CONSIDERATION OF THE ANTERIOR OPENBITE AND DEEP BITE IN CLASS III AND THEIR TREATMENT WITH MULTILOOP EDGEWISE ARCHWIRE (MEAW) (골격형 III급 전치개교와 과개교합에 대한 비교고찰 및 MEAW에 의한 치험예)

  • The purpose of this report is to review vertical dysplasia such as openbite or deep bite in skeletal Class III malocclusion and their treatment modality and to present two cases treated with MEAW. The results obtained were as follows A. Open bite case 1. The treatment time was 3 year 8 months. 2. Upper and lower incisors showed extrusion and especially lower anterior alveolar process showed remodelling. 3. The mesially inclined upper and lower molars were uprighted and especially lower first molars showed extrusion that means remodelling of alveolar bone. 4. Normal overbite and overjet were established. 5. Mandible showed slight clockwise rotation. 6. Maxilla showed slight downward bending of ANS part. 7. Upper lip showed downward drop and lower lip showed retraction and touch between upper and lower lip was established. 8. Tongue posture of post-treatment was more raised than pretreatment. B. Deep bite case 1. The treatment time was 1 year 8 months. 2. Upper incisors showed intrusion and labioversion and lower incisors showed slight intrusion and linguoversion. 3. The lower molars showed distal uprighting and intrusion and upper molars showed mesial movement and extrusion. 4. Normal overbite and overjet were established. 5. Maxilla did not show downward movement. 6. Mandible showed slight clockwise rotation. 7. Lower lip showed retraction and downward drop and upper lip showed downward drop.

THE RELATIONSHIP OF MANDIBULAR CONDYLAR POSITION TO OVERBITE DEPTH (교합 피개 심도와 과두 위치)

  • This study was designed to evaluate the relationship of mandibular condylar position to overbite depth and compare the findings from transcranial radiographs and those, from individualized corrected tomographs in determination of condylar position. The subjects consisted of 20 control subjects (male 8, female 12), and 10 open-bite patients (male 3, female 7) and 23 deep-bite patients (male 17, female 6). The mean age was 23.3 years for the control group, 21.5 years for open-bite group, and 23.2 years for deep-bite group. Transcranial radiographys and individualized corrected tomographys in centric occlusion were taken from right and left temporomandibular joints of each sueject. The results were as follows. 1. In the 20 normal subjects showing no symptoms of TM disorder, the incidence of condylar retrusion was $27.5\%$, middle condylar position $60.0\%$, and anterior displacement $12.5\%$. 2. There was significant correlation between the bite depth and observed condylar position. 3. Only $45.2\%$ of the findings from transcranial radiographs coincided with those from individualized corrected tomographs in determining condylar position.

Orthodontic-prosthodontic interdisciplinary treatment for a patient with multiple missing posterior teeth and unilateral scissors bite (다수의 구치 상실과 편측성 가위교합을 갖는 환자의 보철 교정 협진 치료)

  • This clinical report describes an orthodontic-prosthodontic interdisciplinary treatment for a patient with multiple missing teeth and unilateral scissors bite. A 47-year-old female presented with multiple missing posterior teeth, anterior large overjet, deep bite, and posterior scissors bite on the right premolar area. Periodontal therapy was performed and followed by orthodontic treatment. The maxillary anterior teeth were initially aligned, then two implants were placed for the left mandibular molars to increase occlusal vertical dimension. The scissors bite between the right maxillary and mandibular premolars were corrected using the miniscrews as an anchorage. Other implants were placed for the right maxillary and mandibular molars after the occlusal planes and occlusal relationship were harmonized. The patient adapted well to altered vertical dimension without any specific problems including peri-implant marginal bone loss. Interdisciplinary approach resolve the complex orthodontic-prosthodontic problems and concluded in successful results.

  • Rett syndrome is a neurological disorder seen almost exclusively in females, and found in a variety of racial and ethnic groups worldwide. First described by Dr. Andreas Rett in 1983. The specific features of the Rett syndrome is apraxia. Most of the Rett syndrome has been diagnosed erroneously to autism, cerebral palsy, and unknown developmental disorders. The etiology of the Rett syndrome is not figured out exactly but it seem to have relation with genetic factors. In this case the patient with Rett syndrome had a chief complaint of the injury of palate due to deep bite. We report this case for the satisfactory result using the bite plane to decrease the injury of the palate due to deep bite.

  • Teeth wear and extrusion of antagonist are commonly observed in deep bite patient having severe vertical and horizontal overlap. These problems cause collapse of occlusal plane and abnormal anterior guidance. Without restoring harmonious occlusion, loss of multiple teeth and decreased masticatory function could not be prevented. To resolve problems associated with deep bite, multidisciplinary treatment including oral surgical, orthodontic and prosthetic treatment should be performed. This clinical report describes the results of increasing occlusal vertical dimension with a full-mouth restoration procedure. The treatment procedures include extraoral and intraoral examination, diagnosis, treatment planning, diagnostic wax-up, segmental osteotomy, orthodontic intrusion and prosthodontic rehabilitation. Full mouth rehabilitation with increasing occlusal vertical dimension can solve esthetic and functional problems.

  • Although not all kinds of deep bite occlusions are considered pathological states, they can cause persistent wear and eruption of the anterior mandibular teeth if the anterior occlusal contact is unstable or accompanied by an occlusal disharmony. The patient was a 72-year-old man with no occlusal contact of the remaining anterior teeth in the centric relation position because of a skeletal class II malocclusion and loss of occlusal support. If the vertical dimension needs to be increased for prosthetic restoration in a patient with a deep bite, the amount of vertical dimension increase must be determined through precise diagnosis. This is because the overjet may increase and cause an unstable occlusion. In this review, we report the case of a patient with a stable occlusion and good treatment results achieved by using a removable partial denture prosthesis and applying the Dahl concept.

  • The loss of posterior support and the abnormal jaw relation can cause pathologic findings. If deep bite patients with multiple missing teeth, can not have the stable posterior contact, the mandible moves posteriorly, and consequently the overjet and overbite get worse. And when the mandibular irregular occlusal plane is corrected, it is easier to have the bilateral balanced occlusion with the maxilla. So the treatment goal is to give proper posetrior support and establish appropriate anterior guidance, and ultimately provide improved mastication and esthetics recovery. In this case, a 68 year old man, having deep bite without posterior support was evaluated by the vertical dimesion decision flow-chart. An available prosthetic height, anterior occlusal relation such as overjet, overbite and the esthetic part such as facial height and the cephalometric analysis are the factors to be considered.

Esthetic improvement in patient with deep bite using porcelain laminate veneers and a full veneer crown (과개교합 환자에서 도재 라미네이트 비니어와 전부도재관을 이용한 심미수복 증례)

  • This case is a porcelain laminate veneer (PLV) and all-ceramic crown treatment case on a female patient in her thirties who visited for esthetic improvement. The patient's chief complaint was protrusive and uneven anterior teeth, and she wanted minimum tooth reduction. The patient was in skeletal class II deep bite. Considering this, check bite method was used for mounting on articulator during diagnostic wax-up. Tooth reduction was done using mock-up base on diagnostic wax-up, and the amount of deletion was checked by index. For the accordance of shade, PLVs were restored in prior to all ceramic crown. After using all-ceramic crown, satisfactory esthetic improvement was obtained.

  • Deep overbite patients who do not have proper occlusal relationship may cause problems such as teeth wear and antagonist extrusion. These lead to the collapse of occlusal plane and esthetic problem. Increasing vertical dimension is frequently essential to resolve those problems. This case report demonstrates a full-mouth rehabilitation for a patient with severe deep bite that contacts surface to surface by increasing vertical dimension. Treatment procedures included diagnosis, treatment planning, implant surgery, and prosthodontic rehabilitation. Satisfactory results were obtained in functional and esthetic aspects.
