  • Back pains include pain in one or both side costa portion and lateral abdomen. There are different kinds of flank-related disease such as intercostal nerve pain, chronic gastritis, duodenitis, herpes zoster, pepticulcer, cholecystitis, cholelithiasis and so on. Back pain arises from pathological abnormal activities. On the other hand psychogenic pains are caused by psychogenic factors without pathological abnormal activities. This is similar to KiWol back pain, which resulted from psychogenic factors like stress, anxiety, anger and etc. In oriental medicine, KiWol is the state of depression of Ki, and Shihosogansan used to treat the back pain diagnosed as stagnation of the liver Ki. So, we decided to apply Shihosogansan to a patient who suffered from back pain diagnosed as stagnation of the liver Ki. Therefore the patient treated with Shihosogansan and improved in consciousness symptoms, so we report it for the better treatment.

  • The present study attempted to modify the conventional Melodic Intonation Therapy (MIT) in three aspects: number of syllables of adjacent target utterances (ATU), melody patterns of ATU, and initial listening of melody and intoned speech with the eyes closed. The modified Melodic Intonation Therapy (MMIT) was applied to two severe Korean aphasics. The patients exhibited a severely nonfluent aphasia resulting from a left CVA(Cerebrovascular Accident). The purpose of the modification was to avoid perseveration and improve reflective listening skills. First, the treatment program avoided ATU with the same number of syllables. Second, four different patterns of melody were developed: rising type, falling type, V-type, and inverted V-type. One type of prosodic pattern was preceded and followed by another type of melody. These two variations were to decrease perseverative behaviors. Finally, the patients kept their eyes closed when the clinician played and hummed a target melody at the initial stage of the program in order to improve reflective listening skills. A single-subject alternating treatment design was used. The effects of MMIT were compared to the conventional MIT. Differing the number of syllables and the type of melodic patterns decreased perseverative behaviors and produced more correct names. The initial listening of the target melody with the patients' eyes closed seemed to increase their attentiveness and result in a more fluent production of target utterances. Probable reasons for the effectiveness of MMIT were discussed.

  • Complex Regional Pain Syndrome(CRPS) is a chronic neruopathic pain syndrome, most frequently encountered after trauma, nerve injury, fracture, cerebral vascular accident(CVA), spinal injury, and surgery to a limb, and affected by diverse factors such as disease process, intervention method, and other biopsychosocial components. Occupational therapy as a multidisciplinary treatment approach may be potentially useful when simultaneously targeting such biological, psychological, and social components. Biological treatment can focus on increasing functional use of extremity for promoting independence. Psychological intervention can include relaxation/biofeedback training and cognitive-behavioral treatments, and social approach can focus on recreational therapy and vocational rehabilitation. Occupational therapy will encourage functional restoration, pain management, and regaining of psychological components with collaboration between different disciplines.

  • Tracheo-innominate artery fistula (TIF) is a rare but catastrophic and almost always fatal complication of tracheostomy. TIF can occur anytime but is commonly present 3 to 24 days after tracheostomy. It can first manifest as massive bleeding around and through the tracheostomy tube, but it can also manifest as a small amount of blood with temporary spontaneous resolution. If TIF is suspicious, airway management and prompt surgical intervention are needed. In an 83-year-old man with CVA history 20 years earlier and who had recurrent aspiration pneumonia, tracheostomy was performed for respiratory management and ventilator support. On day 7 post-tracheostomy, the patient had bleeding from the tracheostoma. Immediate surgical exploration was performed to control the bleeding. A defect was seen at the post wall of the innominate artery. The erosive portion of the artery was sutured, but the patient died three weeks after the surgery due to rebleeding and respiratory failure. We present a patient who developed TIF after tracheostomy, with literature review.

  • A multiplex RT-PCR (mRT-PCR) assay was developed for the detection of the recently reported viruses, Cherry virus A (CVA), Little cherry virus 1 (LChV-1), and Little cherry virus 2 (LChV-2), in cherry plants in Korea. Eight sets of primers were designed for each virus and their specificity was tested by using various combinations of mixed primer sets. From the designed primer sets, one combination was selected and further evaluated to estimate the optimum temperature and detection limits of the mRT-PCR. A newly developed mRT-PCR assay was also tested using 20 cherry samples collected in the field. This mRT-PCR assay may be a useful tool for field surveys of diseases and the rapid detection of these three viruses in cherry plants.
