Purpose: Previous studies have solely focused on fresh extraction sockets, whereas in clinical settings, alveolar sockets are commonly associated with chronic inflammation. Because the extent of tissue destruction varies depending on the origin and the severity of inflammation, infected alveolar sockets may display various configurations of their remaining soft and hard tissues following tooth extraction. The aim of this study was to classify infected alveolar sockets and to provide the appropriate treatment approaches. Methods: A proposed classification of extraction sockets with chronic inflammation was developed based upon the morphology of the bone defect and soft tissue at the time of tooth extraction. The prevalence of each type of the suggested classification was determined retrospectively in a cohort of patients who underwent, between 2011 and 2015, immediate bone grafting procedures (ridge preservation/augmentation) after tooth extractions at Seoul National University Dental Hospital. Results: The extraction sockets were classified into 5 types: type I, type II, type III, type IV (A & B), and type V. In this system, the severity of bone and soft tissue breakdown increases from type I to type V, while the reconstruction potential and treatment predictability decrease according to the same sequence of socket types. The retrospective screening of the included extraction sites revealed that most of the sockets assigned to ridge preservation displayed features of type IV (86.87%). Conclusions: The present article classified different types of commonly observed infected sockets based on diverse levels of ridge destruction. Type IV sockets, featuring an advanced breakdown of alveolar bone, appear to be more frequent than the other socket types.
As in all other parts in the body, oral tissue also undergoes dramatic changes with increasing age. Since these changes occasionally go beyond physiological scope, which may result in pathological changes, it is essential for dentist to understand changes caused by normal aging process. With increasing age, tooth morphology and occlusion also varies, especially loss of hard tissue, which is taking place in lifelong time, occurs as a result of tooth wear. When this loss of hard tissue is presented rapidly or excessively, functional and esthetical problems are raised, resulting in lowering quality of life of patient as well as making dental treatment for oral rehabilitation even more complex. Therefore, based on understanding of change in occlusion with increasing age, strategic approaches for maintenance of oral health in both functional and esthetic aspect are required as appropriate restoration and maintenance for progressive tooth wear enables desirable occlusal relationship. Carefully planned-restorative treatment in accordance with changed occlusal relationship is also required in the same context. Instead of taking changes in oral tissue as only a consequence of ageing, it is vital to educate patient and his or her guardian, assuring maintenance of oral hygiene and regular dental check-up are of utmost importance for improved oral health.
Teeth are made up of three hard tissues, enamel, dentin, and cementum. The dental pulp is the only non-mineralized connective tooth tissue that is surrounded by dentin. The dentin-pulp complex is able to respond to injury by producing hard tissue deposition. However, dentin is considered one of the most difficult tissues to regenerate because of its unique anatomic and physiologic nature. Recently, advances in understanding the applicability of bio-active dentin regenerating proteins are emerging with the development of biological-based therapies using bio-active materials. Dentin defects were regenerated by the deposition of tubular physiologic dentin after application of the bio-active protein in a beagle dog model. Therefore, the bio-active protein may be able to serve as a novel dentin regenerating material and improve symptoms of dentin hypersensitivity.
Purpose: The role of hard-type crosslinked hyaluronic acid (HA) with particulate bone substitutes in bone regeneration for combined inlay-onlay grafts has not been fully investigated. We aimed to evaluate the effect of hard-type crosslinked HA used with bone substitute in terms of new bone formation and space maintenance. Methods: A 15-mm-diameter round defect was formed in the calvaria of 30 New Zealand White rabbits. All animals were randomly assigned to 1 of 3 groups: the control group (spontaneous healing without material, n=10), the biphasic calcium phosphate (BCP) graft group (BCP, n=10), and the BCP graft with HA group (BCP/HA, n=10). The animals were evaluated 4 and 12 weeks after surgery. Half of the animals from each group were sacrificed at 4 and 12 weeks after surgery. Samples were evaluated using micro-computed tomography, histology, and histomorphometry. Results: The BCP group showed higher bone volume/tissue volume (BV/TV) values than the control and BCP/HA groups at both 4 and 12 weeks. The BCP and BCP/HA groups showed higher bone surface/tissue volume (BS/TV) values than the control group at both 4 and 12 weeks. The BCP group showed higher BS/TV values than the control and BCP/HA groups at both 4 and 12 weeks. No statistically significant difference in newly formed bone was found among the 3 groups at 4 weeks. The BCP group showed significantly higher new bone formation than the BCP/HA group at 12 weeks. Conclusions: Hard-type crosslinked HA did not show a positive effect on new bone formation and space maintenance. The negative effect of hard-type crosslinked HA may be due to the physical properties of HA that impede osteogenic potential.

Objectives: As the craniofacial and neck regions are prime areas of injury in bear attacks, the careful management of soft and hard tissue injuries and selection of reconstructive options is of the utmost importance. This study will review the incidence and patterns of bear mauling in eastern India reported to our department and the various modalities used for their treatment over a period of 7 years. It also documents the risks of infection in bear mauling cases and the complications that have occurred. Materials and Methods: Twenty cases were treated over the study period. Cases were evaluated for soft and hard tissue injuries including tissue loss and corresponding management in the craniofacial region. Cases were also evaluated for other associated injuries, organ damage and related complications. Results: Various modalities of treatment were used for the management of victims, ranging from simple primary repairs to free tissue transfers. Simple primary repairs were done in 75% of cases, while the management of the injured victims required reconstruction by local, regional or distant flaps in 25%. Free tissue transfers were performed in 15% of cases, and no cases of wound infection were detected in the course of treatment. Conclusion: Knowledge of various reconstructive techniques is essential for managing maxillofacial injuries in bear mauling cases. Modern reconstructive procedures like free tissue transfer are reliable options for reconstruction with minimal co-morbidity and dramatic improvement in treatment outcomes.
This study summarizes the recent cutting-edge approaches for dentin regeneration that still do not offer adequate solutions. Tertiary dentin is formed when odontoblasts are directly affected by various stimuli. Recent preclinical studies have reported that stimulation of the Wnt/β-catenin signaling pathway could facilitate the formation of reparative dentin and thereby aid in the structural and functional development of the tertiary dentin. A range of signaling pathways, including the Wnt/β-catenin pathway, is activated when dental tissues are damaged and the pulp is exposed. The application of small molecules for dentin regeneration has been suggested as a drug repositioning approach. This study reviews the role of Wnt signaling in tooth formation, particularly dentin formation and dentin regeneration. In addition, the application of the drug repositioning strategy to facilitate the development of new drugs for dentin regeneration has been discussed in this study.
The macroscopical and histological findings on a rare of malformed teeth occured in the left first Molar of upper jaw of the sixteen years old boy who is examined at the dental clinic of Soodo medical college are summerized as follows. 1.Occulusal surfare of the malformed teeth formed as circumvallated hard tissue wall and root are hypertrophical type. 2. The tissue of malformed teeth occured dystrophy of calcication distavance.
This paper reflects on the state of the art of two kinds of tooth hard tissue (enamel and dentin) bonding with resin cement. After presenting composition of resin cement, concepts of enamel bonding and resin bonding are addressed. Special attention is devoted to the concept and advantage of self-etching technique. Finally, recommended clinical performance regarding bonding to tooth with resin cement is summarized.
Background: Tooth extraction commonly leads to loss of residual alveolar ridge, thus compromising the room available for the implant placement. To combat the post-extraction alveolar loss, alveolar ridge preservation is practiced, with the advent of the biomaterial available. The purpose of this study was to assess the efficiency of calcium phosphosilicate biomaterial in alveolar ridge preservation. Twenty patients indicated for extraction were selected followed by socket grafting using calcium phosphosilicate. Implant placement was done 6 months postoperatively during which a core was harvested from the preserved sockets. Clinico-radiographic measurements of hard and soft tissues were taken at baseline and 6 months post-grafting. Results: There were no significant changes in the radiographic and soft tissue parameters while significant changes in hard tissue parameters with 1.9 mm (p = 0.013) gain in mid-buccal aspect and 1.1 mm (p = 0.019) loss in horizontal bone width were observed. The histomorphometric evaluation depicted the vital bone volume of 54.5 ± 16.76%, non-mineralized tissue 43.50 ± 15.80%, and residual material 2.00 ± 3.37%. Conclusion: The implants placed in these preserved ridges presented 100% success rate with acceptable stability after a 1-year follow-up, concluding calcium phosphosilicate is a predictable biomaterial in alveolar ridge preservation.
The clinical and statistical evaluation on the patients of maxillofacial deformities who were operated by author were analyzed after several month or years or se. Pre and post operative cephalometric radiographs of 45 orthognathic surgery patients were compared. The post operative radiographs had been taken at least 9 month to several years. Measurements were made between constructed hard tissue and soft tissue points located on each before and after film tracing. The items studied and evaluated are as follows: 1) Classification and divid of patients 2) Operation technic adopted by operator. 3) Motives of patients for operation and their untowards. 4) Self satisfication of patients after operation. 5) Post operative changes of soft and hard tissue. 6) Side action during and after operation.
