Objectives: The purpose of the study was to identify the relationship between metabolic syndrome and oral diseases in the middle aged and elderly in Korea. Methods: The study subjects were 6,390 people over 40 years old from 2010 and 2012 Korea National Health and Nutrition Examination Survey. The survey questionnaire consisted of health, nutrition, and oral examination surveys. The independent variables included general characteristics, health behavior, oral health behavior, and metabolic syndrome. The dependent variables included dental caries experience and periodontal disease. The oral examination was carried out by the dentist based on World Health Organization standard. Results: The average prevalence rate of metabolic syndrome MS was 23.79%, including 54.84% of risk group and 21.37% of normal group. The missing teeth rate was 82.38%, DMFT rate was 90.28% and the periodontal disease rate was 33.15%. Those having abnormal fasting blood glucose had 1.17 fold(95% CI: 1.00~1.37) higher periodontal disease than the normal group. The abnormal HDL cholesterol group had 1.25 times higher odds ratio(95% CI: 1.07~1.46) and the obese group had 1.27 times higher odds ratio(95% CI: 1.07~1.51). The risk group had 1.20 times higher odds ration(95% CI: 1.00~1.44) and that of the metabolic syndrome group was 1.60 times higher(95% CI: 1.29~1.97) in periodontal disease. The high blood pressure group had 1.25 times of missing teeth prevalence rate(95% CI: 1.00~1.37). The metabolic syndrome group had 1.47 times of missing teeth prevalence rate(95% CI: 1.11~1.94). Conclusions: The middle aged and elderly people in Korea had higher rate of metabolic syndrome and oral disease. It is necessary to implement the preventive oral health examination for the control of metabolic syndrome and oral diseases prevalence.
Patients with mental disability who are classified as dental severely disabled, have poor oral health status and many difficulties in taking health services. The purpose of this study was to investigate factors of influencing in visiting oral health care services. After receiving approval from institutional review board in Seoul National University, 39 participants were recruited and the oral health examination and questionnaire were taken. Data were analyzed using one-way ANOVA, independent t-test, hierarchical linear regression for predicting influence of each participants' characteristics on oral examination time and care time. In regression model, the higher the disability grade and the lower the degree of cooperation, the more oral examination time increased. However possibility of communication was not significantly influenced. Also, the higher the disability grade and the lower the degree of cooperation, the more oral care time increased. However possibility of communication was not significantly influenced. These results are expected to provide objective data for introduction and establishment of visiting dental care.
Objectives: The purpose of this study was to analyze the effect of chewing difficulty on depression in Korean adults to provide basic data on improving the quality of life related to oral health of Korean adults. Methods: The raw data of the analysis were obtained from the dataset of the seven Korea National Health and Nutrition Examination Survey conducted between 2016 and 2018. The subjects included 9,535 people who underwent oral examination and answered questions. The collected data were analyzed using the SPSS (ver 21.0) program using composite samples, and the mean crossover analysis and logistic return analysis were performed. Results: An analysis of the effects of chewing difficulty on depression in Korean adults showed that those with chewing difficulty had a 2.24 fold higher risk of depression (95% CI: 1.73-2.90) and a statistically significant difference (p<0.05). Conclusions: The association between the chewing difficulty on depression could be confirmed, and measures improve of the oral health and preventing depression should be prepared.
Objectives: The purpose of this study was to investigate the influence of demographic characteristics and oral health status on unmet dental needs among preschool children and to provide a basis for improvement of the dental care equality and accessibility using data from the $6^{th}$ National Health and Nutrition Survey. Methods: This study was performed using data collected from the $6^{th}$ National Health and Nutrition Survey. The subjects were 1,472 out of 22,940 people, who participated in the survey and under went oral examination. IBM SPSS Statistics (Version 20.0) was used for statistical analyses based on the complex sampling design. Frequency analysis was performed to determine the distribution of unmet dental needs according to the characteristics of the subjects. The Rao-Scott ${\chi}^2$ test was performed to examine the relationship of unmet dental needs with general characteristics and health- and oral health-related variables. Relevant factors were determined using binary logistic regression analysis. Results: The factors that had statistically significant relations with unmet dental needs included age, medical insurance, household income, limited physical activity, history of dental caries in deciduous teeth, and subjective health status. Logistic regression analysis of complex samples was conducted to determine factors related to unmet dental needs. The results of analysis showed that limited physical activity and history of dental caries in deciduous teeth were related to unmet dental needs. Conclusions: The results show the factors affecting, and the reasons for, the unmet dental needs of preschool children. Future studies are needed to develop national projects and oral health education reforms to address inequalities in preschool children's dental care.
Objectives: The aim of this study was to investigate the effect of breastfeeding on the occurrence of early childhood caries in Korean infants and toddlers. Methods: Data on oral examinations of infants and toddlers of the National Health Insurance Service were analyzed. The study subjects were children who participated in both the first, second, and third oral examinations and the first general health examination in 2008-2017 (n=142,185). Based on the responses to the questionnaire, the subjects were classified into breastfeeding, formula feeding, and mixed feeding groups. The participants were monitored for the development of early childhood caries in three sequential oral examinations. Results: Based on the oral examination results conducted at 54-65 months old, the decayed-filled teeth index of the breastfeeding group was the highest (2.03±3.08), followed by the mixed (1.96±3.03) and the formula feeding groups (1.82±2.91). The Cox proportional hazard regression model including all the variables showed that the risk of developing dental caries was significantly lower in the formula (hazard ratio [HR], 0.85) and mixed feeding groups (HR, 0.91) than in the breastfeeding group. Conclusions: Breastfeeding children have a higher risk of early childhood caries; therefore, oral hygiene education and regular dental check-ups are necessary.
The purpose of this study was to investigate the relationship between job performance, job importance and job satisfaction in dental hygienists' preventive dental treatment. The subjects in the questionnaire survey were 275 dental hygienists in the Mokpo from October 7 to October 14, 2016. Statistical analysis was performed using SPSS Ver. 18.0. According to the analysis of the job performance, and job importance of preventive dental care tasks, the total score of 3.35±0.60, 3.85±0.51, respectively. Job performance was highest in scaling (4.24±0.90), and lowest in oral microbiological examination (2.40±1.14). Job importance was highest in scaling (4.49±0.77), and lowest in oral microbiological examination(3.04±0.85). As a result of analyzing the job satisfaction of preventive dental care tasks, the overall average was 3.23±0.40 out of 5 points. Patient relationship was the highest (3.71±0.66), working environment 3.55±0.77, staff relation 3.48±0.63, dental organization 3.27±0.73, professional support 3.21±0.67 and prospect of career 3.13 ± 0.58. remuneration 2.80±0.58, and workload 2.68±0.57, respectively. The correlation coefficients were 0.263 (job performance and job satisfaction), 0.211 (job importance and job satisfaction), and 0.461 (job performance and job importance) statistically significantly. The results of this study suggest that the higher job performance and job importance in preventive dental treatment, the more job satisfaction. Therefore, it is necessary to expand the dental hygienist's preventive dental treatment tasks and provide educational support to do this. Also, it is necessary to develop an oral preventive management program to improve the professionalism of the dental hygienist and improve job satisfaction.
Historically, the screening of patients for signs of oral cancer and precancerous lesions has relied upon the conventional oral examination. A variety of commercial diagnostic aids and adjunctive techniques are developed to potentially assist in the screening of healthy patients for evidence of occult cancerous change. This paper is reviewing the literature associated with current oral cancer screening aids such as spectroscopy, chemoiluminescence, exfoliative cytopathology, vital staining and saliva as a diagnostic tool. Despite the increased public awareness of oral cancer, no technique or technology to date has provided definitive evidence to suggest that it improves the sensitivity or specificity of oral cancer screening beyond clinical oral examination alone.
Hyperdontia or supernumerary teeth without associated syndrome is a rare phenomenon, as supernumerary teeth are usually associated with cleft lip and palate or other syndromes such as Gardner's syndrome, cleidocranial dysplasia, and so on. Five patients with supernumerary teeth visited our department. They had no familial history or other pathology, certain treatment protocols was modified due to the presence of supernumerary teeth. Non-syndromic supernumerary teeth, if asymptomatic, need to have periodical radiographic observation. If they showed no variation as they impacted in the jaw, careful examination is necessary because they may develop into pathological status such as dentigerous cysts. The importance of a precise clinical history and radiographic examination for patients with multiple supernumerary teeth should be emphasized.
A 46-year-old man was referred to our hospital for treatment, complaining of swelling on the right mandibular molar region. Radiographic examination revealed a well defined multilocular radiolucent lesion with root resorption of right lower anteriors and molars. Following biopsy, a diagnosis of unicystic ameloblastoma of mural type was made and hemimandibulectomy was performed under general anesthesia. Histopathological examination of the surgical specimen exhibited a unicystic ameloblastoma of luminal, intraluminal, and mural type. Intraluminal proliferation was of plexiform pattern and mural proliferation showed unusual histopathological findings, which revealed follicular, acanthomatous areas coexisted with desmoplastic areas. This mural picture was similar to the so-called 'hybrid lesion of ameloblastoma', whose biological profile is not elicited due to the lack of adequate published reports. Two years follow up till date has not revealed any signs of recurrence.
Objectives: This study aimed to: 1) investigate the beverage consumption pattern among Korean adults, and 2) analyze the relationship between the frequency of beverage consumption and oral health status. Methods: We used data from the 6th Korean National Health and Nutrition Examination Survey. A general linear model was employed to assess the associations between demographic factors and frequency of beverage consumption; and oral health status and the frequency of beverage consumption. Results: The beverage with the highest frequency of intake was coffee (11.5 times per week). More frequent consumptions of fruit juices and carbonated drinks were associated with higher numbers of decayed teeth. Conclusions: To improve oral health, frequent intake of acidic and sweetened beverages should be reduced, and the consumption of milk should be encouraged.
