Endodontic surgery is a procedure to treat apical periodontitis or abscess in cases that did not heal after nonsurgical treatment or retreatment. This might include situations with persistent intracanal infection after root canal treatment. Other reasons might be found in extraradicular infection, such as bacterial biofilm on the apical root surface or bacteria within the lesion. For many years, the treatment standard was the traditional approach with surgical burs and amalgam for root-end filling. Endodontic microsurgery is the most recent step in the evolution of endodontic surgery, applying not only ultrasonic tip and biocompatible filling materials but also incorporating high-power magnification and illumination. Although many studies have been published that advocate the use of modern technique, the traditional techniques are still widely used in the surgery community. The purpose of this study was to demonstrate the endodontic microsurgery procedure including the root-end preparation and filling with the use of a surgical operating microscope.

20 radicular cysts of primary teeth mimicking dentigerous cyst were reviewed. The following results were obtained. 1. The patients' age ranged from 7 to 14 years. Males(60%) were more involved than females(40%). 2. The mandible(85%) was affected more frequently than the maxilla(15%) The mandibular deciduous molar area(80%) was the most frequently involved. 3. The diameter of the cyst varied from 10 to 30㎜. 55% of permanent successors showed underdeveloped roots less than one-third. 4. The etiologic factors were pulp-treated teeth(65%), severe caries (20%), trauma(10%), deep amalgam filling(5%).
A cracked tooth is defined as an incomplete fracture initiated from the crown and extending subgingivally and usually directed mesiodistally. Cracked teeth were most frequently involved in mandibular and maxillary molars at the age of 50s. Cracks occurred mainly in nonbonded restorations such as gold and amalgam, and majority of cracks were found in intact teeth. A pulpal and periapical diagnosis is dependent on the extent of the crack and duration of the symptom. The pulp of a cracked tooth might become inflamed because of microleakage, which induces thermal sensitivity. Once the crack has extended and exposed the pulp, severe pulp and periapical pathosis will likely be present. In addition, the extended crack can cause a bony dehiscence with a resulting narrow and deep periodontal pocket. Therefore, early diagnosis of the cracked tooth and proper treatment planning are important for clinician.

Performed treatments were 322 scaling, 13 fluoride varnish, 727 preventive resin restoration, 1296 resin restoration, 600 amalgam restoration, 46 GI restoration, 612 extraction, 289 pulp treatment of primary teeth, 75 pulp treatment of permanent teeth and 138 stainless steel crown restoration. Many handicapped patients have some difficulty to dental treatment. They have limited access to dental care, which is compounded by a shortage of skilled dental professionals who are willing to treat these population and financial problems.
The author conducted a dental survey of college girl students and compared with that of college students 20 years and 10 years ago which based on the same criteria. The items of this survey included the prevalence of dental caries, DMFT and the kinds of restored materials. College girl students 2,294 were examined in May 1988, with the recommended criteria and method of W.H.O.. Among them, the data of 2,243 were analyzed and compared with the data of 1968 and 1978. The following results were obtained : 1. The average number of present teeth was $28.86{\pm}1.65$, which does not show much difference when compared to $29.00{\pm}1.94$ of 1968 and $28.97{\pm}1.64$ of 1978. 2. The prevalence of dental caries was 86.22%, which increased in comparison to 83.25% of 1968 and 84.92% of 1978. 3. DMF index was 19.98, which considerably increased in comparison to 14.56 of 1968 and 15.51 of 1978. 4. The number of DMFT was $5.75{\pm}5.04$ per student. Compared to $4.23{\pm}3.88$ of 1968 and $4.49{\pm}3.69$ of 1978, there is a considerable increase. I) The average number of decayed(D) teeth was $1.14{\pm}1.63$, which decreased from $1.16{\pm}1.57$ of 1968 and $2.09{\pm}2.16$ of 1978. II) The average number of missing(M) teeth was $0.23{\pm}0.73$, which also decreased from $0.69{\pm}1.11$ of 1968 and $0.32{\pm}0.79$ of 1978. III) The average number of filled(F) teeth was $4.39{\pm}4.91$, which increased from $2.38{\pm}3.33$ of 1968 and $2.09{\pm}3.17$ of 1978. 5. According to the analysis of filling material, filling baby including amalgam, gold inlay, resin, silicate cement and paladium inlay was 90.5%, which increased in comparison to 72.8% of 1968 and 83.6% of 1978. Among them, amalgam occupied 62.93%, which showed the most increase. On the other hand, crown and bridge was 6.5%, 3.0%, respectively, which showed continuous decrease from 12.1%, 15.1% of 1968 and 8.7%, 7.7% of 1978. Therefore, the prosthodontic treatment reduced due to the decrease of tooth extraction. In contrast, there is continuous increase of conservative treatment which maintaining natural tooth.

Restorative dental materials have advanced rapidly, with improved physical properties that improve survival rates. Accordingly, various materials can be selected. Amalgam, composite resin, glass-ionomer cement, and preformed stainless steel crowns have all been used widely for the restoration of dental caries in primary molars. The various dental materials used to treat proximal caries in the primary molars have distinct advantages and disadvantages. However, few studies have examined their survival rates. This retrospective study examined the 2-year survival rates of more than 700 class II restorations of proximal caries in primary molars clinically and radiologically according to the type of restoration. The study results should help in the selection of class II restorations for molars, one of the biggest concerns of pediatric dentists.
Mineral trioxide aggregate (MTA) is mainly composed of lime and silica. Its four major phases are tricalcium silicate, dicalcium silicate, tricalcium aluminate, and tetracaclcium aluminoferrite. MTA has relatively long initial setting time (2h 45m) and various additives can be added to reduce setting time. Compressive strength of MTA increases with time and reaches 100 MPa after 28 days. MTA has high pH of 9-12.5 because of the formation of calcium hydroxide during its hydration reaction. MTA has superior sealing ability to amalgam and IRM when it is used in perforation repair or root end filling. MTA is safe in cytotoxicity and genotoxicity and have potential to promote pulpal and periapical hard tissue formation.

This study aimed to evaluate the effects of national dental screening for children in Korea in the aspects of prevention of dental caries, the number of treatments related to dental caries, and cost of dental care. The study used the national cohort data on children health screening provided by National Health Insurance Service, and analyzed 32,937 1st dental screening, 22,608 2nd dental screening, 13,708 3rd dental screening, and 241,043 cases from the dental care of 27,944 children born in year 2008 and 2009. The decayed-filled teeth index and decayed-filled person rate, calculated from the results of the 2nd and 3rd dental screening, decreased as the number of preceding dental screening increased. Glass ionomer and amalgam restoration, pulp treatment of primary teeth and extraction of primary canine and molar were shown to decrease as the number of examined dental screening increased, and the total cost of dental care covered by national health insurance also decreased as the number of dental screening increased. In conclusion, national dental screening for children is an efficient and economical oral health care method that prevents dental caries and lowers the possibility of dental caries related treatment, thereby reducing cost of dental care.

Pediatric dentistry is differentiated from other fields of dentistry in that it provides comprehensive dental care for children and adolescents. In early days, pediatric dentistry used to be confined to the caries treatment and extraction of primary teeth However, the practice spectrum of pediatric dentistry has broadened to the orthodontic, surgical, esthetic and preventive treatments A survey that contained questions about practice patterns were mailed to 50 pediatric dentists, and 21 surveys were returned. Results were as follows: 1. The average number of patients per week was 82.4, and the average number of treated teeth per patient was 2.35. 2. Preventive treatments comprised 15.7%, restorative treatments 55.7%, endodontic treatments 15.6%, surgical treatments 10.5%, and orthodontic treatments 2.4%. 3. In restorative treatments, amalgam restoration comprised 3.8%, glass ionomer 5.5%, composite resin 63.0%, and stainless steel crown 27.7%.
The plane strain fracture toughness of a material characterize the resistance to fracture in the presence of a sharp crack under severe tensile condition. Fracture toughness can be determined by indentation method. The purpose of this study was to investigate the fracture toughness of dental amalgams by measuring the plane strain fracture toughness and the fracture toughness from indentation method. Two conventional and four high copper amalgam alloys were employed for this study. The amalgams were prepared according to the A.D.A. spec. No. 1 and inserted into the specially designed mould with the single edge notch specimen to use in 3-point bending method. The specimens (20mm long, 4mm wide, 2mm thick) were stored at $37^{\circ}C$ for 1 week, and tested in 3-point bending by means of Instron at a cross-head speed of 1mm/min. In indentation method, the specimens were made in same manner as single edge notch specimens. The test was conducted with Vickers hardness tester at 10kg load. The following results were obtained. 1. The plane strain fracture toughness and the fracture toughness from indentation method were higher in the low copper amalgams than the high copper amalgams. 2. In high copper amalgams, the fracture toughness of amalgams decreases according as the copper contents increase. 3. In similar copper contents, the single composition amalgams have a higher fracture toughness than the admixed amalgams.
