Research on Actual Condition of Emergency Patients in Farming and Fishing Villages - In southwest coast district center - (농어촌지역 응급환자 실태조사 - 서남해안 지역을 중심으로 -)

  • Purpose: This study aims to examine characteristics and actual conditions of patients using emergency room at farming and fishing villages, solve overcrowding of emergency room at the tertiary hospital and activate local emergency clinics. Methods: It examines department of diagnosis and treatment, vehicles used, sex, age, residential area, visit hour, length of stay, presence or absence of trauma, measures after first aid and degree of severity based on medical records of 6,740 patients using emergency room at farming and fishing villages from Jan. 1 to Dec. 31, 2005. Conclusion : 1. Sex distribution of patients of emergency room was male 54.9% and female 45.1% and age distribution between over 40 and below 50 was most as 15.9%. 2. Transport means to emergency room were 91.4 of private car and others (public transport and going on foot), 7.5 of 119, 129 and police car and 1.0% of ambulance. 3. According to distribution of residential areas of emergency patients, 38.9% were Eup area, 42.1% Myeon area, 11.4% distant area and 7.5% adjacent area. 4. According to distribution of emergency patients by department of diagnosis and treatment, internal medicine was most as 35.8% and 55% of patients visited emergency room from 3:31 p.m. to 11:30 p.m.. 5. According to total hours of diagnosis and treatment of subjects, 51.2% were within 30min. and cases of non-trauma disease were 68.2%. 6. According to degree of emergency of emergency patients, non-emergency cases were 65.3%, urgent cases 27.7% and emergency cases 7.0% and 74.2% of patients returned home after first aid and 20.6% of them hospitalized. In conclusion, characteristics and diversification of patients should be examined and efforts by government and local medical institutions which must organize emergency system and facility and personnel levels suitable to regional conditions are needed in order to prevent overcrowding of emergency center of the tertiary hospital and activate local emergency center.

Leaf Litter Processing and Patterns of Shredder Distribution in Headwater Steams in Southeastern Korea (한국 남동지역 상류 하천에서의 낙엽 분해기작과 shredder 분포 유형)

  • During the period of December 1992 (winter-spring) and from February 1995 (winter-spring), the leaf processing rates of oak (Quercus serrata) and tulip (Liriodendron tulipifera) tree was investigated in the headwater streams in southeastern part of Korea in conjunction with the distribution pattern of macroinvertebrate fauna. Using two types of bags ($10\times30cm$ with 5 g of dry leaves; open bag with holes, closed bag without holes), decomposition rates of oak and tulip tree by shredder and/or microbiota at a reatively undisturbed 2nd-order stream were compared, Regardless of leaf type, leaves in the open bage decomposed slightly faster than those in the closed bags. In the 1992 experiment, osk leaves decomposed much slower than tulip leaves (after 138 degree days, osk : closed, 0.006% loss/day ; open, 0.008 ; tulip: closed, 0.021 ; open, 0.023; n=2). The of the first experiment using oak leaves in 1995 were similar to those of the first experiment (after 151 degree days, oak: closed, 0.005% loss / day; open, 0.006; n=6). Over 50% of invertebrates from 122 leaf pack samples collected from 12 streams during the winter period of 1994 were identified as shredders (shredder, 56.2; collector, 32.7; scraper, 8.65; predator, 2.45%). Among shredders, Gammarus sp. and Tipula sp. were dominant species in terms of number and biomass (8.2 ind./g, 1.0 ind./g AFDW of leaves). Among many physico-chemical parameters, the width of stream channel was found to be the most influential factor in the distribution of Gammarus and Tipula (Gammarus: r=-0.34, P<0.001;Tipula:r=0.40, P<0.001). Considering the fact that oak is one the dominant riparian vegetation in the southeastern part of korea, the patterns of oak processing and shredder distribution shown in theis study may well represent some of the important characteristics of headwater steams in southeastern Korea.

  • In this study, the distribution behaviors of the polystyrene sulfonic acid (PSSA) grafting polymer across the FEP-g-PSSA membranes prepared by a simultaneous irradiation method, were investigated by analyzing the cross-section of the membranes with a SEM-EDX instrument. The effects of irradiation conditions such as the degree of grafting, FEP film thickness, and grafting solvent on the distribution of the grafting polymer were mainly studied in this experiment. The results indicate that to obtain the evenly grafted FEP-g-PSSA membranes, the higher degree of grafting is required as the film thickness increases, and the lower dose rate are more effective than the higher dose rate at the given dose.

Aminoglycoside Dosage in Neutropenic Fever Patients after Tranplantation of Blood Stem Cells (조혈모 세포 이식후 neutropenic fever환자에서의 aminoglycoside dosage에 관한 검토)

  • Pharmacokinetic parameters and dosage of aminoglycosides (AGs) were studied retrospectively in 36 patients with neutropenic fever after stem cell transplantation in Seoul National University Hospital from July 1996 to June 2001. AGs pharmacokinetic parameters were calculated with steady-state peak and trough serum drug concentrations by the method of Sawchuk and Zaske et at. The calculated aminoglycosides volume of distribution and clearance were greater than population value $(0.36\pm0.06\;L/kg,\;116\pm32\;ml/min/1.73\;m^2,\;respectively)$. The average dosage of aminoglycosides required to maintain optimal serum AGs concentration was also greater than recommended dose in insert paper. The average dosage of amikacin was $11\pm2.1$ mg/kg every 12 hours (In case of tobramycin, $2.09\pm0.37$ mg/kg every 8 hours or $2.59\pm0.20$ mg/kg every 12 hours). The relationship between AGs volume of distribution and sex, serum albumin (g/dl), body mass index $(kg/m^2)$, body weight change $(\%)$, the amount of fluid inpu (ml/kg/day), the degree of hematocrit decrease $(\%)$ were studied respectively. Univariate anlysis revealed that body mass index $(kg/m^2)$, the amount of fluid input (ml/kg/day) and the degree of hematocrit decrease $(\%)$ had significant correlation with aminoglycosides volume of distribution. But sex, serum albumin, body weight change $(\%)$ had no significant correlation with aminoglycosides volume of distribution.

Evaluating LIMU System Quality with Interval Evidence and Input Uncertainty

  • The laser inertial measurement unit is a precision device widely used in rocket navigation system and other equipment, and its quality is directly related to navigation accuracy. In the quality evaluation of laser inertial measurement unit, there is inevitably uncertainty in the index input information. First, the input numerical information is in interval form. Second, the index input grade and the quality evaluation result grade are given according to different national standards. So, it is a key step to transform the interval information input by the index into the data form consistent with the evaluation result grade. In the case of uncertain input, this paper puts forward a method based on probability distribution to solve the problem of asymmetry between the reference grade given by the index and the evaluation result grade when evaluating the quality of laser inertial measurement unit. By mapping the numerical relationship between the designated reference level and the evaluation reference level of the index information under different distributions, the index evidence symmetrical with the evaluation reference level is given. After the uncertain input information is transformed into evidence of interval degree distribution by this method, the information fusion of interval degree distribution evidence is carried out by interval evidential reasoning algorithm, and the evaluation result is obtained by projection covariance matrix adaptive evolution strategy optimization. Taking a five-meter redundant laser inertial measurement unit as an example, the applicability and effectiveness of this method are verified.

A Comparative Study on the Efficient Reordering Methods of Sparse Matrix Problem for Large-scale Surveying Network Adjustment (대규모 측지망 조정을 위한 희소 행렬의 효율적인 재배열 방법에 대한 비교 연구)

  • When a large sparse matrix is calculated for a horizontal geodetic network adjustment, it needs to go through the process of matrix reordering for the efficiency of time and space. In this study, several reordering methods for sparse matrix were tested, using Sparse Matrix Manipulation System(SMMS) program, total processing time and Fill-in number produced in factorization process were measured and compared. As a result, Minimum Degree(MD) and Mutiple Minimum Degree(MMD), which are based on Minimum Degree are better than Gibbs-Poole-Stockmeyer(GPS) and Reverse Cuthill-Mckee(RCM), which are based on Minimum Bandwidth. However, the method of the best efficiency can be changed dependent on distribution of non-zero elements in a matrix. This finding could be applied to heighten the efficiency of time and storage space for national datum readjustment and other large geodetic network adjustment.

Emergency Medical Technology Curriculum Development for college (응급구조와 교과과정 개발)

  • Purpose : This study was performed to develop the curriculum of Emergency Medical technology of College. Method : The authors analyzed curricular of EMT in 12 college in Korea. We conducted the distribution, credit, mean, frequency of liberal arts, majors, teaching profession subject by Kim and Lee(2007). We compared curriculum of knowledge and clinical training courses. Results : The numbers of whole subjects were 59.3 and the number of mean and the credits were 130.8 degree of mean. The numbers of liberal arts were 9.5 number of mean and the credits were 15.5 degree of mean. The numbers of major were 47.5 number of mean and the credits were 111.4 degree of mean. The numbers of teaching profession subject were 1.9 number of mean and the credits were 4 degree of mean. Finally, frequency of curriculum of knowledge and clinical training courses were 57.1% and 42%, respectively. Conclusion : This results will represent curriculum development and will standardize curriculum for EMT collage. in addition, it will make EMT curriculum standardized.

Non-Timber Forest Products Consumption Behaviors According to Dietary Lifestyle (식생활 스타일에 따른 단기소득임산물의 소비행태)

  • This study examined differences in consumers' images of short-term income forest products, preference, willingness to pay prices and purchase behavior according to dietary lifestyle as well as investigated what factors influence the degree of satisfaction when purchasing short-term income forest products and willingness to repurchase. According to dietary lifestyle, the results classified consumers as 'frugal housewife type,' 'convenience-seeking type,' and 'food high-involvement type.' A 'food high-involvement group' is defined as a group that wants high quality products regardless of price. In the 'frugal housewife type,' country of origin and hygiene/safety (considered when purchasing food) had positive influences on the degree of satisfaction. In the 'convenience-seeking type,' country of origin (considered when purchasing food) had a positive influence on the degree of satisfaction while country of origin (checked when purchasing food) had a negative influence on degree of satisfaction. Consumers had a lack of perception for short-term income forest products; subsequently, short-term income forest products had a weakness of low access to consumers. Therefore, farms for short-term income forest products need to divide products into 'high-priced' luxury products and 'low-priced' frugal products according to dietary lifestyle characteristics, improve packaging status to enable consumers to check quality certificates, and clearly indicate country of origin as well as improve distribution processes and increase consumer access to products.

POSTEROANTERIOR CEPHALOMETRIC STUDY OF FACIAL ASYMMETRY ABOUT CLINICAL CHARACTERISTICS AND CHANGES AFTER ORTHOGNATHIC SURGERY (안모비대칭환자의 임상적 특성 및 악교정수술후 변화에 관한 연구;정모두부방사선 규격사진의 계측을 중심으로)

  • This study was carried out to evaluate the distribution and the degree of the asymmetry existed in normal persons and asymmetric patients, and to investigate the changes of asymmetric patients after orthognathic surgery. The analysis was performed with the posteroanterior cephalometric radiography of 60 normal persons and 31 facial asymmetric patients. The results were as follows : 1. The degree of normal asymmetry existed in normal persons was not significant except MF and Me measurements. 2. The degree of normal asymmetry according to sex difference was not significant except cranial base area. 3. When normal persons were compared to asymmetric patients, there were more measurements which presented significant asymmetry on mandible than on maxilla. 4. When postoperative state was compared to preoperative state, the degree of asymmetry were usually decreased except AGO and GA measurements, especially the Cd, MF, Me, and Cd-Me measurements decreased significantly. 5. When postoperative state was compared to normal persons, 4 measurements of mandible approached significantly the measurements of normal persons.
