the relationships between job satisfaction with positive psychological capital (r=0.85, p<0.01), as well as job performance with positive psychological capital (r=0.88, p<0.01). There was a positive correlation between positive psychological capital and job satisfaction (r=0.85, p<0.01); and the contributing factors for the same were hope (p<0.05) and optimism (p<0.001). Further, positive psychological capital and job performance were positively correlated (r=0.88, p<0.01); the influencing factors for the same were hope (p<0.01), dynamic stability (p<0.05), and optimism (p<0.001). Conclusions: It is necessary to develop various intervention programs and strategies to strengthen the positive psychological capital in dental hygienists since it affects their job satisfaction and job performance.
