유아의 구강관리에 관한 보호자의 인지도 조사 (A STUDY OF PARENTAL KNOWLEDGE AND ATTITUDE ABOUT INFANT ORAL HEALTH CARE)

  • 전반적인 유아의 구강관리에 관한 보호자의 인지도를 알아 보고, 이를 통한 구강관리에 대한 동기유발을 목적으로 1998년 7월부터 1998년 8월까지 삼성서울병원 소아과에 내원한 아동과 강남구 일원동 일대 유치원 및 어린이집 8곳의 만 3세 이하아동 350명의 보호자를 대상으로 시행한 설문 조사를 통해 다음과 같은 결과를 얻었다. 1. 남성보다는 여성이, 그리고 교육정도가 높을수록 치과 지식정도가 높았으며, 연령이나 경제 수준과는 무관하게 나타났다. 2. 치아의 생성시기에 대해 응답자의 59%가 태아기라고 답하였다. 3. 구강관리 시작시기는 응답자의 76%가 맹출 전이라고 하였으며, 63%가 실제로 만 1세 미만에 구강관리를 시작한다고 하였다. 4. 구강관리 방법에 대해 응답자의 70%가 거즈를 사용한다고 하였다. 5. 응답자의 57%가 만 1세 이후에도 계속 수유한다고 하였다. 6. 우유병 우식증에 대한 인지도는 45%로 나타났다. 7. 유치수복의 필요성에 대하여서는 90%가 필요하다고 하였으며, 31%에서 최초의 치과내원시기를 만 2세 경이라고 하였다. 8. 47%의 보호자가 치아우식이 감염성 질환임을 인식하지 못하였다. 9. 응답자의 33%만이 임신 중 치과 검진을 받았다고 하였고, 소아과나 산부인과에서 구강관리에 관한 지도를 받은 것은 11% 뿐이었다. 10. 주로 육아서적에서 유아의 구강관리에 관한 정보를 얻는 것으로 나타났다. 11. 보호자들의 전반적인 치과지식과 실천정도는 유아들에게 적절한 구강관리를 제공하기에 충분치 않은 것으로 나타났다.

건강보험 데이터를 이용한 14세 이하 소아청소년의 치아 우식 유병률 경향성 (Trends of Dental Caries Prevalence in Children Under 14-Year-Old Using a Health Insurance Database)

  • 이 연구의 목적은 건강보험심사평가원의 자료를 이용하여 만 14세 이하 소아청소년들의 치아 우식 유병률의 경향성과 치료의 현황을 파악하는 것이다. 이 연구는 2011년부터 2020년까지 연도별로 전체 소아청소년이 포함된 모집단에서 약 100만명을 무작위 표본 추출한 집단의 진료내역을 이용하여 분석을 시행하였다. 이 연구에서 K02 치아우식 주상병명을 진단받는 아이들은 연도별로 증가하였고, 전반적으로 모든 연령군에서 치아 우식 치료를 받는 아이들도 증가하는 경향성을 보였다. 반면 중증의 치아 우식으로 인해 치수 치료 또는 발치를 경험한 10 - 14세 어린이들은 감소하는 경향을 보였다. 2021 - 2022년 아동 구강 건강 실태조사에서도 5세와 12세의 우식경험지수는 정체되거나 소폭 증가하는 경향을 보였으나 우식 유병자율은 감소하였다. 치료적 관점에서 개인적, 지역적 환경은 과거에 비해 전반적으로 개선된 것으로 나타났으나 예방적 관점에서 치아 우식 유병 상태와 예방 진료가 지난 10년간 개선되지 않고 정체 상태로 머물러 있던 것으로 해석된다. 따라서 대한민국에서 소아청소년을 대상으로 구강건강 증진 및 예방을 위해 시행되고 있는 구강 보건 사업에 대한 홍보와 효용성에 대한 평가가 필요하다.

자녀의 구강건강 관심도 및 인지도에 따른 치아우식예방법의 인식과 실천 (Awareness and practice of dental caries prevention according to concerns and recognition for off-spring's oral health)

  • Objectives : The purpose of this study was to examine the awareness of mothers on their children's oral health and their concern for that by socio-demographic characteristics and the relationship of their awareness of methods of dental-caries prevention to their practice of the methods. Methods : The subjects in this study were 337 guardians of preschoolers at kindergartens and daycare centers. A self-administered survey was conducted from April 25 to May 27, 2011, and the collected data were analyzed by the statistical package SPSS 18.0. Results : 1.Self-rated concern for children's oral health, 87.7 percent and 12.1 percent replied, "So-so." Whether they were working or not and whether they were mainly responsible for child rearing made significant differences to that(p<.05). 2. As to subjective awareness of their children's oral health, the largest group of the mothers answered "So-so." (44.9%) The second replied that their children were in good oral health(40.5%), and the third group in poor oral health(14.2%). 3. The relationship between self-rated concern for their children's oral health and awareness of methods of caries prevention, statistically significant differences were found according to toothbrushing education and sealant(p<.05). There were no statistically significant differences in practice, but application of fluoride was the least. 4. The relationship between self-rated awareness for their children's oral health and awareness of the preventive methods of caries, there were statistically significant gaps in awareness of toothbrushing education(p<.05). In practice, statistically significant gaps were found in practice of toothbrushing education and sugar-intake restriction(p<.01). 5. In regard to the correlation between awareness and practice of the preventive methods of caries, awareness of all the factors involving toothbrushing education, sealant, application of fluoride and restriction of sugar intake had a significant positive correlation to practice of them. Better awareness led to better practice. Conclusions : In order to ensure children's successful oral health care, more authentic education of how to prevent dental caries should be offered by experts such as dental hygienists and dentists. Especially, detailed information on application of fluoride, restriction of sugar intake and pit and sealant should be provided for mothers to help their children to stay away from dental caries.

학령기 아동을 위한 구강건강프로그램 적용 (Application of Dental Health Program for Elementary School Children)

  • Purpose: In this study an analysis was done of participants who were educated using a dental health program as compared to a control group who only used a booklet on the subject. The participants were elementary school children and the education focused on dental health knowledge, behavior, health belief, and self-efficacy. Methods: Sixth grade students from two different schools in D-city were assigned to an experimental group which was educated using a dental health program over five weeks and a control group which was educated with a dental care booklet. Results: The experimental group showed significant increases in knowledge, behavior, sensitivity, severity, benefit, belief, and self-efficacy indicating that the program was effective, but there were no significant differences in self-efficacy, sensitivity, severity, importance, or disability between the two groups. Significant differences between the groups were found for knowledge, behavior, and benefits regarding dental health. Conclusion: The results of this study indicate that a dental health program for elementary school students is effective in increasing their knowledge and behavior regarding dental health but the lack of significant differences in several of the variables in this study may be related to the education provided to the control group using a booklet.

국민학교 아동의 구강보건관리 및 지식에 대한 실태조사(II) - 전북 부안군 농촌지역을 중심으로 - (The survey of actual condition of pupils care and Knowledge of dental hygiene(II) - Centering the rural area around Booan-Kun Chollabuk-do -)

  • We investigated 974 male and female students who are in 4th, 5th and 6th year of 8 elementary schools are located win Puan-kun, the farm area of Chollabuk-do, and the results are as follows. 1. Visiting dental clinic in order to be treated for dental caries hold the highest rate as 26.1% of male students and 30.3% of female students. 2. On fear when children visit dental clinic, first, 25.6% of male students and 35.1% of female students mark the highest rate as some what fear. Second, 23.1% of male students represent to be usual and 29.4% of female students represent to be very fear. There fore it speaks for dissimilarity in character between male and female students. 3. On reason of developing dental caries, 62.5% of male students and 55.8% of female students replied that it was developed because they frequently ate the sweet things. 4. Children who pull out decidous teeth alone mark the highest rate as 44.7% of male students and 38.6% of female students. 5. On important thing for ordinary dental health, 56.2% of male students and 58.5% of female students as the highest rate replied that they often brushed their teeth.

학교구강보건실 운영 효과에 관한 조사 (Effects of school incremental dental care program)

  • Objectives : This study was conducted to purpose the fundamental data for further developments of the school dental clinic program(SDCP) through comparing results of oral health promotion program for 4 years relating to compare oral health status of children who had experienced and had not experienced the program. Methods : The subjects were 66 children of elementary school in Busan Metropolitan City who had benefited school dental clinic program(SDCP) from 2004 to 2007 and control group was 98 children of elementary school who had not benefited school dental clinic program(SDCP). Data analysis were done with SPSS program (ver 13.0) through basic statistical data, t-test. Results : The DMF rate of elementary school students who had benefited program was 41.5% and it was lower than that of control group(52.5%), however it was not significant statistically. The DMT index of elementary school students who had benefited program was 1.81 and it was lower than that of control group(2.44)(p<0.05). The DMFT rate of elementary school students who had benefited program was 4.44% and it was lower than that of control group(7.45%)(p<0.05). Conclusions : In oder to promote oral health of children, school dental clinic in elementary school should be extended to more elementary schools as possible and the operating system should be changed to improve the efficiency. Oral health educational materials should be developed and utilized to improve educational effect.

초등학생 대상의 국내 구강건강관리 프로그램에 대한 체계적 문헌고찰 (A systematic review of domestic oral health care programs for elementary school children)

  • Objectives: This purpose of this study was to examine the trends in interventional research and analyze the contents and results of intervention through a systematic literature review of the domestic research literature, suggesting the effect of an oral care program on elementary school students. Methods: A total of 262 articles were retrieved initially, and the duplicate articles were then removed, excluded after screening titles, abstracts, and full-text articles. Finally, a total of 16 papers were used in the review. Results: The study included 9 articles with the nonequivalent control group pretest-posttest design and 7 with the one-group pretest-posttest design. All articles were of non-randomized studies. The most common index of the interventional effect was dental plaque. Further, oral health behaviors and knowledge, perception, self-efficacy, and DMFT index were common indices. In the quality assessment, in the "blinding of outcome assessment" among six items of ROBANS, 12 studies showed a high risk of bias. Conclusions: In order to develop an interventional oral health care program for elementary school students and evaluate the effectiveness, a strict research design and qualitative improvement of research reports are required, and continuous research should be conducted to develop a systematic protocol.

장애아동 보호자의 구강건강관리 인식 (A Study on the Oral Halth Care of Children with Disabilites Recognition)

  • 장애아동보호자의 구강건강 인식이 장애아동의 구강건강에 미치는 영향을 연구하여 장애아동의 구강건강향상을 증진시키고 정책적인 예방구강보건사업을 실시하여 질적인 치과의료서비스를 제공하고자 연구를 시행하였다. 병원에 내원하는 장애아동의 보호자 200명을 대상으로 조사분석하여 연구한 결과는 다음과 같다. 1. 장애아동과 보호자와의 관계, 응답자의 최종학력, 아버지 직업, 어머니 직업, 월수입, 거주형태에 따라 일일 간식섭취 횟수는 통계적으로 유의한 관계가 있었다(p < 0.05). 2. 장애아동과의 관계, 응답자의 최종학력, 아버지 직업, 어머니 직업, 월수입, 거주형태에 따라 간식의 유형(신선한 과일류, 과자류, 아이스크림 및 음료수)은 통계적으로 유의한 관계가 있었다(p < 0.05). 3. 장애아동과의 관계, 응답자의 최종학력, 아버지 직업, 어머니 직업, 월수입, 거주형태에 따른 구강보건교육 참여정도는 통계적으로 유의한 관계가 있었다(p < 0.01).

충남 일부지역유치원 및 보육시설 아동 어머니의 구강보건 지식과 행동에 관한 연구 (A study on oral health knowledge and behavior of mothers of children attending kindergartens and child care facilities in some parts of Chungnam)

  • Objectives : Since most oral diseases are not life threatening, it can be assumed that an individual's behavior to maintain one's oral health is most likely influenced by one's oral health knowledge. Methods : Mothers whose children are attending five kindergartens and child care facilities in the city of Danjin, Chungnam were selected as subjects of this study. Out of a total of 230 questionnaires collected. A self-administered survey was conducted from April 1 to April 30, 2012. The data were analyzed using SPSS 19.0 (SPSS 19.0 K for window, SPSS Inc, USA) with the level of significance as 0.05. Results : 1. Most of the participants correctly answered questions on the oral health knowledge with a mean of 7.37 out of 9. 2. In the section of caring a child's oral health behavior, 'caring a child to brush his or her teeth three times a day' had the highest score of 3.45 out of 5. 3. Looking at caring a child's oral health behavior and a mother's oral health knowledge by general characteristics of mothers, there were no significant differences based on the mother's age, whether a mother is currently working or not, her level of education, and her monthly average income. Conclusions : Based on the above results, we can draw a conclusion that the lower the difference between the rate of correct answers and that of incorrect answers on mother's oral health knowledge to her child, the higher the mother's score of caring her child's oral health behavior, leading to better caring.

부모의 자녀에 대한 구강건강 관심도 조사 (Parents' concern for child oral health)

  • The purpose of this study was to examine the interest level of mothers in oral health. The subjects in this study were 184 mothers whose children attended daycare centers in the city of K. A survey was conducted from March 15 through April 30, 2007, and SPSS WIN 11.5 program was employed to analyze their answer sheets. The findings of the study were as follows: 1. A group of mothers whose children brushed their teeth three times a day on the average made up 55.7 percent of those who were at the age of 35 and under. Among the mothers having three or more children, those mothers accounted for 59.1 percent. The daily mean toothbrushing frequency of children was higher among the better-educated mothers. 2. Concerning dietary and oral guidance, the mothers whose academic background was better curbed their children's excessive sugar intake more often, and the gap between them and the others was statistically significant(pE.05). As to the importance of oral health by the number of child and academic credential, those who had two children(36.0%) taught them the importance of oral health frequently(pE.01), and the better-educated mothers taught about it often(pE.05). The gap between the groups and the others was significant. 3. As for experience of visiting dental institutions, the working mothers visited dental institutions more often than the stay-at-home mothers, and the gap between them was statistically significant(pE.05). Regarding purpose of the visit, the largest group aimed to get treatment(64.6%), followed by having a dental checkup(14.6%) and receiving preventive treatment(13.5%). As to any inconveniences in using dental institutions, those who were working and who were in the upper income bracket found it more inconvenient to do that because of a long distance or time constraints. The gap between them and the others was significant. 4. In regard to concern for dental care, the mothers who were better educated(pE.001) and whose family income was 3 million won or more had their teeth scaled more often, and the gap between them and the others was statistically significant(pE.01). The rate of the regular visitors of dental institutions stood at 32.7 percent of the junior-college graduates, 31.0 percent of the mothers receiving college or higher education and 10.1 percent of the high-school graduates(pE.01). And those who earned larger income paid a visit to dental institutions more often and on a regular basis. The gap between them and the others was statistically significant(pE.05).
