난소예비력 저하 여성에 대한 한약 단독 치료의 임신 관련 지표 개선 효과에 관한 체계적 문헌 고찰 및 메타분석 (East Asian Herbal Medicine (EAHM) Alone for the Treatment of Women with Diminished Ovarian Reserve (DOR): A Systematic Review and Meta-analysis)

  • Objectives: The aim of this study was to find out whether east asian herbal medicine (EAHM) treatment alone is effective in improving pregnancy-related indicators compared to conventional medicine in women with Diminished Ovarian Reserve (DOR). Methods: We searched eligible studies from PubMed, Cochrane Library, EMBASE, Chinese National Knowledge Infrastructure Database, CiNii, Korean Medical Database, Korean Studies Information Service System, Oriental Medicine Advanced Searching Integrated System, ScienceOn. GRADE pro was used to evaluate the current evidence of the study. Result: A total of 5 studies, 325 women with DOR were included. EAHM showed a significant effect on improvement of pregnancy rate (n=270, RR 2.13 [95% CI 1.44 to 3.15], Z=3.78, p=0.0002, I2=0%) and Anti-Mullerian Hormone (AMH) level (n=211, SMD 0.82 [95% CI 0.40 to 1.25], Z=3.80, p=0.0001) compared to conventional medicine. In ovulation rate (n=156, RR 0.86 [95% CI 0.70 to 1.06], Z=1.43, p=0.15, I2=0%), Antral Follicle Count (n=245, SMD 0.27 [95% CI -0.25 to 0.79], Z=1.01, p=0.31), and follicle stimulating hormone (n=245, SMD 0.29 [95% CI -0.13 to 0.70], Z=1.36, p=0.17) level, EAHM showed similar effects to conventional medicine. In this study, the most frequently used herbal medicines were Cuscutae Semen, Dipsaci Radix, and Angelicae Gigantis Radix. Conclusion: This meta-analysis showed that EAHM could improve pregnancy rates and AMH levels in women with DOR. However, more well-designed RCTs will have to be performed further in the future.

  • Objectives The purpose of this study is to investigate the latest research trends regarding the traditional Chinese medicine treatment of plantar fasciitis and suggest the direction for future research. Methods We investigated recent clinical studies about traditional Chines medicine treatment for Plantar fasciitis through searching the electronic database of China National Knowledge Infrastructure. We analyzed the characteristics of selected studies according to study design, criteria for diagnosis and evaluation, periods, treatment methods. Results Twenty-six clinical studies published from 2018 to 2022 were analyzed. Acupuncture, herbal medication, herbal fumigation and washing, herbal integration, herbal external application, and manual therapy were performed for the treatment of plantar fasciitis in China. In particular, studies on acupuncture, herbal fumigation and washing, and manual therapy were actively conducted and all of these treatment methods were effective. Conclusions Various Chinese medical interventions are being studied in China for the treatment of plantar fasciitis. However, in order to reinforce evidence for effectiveness of treatment, additional high-level clinical studies are required. It is considered that additional research related to the Korean medicine treatment for plantar fasciitis is needed in the future in Korea.

최적화된 확률 모델을 이용한 다양한 품질의 지문분류 (Various Quality Fingerprint Classification Using the Optimal Stochastic Models)

  • 지문분류는 1:N 지문인식 시스템의 효율성을 높이는 단계로 지문의 매칭 시간 단축과 인식의 정확성을 높여주는 역할을 한다. 지문 각 클래스의 융선 패턴은 한 개 이상의 클래스와 중복되는 성질을 가지기 때문에 지문분류 작업은 어렵다. 또한 잡음을 많이 포함하거나 예외적인 입력 상태인 경우에도 분류 작업은 어려워진다. 본 논문에서는 다양한 품질의 지문을 효과적으로 분류하기 위해 지문의 방향특징을 이용해 확률 모델을 설계하고, 이를 최적화 하여 지문분류를 수행하는 방법을 제안하였다. 지문 융선을 픽셀단위로 탐색하여 방향 값을 산출하고, 산출된 방향 값을 일정 픽셀 단위로 병합하여 지문의 방향특징을 추출한다. 추출된 방향 특징을 이용해 확률론적 정보추출 및 인식 방식인 마코프 모델을 이용하여 지문의 클래스별 마코프 모델을 생성한다. 생성된 클래스별 마코프 모델의 상태전이 행렬을 분석하여 클래스별 분류 모델의 가중치 항목을 결정하고 유전자 알고리즘을 이용하여 지문분류 성능을 향상시킬 수 있는 최적의 수치를 찾아낸다. 유전알고리즘에 의해 최적화된 분류모델에 다양한 품질의 지문 데이터베이스를 적용하여 실험해 본 결과 최적화 되기 전의 분류 모델에 비해 우수한 분류성능을 보였다. 또한 실험에 사용한 다양한 품질의 데이터베이스를 분석해본 결과 제안한 방법은 특이점 유, 무 및 상태에 독립적으로 예외적인 입력상황의 지문에 대해 효율적으로 지분분류를 수행했다.

GB-색인: 고차원 데이타의 복합 유사 질의 및 적합성 피드백을 위한 색인 기법 (GB-Index: An Indexing Method for High Dimensional Complex Similarity Queries with Relevance Feedback)

  • 멀티미디어 데이타베이스와 같은 고차원 응용에서 유사 색인과 검색은 어려운 문제이며, 특히, 다수의 특성을 함께 색인하는 경우에는 더욱 어렵다. 본 논문에서는 고차원 이미지 데이타베이스에서 복합 유사 질의 및 적합성 피드백을 효율적으로 처리하기 위한 새로운 색인 기법인 GB-색인을 제시한다. GB-색인은 각 특성 차원을 독립적으로 처리함으로써 다수의 특성과 다수의 질의 객체를 유연하게 제어한다. 아울러, 비트맵 색인을 통해 데이타베이스에 있는 모든 객체를 비트맵의 집합으로 표현하여 질의를 효율적으로 처리한다. GB-색인의 기술적인 주된 공헌은 다음과 같다: (1) 고차원 데이타를 위한 효율적인 색인, (2) 효율적인 복합 유사 질의 처리, (3) 적합성 피드백을 위한 분리형 질의의 효과적 처리. 실험 결과에 따르면 GB-색인은 순차 탐색 및 VA-파일에 비해 큰 성능 향상을 보였다.

지역개발사업의 효율적 관리 및 활용을 위한 관리체계 구축에 관한 연구 (A Study on Construction of Management System for Efficient Management and Application of Regional Development Project)

  • 본 연구는 '지역 개발 및 지원에 관한 법률' 시행에 맞춰 이전보다 강화된 지역개발사업 관리방안을 모색하는데 연구의 목적을 갖고 있다. 이를 위해 그동안 지적되어 온 지역개발사업 관리의식의 부족, 통합관리체계의 부재, 사업관리 정보의 신뢰성 부족 등을 개선하고자 다음과 같은 개선방안을 제시하였다. 첫째, 지역개발사업의 관리는 관계법령에 기초한 사업평가관리 체계로 운영할 필요가 있다. 둘째, 지역개발사업의 다양한 정보를 DB화하여 중앙정부 및 지방자치단체에서 활용 가능한 정보시스템을 구축하여야 한다. 셋째, 지역개발사업 관리의 효율성과 전문성을 제고하기 위해 전문위탁 기관 등을 활용한 사업관리 체계마련이 필요하다. 마지막으로 담당 공무원의 전문성 및 책임성을 제고하기 위한 업무환경 개선이 필요하다.

Association between Ras association domain family 1A Promoter Methylation and Esophageal Squamous Cell Carcinoma: a Meta-analysis

  • RASSF1A has been reported to be a candidate tumor suppressor in esophageal squamous cell carcinoma (ESCC). However, the association between RASSF1A promoter methylation and ESCC remains unclear. Eligible studies were identified through searching PubMed, Medline, Web of Science, and the China National Knowledge Infrastucture database. Studies were pooled and odds ratios (ORs) with corresponding confidence intervals (CIs) were calculated. Funnel plots were also performed to evaluate publication bias. Twelve studies involving 859 cases and 675 controls were included in this meta-analysis. A significant association was observed between RASSF1A methylation and ESCC overall (OR = 11.7, 95% CI: 6.59-20.9, z=8.36, P<0.00001). Subgroup analysis showed that the OR for heterogeneous tissues was 5.35 (95% CI = 2.95-9.71) while for autologous tissues it was 16.0 (8.31-30.96). For patient sample size, the OR for the <50 subgroup was 9.92 (95% CI = 2.88-34.2) and for the 50 case group was 13.1 (95% CI = 6.59-25.91). The OR for a relationship between RASSF1A methylation and TNM stages was 0.27 (95% CI=0.10-0.77), whereas there were no significant differences in RASSF1A methylation in relation to gender and differentiation among ESCC cases. This meta-analysis suggests a significant association between RASSF1A methylation and ESCC.

다성음원 기반 QbSH 시스템을 위한 매칭엔진의 설계 및 구현 (Design and Implementation of Matching Engine for QbSH System Based on Polyphonic Music)

  • 본 논문은 다성음원에서 추출된 특성정보 기반 QbSH (Query-by-Singing/ Humming) 시스템의 매칭엔진에 대해 제안하였다. 다성음원 기반 QbSH 시스템은 사람의 노래나 허밍에서 추출된 특성정보와 MP3 파일과 같은 다성음원에서 추출된 특성정보를 비교하여, 가장 유사한 음원을 검색하는 시스템이다. 제안된 매칭엔진에는 다성음원에서 특성 추출시 발생하는 오류를 줄이고, 매칭성능을 향상시키기 위해 크로마-스케일 표현기법 (Chroma-Scale Representation), 보상기법 (Compensation) 및 비대칭적 DTW (Asymmetric Dynamic Time Warping) 알고리즘을 적용하였다. 또한 다양한 거리 함수 (Distance Metric)를 적용하여 매칭엔진의 성능향상을 확인하였다. 1,000개의 허밍 질의와 450곡의 다성음원 데이터베이스를 기반으로 제안한 QbSH 시스템의 성능 실험을 수행하다. 성능 평가를 통해 제안한 QbSH 시스템이 MRR (Mean Reciprocal Rank) 0.718의 정확도를 가지는 것으로 확인되었다.

암 환자 식욕부진 증상 평가 도구의 활용 및 특성에 대한 분석 (The Analysis of usage and characteristic of Cancer-Related Anorexia Symptom Assessment Tool)

  • Objectives : Anorexia is the primary symptom impinging cancer patients' Quality of Life. It is usually accompanied by gastrointestinal symptoms(GI symptoms). Thus, to measure anorexia symptom precisely, assessing anorexia and GI symptoms together is recommended. This study was designed to analyze cancer-related anorexia assessment tools, extract GI symptoms included in these tools and investigate usefulness of instruments in clinical trials. Methods : Instruments were selected by searching PubMed, PROQOLID database. We analyzed instruments by number of items, assessment method, type of question, GI symptoms. Results : 9 instruments were selected to assess cancer-related anorexia symptom. Most tools adopt Likert scale as response scale and 'during past week' as recall period. Assessment method of all 9 instruments is the self-administration. Questions measuring anorexia are able to be sorted into 3 forms (frequency, severeness, distress of anorexia symptom). Among the GI symptoms, nausea is included in all 9 instruments. In clinical trials of cancer-related anorexia, Edmonton Symptom Assessment Scale(ESAS) and Functional Assessment of Anorexia/Cachexia Therapy Questionnaire(FAACT) were selected as endpoint measure. Conclusions : The result showed that FAACT is the only specialized tool to assess cancer-related anorexia. To measure cancer-related anorexia precisely, the need to develop new instrument exists.

High Affinity Pharmacological Profiling of Dual Inhibitors Targeting RET and VEGFR2 in Inhibition of Kinase and Angiogeneis Events in Medullary Thyroid Carcinoma

  • Clinical evidence shows that dual inhibition of kinases as well angiogenesis provides ideal therapeutic option in the treatment of medullary thyroid carcinoma (MTC) than inhibiting either of these with the events separately. Although treatment with dual inhibitors has shown good clinical responses in patients with MTC, it has been associated with serious side effects. Some inhibitors are active agents for both angiogenesis or kinase activity. Owing to narrow therapeutic window of established inhibitors, the present study aims to identify high affinity dual inhibitors targeting RET and VEGFR2 respectively for kinase and angiogenesis activity. Established inhibitors like Vandetanib, Cabozantinib, Motesanib, PP121, RAF265 and Sunitinib served as query parent compounds for identification of structurally similar compounds by Tanimoto-based similarity searching with a threshold of 95% against the PubChem database. All the parent inhibitors and respective similar compounds were docked against RET and VEGFR2 in order to retrieve high affinity compounds with these two proteins. AGN-PC-0CUK9P PubCID: 59320403 a compound related to PPI21 showed almost equal affinity for RET and VEGFR2 and unlike other screened compounds with no apparent bias for either of the receptors. Further, AGNPC- 0CUK9P demonstrated appreciable interaction with both RET and VEGFR2 and superior kinase activity in addition to showed optimal ADMET properties and pharmacophore features. From our in silico investigation we suggest AGN-PC-0CUK9P as a superior dual inhibitor targeting RET and VEGFR2 with high efficacy which should be proposed for pharmacodynamic and pharmacokinetic studies for improved treatment of MTC.

Expression Analyses Revealed Thymic Stromal Co-Transporter/Slc46A2 Is in Stem Cell Populations and Is a Putative Tumor Suppressor

  • By combining conventional single cell analysis with flow cytometry and public database searches with bioinformatics tools, we extended the expression profiling of thymic stromal cotransporter (TSCOT), Slc46A2/Ly110, that was shown to be expressed in bipotent precursor and cortical thymic epithelial cells. Genome scale analysis verified TSCOT expression in thymic tissue- and cell type- specific fashion and is also expressed in some other epithelial tissues including skin and lung. Coexpression profiling with genes, Foxn1 and Hoxa3, revealed the role of TSCOT during the organogenesis. TSCOT expression was detected in all thymic epithelial cells (TECs), but not in the $CD31^+$endothelial cell lineage in fetal thymus. In addition, ABC transporter-dependent side population and Sca-$1^+$ fetal TEC populations both contain TSCOT-expressing cells, indicating TEC stem cells express TSCOT. TSCOT expression was identified as early as in differentiating embryonic stem cells. TSCOT expression is not under the control of Foxn1 since TSCOT is present in the thymic rudiment of nude mice. By searching variations in the expression levels, TSCOT is positively associated with Grhl3 and Irf6. Cytokines such as IL1b, IL22 and IL24 are the potential regulators of the TSCOT expression. Surprisingly, we found TSCOT expression in the lung is diminished in lung cancers, suggesting TSCOT may be involved in the suppression of lung tumor development. Based on these results, a model for TEC differentiation from the stem cells was proposed in context of multiple epithelial organ formation.
