The Relationship between Hormone Replacement Therapy and Periodontal Disease in Postmenopausal Women

  • The purpose of this study was to investigate the relationship between hormone replacement therapy (HRT) and periodontal disease in postmenopausal women using data from the 4th and 5th Korea National Health and Nutrition Examination Survey. A total of 5,482 postmenopausal women aged 45~75 years were included as study subjects in the final analysis. The HRT group comprised 1,035 postmenopausal women who had received HRT for at least one month, and the non-HRT group comprised 4,447 postmenopausal women who did not receive HRT. The chi-square test was used to confirm the bivariate relationship between the variables. Multinomial logistic regression analysis was used to adjust for covariance (age, education, family income, body mass index, age of menopause, alcohol, smoking, dental visit <1 per year, use of oral care products, and frequency of tooth brushing per day). After adjusting for all covariates, HRT was found to be associated with periodontal disease. In particular, the relationship between HRT and periodontal disease was more evident in older women and women younger than 45 years of menopausal age. The relationship between HRT and periodontal disease was stronger in women who brushed their teeth less than 3 times per day, women without regular oral examination, and women who did not use oral hygiene products. The results of this study confirmed the importance of actively considering hormone therapy when determining policy recommendations for postmenopausal women. Especially, health programs such as HRT, regular dental examination, and oral care are needed for older women who have undergone premature menopause.

Characteristics and Treatment of Temporomandibular Disorder in Children and Adolescents: An Analytic Review

  • Purpose: The purpose of this study is to investigate the prevalence of temporomandibular disorders (TMDs) in children and adolescents, their characteristic contributing factors, the characteristic features of symptoms and symptoms, and the response to treatment. Methods: We studied the researches, that were the results of the searches for words such as temporomandibular disorder, TMD, children, adolescents, and juvenile through PubMed and DBpia. Results: According to a study conducted in Busan, the ratio of adolescents increased from 18.3% to 21% in 2008 compared to 2000, and the proportion of boys increased from 38.58% to 45.38%. One of the characteristic contributing factors for adolescents is the macrotrauma such as jaw trauma, vehicle accidents, sports, physical abuse, forceful intubation, and third molar extraction. The second is a microtrauma from parafunctional habit such as bruxism, clenching, hyperextension, wind instrument, and fingernail biting that can cause joint overload, cartilage breakdown, synovial fluid alterations, and other changes within the joint. The diagnosis of TMDs in juvenile adolescents is not significantly different from that of adults. Medical history, clinical examination and radiological examinations are required. Conclusions: In the temporomandibular joint history and assessment, all comprehensive dental history examination is required, including head and neck pain, mandibular dysfunction, previous orofacial trauma, history of present illness with an account of current symptoms. For the treatment and management of temporomandibular arthritis in juvenile adolescents, understanding the characteristics of TMDs in juvenile adolescents and thoroughly analyzing appropriate diagnosis and possible contributing factors through comprehensive history taking & examination, conservative treatment, including fast and active cautions education, will be essential.

  • Objectives: The purpose of this study was to confirm the relationship between socioeconomic characteristics and the prevalence of periodontal disease, with a focus on employment status, in a representative sample of Korean adults aged 19-39 years. Methods: Data were obtained from 3,178 adults who completed the Korean National Health and Nutrition Examination Survey, health-related questionnaires, and an oral examination. Socioeconomic, demographic, and oral health-related behavior data were adopted as independent variables. The chi-square test was used to assess differences in the distribution of socioeconomic status, oral health behavior, and nutrition factors according to employment status. Logistic regression analysis was performed to confirm the relationships between the prevalence of periodontitis and the related variables. Results: Significant differences in sex, educational level, income, marital status, smoking status, prevalence of periodontitis, number of dental check-ups in a year, and unmet dental care were confirmed according to employment status. The prevalence of periodontal disease was not significantly related to employment status. Conclusions: These results suggest a significant difference in periodontal disease according to socioeconomic factors in Korean adults. However, the prevalence of periodontal disease was not related to employment status. To prevent the progression of periodontal disease, attention should be paid to oral health, time available for dental treatment, smoking cessation, and use of floss.

사회경제적 수준과 구강건강과의 관련성 연구: 제6기 국민건강영양조사 자료를 이용하여 (A study on the relationship between socioeconomic level and oral health: analysis of data from the Sixth Korean National Health and Nutritional Examination Survey)

  • Objectives: In this study, both subjective and objective levels of oral health were used to identify the relationship between oral health inequalities. Methods: Korean National Health and Nutritional Examination Survey data from 2013 to 2015 were combined to create an analysis plan. Oral health questions categorized as subjective oral health conditions and oral health-related diseases used dental tissue disease status as data measured by the Community Periodical Index(CPI) and decayed, missing, filled teeth(DMFT) experience. Other data on oral health behaviors such as toothache experience, the frequency of toothbrush use, chewing problems, oral examination status, and unmet dental care needs were classified and analyzed according to the socioeconomic level. Data were analyzed using frequency and cross analyses, and the statistical significance level was set at 0.05. Results: It was found that higher the economic and educational level, better was the subjective oral health, lower the CPI, lower the experience of toothache, higher the frequency of toothbrush use, lower the number of people having chewing problems, and higher the frequency of oral checkups. Conclusions: Oral health inequality exists among social classes. It is suggested that continuous research and efforts be carried out to promote oral health while considering socioeconomic and educational levels. Further, active government efforts will be needed to address polarization by social class.

Clinical assessment of various imaging systems for dental plaque scoring after the use of 3 different toothpastes

  • Purpose: This study was conducted to compare dental plaque scores obtained through clinical examinations and various imaging techniques, as well as to assess the effectiveness of herbal and conventional toothpastes for plaque removal. Materials and Methods: Thirty volunteers were divided into 3 groups. Each group was given a different toothpaste (from 2 herbal toothpastes and a conventional toothpaste) with which to brush their teeth for 21 days. Both initially and after brushing, dental plaque samples were collected, and plaque on the buccal surfaces of anterior teeth was scored using several imaging systems after staining with a disclosing agent. Specifically, digital dental photography, intraoral digital scanning, and FluoreCam imaging were employed to capture intraoral images. The Turesky Modified Quigley-Hein Plaque Index was used for clinical examination and image analysis. Quantitative polymerase chain reaction analyses and correlational assessments between clinical examination and imaging scores were conducted before and after toothpaste use. The Shapiro-Wilk test and Pearson correlations were utilized. Results: The lowest mean value was observed in the clinical examination without staining, while the highest was obtained using the FluoreCam method. No significant change was found in the level of any microorganism assessed following toothpaste use (P<0.05), with the exception of a decrease in S. mutans levels after using conventional toothpaste (P<0.05). Conclusion: Herbal toothpaste demonstrated plaque-removal effectiveness comparable to that of conventional toothpaste. The use of imaging methods for measuring plaque index has been suggested as a means to educate patients about plaque control and promote ongoing oral care.

13-18세 청소년의 비만이 치아우식에 미치는 영향 (Effects of obesity on dental caries in adolescents of 13-18 years)

  • Objectives: This study investigated the relationship between obesity and dental caries of Korean adolescents. Methods: Data from the Korea National Health and Nutrition Examination Survey (KNHNES VI) (2013 to 2015) were used and targeted adolescents aged 13 to 18 years. For statistical analysis, the SPSS 18.0 program was used, and the significance level was set at 0.05. Results: For the general and health-related characteristics, female students (p=0.001) showed higher decayed, missing, and filled teeth (DMFT) index than male students. Furthermore, DMFT showed higher tendency, as age was higher (p<0.001). Smoking (p=0.011) and drinking experience (p=0.027) revealed higher DMFT. The dental caries experience was significantly different among students who were female (p=0.033), older (p=0.001), and overweight or underweight (p=0.022). In the BMI for the general and health-related characteristics, male students had higher obesity rate than female students (p=0.003), and obese subjects had higher dental checkup in the last year (p=0.013). In multiple linear regression analysis of obesity and DMFT, the unadjusted model showed that the obesity group's DMFT was about 0.8 higher than that of the normal group (p=0.024). However, statistical significance was not shown in the adjusted models. In the logistic regression analysis of obesity and dental caries experience, the unadjusted model 1 (1.93 times), the adjusted model 2 (1.79 times), and model 3 (1.76 times) showed significantly higher dental caries experience. Conclusions: Both obesity and dental caries are related to diet habits and lifestyles and have a negative impact on adult health. Therefore, it is necessary to create a healthy social environment around the adolescent, and proper intervention through education is required to form healthy habits.

  • Objectives: The purpose of this study was to compare the general health status and oral health status based on periodical dental preventive experience, and to find methods for regular dental prophylaxis, using the data obtained from the $7^{th}$ National Health and Nutrition Survey. Methods: This study was performed using data collected from the $7^{th}$ National Health and Nutrition Examination Survey (KNHANES). A total of 9,015 participants were selected for the study. Statistical analysis was performed using SPSS 21.0, with data obtained from a complex sampling design, and a significance level of 0.05 was set. General characteristics of the participants, oral health status, and recent preventive dental treatment were examined for a complex sampling frequency analysis, and the differences in distribution were analyzed using chi-square test. Results: The group that recently under went dental prophylaxis, when compared to the group that had not under gone dental prophylaxis recently, reported several 'no obstacles' for all items of the EQ-5D (p<0.05). Moreover, the group that recently underwent dental prophylaxis used all the oral care products (p<0.05). Conclusions: It is necessary to actively expand institutional supervision so that regular dental preventive measures can be undertaken to promote oral health that affects general health.

치과위생사의 치과보존분야 직무수행 현황 분석 (An Analysis of the Job Performance in Operative Restoration by Dental Hygienists)

  • The purpose of this study is to analyze the dental hygienists' overall performance in operative restoration and the clinical performance in operative restoration according to dental hygienists' career and to provide basic data for establishing the appropriate range of dental hygienists' work. Subjects of this study are 339 dental hygienists working at dental clinic and hospital nationwide, selected by their working place, career, type of clinic, and location of clinical institution. The distribution of people who responded to the survey shows that 81 belong to beginner level(less than 2 years since entering clinic), 115 intermediate level(2 to 3 years since entering clinic), 81 higher level(4 to 5 years since entering clinic) and 62 advanced level(more than 6 years since their entering clinic). In terms of the types of clinical institution, 178 belong to dental clinics and 161 belong to dental hospitals. The survey used in this study are focused on perception about clinical performance in operative dentistry and adequacy of the work. Operative dentistry consists of operative restoration and endodontic therapy. The operative restoration consists of 15 categories such as patient welcoming, examination and diagnosis, planning of treatment, anesthesia, control of moisture, cavity preparation, pulp protection, matrix band application, amalgam filling, resin filling, glass ionomer cement filling, abrasive strip removal, rubber dam removal, bite check and polishing, patient education, and arrangement. The reliability was Cronbach's Alpha .9453. SPSS 10.0 for Windows was used to analyze the responses. One way ANOVA was utilized to verify the differences in the dental hygienists' job performance in operative restoration and their job performance according to career. When significant difference was found. Duncan multi comparison post hoc was done. To sum up the results of this study, patient welcoming look the first place in the operative restoration. It was followed by patient education, examination and diagnosis, introducing treatment plan, resin filling, glass ionomer cement filling, amalgam filling, bite check and polishing, anesthesia, pulp protection, control of moisture, abrasive strip removal, cavity preparation, matrix band application, rubber dam removal, and anesthesia. In terms of the clinical performance by career, there were significant differences in 19 activities such as medical eraluation, oral examination, patient charting, intra oral readio graphs, firm developing fixing mounting, curing light gun, education of attention content after operation. Based on the results of this study, the specific range of operative restoration for dental hygienists should be focused on providing basic data for dentists' diagnosis, alleviation of fear and aching accompanied by injection and anesthesia, data providing for dentists' decision of anesthesia degree, and maximization of control of moisture.

국내 의료기관에서 측방두부규격방사선촬영시 임상에서의 촬영조건 및 환자 선량 (Radiographic examination protocol and patient dose in lateral cephalometric radiography in Korea)

  • Purpose : To survey the radiographic examination protocol for lateral cephalometric radiographic examinations and to measure their patient doses in Korea and to compare the dose according to the size of hospital, the type of image receptor system, and the installation duration. Materials and Methods : The radiographic examination protocols (kVp, mA, and exposure time) for lateral cephalometric radiography were surveyed with 61 cephalometric radiographic equipments and their patient dose-area product (DAP) measured with a DAP meter (DIAMENTOR M4-KDK, PTW, Freiburg, Germany) for 51 cephalometric radiographic equipments. The radiographic examination protocols and patient doses were compared according to the size of hospital (university dental hospital, dental hospital, and dental clinic), the type of image receptor system (film-based, DR and CR type) and the installation duration, respectively. SPSS 12.0.1 for Windows (SPSS Inc., Chicago, USA) was used for independent t-test and ANOVA test. Results : The average protocols were 77.0 kVp, 12.7 mA, 6.2 second for cephalometric radiography. The average patient dose (DAP) was $128.0mGy\;cm^2$ and 3rd quartile dose (DAP) $161.1mGy\;cm^2$ for cephalometric radiography for adult male. There was no statistically significant difference at average patient DAP according to the size of hospital, the type of image receptor system, and the installation duration, repectively. Conclusion : The average patient dose was $128.0mGy\;cm^2$ and the third quartile patient dose $161.1mGy\;cm^2$ for lateral cephalometric radiography for adult male in Korea.

치주낭 측정시 probe 굵기에 따른 환자의 통증과 측정값의 비교분석 (Comparison analysis of pain experienced by patients during periodontal examination using thinner versus thicker probes)

  • During periodontal examination and periodontal recall visit, patients might feel pain. Probing with 0.45mm diameter probe, "overprobing" into connective tissue underlying periodontal pocket could be the primary aspect of pain. The purpose of present study, performed during recall visit, was to compare the levels of pain experienced by patients during periodontal probing using probes with the tip diameter of either 0.45mm or 0.63mm. Twenty patients were enrolled: 3 periodontists each exammined 20 patients at the Dankook university dental hospital. in each patient at six sites per tooth, diagonal maxillary/mandibular quad-rants were probed with 0.45mm diameter and 0.63mm diameter probe. Lower pain responses following probing with the 0.63mm diameter probe as compared to the 0.45mm probe were observed for patients of therapist 2 and the upper VAS quartile Groups(P<0.05). For therapist 1 and 3 groups, no differences were found. Patient worry about the pain while dental treatment procedures. If therapist use 0.63mm diameter probe while periodontal examination and recall visit, the patient's pain and discomfort could be reduced. Therefore, patient will cooperate more during the treatment, which leads to a better result. Dental therapists should make an effort to minimize the degree of discomfort during treatment. Dental therapist periodically should evaluate this part of their skill by asking patients and promote this skill.
