Fine Needle Aspiration Cytopathology of Pediatric Lympha denopathy among Sudanese Children

  • Background: Pediatric lymphadenopathy is a challenging medical situation for the child patient, the parents, and the physician. Although the bulk of masses will be benign the fear of malignancy is omnipresent. Therefore, the objective of this study was to identify the common cytopathological patterns of lymphadenopathy among Sudanese children. Methods: One hundred pediatric patients presenting with peripheral lymphadenopathy were included in the study, their ages ranging from 2 to 14 years, with a mean age of 7 years. Demographic characteristics, clinical manifestations and FNA materials were prospectively obtained. Results: FNA was performed in 100 cases (100%). There were no technical complications. All cases confirmed adequacy of specimen. Overall, FNA demonstrated 90 (90%) benign lesions and 10 (10%) malignant diagnosis. The benign lesions were reactive lymphoid hyperplasia (n=64), followed by benign granulomatous disease (n=26). Of the 10 cases diagnosed with malignancy, 7 (7%) were cases of non-Hodgkin`s lymphoma and the remaining 3 (3%) were Hodgkin's lymphomas. Conclusion: Pediatric lymphadenopathy is common in Sudan. CLA is the common frequent site. Lymphoma represents a major challenge in this setting.

  • Grapevine leafroll-associated 3 virus (GLRaV-3) and Grapevine fanleaf virus (GFLV) are important viral diseases of grapevine in the world. In this study, the most reliable woody indicator plants were selected for virus indexing. Two grapevines, LN33 (Couderc 1613x vitis berlandieri) and Vitis riparia Gloire, were selected for CLRaV-3 and CFLV graft indexing, respectively. The specific characteristics of Closterovirus isolated from grapevines cultivated in Korea were identified. filamentous virus-like particles only existed in the phloem parenchyma cell. In particular, the vesiculation of mitochondria was observed. This mitochondrial vesicu-lation was considered to be one of the most reliable cytopathic features of Closterovirus. During observation of GFLV-infected Chenopodium quinoa sections, virus-like particles arranged consistently were found forming several layers in cytoplasm. Moreover, virus-like particles in tubules were observed and were associated with plasmodesmata in cytoplasm. This is the first report on cytopathological characteristics of Closterovirus and Nepovirus identified from grapevines in Korea.

  • Historically, the screening of patients for signs of oral cancer and precancerous lesions has relied upon the conventional oral examination. A variety of commercial diagnostic aids and adjunctive techniques are developed to potentially assist in the screening of healthy patients for evidence of occult cancerous change. This paper is reviewing the literature associated with current oral cancer screening aids such as spectroscopy, chemoiluminescence, exfoliative cytopathology, vital staining and saliva as a diagnostic tool. Despite the increased public awareness of oral cancer, no technique or technology to date has provided definitive evidence to suggest that it improves the sensitivity or specificity of oral cancer screening beyond clinical oral examination alone.

Graft Transmission and Cytopathology of Pear Black Necrotic Leaf Spot (PBNLS) Disease

  • Graft transmission and cytopathological studies of a severe pear disease, pear black necrotic leafspot(PBNLS), were carried out to determine the causal agent of the disease. No evidence was found that a fungal or bacterial pathogen could be the causal agent of the disease. Attempts to transmit the agent by sap-inoculation to other plants including herbaceous hosts failed. How-ever, the pathogen was readily graft-transmitted from symptomatic diseased pears to healthy pears. Graft transmission of the pathogen was also demonstrated by using an indicator plant, PS-95, developed in the laboratory through various grafting methods. Ultrastructural study of the disease revealed the consistent presence of flexuous rod-shaped virus-like particles (VLP) in the symptomatic leaves of both Niitaka cultivar and indicator pear, PS-95. The particles, approximately 12 nm in diameter with undetermined length, occurred in the cytoplasm of mesophyll parenchyma cells. Cells with VLPs also contained fibril-containing vesicles, which are common in cells infected with plant viruses with ssRNA genome. The vesicles were formed at the tonoplast. Based on the symptomatology, the presence of fibril-containing vesicles, and graft-transmissibility, it is believed that the VLPs that occurred on symptomatic leaves of black necrotic leafspot of pear are viral in nature, possibly those of a capillovirus.

갑상샘 악성결절의 초음파영상에서 GLCM 알고리즘을 이용한 세포병리 진단의 후향적 분석 (Retrospective Analysis of Cytopathology using Gray Level Co-occurrence Matrix Algorithm for Thyroid Malignant Nodules in the Ultrasound Imaging)

  • 본 연구는 갑상샘 초음파 영상에서 정상 및 악성결절의 세포병리 진단결과를 바탕으로 GLCM 알고리즘분석을 통한 후향적 연구를 시행하여 컴퓨터보조진단의 적용 가능성을 평가하였다. GLCM 알고리즘의 6가지 파라미터를 이용한 갑상샘 악성결절의 인식률 평가와 ROC 곡선을 분석하였다. 실험 결과는 에너지 97%, 대조도 93%, 상관관계 92%, 동질성 92%, 엔트로피 100%, 분산 100%의 높은 질환인식률을 나타내었다. ROC 곡선 분석에서 각 파라미터의 곡선아래면적이 0.947(p=0.001) 이상을 나타내어 갑상샘 악성결절의 인식에 의미가 있는 결과로 나타났다. 또한 GLCM에서 각 파라미터의 cut-off값 결정으로 정량적인 컴퓨터보조진단의 분석을 통한 질환예측이 가능할 것으로 판단된다.

Double membrane-bound particles associated with eriophyid mite-borne plant diseases of unknown etiology : a potentially new group of plant viruses\ulcorner

  • Unique virus-like particles were associated with five eriophyid mite-borne plant diseases of unknown etiology; fig mosaic, redbud yellow ringspot, rose orsette, thistle mosaic, and high plains disease of corn and wheat. Quasi-spherical, double membrane-bound particles (DMPs), 120 - 200 nm in diameter, were observed in the cytoplasm of all cell types in symptomatic leaves of infected plants. No DMPs were observed in symptomless plants. The DMPs in symptomatic thistles were associated with two types of inclusions, electron-dense amorphous material and tubular aggregates. Similar amorphous inclusions were also found in corn and wheat with high plains disease, while tubular inclusions were observed in figs with mosaic symptoms. The particles and inclusions were similar in some aspects to immature particles associated with viroplasms of animal and insect poxviruses and also to the double-enveloped particles of tomato spotted wilt virus associated with viroplasms during early stages of infection, but were unique and unlike any known plant viruses. The DMPs and associated viroplasm-like inclusions in the high plains disease were specifically immunogold labeled in situ with the disease-specific antiserum. Thread-like structures, similar to tenuivirus particles, present in the partially purified virus preparations were also immunogold labeled with the antiserum. It is suggested that the thread-like structures are derived from the DMP. In many cells of symptomatic corn and wheat samples, DMPs occurred together with flexuous rod-shaped particles and cylindrical inclusions of wheat streak mosaic potyvirus (WSMV), suggesting that the disease is caused by a mixed infection of WSMV and the agent represented by the DMPs. Based on cytopathology, symptomatology and mite and/or graft-transmissibility, the five diseases described in this paper are potentially caused by virus(es) and the DMPs associated with these diseases may represent virus particles. If the DMPs are indeed viral in nature, they would comprise a new group of plant viruses.

어린 Siberian Husky 견의 섬유성 골형성장애의 세포학적 고찰 증례 (Cytologic Aspect of Fibrous Osteodystrophy in a Juvenile Siberian Husky)

  • 5개월령의 거세하지 않은 암컷 시베리안 허스키가 상악 병변의 평가를 위해 내원하였다. CBC, 혈청 화학, 요검사 소견에서 염증과 신부전이 지시되었다. 병변의 세포학 검사에서는 거대 다핵 세포들과 원형$\sim$방추형의 다향한 모양의 세포들이 개별적으로 탈락되거나 군집을 형성하였다. 두 종류의 세포들 모두 중등도의 핵 대소부동증과 세포부동증을 보였고, 뚜렷한 한 개의 핵소체 또는 여러 개의 작은 핵소체를 갖고 있었으며 성긴 염색질이었다. 조직병리학적 검사 결과, 상악과 비갑개는 전반적으로 확장되어 있었으며 결합조직에 의해 대체되어 있었으며 신장에서는 형성 이상이 관찰되었다. 이러한 결과를 바탕으로, 신장형성이상과 섬유화로 인한 섬유성 골형성장애로 진단되었다. 대증적인 처치에도 불구하고 환자의 상태는 악화되어 안락사 되었다.

Inflammation Enhanced X-irradiation-Induced Colonic Tumorigenesis in the Min mouse

  • Inflammation is potential risk factor of various human malignancies. Inflammatory bowel syndromes such as ulcerative colitis are well known as risk factors for colon cancer. Here, we examined enhancing effects of dextran sulfate sodium (DSS)-associated inflammation on X-irradiation induced colonic tumorigenesis in Min and wild-type (WT) mice. Animals were X-irradiated at 1.5 Gy at 5 weeks of age (at 0 experimental week) and 2% DSS in drinking water was administered at 5 or 11 experimental weeks. Mice were sacrificed at 16 weeks and incidence and multiplicity of colonic tumors were assessed. Incidence of colonic tumors in Min mouse was increased from 33.3% to 100% (p<0.05) with X-irradiation alone, whereas no tumors were developed in WT mice. In DSS-treated Min mice, X-irradiation increased the number of colonic tumors. Total number of colonic tumors was increased 1.57 times to $30.7{\pm}3.83$ tumors/mouse with X-irradiation+DSS at 5 weeks comapared to $19.6{\pm}2.9$ in corresponding DSS alone group (p<0.05). When the duration of inflammation was compared, longer period of DSS effect promoted more colonic tumorigenesis. Collectively, we conclude that X-irradiation and DSS-induced inflammation act synergistically for colonic tumorigenesis.

Application of Immunophenotyping and Heteroduplex Polymerase Chain Reaction (hPARR) for Diagnosis of Canine Lymphomas

  • Background: Canine malignant lymphoma is classified into B- or T-cell origin, as in the human case. Due to differences in prognosis, a suitable method needs to be developed for lineage identification. Aims: To determine the accuracy of immunophenotypic and molecular information between three methods: immunocytochemistry (ICC), immunohistochemistry (IHC) and heteroduplex polymerase chain reaction for antigen receptor rearrangements (hPARR) in spontaneous canine lymphomas. Materials and Methods: Peripheral blood, fine needle aspiration and tissue biopsies from enlarged peripheral lymph nodes prior to treatment of 28 multicentric lymphoma patients were collected. Cytopathology and histopathology were examined and classified using the updated Kiel and WHO classifications, respectively. Anti-Pax5 and anti-CD3 antibodies as B- and T-cell markers were applied for immunophenotyping by ICC and IHC. Neoplastic lymphocytes from lymph node and white blood cell pellets from peripheral blood were evaluated by hPARR. Results: In this study, low grade B-cell lymphoma accounted for 25% (7/28), high grade B-cell lymphoma for 64.3% (18/28) and high grade T-cell lymphoma for 10.7% (3/28). According to the WHO classification, 50% of all cases were classified as diffuse large B-cell lymphoma. In addition, ICC showed concordant results with IHC; all B-cell lymphomas showed Pax5+/CD3, and all T-cell lymphomas exhibited Pax5-/CD3+. In contrast to hPARR, 12 B-cell lymphomas featured the IgH gene; seven presented the $TCR{\gamma}$ gene; five cases showed both IgH and $TCR{\gamma}$ genes, and one case were indeterminate. Three T-cell lymphomas showed the $TCR{\gamma}$ gene. The percentage agreement between hPARR and ICC/IHC was 60%. Conclusions: Immunophenotyping should not rely on a single method. ICC or IHC with hPARR should be used concurrently for immunophenotypic diagnosis in canine lymphomas.

The Usefulness of Immunocytochemistry of CD56 in Determining Malignancy from Indeterminate Thyroid Fine-Needle Aspiration Cytology

  • Background: Fine-needle aspiration cytology serves as a safe, economical tool in evaluating thyroid nodules. However, about 30% of the samples are categorized as indeterminate. Hence, many immunocytochemistry markers have been studied, but there has not been a single outstanding marker. We studied the efficacy of CD56 with human bone marrow endothelial cell marker-1 (HBME-1) in diagnosis in the Bethesda System for Reporting Thyroid Cytopathology (TBSRTC) category III. Methods: We reviewed ThinPrep liquid-based cytology (LBC) samples with Papanicolaou stain from July 1 to December 31, 2016 (2,195 cases) and selected TBSRTC category III cases (n=363). Twenty-six cases were histologically confirmed as benign (six cases, 23%) or malignant (20 cases, 77%); we stained 26 LBC slides with HBME-1 and CD56 through the cell transfer method. For evaluation of reactivity of immunocytochemistry, we chose atypical follicular cell clusters. Results: CD56 was not reactive in 18 of 20 cases (90%) of malignant nodules and showed cytoplasmic positivity in five of six cases (83%) of benign nodules. CD56 showed high sensitivity (90.0%) and relatively low specificity (83.3%) in detecting malignancy (p=.004). HBME-1 was reactive in 17 of 20 cases (85%) of malignant nodules and was not reactive in five of six cases (83%) of benign nodules. HBME-1 showed slightly lower sensitivity (85.0%) than CD56. The specificity in detecting malignancy by HBME-1 was similar to that of CD56 (83.3%, p=.008). CD56 and HBME-1 tests combined showed lower sensitivity (75.0% vs 90%) and higher specificity (93.8% vs 83.3%) in detecting malignancy compared to using CD56 alone. Conclusions: Using CD56 alone showed relatively low specificity despite high sensitivity for detecting malignancy. Combining CD56 with HBME-1 could increase the specificity. Thus, we suggest that CD56 could be a useful preoperative marker for differential diagnosis of TBSRTC category III samples.
