치과 병·의원의 의료서비스 품질이 환자만족도와 재이용 의도에 미치는 영향 (The influence that the medical service quality of the dental clinic affects to the patient satisfaction and use intention)

  • Purpose: This aim of this paper is to research the influence of customer satisfaction and reutilization on Dental hospitals and clinics of medical service quality. Methods: The study examines 140 Patients who received treatment in Daegu from 1 July 2012 to 30 July 2012. The methods of research were used to factor analysis, reliability test using Cronbach's ${\alpha}$ factor, correlation analysis, multiple regression analysis and spss/win18.0. Results: The factor of quality of care has the highest impact on customer satisfaction. the influence of reutilization depends on the highest reliability and the physical environment was significant. Conclusion: To improve customer satisfaction and reutilization is required to maintenance of hospital facilities, medical services mind and quality of care. The medical service providers are important to develop patient education programs, self-development and customer satisfaction training techniques.

  • This study is to make legal suggestions concerning the legal status of the physical therapists and the their clinics io Korea. This study compares the legal status of the physical therapists with that of the optical and dental technicians, and reviews the current system of the American physical therapeutic clinic. 1. Under the supervision of the doctor or dentist' in Article 1 of Law concerning the Medical Technician should be deleted or changed into 'by the request of the doctor or dentist'. A new independent law should be mode only fer the physical therapist from the general law that stipulates the legal status of other similar medical technicians. 3. The legal status of the physical therapeutic clinic should be stipulated in the regulations for the application of the law as that of the dental technician does. The modification of the medical services, the medical expences, waiting time, the easy access to the clinic of farmers, fishermen, urban laborers, and handicapped people, and the development of the area of the physical therapy.

서울대학교 치과병원 장애인진료실의 외래환자마취 실태 분석 (ANALYSIS ON THE OUTPATIENT ANESTHESIA AT DENTAL CLINIC FOR DISABLED IN SEOUL NATIONAL UNIVERSITY DENTAL HOSPITAL)

  • 치과적 장애인이란 치과진료 시 치과의사에게 자발적인 협조가 힘든 사람을 의미한다. 본 연구는 서울대학 치과병원 장애인진료실에서 치료받은 치과적 장애인 환자들의 임상진료 실태를 마취과적으로 분석하고 향후 장애인 치과치료에 대한 지침을 마련하고자 하였다. 1999년 1월부터 2002년 10월까지 서울대학교 치과병원 장애인진료실에 내원한 장애인 환자들을 대상으로 조사한 결과, 총 89명의 환자들에게 93건의 치과진료가 시행되었다. 환자의 대다수는 정신지체였고 전신마취 하 보존치료를 받았다. 주로 마취유도제는 thiopental, 근이완제는 vecuronium을 이용하였으며 산소, 아산화질소, enflurane을 함께 흡입시켜 마취를 유지하였다. 총 마취지속시간과 회복실 체류시간은 각각 $164.4{\pm}57.2$ 분과 $106.2{\pm}50.5$ 분이었고 회복 과정에서 심각한 합병증은 관찰되지 않았다. 본 연구는 외래환자마취에 기반한 서울대학교 치과병원 장애인진료실의 치료방침에 따라 특별한 문제없이 성공적인 마취가 가능하였음을 보여 주었다. 또한 증가하는 장애인 치과치료에 대한 수요를 충족시키기 위해서 적절한 시설과 인력을 갖춘 보다 맡은 외래환자마취 센터의 필요성도 제시한다고 할 수 있다.

Recruitment and enrollment in a randomized clinical trial of mandibular two-implant overdenture

  • PURPOSE. The purpose of this study was to evaluate the effectiveness of a promotion campaign for subject recruitment and selection, and reasons of withdrawal from a prospective clinical trial of mandibular two-implant supported overdenture. MATERIALS AND METHODS. The subjects of this study were participants in a randomized controlled clinical trial for investigating prognosis of implants and overdentures with attachments. Recruited subjects were classified by gender, age, and participation motives. Withdrawal rate of the participants before and after enrollment were evaluated. RESULTS. 177 patients were recruited and 51 patients were enrolled for the trial. Among them, 40 participants eventually took part in the trial. 116 subjects (65.5%) were recruited by advertisement and 61 (34.5%) were referred by patients of the hospital or local clinics. Regarding recruitment effectiveness, newspaper recruited the largest number of participants. With respect to referral patients, the proportion of our hospital patients was higher (37/61). Subjects in their 70s comprised the largest proportion (22/51). The male to female ratio was similar (25:26). Final withdrawal rate of all subjects were 74.0%. Among the reasons for withdrawal from enrollment (n=126) presence of remaining teeth and lack of motivation were the most common reasons. CONCLUSION. To facilitate recruitment of clinical trial subjects and improve enrollment rate, it is important to obtain a sufficient number of researchers, perform promotion activity with diverse strategies, cooperate with local dentists, increase the research funding, and alleviate subjects' fear against clinical trials by thorough consultation.

국소마취하 상악 측절치 치근단 절제술 후 인식된 신경병성 비정형 치통 1예 -증례 보고- (A Neuropathic Atypical Odontalgia Recognized after the Apicoectomy under Local Anesthesia on the Maxillary Lateral Incisor -A Case Report-)

  • This type of neuropathic pain(atypical odontalgia) is seen most often in middle-aged women or men after dentoalveolar operation. Atypical odontalgia probably is caused by deafferentation leading to intraneural changes in the medullary dorsal horn. Treatment of this problem is difficult, but some success has been reported in uncontrolled, open-labeled studies using high doses of tricyclic antidepressants. This is the management report of a patient case, that had a neuropathic atypical odontalgia recognized with the right maxillary lateral incisor. The patient was consulted to the Department of Pain Clinics, ENT & Neurology and diagnosed the adenoid cystic carcinoma on left cerebellum and right paranasal sinus with extension to the cavernous sinus. In spite of the osteoplastic craniotomy, neurosurgical mass removal and radiation therapy were done with chemotherapy, atypical odontalgia was continued. In addition to the consultation to Psychology, stress management and antidepressant medication were done and improved slowly.

치과내원 환자들의 치주조직건강인식도 분석 (An Analysis on Dental Clinic Inpatients' Awareness about Their Periodontal Health)

  • 본 연구는 치과의료기관 내원환자들의 치주조직건강 인식도를 알아보기 위해 서울지역에 위치한 2개 치과의원과 1개의 치과병원에 내원한 환자 212명을 대상으로 설문조사하였으며, SPSS 14.0통계프로그램을 이용하여 분석한 결과 다음과 같은 결론을 얻었다. 1. 전체 치주조직인식정도는 3.40점으로 예방인식이 3.95점으로 가장 높게 나타났고, 관리인식은 3.09점으로 가장 낮게 나타났다. 2. 일반적 특성과 치주조직건강인식도 차이에 있어서는 성별과 직업에서 전체 치주조직건강인식도 및 각 세부영역별로 집단 간에 통계적으로 유의한 차이가 나타났다. 3. 각 세부영역별 상관관계는 예방인식과 원인인식과의 상관관계가 가장 높게 나타났고, 예방인식과 증상인식과는 상관관계가 없는 것으로 나타났다. 4. 구강보건 실천정도와 치주조직건강 인식정도의 차이에 있어 잇솔질 횟수가 많을수록 스켈링 경험이 있는 경우가 모든 영역에 있어 통계적으로 매우 유의한 차이가 나타났다.

치위생(학)과 학생의 성희롱 인식 및 임상실습 시 성희롱 경험 실태 (Experience and perception of sexual harassment during clinical practice among dental hygiene students)

  • Objectives: The purpose of this study was to contribute to the provision of a safe practice environment and practical sexual harassment prevention education for dental hygiene students who practice clinically. Methods: A total of 260 students with experience in clinical practice at dental hygiene colleges and universities in Gwangju and Jeonnam was included in this study. From May 1, 2018 to June 30, 2018, subjects were surveyed about sexual harassment experience, sexual harassment related information, self-esteem, and self-assertiveness. Statistical analysis was performed by frequency analysis, independent t-test, Mann-Whitney test, one way ANOVA, Pearson's correlation analysis, and multiple regression analysis. Results: In the sexual harassment survey revealed that 64.3% of offenders were patients and 50.0% of the offenders were in their 40s. A total of 78.6% of the offenders reported harassment in hospital type "dental clinics." Most respondents disclosed that "I did not do anything" (60.7%). Psychologically, 75.0% felt "shame and insult." Self-esteem was high in the fourth grade (3.68). Self-esteem and self-assertiveness were higher among those with no sexual harassment experiences (131.77 and 132.81, respectively; p<0.05). Correlation analysis indicated that self-esteem and self-assertiveness were correlated (r=0.509, p<0.01). Factors influencing the perception of sexual harassment include: three to four months of clinical practice (${\beta}=0.361$), experience of sexual harassment (${\beta}=-0.551$), and self-esteem (${\beta}=0.503$). Conclusions: It is necessary to provide systematic and diverse information to address sexual harassment. A proper perception of what constitute sexual harassment in clinical practice is also relevant. Furthermore, practical sexual harassment prevention education program should be developed and implemented.

치위생과 학생들의 임상교육성취도에 미치는 영향 (Study on the Influential Factors Regarding Achivement of Clinical Practice in Dental hygiene)

  • The purpose of this study was to serve as a basis for better student practice in dental hygiene and for determining educational direction for dental hygiene clinical education by examining what students and clinical-practice instructors thought on clinical practice and what factors gave an impact to clinical education. For attaining the purpose, an investigation was made of 449 third-grade students in the department of dental hygiene at seven educational institutes in Seoul and Kyoung-gi province. 246 instructors in charge of the clinical practice of the students at dental hospital or clinics were also examined. The findings of this study were as follows: 1. The instructors and students had different opinions on the number of trainees, training time, beginning of training, and assessment(p<0.05). The instructors thought that a trainee was appropriate for an instructor(59.8%), and that training time was not sufficient(30.8%). They considered it proper for students to start practice during the summer vacation of second year(21.5%), and responded that assessment should be done by the dentist, dental hygienist or professor in charge(44.7%). However, the students had an idea that an instructor should take charge of 2 or 3 trainees(47.4%), and that there was a lot of training time(55.7%). They considered it appropriate to start practice in the first term of second grade(l9A%), and thought the assessment should be made by the dentist and dental hygienist in charge(44.8%). 2. The students' satisfaction was clinical practice was affected(p<0.05) by their own selection of training institutes(28.1%), the absence of trainee from other colleges(29.4%), ample practice time(28.3%), implementation of student assessment once a day(45.3%), diverse practice opportunities (45.5%), and easy traffic to the training institute(32.9%). 3. The achivement of clinical practice was influenced by practice opportunities, the degree of faithful treatment performance, the frequency of student assessment, the number of trainees, traffic to training institutes, assistance in understanding theoretical knowledge of clinical practice, and assessment methods(p<0.05).

치과의료기관 종사자의 감염예방 실천에 관한 연구 (A Study on the Practice for Infection Prevention of Dental Clinic Worker)

  • 본 연구는 치과위생사의 감염예방 실천도를 분석하고자 2014년 1월 15일부터 2월 25일까지 광주광역시와 전남지역의 치과병 의원에 근무하는 치과의료기관 종사자를 대상으로 편의표본추출법을 이용하여 선정하였으며 설문조사를 실시하여 다음과 같은 결론을 얻었다. 일반적 특성과 직업적 특성에 따른 감염예방 실천은 근무처가 대학병원 및 치과병원인 경우, 건강검진을 받은 경우 실천도가 높게 나타났으며 통계적으로 유의한 차이를 보였다(p<0.05). 감염사고실태에 따른 감염예방 실천은 진료 전 문진을 시행하는 경우, 감염성 질환자 진료 시 별도의 개인방호 장비를 착용하는 경우, 감염성 환자 진료 후 진료복을 매회 세탁하는 경우에 통계적으로 유의한 차이가 있었다(p<0.05). 감염관리에 대한 교육이 필요하다고 응답한 경우, 감염관리에 대한 이론 및 시술법에 대한 교육 경험이 있는 경우, 감염관리에 대한 교육시행 시 참여 의향이 있는 경우에 실천도가 높게 나타났으며 통계적으로 유의한 차이를 보였다(p<0.05). 감염관리에 대한 이론 및 시술법 교육경험과 교육 후 인식변화는 치과의원보다 치과병원이 더 높은 것으로 나타났으며 근무처에 따라 통계적으로 유의한 차이가 있었다(p<0.05). 감염예방 실천에 영향을 미친 요인은 감염관리 교육 필요여부, 감염성 환자 진료 후 진료복 세탁 여부, 근무처로 나타났다. 감염관리 교육이 필요하다고 생각하는 경우, 감염성환자 진료 후 진료복을 세탁하는 경우가 감염예방 실천을 잘하고 있는 것으로 나타났고, 치과의원이 치과병원보다 실천이 낮았으며 설명력은 15.8%였다. 이상의 결과로 볼 때 치과진료 감염예방을 위해 치과의료 기관 종사자들에게 지속적인 교육 프로그램이 제공되어 감염예방 실천으로 이어지도록 노력해야 할 것으로 생각된다.

치과위생사의 근로생활의 질(QWL)에 관한 연구 (A study of quality of working life to dental hygienist's)

  • Objectives : Dental hygienist's work satisfaction and stress affect the overall quality of work life(QWL). Therefore, this research is intended to suggest fundamental data to improve QWL by finding out characteristics of each work satisfaction and stress element. To this end, a total of 327 dental hygienists working at general hospitals, university hospitals, dental hospitals and dental clinics across Seoul, Gyeonggi and Incheon were surveyed. Results of survey are as follows. Methods : The collected data were analyzed by using an SPSS 12.0 statistical program, obtaining the following results. The collected data conducted a questionnaire survey for 327 dental hygienists who work at the hospitals, university hospitals, dental hospitals, and dental clinics located at Seoul, Gyeonggi-do, and Incheon district from January until March, 2009, and drew the conclusions as follows. Result : 1. Demographic characteristics, income from 1.5 to 1.99 million were the whole lot, more than 2 million to less than 1.5 million was similar. Marital status Married Unmarried higher than the atheist religion, Christianity, Catholicism, Buddhism, and other, respectively. Classification by level of education in the college graduate, university graduate, graduate diploma, respectively. 2. Are working in a job-related characteristics of dentistry, dental hospital, general and university hospital, respectively. The making in position, Mount, contractor, responsible, senior, was an intern in the order. The five-day workweek whether working at night and is not going to care whether the conduct was similar. Classification of working hours and 8 hours, 8 hours, 8 hours or less orderly, and total of less than 1-3 years of clinical experience, 5 years, less than one year, less than 3-5 years, respectively. 3. There comes out a significant difference according to age, income, position, gross clinical experience, and whether to put night shift into practice in job stability in terms of the quality subsequent to general characteristics(p<.05). 4. There comes out a significant difference according to marital status, one's place of work, position, whether to put a five-day workweek into practice in work environment and benefits package in terms of the quality subsequent to general characteristics (p<.05). 5. There comes out a significant difference according to age, marital status, income, position, and gross clinical experience in education & training and benefits packages in terms of the quality subsequent to general characteristics(p<.05). 6. There comes out a significant difference according to whether to put night medical treatment into practice in social usefulness in terms of the quality subsequent to general characteristics(p<.05). 7. There comes out a significant difference according to marital status, income, one's place of work, gross clinical experience, work hours, and whether to put a five-day workweek into practice in leisure activity in terms of the quality subsequent to general characteristics(p<.05). 8. There comes out a significant difference according to income, one's place of work, and position in wage level in terms of the quality subsequent to general characteristics(p<.05). 9. There was no significant difference in all items related to human relations and free communication in terms of the quality subsequent to general characteristics(p>.05). Conclusions : It is necessary to analyze factors related to work satisfaction and stress in order to improve dental hygienist's quality of work life. Hospitals must support them systematically and institutionally and related organizations must conduct practical research.
