Purpose : To assess the feasibility and the toxicity of the neoadjuvant chemotherapy on the treatment of patients with locoregionally advanced nasopharyngeal carcinoma. Methods and Materials : We analyzed 77 previously untreated and histologically confirmed advanced stage nasopharyngeal carcinoma patients treated with neoadjuvant chemotherapy followed by radiation therapy at the Seoul National University Hospital between 1984 and 1996. The stage distribution was as follows : AJCC stage III-2, stage IV-75. Sixty-six patients received infusion of 5-FU (1000 mg/m$^2$, on Day 1$\~$5) and cisplatin (100 mg/m$^2$, on Day 1), eleven patients received infusion of 5-FU (1000 mg/m$^2$, on Day 1 $\~$5) and carboplatin (300 mg/m$^2$, on Day 1) as neoadjuvant chemotherapy Prior to radiation therapy. The median follow-up for surviving patients was 44 months. Results : The overall chemotherapy response rates were 87$\%$. The toxicities of chemotherapy were mild. Only 3 patients experienced Grade 3 toxicities (1 for cytopenia, 2 for nause/vomiting). The degree of radiation induced mucositis was not severe, and ten patients developed Grade 2 mucositis. The 5-year overall survival rates were 68$\%$ and the 5-year disease free survival rates were 65$\%$. The 5-year freedom from distant metastasis rates were 82$\%$ and 5-year locoregional control rates were 75$\%$. Conclusion : This single institution experience suggests that neoadjuvant chemotherapy improves overall survival and disease free survival for patients with advanced stage nasopharyngeal carcinoma without increase of toxicity.
Though many genetic and epigenetic alterations have been identified in hamster oral carcinogenesis model, there is no information about the possible role of transforming growth factor related with oral cancer. The purpose of this paper was to find the expression patterns of transforming growth factor alpha and beta during the stages of complete oral carcinogenesis model in hamster. 0.5% 9, 10-dimethyl-1, 2-benzanthracene(DMBA) in mineral oil was topically applied to the buccal pouch of 75 hamster three times a week during the experimental periods. The experimental animals were subdivided into two groups of control and experiment. Only the mineral oil was applied to the control group. 0.5% DMBA in mineral oil was applied to the experimental groups of 6, 8, 10, 12, 14, 16, 18 and 20 weeks. The expression of the $TGF-{\alpha}$ and $TGF-{\beta}$ protein were evaluated by the distribution and intensity of positive cells during the carcinogenesis using the immunohistochemical study. The following results were obtained ; 1. The buccal pouch epithelium of hamster was histologically changed to the dysplasia at 6, 8, 10 weeks, carcinoma in situ at 12 weeks, and squamous cell carcinoma at 14 weeks. 2. The expression of the $TGF-{\alpha}$ was restricted to the parabasal and basal layers of the normal and dysplastic mucosa, but those positive cells were extended to the spinous layers of the epithelium in the carcinoma. 3. The degree of $TGF-{\alpha}$ expression was markedly decreased in the carcinoma at 16, 18, 20. The strong positive staining in the center of cancer islands and weak positive staining in periphery of tumor were seen at the stage of squamous cell carcinoma. 4. The positive index of the $TGF-{\alpha}$ had a tendency to increase with DMBA- applied time. There was a statistically significant difference between 12, 18, 20 experimental group and control group (p<0.05). 5. The expression of the $TGF-{\beta}$ was shown at the cytoplasm of all control and experimental groups, and the parabasal and basal layers of the normal and dyslastic mucosa, but it was shown at the basal layers of the epithelium in the carcinoma. 6. $TGF-{\beta}$ was expressed diffusely at 16, 18, 20 experimental group. The strong positive staining in the center of cancer islands and positive staining in periphery of tumor were seen at the stage of squamous cell carcinoma. From the above findings, the expression of $TGF-{\alpha}$ and ${\beta}$ in oral carcinogenesis model seems to have two formal stages, the first being an overexpression step as reaction to uncontrolled growth and the second being one in which external protein accumulate in the surrounding stroma and intracytoplasm. Overexpression of $TGF-{\alpha}$ and ${\beta}$ may have important cooperative roles for the promotion of cancer and factor of prognosis.
Major cities have strengthened their efforts to improve the environmental function of the urban forest through aspects of quantity and quality. Yet, it is difficult to propose both long-term improvement and management policy direction continuously due to a lack of evaluating method that social needs could be reflected. After all, effective strengthening of the function of an urban forest, composed of social and biotic environments, is essential to create evaluation criteria. Such data reflects a quantity variation and identifies the distribution and habitation of biotic sources as well. An assessment of utilization possibilities presents such values as a green connectivity, an ecological healthiness, and usage and access opportunities. Consequently, this research was executed to create an urban forest sustainability index that proposes a management and policy direction based on the evaluation of quantity and quality aspects. This study was processed by the following steps: a degree of importance of ecological aspect, a sub-classification index choice, and an examination of adequacy. This research was conducted from premises that urban forest had another functions comparisons to a natural forest. As a result, a criteria of sustainability should be considered to evaluate an ecological aspect, as well as the social and management aspects regarding human disturbance. This research is designed to develop an evaluation index for ecological aspect. In order to evaluate the ecological healthiness of an urban forest, a vegetation index considers aspects of related rarity, damage possibility, naturalness, area, and diversity. In a wildbird index-related study, forest duration and area were selected. Finally, a variance analysis is presented, inclusive of stability, naturalness, diversity, potentiality, and possibility of wildlife inhabitation, which is indicative of an urban forest sustainability index.
In the 82 apple varieties tested, the distribution of varieties grouped on the damage degree to Alternaria leaf spot were 15 varieties(l8.3%), which damaged above 50%, and 51 varieties(62.2%) damaged below 5%. Susceptible varieties to Alternaria leaf spot were Raritan, Indo, Winesap and Starkrimson, moderately resistant varieties Sekaiichi and Starking, and resistant Jonathan, Tsugaru, Molliies, Jonagold, Gala etc. The leaf hair density on the backside of leaf were rarer inresistant varieties than susceptible one. In inoculation test, penetration and infection of Alternaria mali were easier on the backside than surface of apple leaf at the same varity, and infection rate with Alternaria mali were higher at removal treatment than non-removal one of leaf hairs, so we can guess that leaf hair are related to resistance of variety to Alternaria mali.
The data on the striped rice, borer populations, collected in the four years from 1973 to 1976, was reanalyzed to study the characteristics in the aggregation pattern of the insect larvae in the paddy fields. The distribution pattern of the larvae was well fitted to the negative binomial model in both the first and the second generation. With reference to the Green's coefficient of dispersion estimated, the aggregation of the larvae was categorized into three phases: initial high aggregation, changing, and dispersed stable phase. Except the changing phase, each phase in each generation could be defined by a common k. The phase-change was initiated by the larval dispersal between hills of the rice plants: the larvae of the third stage and those of the fifth stage, for the first and second generation, respectively. The characteristics of the aggregation pattern were stable in the second generation. In the first genaration, the pattern was more or less variable, indicating that the life system of the insect in the first generation was more susceptible to the variations of the external physical factors than that in the second generation.
The purpose of this study was the acquisition of the optimum scale of the apportionment of standard & high-class bed for the maximum profit representative of the desire of customers in a General Hospital with 1,100 beds located in Seoul. This investigation was proceeded by the analysis of the result of the simulation with the survey of both the patients' needs for bed and the degree of the medical service by the grade of the ward. And finally the consequence was obtained as follows: 1. The result of the investigation of the inpatients' preference for the grade of ward classes shows that a private ward reflected 4.3 percent, a semi-private ward 1.7 percent, a three-bed ward 0.1 percent, and a ward with six beds 93.9 percent each other. 2. A questionnaire poll was paralleled of service terms of a medical doctor and a nurse by ward class, the data were used for the standard of the allotment of labor cost by the ward class. The poll shows that the service tenn of a medical doctor and a nurse based on a ward with six beds by ward class showed 1.7 times in internal medicine and 1.9 times in surgery at a private ward; 1.4 times in internal medicine and 1.7 times in surgery at a semi-private room; and 1.2 times both in internal medicine and in surgery at a three-bed ward 3. The resultant findings revealed the most profit per bed and per patient in a private ward. However, an analysis of profit with a standard of unit area by ward class represented a higher profit in both the internal medicine and the surgery semi-private ward than other ward classes. 4. The result of the analysis through simulation based on the data of the prime cost per the ward class proved the optimum scale of the distribution of beds by class as follows: sixteen beds of the internal medicine and twenty three beds of the surgery in the private ward; two hundreds and two of the internal medicine and one hundred and ninety eight of the surgery in the semi-private room; three of both the internal medicine and the surgery each other in the three-bed ward; one hundred and ninety eight of the internal medicine and two hundred and fifty two of the surgery in the ward with six beds. The result of this research exhibits that the income and expenditure of the hospital could be improved by changing parts of wards into private ones(containing the maximum profit per a unit of width) in case the scale of the number of beds is reset with the consideration of the profit per the unit width. In the near future it's strongly expected that the research for the more scientific standard of the allotment of labour cost by ward class and for definition of the optimum scale of the number of beds that actualize the maximum profit with the change of the three elements of the prime cost: cost of materials; labor costs; management expenses.
In Korea, it is significantly deficient in the study about tourism geology, which is a new branch of applied geology that would support the growth of ecotourism world-wide. The objectives of this study include (1) the promotion in geodiversity of tourism geology using the data for type and distribution patterns of geological and geomorphological landscape resources and (2) the redoubling of diversity in the environmental interpretation programs offered by the Korea National Parks Service (KNPS). Our field study discovered 212 geological and geomorphological landscape resources distributed in the area of Taean seashore national park. Coastal topography is the most discovered type followed by weathering topography. It is our belief that the aforementioned resources can be utilized as a tourism geological site in assisting the public to understand geological science and to draw their attention and interests after sorting and filtering it out through discussions with geologists and geomorphologists of a consortium. Furthermore, in order to promote the activation of developing user-oriented geotourism sites, it is recommended to keep monitoring on demographical characteristics of geotourists, behavioral characteristics of geotouconrists within the geotourism site and ducting analysis for developing geotourism program and events. And, the research support of geological engineering dealing with the estimation of weathering degree and the development of conservation techniques for the object of geotourism along with the research of environmental science aspects will improve the activation of tourism geology.
Parents of handicapped children are experiencing difficulties in their children's care, social isolation, change of life style and lirnited leisure time. Because the parents should take care of the children's daily life, they have lots of psychological and physical stress. Chronic stress of parents puts stress to the other family members and affects the development of children with handicap. The purpose of this study were to identify the level of stress in each of parents of children with motor problem, the characteristics of the children and general information related with the children, and to analyse the stress by reasons. Specially organized questionnaire were used for an investigation method. "Test of stress in mother who has children with chronic illness" by Kim Hee-soon were modified and used. The questionnaire answered by 43 mothers and 35 fathers were analyzed. Data analysis includes frequency analysis, Pearson correlation coefficients, paired-samples t-test and MANOV A by SPSSWIN. The results were as follows: 1) Degree of handicap was most moderate (46.5%), level of motor development was most pull to walk (34.9%), and combined handicap was 69.8%. 2) Sexual distribution represented that 51.2% male and 48.8% female. The cost of physical therapy was 69.8% in no more than 100,000 won. 3) The mean of age, for the mother was 32.8 years and 35.3 years. Level of motor development that mother and father expect was 88.4%, 83% walk alone. 4) Both mother and father experienced stress in other of Part II (changes in father was the illness status of the child and difficulty in taking care of child), Part III (prognosis of the child's condition), Part I (social-personal relationships and the responsibility of the care givers). In the total score of stress, mother's stress is indicated higher level than father's stress. 5) There was no correlationship between characteristics and stress of mother and father. 6) There was no statistically significant difference between characteristics and related general information of children with handicap and stress of mother and father. As a results, the mother of children with handicap are experiencing more stress than the father. Both of parents have the most difficulties in the changes in the illness status of the child and difficulty in taking care of child. This study can be used as resources of education, therapy and counselling for children with handicap and their parents. This study, also, can be used to encourage the quality of Iife for the children with handicapped and their family.
This study, to provide quantitative data related to compaction characteristics, identifies the compaction characteristics of various types of soil samplers, in relation to their particle-size distribution and plasticity degree, and the compaction characteristics of artificially created granular materials, in relation to their A & D compaction. The results of the experiments show as follows. $r_{dmax}$ of clay is less than those of both sand and gravel approximately by 10%. O.M.C of clay has turned out to be greater than sand and gravel approximately by 20% and 30%, respectively. Changes in the compaction characteristics can be observed clearly around 30~60% of sand and 30~50% of passing No.200 sieve. It has also been shown that the compaction characteristics related to LL and PL are similar to each other in changes, and that the compaction characteristics become less clear with higher percent of fine grained soil. The compaction characteristics of the artificially created granular materials and field materials have appeared almost similar to each other. $r_{dmax}$ is less approximately by 30% and O.M.C greater approximately by 20% in A compaction than in D compaction. As $r_{dmax}$ and O.M.C become greater, its rate increases.

Activity durations retain probabilistic and stochastic natures due to diverse factors causing the delay or acceleration of activity completion. These natures make the final project duration to be a random variable. These factors are the major source of financial risk. Extending the Stochastic Project Scheduling Simulation system (SPSS) developed in previous research; this research presents a method to estimate how the final project duration behaves when activity durations change randomly. The final project cost is estimated by considering the fluctuation of indirect cost, which occurs due to the delay or acceleration of activity completion, along with direct cost assigned to an activity. The final project cost is estimated by considering how indirect cost behaves when activity duration change. The method quantifies the amount of contingency to cover the expected delay of project delivery. It is based on the quantitative analysis to obtain the descriptive statistics from the simulation outputs (final project durations). Existing deterministic scheduling method apply an arbitrary figures to the amount of delay contingency with uncertainty. However, the stochastic method developed in this research allows computing the amount of delay contingency with certainty and certain degree of confidence. An example project is used to illustrate the quantitative analysis method using simulation.
