Dental Care Utilization for Examination and Regional Deprivation

  • Objectives: Receiving proper dental care plays a significant role in maintaining good oral health. We investigated the relationship between regional deprivation and dental care utilization. Methods: Multilevel logistic regression was used to identify the relationship between the regional deprivation level and dental care utilization purpose, adjusting for individual-level variables, in adults aged 19+ in the 2008 Korean Community Health Survey (n=220 258). Results: Among Korean adults, 12.8% used dental care to undergo examination and 21.0% visited a dentist for other reasons. In the final model, regional deprivation level was associated with significant variations in dental care utilization for examination (p<0.001). However, this relationship was not shown with dental care utilization for other reasons in the final model. Conclusions: This study's findings suggest that policy interventions should be considered to reduce regional variations in rates of dental care utilization for examination.

  • The purpose of this study was to examine dental care utilization and expenditures among children aged 0 to 19 years using Korea Health Panel Survey (KHPS) data in 2008 - 2013. Weighted estimates were nationally representative and statistical significances were analysed by $x^2$ test or adjusted Wald test using STATA 13 software. While dental care utilization increased steadily among children from 2008 through 2013, less than one-third the children received dental care in 2013. Compared with 2008, the proportion of restorative care, oral surgery, preventive care decreased and the proportion of orthodontic care, prosthetic care increased. While the proportion of amalgam markedly decreased, the proportion of resin increased. This study highlight socioeconomic disparities in dental utilization and expenditures among children. Especially the findings provide a baseline assessment for examining trends on dental care for children in the future.

산업장 근로자의 치과의료이용양상 및 관련요인 (Dental care utilization patterns and its related factors of industrial workers)

  • Objectives : This purpose of this study was to examine worker's Dental care utilization patterns and its related factors, to prepare basic data for oral health promotion of workers. Methods : The study subject were 1016 industrial workers from7 working places in Gyung-Sang-Buk Do and Daegu-City. Data for this study was obtained by self-administrated questionnaire. The questionnaire consists of general characteristics, yes or no of dental care utilization before six month, purpose of visit, yes or no of dental disease form the results of dental examination screening before one years, yes or no of and patterns dental care utilization of workers with dental disease, etc. The data was utilized using frequency test and $x^2-test$. Results : Rate of dental care utilization was 40% within six month last, among the general characteristics, it were statistically significant that higher age, married state, bad oral health state, high Work period. Rate of dental disease as a result of their dental examination last was 53.1% among the general characteristics, it were statistically significant that enough economic state, bad oral health state, blue collar work type. Dental care utilization's rate of dental disease as a result of their dental examination last was 48.6% among the general characteristics, it were statistically significant that Enough economic state, higher age, married state, high Work period. Conclusions : In order to promote workers of abnormal opinions to take the medical treatment on their dental diseases later dental examination, it is necessary to educate them consistently for the importance of early medical treatment.

우리나라 성인의 치과의료이용 미수진 관련 요인 (Influencing factors of non-utilization of dental care among Korean adults: Using 2012 Korean National Health and Nutrition Survey)

  • Objectives: The objective of the study was to investigate the related factors of non-utilization of dental care among Korean adults. Methods: Korean National Health and Nutrition Survey 2012 was carried out in South Korea. A total of 5,589 adults were interviewed and examined. A multinominal logistic regression model was used to estimate the odds ratio with 95% confidence intervals for the non-utilization of dental care. Results: The non-utilization of dental care was shown to have a badly perceived oral health status, less toothbrushing before sleeping(OR=1.18: 95% CI=1.00-1.39), periodontitis symptoms (OR=1.82: 95% CI=1.50-2.22), toothache experience (OR=2.03: 95% CI=1.75-2.34), TMJ symptom experience (OR=3.31: 95% CI=2.74-3.99), speaking problem (OR=1.87: 95% CI=1.75-2.34) and a partial dentures needs (OR=2.49: 95% CI=1.93-3.19). Conclusions: The non-utilization of dental care tended to have badly perceived oral health status, less toothbrushing before sleeping, periodontitis, toothache experience, TMJ symptom experience, speaking problem, and partial dentures needs.

건강보험 보장성 확대정책에 따른 치과 이용 및 의료비 지출 변화 : 한국의료패널(2012-2016)을 이용하여 (Changes in dental care utilization and expenditure by the expansion policy of the health insurance coverage : Korea Health Panel Survey 2012-2016)

  • Objectives: This study aimed to verify the impact of dental health care insurance coverage policy by analyzing the changes in dental care utilization and expenditures over 5 years from 2012 to 2016, when the dental health insurance coverage expansion policy was implemented. From the national cohort data collected by the Korea Health Panel Survey, a retrospective study was conducted for all household members using dental services. Methods: This study statistically verified the difference in the dependent variables by frequency analysis, chi-square test, t-test, and one-way analysis of variance (SPSS version 22, IBM Co. USA, p<0.05). Results: The annual utilization rate steadily increased from 23.4% to 26.1% between 2012 and 2016. Although there were differences in utilization rates by gender, age, and income level, patients kept using the dental services in 2016 regardless of the type of health insurance. The average annual copayment for patient expenditures (out-of-pocket amount) increased from ₩463,844 to ₩537,401 in 2012 and 2016, respectively. Of the dental care expenditures over 5 years, the ratio of uninsured expenses by the elderly decreased from 38.5% to 25.9%, and the national health insurance service coverage increased from 40.3% to 49.1%. Conclusions: Although this policy did not reduce overall patient expenditures, it has been found that there was a positive effect on the elderly and low-income groups; it increased the utilization and access to dental services.

Changes in dental care access upon health care benefit expansion to include scaling

  • Purpose: This study aimed to evaluate the effects of a policy change to expand Korean National Health Insurance (KNHI) benefit coverage to include scaling on access to dental care at the national level. Methods: A nationally representative sample of 12,794 adults aged 20 to 64 years from Korea National Health and Nutritional Examination Survey (2010-2014) was analyzed. To examine the effect of the policy on the outcomes of interest (unmet dental care needs and preventive dental care utilization in the past year), an estimates-based probit model was used, incorporating marginal effects with a complex sampling structure. The effect of the policy on individuals depending on their income and education level was also assessed. Results: Adjusting for potential covariates, the probability of having unmet needs for dental care decreased by 6.1% and preventative dental care utilization increased by 14% in the post-policy period compared to those in the pre-policy period (2010, 2012). High income and higher education levels were associated with fewer unmet dental care needs and more preventive dental visits. Conclusions: The expansion of coverage to include scaling demonstrated to have a significant association with decreasing unmet dental care needs and increasing preventive dental care utilization. However, the policy disproportionately benefited certain groups, in contrast with the objective of the policy to benefit all participants in the KNHI system.

치과 임플란트 국민건강보험 급여화 이후 노인의 치과 임플란트 이용에 대한 예측 모형: 사회경제적 요인 중심으로 (Prediction model for dental implants utilization in the elderly after the national health insurance coverage of dental implants: focusing on socioeconomic factors)

  • Objectives: The demand for dental care is expected to increase as the population ages. This study aimed to predict the utilization of dental implant care following the expansion of national health insurance benefits for dental implants. Methods: Multiple linear regression analysis was performed on HIRA big data open portal data and DNN-based artificial intelligence models to forecast the utilization of dental care in relation to the national health insurance coverage for dental implants. Results: National health insurance coverage of dental implants was found to be associated with the number of patients using dental implant services and demonstrated a statistical significance. The dental implant services utilization increased with the increased dental implant health insurance benefits for the elderly population, increased mean by region, increased number of dental institutions by region, and increased health insurance coverage rate for dental implants. However, the dental implant services utilization decreased with the increased number of older people living alone and increased size of dental institutions. Conclusions: With the expansion of the national health insurance coverage for dental implants, it is predicted that the utilization of dental implant medical services will increase in the future.

대구시 일부 근로자의 구강검진후 치과진료 이용의사에 영향을 미치는 요인 (Factors Influencing on Intention of Dental Care Utilization after Annual Dental Examination for Workers in Daegu, Korea)

  • 본 연구는 계속구강건강관리가 잘 이루어지지 않는 근로자를 대상으로 계획된 행동이론에 기초하여 구강질환이 있을 경우 치과치료이용을 하려는 의도에 미치는 요인을 조사를 하였다. 이에 치과의료를 이용할 의도를 높일뿐만 아니라 실제 치과의료 이용으로 실행될 수 있는 방안을 마련하는데 기초자료로 쓰이고자 시도되었다. 연구대상상자는 1,016명 중 현재 치과치료를 받는 환자 53명과 지난 6개월간 치과방문을 경험한 사람 355명을 제외한 총 608명을 대상으로 조사 분석하여 얻은 결론은 다음과 같다. 1. TPB 변인과 의도의 상관관계는 행동에 대한 태도, 주관적 규범, 인지된 행동통제의 순으로 나타났으나 인지된 행동통제는 유의하지 않았다. 2. 치과의료이용 의도에 영향을 미치는 일반적 특성은 경제상태로서 여유가 있다고 생각할수록 의도가 높게 나타났으며, TPB 변인 중에서는 치과의료이용과 관련된 행동에 대한 태도와 주관적 규범이 유의한 영향을 주었다. 3. 일반 선형회귀분석한 결과 모형의 설명력은 21%로 나타났다. 구강질환이 있을 경우 치과의료를 이용할 의도가 높다고 하더라도 실제 상황에서는 행동으로 실행이 되지 않을 가능성이 높을 것으로 간주된다. 하지만 의도가 낮은 사람에 비하여 의도가 높은 사람이 실제 치과의료이용을 할 가능성이 높다. 따라서 의도에 영향을 주는 치과의료이용에 관련된 태도와 주관적 규범이 긍정적으로 형성되며 인식할 수 있도록 구강보건교육을 통하여 동기부여가 되어야 하겠다. 그렇게 위해서는 산업장 구강검진이 일회성 형식에 그치지 않으며 구강질환자는 검진치의사의 강력한 치료권고를 통하여 치과치료를 받을 수 있도록 유도하여야 하고, 계속구강건강관리프로그램이 전면적으로 실행되면서 활성화되어 근로자들의 구강건강증진이 이루어져야 하겠다.

Association between Unmet Dental-Care Needs and the Intention to Use Mobile Healthcare Services among Soldiers

  • Background: Although a direct association has been established between oral health management and maintaining military combat readiness and fulfilling defense duties, unmet dental-care needs have been consistently reported, and there has been little research on the role of mobile healthcare services in addressing this issue. This study explored the association between unmet dental-care needs and the intention to use mobile healthcare services among military personnel. Methods: This study was conducted on military personnel who visited a military hospital in Gyeonggi-do, Korea, from August 19 to August 31, 2024. A total of 150 self-administered questionnaires were distributed and 135 valid responses were analyzed. The analysis included general characteristics, dental care utilization, and intention to use mobile healthcare services. Statistical analyses, such as t-tests, ANOVA, and multiple regression, were employed to determine the factors influencing the intention to use mobile healthcare services. Results: This study found that military personnel with unmet dental-care needs had significantly higher intentions to use mobile healthcare services than those without such needs. The analysis revealed that the key factors influencing this intention were perceived health status and previous dental-care utilization. Specifically, participants who rated their overall health as better and those who had used dental care services in the past year showed a greater tendency to express interest in mobile healthcare services than those who did not. Furthermore, unmet dental-care needs were more prevalent among soldiers who reported limited access to dental facilities owing to time constraints or a lack of prioritization of oral health, which increased their inclination toward utilizing mobile healthcare solutions as an alternative. Conclusion: Mobile healthcare solutions can offer personalized and timely care as viable alternatives for improving oral health management in the military. Moreover, integrating mobile healthcare services into military health systems could significantly reduce unmet dental-care needs and enhance overall combat readiness by promoting better health outcomes.

외국인 이주노동자의 구강건강수준과 치과 의료이용 관련요인 (Related factors to dental care utilization and oral health status in immigrant workers in Korea)

  • Objectives: The purpose of the study is to investigate the related factors to dental care utilization, oral health behaviors, and oral health status in immigrant workers in Korea. Methods: The subjects were 504 foreign immigrant male workers over 20 years old who visited Daegu labor consultation center for oral health survey and oral examination. The questionnaire included 5 questions of socioeconomic characteristics, 8 questions of oral health practice behavior, 6 questions of dental clinic visit, 8 questions of social relations and Korean language proficiency. The question for health behavior was measure by body mass index(BMI). Social relations and Korean language proficiency instrument was modified by Seol from "Family welfare survey in Korean international marriage" and scored by Liker 5 scale. Results: The oral health examination of the immigrant workers was as follows: decayed teeth - 76.6%, filling teeth - 27.4%, missing teeth - 69.8%, dental caries experience above five or more - 60.2%, periodontal pocket tissues - 58.9%. Simplified Oral Hygiene Index was very poor and accounted for 49.0%. Dental care utilization experience was closely associated with social relation indexes including attendance in family events, household stuff help, financial help and counseling for hard work(p<0.01). Dental care utilization experience proportionally increased with proficiency in Korean literacy including speaking, listening, and writing abilities of Korean language(p<0.01). Conclusions: In order to improve the oral health condition of the immigrant workers, it is important to provide social network, Korean language proficiency support, and health insurance coverage through economic burden reduction by the Korean government.
