Clinical and Mycological Studies on Tinea cruris (완선(頑癬)의 임상(臨床) 및 균학적(菌學的) 연구(硏究))

  • The authors performed clinical and mycological studies on Tinea cruris of 842 outpatients who had visited the dermatology clinic of Seoul National University Hospital from 1975 to 1979. 1) The incidence of Tinea cruris was 4.4% in maximum and 3.0% in minimum of the total superficial mycotic diseases cases, so there are no remarkable changes of the yearly prevalence rate. 2) Tinea cruris was more frequent among males, and the male cases were 15.2 times higher than those of female cases. 3) By the monthly distribution, Tinea cruris shows most high in summer season (June, July and August). 4) The age distribution group for Tinea cruris varied from the first to the eighth decade, but most of them were in their third decade. 5) Concurrent infection of Tinea cruris with other types of dermatophytoses(Tinea) was noted in 19.0% of the cases. Patients with Tinea cruris and Tinea pedis were most highly observed in 51.3%, and on next. Tinea cruris and Tinea corporis 25.6% in all of the concurrent infection cases. 6) The strains were identified by culture on ordinary Sabouraud's glucose agar media and abtained three species and 99 strains. a) Trichophyton rubrum was most common causative organism of Tinea cruris. Trichophyton rubrum was isolated 64 strains (64.6%). b) Trichophyton mentagrophytes was isolated 27 stains (27.3%) and Epidermophyton floccosum was 8 strains (8.1%).

Conduits for Coronary Bypass: Internal Thoracic Artery

  • This second report in the series on coronary artery bypass presents the authors experience and personal views on the internal thoracic artery (ITA) which date to 1966. There has been a very gradual evolution in the acceptance of this conduit which was initially compared with the saphenous vein and viewed as an improbable alternative to it. As is common with concepts and techniques which are 'outside the box' there was skepticism and criticism of this new conduit which was more difficult and time consuming to harvest for the surgeon who had to do it all. It was viewed as small, fragile, spastic and its flow capacity was questioned. Only a few surgeons employed it because of these issues and some of them would frequently graft it to the diagonal artery as it was thought not to supply adequate flow for the left anterior descending unless it was small. After a decade, angiographic data revealed superior patency to vein grafts. Even this evidence and survival benefit reported a few years later did not convince many surgeons that their concerns about limitations justified its use. Thus widespread adaption of the ITA as the conduit of choice for the anterior descending required another decade and bilateral use is only now expanding to more than 5% of patients in the US and somewhat faster in other countries.

A Fingerprint of Global Warming Appeared in Winter Precipitation across South Korea (우리나라 겨울철 강수에 나타난 지구온난화의 징후)

  • In this study, changes in precipitation across South Korea during snow seasons (November-April) and their potential are examined. Current (1973/74-2006/07) and future (2081-2100) time series of snow indices including snow season, snow-to-precipitation ratio, and snow impossible day are extracted from observed snow and precipitation data for 61 weather stations as well as observed and modeled daily temperature data. Analyses of linear trends reveal that snow seasons have shortened by 3-13 days/decade; that the snow-to-precipitation ratio (the percentage of snow days relative to precipitation days) has decreased by 4-8 %/decade. These changes are associated with pronounced formations of a positive pressure anomaly core over East Asia during the positive Arctic Oscillation winter years since the late 1980s. A snow-temperature statistical model demonstrates that the warming due to the positive core winter intensifies changes from snow to rain at the rate of $4.7cm/^{\circ}C$. The high pressure anomaly pattern has also contributed to decreases of air-sea thermal gradient which are associated with the reduction of snow could formation. Modeled data predict that a fingerprint of wintertime global warming causing changes from snow to rain will continue to be observed over the 21st century.

  • Recently, graphene has attracted considerable attention owing to its unique electrical, optical, thermal, and mechanical properties. The broad spectrum of applications from optics, sensors, and electronics to biodevice have been proposed based on these properties. In particular, graphene has been proposed as a protective coating layer and solid lubricant for microdevices and nanodevices because of its high mechanical strength, chemical inertness, and low friction characteristics. During the past decade, extensive efforts have been made to explore the tribological characteristics of graphene under various conditions and to expand its applicability. In addition to the experimental approaches, the molecular simulations performed provide fundamental insights into the friction and wear characteristics of graphene resulting from molecular interactions. This work is a review of the studies conducted over the past decade on the tribological characteristics of graphene using molecular simulation. These studies demonstrate the principal mechanisms of the superlubricity of graphene and help clarify the influences of surface conditions on tribological behavior. In particular, the investigation of the effects of the number of layers, strength of adhesion to the substrate, surface roughness, and commensurability provides deeper insights into the tribological characteristics of graphene. These fundamental understandings can help elucidate the feasibility of graphene as a protective coating layer and solid lubricant for microdevices and nanodevices.

The relationship between malocclusion and menarcheal age, and its secular trend for Korean women

  • Objective: This study aimed to evaluate the mean age of menarche, its secular trend in Korean women, and the relationship between malocclusion and the rate of skeletal maturation, as defined by menarcheal age. Methods: We retrospectively collected data on menarcheal age from 931 Korean women born during 1961 - 1997. Subjects were divided by the malocclusion type and birth-year decade into 3 and 4 groups, respectively. The mean menarcheal age for each group was determined, and one-way ANOVA was performed for intergroup comparison (p = 0.05). Two-way ANOVA was also performed to compare all the 12 subgroups (p = 0.05). Results: The mean age of menarche was 12.82 years for Korean women born during 1961 - 1997. A distinct downward secular trend of menarcheal age was noticed (p < 0.05). For the birth-year decade 1961 - 1970, the Class III malocclusion group showed earlier onset of menarche than the other malocclusion groups (p < 0.05), but the other birth-year groups did not show any significant difference in the type of malocclusion (p > 0.05). Conclusions: A positive secular trend towards earlier menarche exists among Korean women. Malocclusion does not show any significant relationship with the rate of skeletal maturation, as defined by menarcheal age.

CLINICAL STUDY OF CYST IN THE JAWS (악골의 낭종에 관한 임상적 연구)

  • The purpose of this study is to find the histopathological pattern of cysts. We reviewed the hospital chart, out-patient chart, roentgenogram, histopathologic report and operation report of 152 patients who had been diagnosed as cyst and treated at the department of Oral and Maxillofacial Surgery of Korea university hospital between Jan. 1, 1995 and Dec. 31, 1998. And then we studied clinically with regard to pathological classification, age and sex distribution, anatomical distribution and so on. The results were as follows : 1. In pathologic classification, radicular cyst (97cases, 64%), dentigerous cyst (35cases, 23%), odontogenic keratocyst (8cases, 5.3%) were dominant among cases of cyst. 2. The pattern of age distribution in cases of radicular cyst, dentigerous cyst and odontogenic keratocyst was similar to that found in previous studies. The peak incidence was in the second decade (27%) and third decade (29%) in overall cases. 3. The male-to-female ratio was 1.9 : 1. 4. Radicular cyst occured most frequently in the maxillary incisor teeth area, dentigerous cyst in mandibular wisdom teeth area, and odontogenic keratocyst in mandibular molar area.

A Study of Mandibular Foramen and Mandibular Canal using Orthopantomograms. (Orthopantomogram을 이용한 하악공 및 하악관에 관한 연구)

  • The mandibular canal must be considered carefully during surgical treatment, especially surgical extraction of the impacted tooth and intraosseous implant because it contains the important inferior alveolar nerve and vessels. The author investigated the curvatUre of the mandibular canal, the positional frequency of mandibular foramen to the occlusal plane and gonial angle and the positional frequency of the mental foramen to the tooth site using orthopantomograms. The materials consisted of 295 orthopantomograms divided into seven groups ranging from the first decade to 6th. decade. The results were as follows: 1. The position of mandibular foramen was most frequently below occlusal plane in Group Ⅰ (78.6%) and Group Ⅱ (71.2%), above occlusal plane in Group Ⅲ (63.0%), Group IV (71.1%), Group V (57.6%), Group (76.7%) and Group VII (70.0%). 2. The curvature of mandibular canal was 142.8° in Group Ⅰ, 142.09° in Group Ⅱ, 139.34° in Group Ⅲ, 141.48° in Group Ⅳ, 138.45° in Group Ⅴ, 140.77° in Group Ⅵ and 143.89° in Group Ⅶ. 3. The gonial angie was 125.82° in Group Ⅰ, 123.18° in Group Ⅱ, 124.06° in Group Ⅲ, 120.45° in Group Ⅳ, 121.12° in Group Ⅴ, 121.63° in Group Ⅵ and 121.24° in Group Ⅶ. 4. The position of the menta] foramen was most frequently below the apex of mandibular first premolar in Group Ⅰ (57.2%), between the apex of mandibular first and second premolar in Group Ⅱ (59.6%) and Group Ⅲ (48.9%), and below the apex of mandibular second premolar in Group Ⅳ (39.2%), Group Ⅴ (48.5%) Group Ⅵ(46.6%) and Group Ⅶ(56.4%)

Birth of a healthy baby after preimplantation genetic diagnosis in a carrier of mucopolysaccharidosis type II: The first case in Korea

  • Mucopolysaccharidosis type II (MPS II) is a rare X-linked recessive lysosomal storage disease caused by mutation of the iduronate-2-sulfatase gene. The mutation results in iduronate-2-sulfatase deficiency, which causes the progressive accumulation of heparan sulfate and dermatan sulfate in cellular lysosomes. The phenotype, age of onset, and symptoms of MPS II vary; accordingly, the disease can be classified into either the early-onset type or the late-onset type, depending on the age of onset and the severity of the symptoms. In patients with severe MPS II, symptoms typically first appear between 2 and 5 years of age. Patients with severe MPS II usually die in the second decade of life although some patients with less severe disease have survived into their fifth or sixth decade. Here, we report the establishment of a preimplantation genetic diagnosis (PGD) strategy using multiplex nested polymerase chain reaction, direct sequencing, and linkage analysis. Unaffected embryos were selected via the diagnosis of a single blastomere, and a healthy boy was delivered by a female carrier of MPS II. This is the first successful application of PGD in a patient with MPS II in Korea.

The bone density of mandible as the aging process in Koreans (한국인 연령에 따른 하악 치조골 골밀도)

  • Introduction: This study compared the alveolar bone density of the mandible according to gender, age and position using Cone-beam computed tomography (CT). Materials and Methods: The maxillofacial CT scan data was obtained from 60 Korean patients. In addition, the alveloar bone density of 5 males and 5 females with normal occlusion aged from 10 to 70 years was measured at the buccal cortical bone, cancellous bone and lingual cortical bone, as well as at the position of the incisors, canines, premolars and molars. Results: The age-specific mean bone density was highest in patients in their third decade. The buccal cortical bone of the molars showed the highest bone density. Males in their fifties and sixties had a higher bone density in the cancellous bone in the region of the premolars and the buccal cortical bone of the molars, respectively, than females but there was no significant difference between males and females in the other parts. The cancellous bone density was highest in those in their twenties and thirties, and tended to decline up to their seventh decade. Conclusion: These results revealed a significantly different bone density according to gender, age and position in the Korean population. In addition, it is possible to predict the bone density based on these results.

Increased Tuberculosis Burden Due to Demographic Transition in Korea from 2001 to 2010

  • Background: Notified tuberculosis (TB) cases in Korea have not decreased over the last decade (2001-2010). Methods: To clarify the reasons, we analyzed an annual report on notified tuberculosis patients and age-specific population drift in Korea. Results: Compared to the age-specific notified TB cases between 2001 and 2010, distinctive features in notified TB cases and new cases increased markedly in people aged 45-54 years and in patients over 65 years old, whereas those between 15-34 years in 2010 decreased drastically. In particular, notified TB individuals over 65 years old occupied 29.6% of the cases in 2010, which was 1.5 times higher than that in 2001. The main reason not to decrease in notified TB patients for the last decade (2001-2010) was due to the increasing elderly population as well as the aging of baby boomers, which have a higher risk of TB development. Conclusion: Korea needs to pay attention to the older population in order to successfully decrease the burden of TB in the future.
