Reliability of Transcranial Doppler Examination in the Diagnosis of Delayed Ischemia after Subarachnoid Hemorrhage (지주막하출혈에 의한 지연성 허혈신경장애의 진단에 있어서 경두개도플러 검사의 신뢰도)

  • Objective : The reliability of transcranial doppler sonography(TCD) for predicting delayed ischemic neurologic deficit( DIND) was investigated in patients with aneurysmal subarachnoid hemorrhage(SAH). Methods : The velocity of blood flow through the middle cerebral artery was measured by TCD in 70 patients treated surgically within 72 hours due to ruptured cerebral aneurysm. A correlation between measured maximal mean blood flow velocities and clinical factors including age, hypertension, Hunt-Hess grade, Fisher grade, DIND, and outcome was made. Results : An age-dependent reduction of the measured maximal mean velocities was found(r=-0.4043, p<0.001). Flow velocities in hypertensive patients were significantly lower than in the normotensive individuals(two-tailed T test, p<0.05). There was no significant difference between the flow velocities and evaluated other clinical factors. When the flow velocities of 14 patients who developed DIND were compared with those of patients without deficits, no significant difference was seen. A significant increase in flow velocities in the days before the onset of DIND was found only in 4 of 14 cases. High flow velocities did not necessarily mean impending neurological deficits : 6 of 70 patients tolerated flow velocities over 160cm/s. Conclusion : We suggest that TCD readings have limited utility for predicting DIND following SAH, especially in older or hypertensive patients. More direct measurements of blood flow are necessary to adequately predict which patients are at high risk of DIND.

  • We have performed two experiments in the fast critical assembly BFS to measure the effective delayed neutron fraction $\beta$$_{eff}$ values and compared the results to validate the $\beta$$_{eff}$ computation code, BETA-K. Measurements of $\beta$$_{eff}$ were carried out in a metallic plutonium core and a metallic uranium core with Cf$^{252}$ source pseudo-reactivity method. Fission integrals and correction factors, which were used to obtain the experimental $\beta$$_{eff}$ values, were calculated by using the LMR core design computation code system of KAERI. BETA-K has been developed consistently with the hexagonal Nodal Expansion Method (NEM) and it used delayed neutron data of ENDF/B-VI. By comparing the computed $\beta$$_{eff}$ values with the measured ones, we found that the results from BETA-K agreed with the experimental values within the experimental error bound.ror bound.

A study on the Delayed Muscle Soreness after Exercise and the Indicators of Muscle Damage (운동 후 지연된 근통종과 근손상의 지표에 관한 연구)

  • The purpose of this study was to analyze the delayed muscle soreness after exercise and the indicators of muscle damage. Subjects of this study were 9 male students. Some muscle enzymes and others(GOT, LDH, ALD, CPK in serum, WBC, ESR) that known as the indicators of muscle damage were measured before exercise. After measurements they run on a treadmill ($incline:25\%$, speed;3.5miles/h) for 7 min. on an average. The intensity of this kind of exercise was $90.5\%$ of maximal exercise intensity. After exercise, same indicators were measured at the just after exercise, 24hr after, 48hr after, 72hr after. Also muscle soreness level was evaluated at same intervals by make use of Modified Abraham's scale. The result was as follow: There was no relationships between muscle soreness and the indicators of muscle damage but ALD was the most important indicator that can be explains the muscle soreness very well.

EMG-based Prediction of Muscle Forces (근전도에 기반한 근력 추정)

  • We have evaluated the ability of a time-delayed artificial neural network (TDANN) to predict muscle forces using only eletromyographic(EMG) signals. To achieve this goal, tendon forces and EMG signals were measured simultaneously in the gastrocnemius muscle of a dog while walking on a motor-driven treadmill. Direct measurements of tendon forces were performed using an implantable force transducer and EMG signals were recorded using surface electrodes. Under dynamic conditions, the relationship between muscle force and EMG signal is nonlinear and time-dependent. Thus, we adopted EMG amplitude estimation with adaptive smoothing window length. This approach improved the prediction ability of muscle force in the TDANN training. The experimental results indicated that dynamic tendon forces from EMG signals could be predicted using the TDANN, in vivo.

  • For measuring nonlinear Vehicle Modeling based on 7-Depth Sensor, the neural networks are proposed m adaptive and in realtime. The structure of it is similar to recurrent neural networks; a delayed output as the input and a delayed error between the output of plant and neural networks as a bias input. In addition, we compute the desired value of hidden layer by an optimal method instead of transfering desired values by backpropagation and each weights are updated by RLS(Recursive Least Square). Consequently, this neural networks are not sensitive to initial weights and a learning rate, and have a faster convergence rate than conventional neural networks. This new neural networks is Error Estimated Neural Networks. We can estimate nonlinear models in realtime by the proposed networks and control nonlinear models.

  • The purpose of this study was to determine the effects of transcutaneous electrical nerve stimulation(TENS) on delayed onset muscle soreness(DOMS). Twenty males performed eccentric exercise of the elbow flexor. Subjects were randomly assigned to one of three groups: 1) a group ($n_1$=7) that received low frequency TENS (7 Hz), 2) a group ($n_2$=7) that received high frequency TENS (500 Hz), 3) a control group ($n_3$=6) that received no treatment. DOMS was induced in a standardised fashion in the non-dominant elbow flexor of all subjects by repeated eccentric exercise. Treatments were applied immediately following exercise and again at 24 hours and at 48 hours after. Subjects attended on three consecutive days for treatment and measurement of elbow flexion, extension and resting angle(universal goniometer), and pain(Visual Analogue Scale; VAS) on a daily basis. Measurements were taken after treatment. Analysis of results using repeated measures analysis of variance(ANOVA) and post hoc tests were as follows: 1) there were between groups differences in pain value at 48 hours after (p<0.05), 2) one-way ANOVA with repeated measurement for pain, resting angle, flexion angle and extension angle revealed significant differences within low frequency TENS group, 3) one-way ANOVA with repeated measurement for flexion angle revealed significant difference within high frequency TENS group.

The Vortical Flow Field of Delta Wing with Leading Edge Extension

  • The interaction and breakdown of vortices over the Leading Edge Extension (LEX) - Delta wing configuration has been investigated through wing-surface pressure measurements, the off-surface flow visualization, and 5-hole probe measurements of the wing wake section. The description focused on analyzing the interaction and the breakdown of vortices depending on the angle of attack and the sideslip angle. The Effect of angle of attack and sideslip angle on the aerodynamic load characteristics of the model is also presented. The sideslip angle was found to be a very influential parameter of the vortex flow over the LEX-delta wing configuration. The introduction of LEX vortex stabilized the vortex flow, and delayed the vortex breakdown up to a higher angle of attack. The vortex interaction and breakdown was promoted on the windward side, whereas it was suppressed on the leeward side.

  • The strengthened aviation security measurements caused by increased terror threat all around the world have air passengers annoyed by uneasy treatment and some delayed process. The service level of air travel and competitiveness of airlines industry has been weakened also. The aviation security process is mainly conducted at an airport. The threat level of airport is not usually same airport to airport, and the quality of aviation security activity is neither same. However, ICAO requires every international airport to conduct similar process to ensure aviation security without considering situational difference of each airport. The paper has objectives to develop reasonable security measurements based on risk management concept. It studied the ways of achieving target level of security of an airport, considering the threat situation of the airport and its ability to conduct security procedures.

Uveitis as an important ocular sign to help early diagnosis in Kawasaki disease

  • Purpose: Incomplete Kawasaki disease (KD) is frequently associated with delayed diagnosis and treatment. Delayed diagnosis leads to increasing risk of coronary artery aneurysm. Anterior uveitis is an important ocular signs of KD. The purpose of this study was to assess differences in laboratory findings, including echocardiographic measurements, clinical characteristics such as fever duration and treatment responses between KD patients with and those without uveitis. Methods: We conducted a prospective study with 110 KD patients from January 2008 to June 2013. The study group (n=32, KD with uveitis) was compared with the control group (n=78, KD without uveitis). Laboratory data were obtained from each patient including complete blood count (CBC), erythrocyte sedimentation rate (ESR), platelet count, and level of alanine aminotransferase, aspartate aminotransferase, serum total protein, albumin, C-reactive protein (CRP), and N-terminal probrain natriuretic peptide (NT-pro BNP). Echocardiographic measurements and intravenous immunoglobulin responses were compared between the two groups. Results: The incidence of uveitis was 29.0%. Neutrophil counts and patient age were higher in the uveitis group than in the control group. ESR and CRP level were slightly increased in the uveitis group compared with the control group, but the difference between the two groups was not significant. No significant differences in coronary arterial complication and treatment responses were observed between the two groups. Conclusion: Uveitis is an important ocular sign in the diagnosis of incomplete KD. It is significantly associated with patient age and neutrophil count.

Effects of branched-chain amino acid supplement on knee peak torque and indicators of muscle damage following isokinetic exercise-induced delayed onset muscle soreness

  • [Purpose] This study aimed to investigate the effects of branched-chain amino acid (BCAA) supplement on delayed onset muscle soreness (DOMS) by analyzing the maximum muscle strength and indicators of muscle damage. [Methods] Twelve men with majors in physical education were assigned to the BCAA group and placebo group in a double-blinded design, and repeated measurements were conducted. DOMS was induced with an isokinetic exercise. Following BCAA administration, the changes in the knee extension peak torque, flexion peak torque, aspartate aminotransferase (AST), creatine kinase (CK), and lactate dehydrogenase (LDH) concentrations were analyzed. The maximum knee muscle strength was measured at the baseline (pre-D0) following BCAA administration for 5 days before exercise (-D5, -4D, -3D, -2D, -1D). In contrast, the post-treatment measurements (D3) were recorded after BCAA administration for 3 days (post-D0, D1, D2). Blood samples were obtained before (pre-D0), immediately after (post-D0), 24 h (D1), 48 h (D2), and 72 h (D3) after the exercise to analyze the indicators of muscle strength. BCAA was administered twice daily for 8 days (5 days and 3 days before inducing DOMS and during the experimental period, respectively). [Results] There was no difference in the flexion peak torque between the groups. However, the BCAA group showed a significantly higher extension peak torque at D3 (second isokinetic exercise), compared to the placebo group (p<.05). There was no difference in AST changes between the groups. Nonetheless, the CK and LDH were significantly reduced in the BCAA group, compared to the placebo group. There was no correlation between the extension peak torque and flexion peak torque. However, the CK and LDH increased proportionately in DOMS. Moreover, their concentrations significantly increased with a decreasing peak torque (p<.01). [Conclusion] An exercise-induced DOMS results in a decrease in the peak torque and a proportional increase in the CK and LDH concentrations. Moreover, the administration of BCAA inhibits the reduction of the extension peak torque and elevation of CK and LDH concentrations. Therefore, BCAA might be administered as a supplement to maintain the muscle strength and prevent muscle damage during vigorous exercises that may induce DOMS in sports settings.
