인영촌구비교맥진(人迎寸口比較脈診)에 관한 연구 (A Study on Comparative Pulse Diagnosis of Renying Pulse(人迎脈) and Cunkou Pulse(寸口脈))

  • Objectives : While Comparative Pulse Diagnosis of Renying pulse(人迎脈) and Cunkou pulse(寸口脈) is one of the three major pulse diagnostic methods in "Huangdineijing" along with Three Positions and Nine Indicators Pulse Diagnosis(三部九候脈診法) and Cunkou Pulse Diagnosis(寸口脈診法), it has died out in later periods. This study aims to examine this lost method. Methods : Annotations of "Huangdineijing" were examined along with descriptions of the author's own experience. Results & Conclusions : Renying is the Renying(人迎) point from the Stomach Channel(ST), while Cunkou is the Taiyuan(太淵) point from the Lung Channel(LU). These two points are compared in order to determine the deficiency and excess of the Zangfu(臟腑). Normal pulses(平脈) are Soft(軟脈) or Moderate(緩脈), while Stirred pulses(躁脈) are Stringy(弦脈), Tight(緊脈), Slippery(滑脈) or Long(長脈). If the Renying is once active where Shaoyang pulse is active, purge the Gallbladder and supplement the Liver. If there is Stirred pulse, purge the Triple Burner and supplement the Pericardium. If the Renying is twice active where Taiyang pulse is active, purge the Bladder and supplement the Kidney. If there is Stirred pulse, purge the Small Intestine and supplement the Heart. If the Renying is three times active, where Yangming pulse is active, purge the Stomach and supplement the Spleen. If there is Stirred pulse, purge the Large Intestine and supplement the Lung. If the Cunkou is once active where the Jueyin pulse is active, purge the Liver and supplement the Gallbladder. If there is Stirred pulse, purge the Pericardium and supplement the Triple Energizer. If the Cunkou is twice active where the Shaoyin pulse is active, purge the Kidney and supplement the Bladder. If there is stirred pulse, purge the Heart and supplement the Small Intestine. If the Cunkou is three times active where the Taiyin pulse is active, purge the Stomach and supplement the Spleen. If there is Stirred pulse, purge the Lung and supplement the Large Intestine.

보중익기탕(補中益氣湯)의 형상의학적 활용 (Application of Bojungikki-tang in Hyungsang Medicine)

  • The following conclusions are produced by the consideration of Bojungikki-tang(補中益氣湯) from the view point of Hyungsang medicine. A review on the explanation and provisions of Bojungikki-tang stipulated in 'Donguibogam(東醫寶鑑)' results in the idea that the main purpose of Dongwon(東垣) and various application in clinic by Jeesan(芝山) cannot be understood without the deep comprehension of ki(especially Jungki and Wonki) and the fire. Jungki(中氣, the middle warmer ki) is the ki of stomach. Wonki(元氣, the original ki) is a combination of innate Jung(先天之精, Ki stored between the two kidneys) and acquired Jung(後天之精, Ki of the stomach) and the sources of five Jang and six Bu, twelve meridians and triple warmer. Yin fire is referred to the ministerial fire of spleen, liver and kidney. Excess or lack of the ministerial fire can cause a disease and especially the excess of it is harmful to original ki. Deficiency of original ki is thought to be a cause of allergic diseases. Damages on the original ki by the abdominal surgery lead to a disease. Pale complexion, long face and the long limbs are considered as the Hyungsang of deficiency of Jung ki by Jeesan. Clinical cases of Bojungikki-tang in Hyungsang medicine draw the following conclusions ; Bojungikki-tang is usually prescribed to the person who has one of the following Hyungsang : Bangkwang type, pale complexion, long face, short-sight, astigmatism and wrinkles on face. Bojungikki-tang can be applied to the every disease caused by the deficiency of original ki regardless of name and symptoms of disease.

폐암 변증도구 개발을 위한 기초연구 (Preliminary Study to Develop the Instrument on Pattern Identification for Lung Cancer)

  • This study is to develop a standard tool for pattern identifications in Korean Medicine for Lung Cancer. The advisor committee for this study was organized by 12 Korean Medicine professors from the Korean Association of Traditional Oncology. The item and structure of instrument were based on review of published literature. We took the consultation twice from the advisor committee and additional advices by e-mail correspondences. We divided the symptoms and signs of lung cancer into 6 pattern identifications. - lung spleen both deficiency (肺脾兩虛), lung kidney both deficiency (肺腎兩虛), dampness phlegm obstructing the lung (濕痰蘊肺), qi stagnation blood stasis (氣滯血瘀), yin deficiency toxin heat (陰虛毒熱), and heat toxin accumulation (熱毒蘊結). We obtained the mean weights which reflected the standard deviations from each symptoms of the 6 pattern identifications which were scored on a 5-point scale by 12 experts. We designed the Korean medicine pattern identification tool for lung cancer. It was composed of 57 questions in the question-and-answer format. Though there are some limits that this study is not proved about validity and reliability, the instrument is meaningful and expected to be applied to the subsequent research.

Cholesteryl Ester Transfer Protein (CETP) Deficiency and CETP Inhibitors

  • Epidemiologic studies have shown that low-density lipoprotein cholesterol (LDL-C) is a strong risk factor, whilst high-density lipoprotein cholesterol (HDL-C) reduces the risk of coronary heart disease (CHD). Therefore, strategies to manage dyslipidemia in an effort to prevent or treat CHD have primarily attempted at decreasing LDL-C and raising HDL-C levels. Cholesteryl ester transfer protein (CETP) mediates the exchange of cholesteryl ester for triglycerides between HDL and VLDL and LDL. We have published the first report indicating that a group of Japanese patients who were lacking CETP had extremely high HDL-C levels, low LDL-C levels and a low incidence of CHD. Animal studies, as well as clinical and epidemiologic evidences, have suggested that inhibition of CETP provides an effective strategy to raise HDL-C and reduce LDL-C levels. Four CETP inhibitors have substantially increased HDL-C levels in dyslipidemic patients. This review will discuss the current status and future prospects of CETP inhibitors in the treatment of CHD. At present anacetrapib by Merck and evacetrapib by Eli Lilly are under development. By 100mg of anacetrapib HDL-C increased by 138%, and LDL-C decreased by 40%. Evacetrapib 500 mg also showed dramatic 132% increase of HDL-C, while LDL-C decreased by 40%. If larger, long-term, randomized, clinical end point trials could corroborate other findings in reducing atherosclerosis, CETP inhibitors could have a significant impact in the management of dyslipidemic CHD patients. Inhibition of CETP synthesis by antisense oligonucleotide or small molecules will produce more similar conditions to human CETP deficiency and may be effective in reducing atherosclerosis and cardiovascular events. We are expecting the final data of prospective clinical trials by CETP inhibitors in 2015.

소양증(瘙痒症)의 침구치료(鍼灸治療)에 관한 고찰(考察) (A Literature Review on the Itching in Acupuncture and Moxibustion Treatments)

  • Objective : The purpose of this study is to review the acupuncture therapy and moxibustion on itching Method : We searched and reviewed chinese journals of and books of the acupuncture therapy and moxibustion on itching. Results & Conclusions : 1. Dermatopathy of itching is described as eczema, neurotic-dermititis, itching in orient medical text. 2. According to text, itching causes were wind symptom produced by deficiency of blood, deficiency syndrom of the spleen, and damp-heat. 3. Frequently used acupoints were LI10(41 frequency), SP10(29), ST36(27), SP6(22), GB20(19), Asi point(18), BL(17), back Su acupoints(13), LI4(13), SP9(10), PC6(7), HT7(7). etc 4. The practice of acupuncture was used by body acupuncture, auriculo-theraphy, loss of bleeding with needle, blum-blosson needle, dermal needle, fire needle. etc. 5. Moxibustions are mild moxibustion, non-scar producing moxibustion, herb- moxibustion. etc.

  • Patients with neurogenic bladder undergo morphometric and functional changes of their bladder and urethra. As a result, voiding symptoms such as frequency, nocturia, urgency and incontinence appear. We experienced two cases of the urinary incontinence treated with Samboo-tang(Sanfu-tang). In the point of Differentiation of Syndrome, two cases were diagnosed as Deficiency Syndrome of Yang of the kidneys. After Samboo-tang(Sanfu-tang) administration, the frequency of the urinary incontinence were decreased and the accompanied symptoms also improved. So, we report two cases with a brief view of related literatures.

The Role of Genetic Diagnosis in Hemophilia A

  • Hemophilia A is a rare X-linked congenital deficiency of clotting factor VIII (FVIII) that is traditionally diagnosed by measuring FVIII activity. Various mutations of the FVIII gene have been reported and they influence on the FVIII protein structure. A deficiency of or reduction in FVIII protein manifests as spontaneous or induced bleeding depending on the disease severity. Mutations of the FVIII gene provide important information on the severity of disease and inhibitor development. FVIII mutations also affect the discrepant activities found using different FVIII assays. FVIII activity is affected differently depending on the mutation site. Long-range PCR is commonly used to detect intron 22 inversion, the most common mutation in severe hemophilia. However, point mutations are also common in patients with hemophilia, and direct Sanger sequencing and copy number variant analysis are being used to screen for full mutations in the FVIII gene. Advances in molecular genetic methods, such as next-generation sequencing, may enable accurate analysis of mutations in the factor VIII gene, which may be useful in the diagnosis of mild to moderate hemophilia. Genetic analysis is also useful in diagnosing carriers and managing bleeding control. This review discusses the current knowledge about mutations in hemophilia and focuses on the clinical aspects associated with these mutations and the importance of genetic analysis.

심병변증(心病辨證)의 형성과정(形成過程)에 대한 문헌적(文獻的) 고찰(考察) (Bibliographical study on formation process of the differentiation of syndrome of heart-disease)

  • 오장육부중(五臟六腑中)에서 심(心)은 인체(人體) 생리활동(生理活動)의 주재(主宰)으로서 장부(臟腑) 가운데에서도 수위(首位)를 차지하여 인체(人體)의 사추활동(思推活動)이나 장부기능(臟腑機能)의 협조(協調) 및 기혈(氣血)의 통창(通暢)등도 모두 심(心)의 기능(機能)에 의존(依存)하는 바이므로 심(心)을 생명활동(生命活動)의 중심(中心)이라고 한다. 본(本) 논문(論文)은 변증체계(辨證體系)의 형성과정(形成過程)을 심병변증(心病辨證)에 한(限)하여 문헌적(文獻的)으로 고찰(考察)한 것으로써, 첫째 심병(心病)의 허증분류(虛症分類)에 있어 심허증(心虛症)이라고 포괄적(包括的)으로 언급(言及)되어지던 것이 심음허증(心陰虛症)과 심양허증(心陽虛症)으로 분류(分類)되었으며, 다시 심기허증(心氣虛症) 심양허증(心陽虛症) 심혈허증(心血虛症) 심음허증(心陰虛症)으로 분류(分類)되었다가, 최근(最近)에 변증분류(辨證分類)에서는 이를 더욱 세분화(細分化)시켜 심기허증(心氣虛症) 심양허증(心陽虛症) 심혈허증(心血虛症) 심음허증(心陰虛症) 심기음양허증(心氣陰兩虛症) 심기혈양허증(心氣血兩虛症) 심음양양허증(心陰陽兩虛症) 심양포탈증(心陽暴脫證)으로 분류(分類) 발전(發展)시키고 있다. 둘째 심병(心病)의 실증분류(實證分類)에 있어 가장 중요한 것은 담(痰)과 화(火) 열(熱)의 문제(問題)였으며 이것들을 가지고 다양한 변증분류(辨證分類)를 하였는데, 초기(初期)에는 담증(痰證)과 열증(熱證)을 단지 분리(分離)하여 변증(辨證)하였던 것을, 최근(最近)에는 담증(痰證)과 화증(火證)뿐만 아니라 담화(痰火)를 같이 묶어 변증(辨證) 하였으며, 심기허심양허(心氣虛心陽虛)에서 기인(起因)된 심어증(心瘀證)을 점차 중요(重要)하게 여기는 방향(方向)으로 변증분류(辨證分類)를 하였다. 이러한 변증분류(辨證分類)의 다양화(多樣化) 세분화(細分化)는 점점(漸漸) 다양(多樣)해지는 질병양상(疾病樣相)에 보다 잘 대처하려는 연구결과(硏究結果)로 보여지며 이후로도 보다 실증적(實證的)인 연구(硏究)가 더욱 더 요망(要望)된다.

  • Object : The purpose of this study is assistant to medical treatment for patient, who suffers from headache, by classifing etiologies of headache and investigating using meridian and acupuncture point.Method : By considering through the oriental literature, we investigated etiologies and frequency of using meridian and acupuncture point on headache.Result:1. The Oriental etiologies of headache is classified in 'wind(風)', 'hot and feverish(熱)', 'humidity(濕)', 'cold(寒)', 'defidiency of qi(氣處)', 'deficiency of blood(血虛)', 'extravasated blood(瘀血)', 'asthenia of kidney(賢處)', 'anger by depression(鬱怒)', 'Damhwa(痰火)'2. The frequently used meridians on headache are followings : the 1st is Choksoyang-Tam-Kyong(足少陽膽經), the 2nd Choktaeyang-Pabggwabg-Kyong(足太陽膀胱經), the 3rd Tok-maek(督脈), and the 4th Chokyangmyong Wi-Kyong(足陽明胃經).3. The frequently used acupuncture points on headache are followings : the 1st is paek'oe(百會), the 2nd Hapkok(合谷) and the 3rd P'ungji(風池).Conclusion:1. The books about treatment of headache by using acupuncture are The Yellow Emperor's Classic on internal Medicine(黃帝內經) and Gab-UI-Kyoung(甲乙經) and so on.2. In The Yellow Emperor's Classic on Internal Medicine(黃帝內經), they mainly used treatment by following the stream of meridian on headache.3. After Gab-U1-Kyoung(甲乙經), they suggested specialized acupunctre point.4. Three Yang meridians(三陽經) that has many acupuncture point located on head area, are related to medical treatment on headache.

활성화 반응으로 제작된 TiO2의 박막특성 (Film Properties of TiO2 Made by Activated Reactive Evaporation)

  • $TiO_2$ thin film has wide application because of its high capacitanca, reflection, and good transmissivity in visible range. $TiO_2$ thin film can be made by thermal deposition method, reactive evaporation method, activated reactive evaporation(ARE) method. In the case of thermal deposition, the oxygen deficiency can occur because the melting point of Ti is very high. While in the case of reactive evaporation, high density $TiO_2$ can not be made, because reactive gas($O_2$) and evaporated material(Ti) are not fully combined, activated reactive evaporation, $TiO_2$ is easily deposited at lower gas pressure compared with reactive evaporation because the ionized reactive gas is made by plasma. Therefore, activated reactive evaporation is very useful to deposit the material having the high melting point. In this work, we formed $TiO_2$ thin film by activated reactive evaporation method. The surface of $TiO_2$ thin film was analyzed by X-ray photoelectron spectroscopy. The surface morphology which was analyzed by atomic force microscopy(AFM) shows that feature of the film surface is uniform. The dielectric capacitance, withstanding voltage were $600{\mu}F/cm^2$, 0.4V respectively. In further work, we can increase the withstanding voltage by improving the deposition parameter of substrates.
